ABSTRACT

Introduction: Subepithelial lesions (SELs), described as bulges or masses covered by healthy-looking mucosa, are usually found incidentally during endoscopic studies. They are typically asymptomatic and are estimated to be identified in 1% of esophagogastroduodenoscopies performed. Materials and methods: A descriptive study was conducted with retrospective data collection. We included all patients treated at the Unión de Cirujanos, a referral gastroenterology unit of the Coffee Region in Manizales, between January 2020 and January 2022, who underwent endoscopic ultrasonography to study subepithelial-looking lesions located in the esophagus, stomach, and duodenum. Results: 152 endoscopic ultrasounds were performed, finding 108 SELs; 66.6% of the patients were women, and the average age was 58. Most SELs were located in the stomach (78.7%), the antrum being the most frequent location. The average diameter of the gastric SELs was 14.6 mm, and 47% of the lesions depended on the fourth echolayer; the most frequent presumptive diagnoses were gastrointestinal stromal tumor (GIST; 65.8%) and lipoma (11.7%). Conclusions: SELs of the GI tract originate in the muscularis mucosae, submucosa, or muscularis propria. They are most frequently located in the stomach, and their characterization usually requires endoscopic ultrasonography and histopathology. Treatment of these lesions remains controversial due to their low frequency, histological variety, and low malignant potential.


Introducción: las lesiones subepiteliales (LSE), descritas como abultamientos o masas cubiertas por mucosa de aspecto sano, se encuentran usualmente de manera incidental durante estudios endoscópicos; suelen ser asintomáticas y se estima que se identifican en el 1% de las esofagogastroduodenoscopias realizadas. Métodos: se realizó un estudio descriptivo con recolección retrospectiva de la información. Se incluyeron todos los pacientes atendidos en Unión de Cirujanos, unidad de gastroenterología de referencia del Eje Cafetero ubicada en la ciudad de Manizales, entre enero de 2020 y enero de 2022, a quienes se les realizó ultrasonografía endoscópica como parte del estudio de lesiones de aspecto subepitelial localizadas en el esófago, estómago y duodeno. Resultados: se realizaron 152 endosonografías y se encontraron 108 lesiones subepiteliales, 66,6% de los pacientes eran mujeres y el promedio de edad fue 58 años. La mayoría de las LSE se localizaron en el estómago (78,7%) y, de estas, la localización más frecuente fue el antro; el diámetro promedio de las LSE gástricas fue de 14,6 mm y el 47% de las lesiones eran dependientes de la cuarta ecocapa; los diagnósticos presuntivos más frecuentes fueron el tumor del estroma gastrointestinal (GIST; 65,8%) y lipoma (11,7%). Conclusiones: las LSE del tracto gastrointestinal se originan en la muscular de la mucosa, submucosa o muscular propia, de manera más frecuente se localizan en el estómago y su caracterización suele requerir la realización de ultrasonografía endoscópica y estudio histopatológico. El tratamiento de estas lesiones sigue siendo controversial debido a su baja frecuencia, variedad histológica y bajo potencial maligno.

ABSTRACT

Objetivo: Evaluar los resultados refractivos y visuales post cirugía refractiva corneal con láser de excímero (LASEK-MMC o PRK-MMC) en pacientes miopes con o sin astigmatismo asociado. Método: Se realizó un estudio preexperimental del tipo antes y después, en el que fueron incluidos 81 pacientes (162 ojos) tratados con LASEK-MMC (65 pacientes) o PRK-MMC (16 pacientes) seguidos por tres meses. Se les realizó un examen oftalmológico completo y se evaluaron las variables: edad, sexo, diagnóstico y tipo de miopía, así como agudeza visual, esfera, cilindro, equivalente esférico precirugía y poscirugía. Además, se analizó la función visual y los resultados de la refracción. Resultados: El valor de la mediana de edad fue de 24,0 años en los pacientes tratados con LASEK-MMC y en el grupo con PRK-MMC fue de 23,0 años. En ambos grupos existió un predominio del sexo femenino. La totalidad de los pacientes tratados presentaba un astigmatismo miópico compuesto con niveles de miopía leve. A los tres meses el 96,9 por ciento de los pacientes tratados con LASEK-MMC y el 93,8 por ciento de los tratados con PRK-MMC tenían una agudeza visual no corregida de 20/20 o más y todos tenían una visión de 20/40 o más. Conclusiones: La cirugía fotoablativa con láser tiene buenos resultados refractivos y visuales ya que la mayoría de los pacientes quedaron emétropes con una marcada mejoría de la agudeza visual sin corrección y de la refracción en el posoperatorio(AU)


Objective: To evaluate refractive and visual outcomes after excimer laser corneal refractive surgery (laser-assisted subepithelial keratectomy [LASEK] or photorefractive keratectomy [PRK], with intraoperative application of mitomycin-C [MMC]) in myopic patients with or without associated astigmatism. Methods: A preexperimental, before-and-after study was carried out with 81 patients (162 eyes) treated with LASEK-MMC (65 patients) or PRK-MMC (16 patients) and followed for three months. A complete ophthalmologic examination was done and the following variables age, sex, diagnosis and type of myopia were assessed, as well as pre-surgery and post-surgery visual acuity, sphere, cylinder and spherical equivalent. In addition, visual function and the refraction results were analyzed. Results: The median age was 24.0 years in the LASEK-MMC group and 23.0 years in the PRK-MMC group. In both groups there was a predominance of the female sex. All treated patients had compound myopic astigmatism with mild myopia. At three months, 96.9 percent of patients treated with LASEK-MMC and 93.8 percent of patients treated with PRK-MMC had uncorrected visual acuity of 20/20 or better, and all had vision of 20/40 or better. Conclusions: Laser photoablative surgery has good refractive and visual outcomes as most patients became emmetropic with marked improvement in uncorrected visual acuity and refraction postoperatively(AU)

ABSTRACT

Objective: To assess the effectiveness and safety of cap-assisted endoscopic resection and the usefulness of endoscopic ultrasonography (EUS) for managing small rectal subepithelial tumors (SETs). Patients and methods: Patients with small rectal SETs≤10mm in diameter were enrolled in this study at our hospital from October 2014 to December 2017. First, EUS was performed to evaluate the lesions. Then, cap-assisted endoscopic resection was performed by suctioning the SET into a transparent cap, ligating with a metal snare and then resecting the tumor. The wound was closed using endoclips if necessary. Results: Forty patients were enrolled in the study. EUS showed lesions originating from muscularis mucosa or submucosa with an average diameter of 5.4×3.1mm. The en bloc resection rate was 85.0% obtained by cap-assisted endoscopic resection, with a mean total procedure time of 17.6min. No immediate perforation happened. Immediate bleeding occurred in five patients; all cases were managed successfully by endoscopy. No delayed bleeding was observed. Pathology examination showed that 70.0% of the lesions were neuroendocrine tumors (G1). One case of recurrence was seen in follow-up; it was managed successfully by endoscopic submucosal dissection. There was no tumor recurrence in a median follow-up period of 41 months in the remaining 39 patients. Conclusions: Most small rectal SETs arising from the muscularis mucosa or submucosa are neuroendocrine tumors and require proper treatment. Cap-assisted endoscopic resection is simple, effective and safe for resecting such lesions, and EUS is useful for case screening.(AU)


Objetivo: Evaluar la eficacia y la seguridad de la resección endoscópica asistida por capuchón y la utilidad de la ultrasonografía endoscópica (USE) para el tratamiento de pequeños tumores subepiteliales (TSE) rectales. Pacientes y métodos: Los pacientes con TSE rectales pequeños ≤10 mm de diámetro se enrolaron en este estudio en nuestro hospital desde octubre de 2014 hasta diciembre de 2017. Primero, se realizó una USE para evaluar las lesiones. Luego, se realizó una resección endoscópica asistida por capuchón aspirando el TSE en un capuchón transparente, ligándolo con una asa metálica de polipectomía y luego resecando el tumor. La herida se cerró usando endoclips, si ello era necesario. Resultados: Cuarenta pacientes fueron enrolados en el estudio. La USE mostró lesiones originadas en la muscularis mucosae o submucosa con un diámetro promedio de 5,4 × 3,1 mm. La tasa de resección en bloque fue del 85,0% obtenida mediante resección endoscópica asistida por capuchón, con un tiempo total medio de procedimiento de 17,6 min. No se produjo ninguna perforación en el momento. Se produjo una hemorragia inmediata en cinco pacientes; todos los casos se trataron con éxito mediante una endoscopia. No se observó ningún retraso en el sangrado. El examen patológico mostró que el 70% de las lesiones eran tumores neuroendocrinos (G1). En el seguimiento se observó un caso de recurrencia, el cual se trató con éxito mediante una disección endoscópica de la submucosa. No hubo recurrencia de tumores en un período de seguimiento medio de 41 meses en los 39 pacientes restantes. Conclusiones: La mayoría de los TES rectales pequeños que surgen de la muscularis mucosae o submucosa son tumores neuroendocrinos y requieren de un tratamiento adecuado. La resección endoscópica asistida por capuchón es simple, eficaz y segura para resecar tales lesiones y la USE es útil para la detección de casos.(AU)

ABSTRACT

The pre-operative differential diagnosis of gastric subepithelial lesions is complex. We can find pathologies with a very different behavior. Some of them, like gastrointestinal (GI) stromal tumors, can present a malignant behavior, and others like schwannomas are practically benign. Schwannomas of the GI tract originate from the Schwann cells of the Auerbach plexus and their most frequent location is the stomach. The definitive diagnosis is made by immunohistochemical analysis of the surgical specimen and its resection is curative. We report two cases of gastric subepithelial lesions with a definitive diagnosis of schwannoma.


El diagnóstico diferencial preoperatorio de las lesiones subepiteliales gástricas es complejo. Podemos encontrar patologías con un comportamiento muy diferente. Algunas de ellas, como los GIST (gastrointestinal stromal tumours), pueden presentar un comportamiento maligno, y otras, como los schwannomas, son prácticamente benignas. Los schwannomas del tracto gastrointestinal se originan de las células de Schwann del plexo de Auerbach y su localización más frecuente es el estómago. El diagnóstico definitivo se realiza mediante el análisis inmunohistoquímico de la pieza quirúrgica, y su resección es curativa. Reportamos dos casos de lesiones subepiteliales gástricas con diagnóstico definitivo de schwannoma.

ABSTRACT

BACKGROUND: Gastric lipomatosis (GL) is defined as a lipomatous lesion with diffuse infiltration of the submucosal layer by adipose tissue with multiple lesions and must be differentiated from gastrointestinal lipomas, which are solitary submucosal masses composed of well-differentiated adipose tissue surrounded by a fibrous capsule. GL are uncommon, represents 5 % of gastrointestinal tract lipomas and less than 1∼3 % of all gastric tumors. CASE PRESENTATION: A 65-year-old woman, who underwent non-operatively approach by Gastric lipomatosis. Non-specific gastrointestinal symptoms and endoscopic examination ruled out the possibility of mucosal ulceration or bleeding. Endoscopic ultrasonography and abdominal CT that confirmed gastric lipomatosis. DISCUSSION: Gastric lipomas are usually small, solitary and asymptomatic, and are hence often detected incidentally, surgical management in asymptomatic patients still be a in debate. CONCLUSION: Non operative management in massive gastic lipomatosis is appropiate when the patient is asymptomatic without mucosal ulceration, regardless of the amount of gastric lipomas. Avoiding patient the morbidity of a total gastrectomy.

ABSTRACT

Subepithelial lesions are generally an incidental diagnosis with an prevalence of 0.4%. These tumors represent a great diagnostic challenge, mainly when ruling out potentially malignant lesions, such as gastrointestinal stromal tumor (GIST), lymphomas and carcinomas. Among the many differential diagnosis, the ectopic pancreas arises with an prevalence of 1-2% in general population. The first diagnostic approach is performed using upper digestive endoscopy, computed tomography and endosonography. This last one has a diagnostic performance of less than 50%, which increases to 90% when it is associated with a histopathological examination. There is no current consensus regarding the management and monitoring of these lesions. Based on the imaging and histological characteristics, the possibilities range from observation to endoscopic or surgical resection. In this context, we will present a clinical case of ectopic pancreas as an incidental finding, and afterwards the diagnostic and therapeutic breakdown of subepithelial lesions.


Las lesiones subepiteliales son pesquisadas generalmente de manera incidental, con una prevalencia de 0,4%. Estos tumores suponen un gran desafío diagnóstico, principalmente al momento de descartar lesiones potencialmente malignas, como el tumor estromal gastrointestinal (GIST), linfomas y carcinomas. Dentro de los posibles diagnósticos, surge el páncreas ectópico, con una prevalencia de hasta 1-2% en la población general. La primera aproximación diagnóstica se realiza mediante endoscopia digestiva alta, tomografía computarizada y la endosonografía, ésta última con un rendimiento diagnóstico menor del 50%, que aumenta hasta el 90% al asociar el examen histopatológico. No existe consenso actual respecto al manejo y seguimiento de estas lesiones, que según sus características imagenológicas e histológicas, va desde la observación hasta la resección endoscópica o quirúrgica. En este contexto, se presenta un caso clínico de páncreas ectópico como hallazgo incidental y el desglose diagnóstico y terapéutico de las lesiones subepiteliales.

ABSTRACT

RESUMEN Objetivo: Evaluar los resultados relacionados con la efectividad y la seguridad en el tratamiento con queratectomía subepitelial asistida por láser con mitomicina C versus queratectomía fotorreactiva con mitomicina C en ojos con miopía o astigmatismo miópico compuesto. Métodos: Se realizó un estudio experimental, longitudinal y prospectivo en el Instituto Cubano de Oftalmología Ramón Pando Ferrer entre abril del año 2016 y abril de 2017. Se empleó un muestreo aleatorio simple por el cual se obtuvo la técnica quirúrgica a realizar. El primer ojo operado fue el derecho y a la semana se realizó la cirugía en el ojo izquierdo, en el cual se aplicó la otra técnica quirúrgica. Esto permitió realizar en cada paciente ambas técnicas quirúrgicas. La muestra quedó constituida por 146 ojos (73 pacientes) que cumplían con los criterios de selección. Resultados: El comportamiento preoperatorio entre los dos grupos fue muy similar. En ninguno de los casos existieron diferencias en las variables que se evaluaron (agudeza visual sin corrección, agudeza visual mejor corregida, esfera, cilindro y equivalente esférico). La diferencia entre los valores preoperatorios y los encontrados a los 6 meses del tratamiento quirúrgico en cada grupo fueron estadísticamente significativos (p= 0,000) para todas las variables analizadas, excepto para la agudeza visual mejor corregida en el grupo de ojos tratados con láser con mitomicina C (p= 0,083). El haze y el defecto de epitelización fueron los dos tipos de complicaciones observadas. Conclusiones: Se demostró que ambas técnicas quirúrgicas son efectivas y seguras(AU)


ABSTRACT Objective: Evaluate the effectiveness and safety of laser-assisted subepithelial keratectomy with mitomycin C vs. photoreactive keratectomy with mitomycin C in eyes with myopia or compound myopic astigmatism. Methods: An experimental prospective longitudinal study was conducted at Ramón Pando Ferrer Cuban Institute of Ophthalmology from April 2016 to April 2017. Simple random sampling was used to decide on the surgical technique to be applied. Surgery was first performed on the right eye using one of the techniques, and then one week later on the left eye with the other technique. That way each patient could undergo both surgical techniques. The sample was composed of 146 eyes (73 patients) meeting the inclusion criteria. Results: Preoperative behavior was very similar in the two groups. In neither case were differences found in the variables analyzed (uncorrected visual acuity, best corrected visual acuity, sphere, cylinder and spherical equivalent). The differences between preoperative values and those found six months after surgery in each group were statistically significant for all the variables analyzed (p= 0.000), except for best corrected visual acuity in the group of eyes treated with laser with mitomycin C (p= 0.083). Haze and epithelization defect were the two types of complications observed. Conclusions: It was demonstrated that both surgical techniques are effective and safe(AU)

ABSTRACT

Algumas das principais indicações para o recobrimento de superfícies radiculares expostas por recessão gengival compreendem a hipersensibilidade dentinária cervical e o tratamento de defeitos estéticos. Este trabalho descreveu o tratamento de recessão gengival múltipla classe III de Miller em um periodonto delgado, após 36 meses de uma intervenção cirúrgica com enxerto gengival livre. O tratamento cirúrgico periodontal proposto foi o recobrimento radicular por meio de enxerto de tecido conjuntivo subepitelial associado a retalho reposicionado coronalmente na região vestibular dos elementos 31, 41 e 42. Após 90 dias, foi possível observar o recobrimento parcial da recessão, aumento da espessura tecidual e o controle do biofi lme pelo paciente, promovendo maior proteção da região e redução do risco de recidiva das recessões. Diante da resolução do caso clínico apresentado, pôde-se concluir que a técnica do enxerto de tecido conjuntivo subepitelial associado ao retalho reposicionado coronalmente, quando bem indicada e realizada, pode ser empregada com sucesso em áreas de recessões múltiplas e periodonto com espessura reduzida.


Some of the main indications for the root coverage of gingival recessions include cervical dentin hypersensitivity and treatment of aesthetic defects. This manuscript describes the treatment of a Miller's Class III multiple gingival recession 36 months after a free gingival graft surgical procedure. The periodontal surgical treatment proposed was the subepithelial connective tissue graft associated with a coronally repositioned fl ap at vestibular region of elements 31, 41 and 42. After 90 postoperative days, it was possible to observe an increase on tissue thickness and biofi lm control by the patient, promoting a greater protection of the region and reducing the risk of recession recurrence. It can be concluded that the subepithelial connective tissue graft technique associated with the coronal repositioned fl ap, when well indicated and performed, can be successfully used in multiple recessions and periodontal regions with reduced thickness.

ABSTRACT

Objetivo: apresentar um relato de caso cirúrgico para tratamento de uma retração unitária classe II de Miller, com um acompanhamento de seis meses. Relato de caso: retração classe II de Miller no dente 31 foi submetida a um procedimento cirúrgico com enxerto subepitelial, associado ao deslocamento coronário do retalho, em uma paciente com queixa de sensibilidade dentinária e dificuldade de higienização local. Resultados: os resultados demonstraram recobrimento radicular satisfatório, atingindo cerca de 83% da superfície radicular, e um evidente aumento da altura e espessura de tecido gengival queratinizado, bem como melhora da sensibilidade dentinária e capacidade de higienização local. Conclusão: o enxerto subepitelial associado ao deslize coronário do retalho proposto neste caso foi uma abordagem eficiente para o recobrimento radicular de uma lesão classe II de Miller, pois proporcionou alta taxa de recobrimento, ganho de tecido queratinizado e satisfez o anseio da paciente.


Objective: to present a surgical case report for treatment of Miller Class II single defect at 6 months follow-up. Case report: a Miller Class II recession at tooth 31 underwent a surgical procedure with subepithelial connective tissue associated with coronally advanced flap in a patient complaining of dentin hypersensitivity and local hygiene difficulty. Results: the results showed satisfactory root coverage reaching about 83% of the root surface and an evident increase in width and thickness of keratinized tissue, as well as improvement of dentin hypersensitivity and local hygiene ability. Conclusion: the subepithelial connective tissue associated with coronally advanced flap proposed in this case was an efficient approach to root coverage of a Miller class II lesion because it provided a high level of root coverage, keratinized tissue gain and satisfied the patient's longing.

ABSTRACT

Objetivo: describir, mediante un caso clínico, una técnica quirúrgica para la optimización del injerto conectivo libre subepitelial en la cobertura de recesiones gingivales contiguas múltiples. Caso clínico: Una paciente de sexo femenino, de 57 años de edad, concurrió a la consulta con recesiones clase III de Miller, abfracciones, erosiones y malposición. El procedimiento quirúrgico elegido fue la técnica de desplazado coronal con incisiones oblicuas. Se realizó la toma de un injerto conectivo subepitelial y se lo optimizó en seis porciones trian-gulares, de manera tal que cada una abarcara una recesión desde el 13 hasta el 23. Se efectuó el seguimiento de la cicatrización y de la estabilidad de la cobertura radicular a los 90 días.Conclusión: Este nuevo abordaje quirúrgico es una alternativa válida para cubrir recesiones gingivales múltiples en un solo procedimiento, al poder aprovechar al máximo la utilización de un injerto conectivo libre subepitelial.


Aim: to present the surgical resolution of a clinical caseof multiple gingival recessions treated with a novel approach.Case report: A 57 year-old non-smoking female patientwith Miller Class III gingival recessions abfractions, erosionsand tooth malposition came to the consultation. The treatmentthat was chosen consisted in covering the gingival recessionswith a coronal advanced flap with oblique incisions. A subephitelialgingival graft was harvested from the palate andan optimization in triangular portions was made, in order touse one portion for each recession. Healing and stability wererecorded for a 90 days follow-up period.Conclusion: This new surgical approach could be analternative for the treatment of multiple gingival recessionsin a single procedure, as the use of the ILS could be maximized.

ABSTRACT

OBJECTIVE: To determine the anatomical and functional outcomes of corneal refractive surgery in patients on amiodarone, a drug listed as being contraindicated in patients undergoing this procedure. MATERIAL AND METHODS: A retrospective observational study was conducted on all consecutive patients who took amiodarone and who underwent LASIK or surface ablation from January 2003 to December 2014. Functional (visual and refractive) outcomes are described. RESULTS: A total of 20 patients (33 eyes) were included. No significant intraoperative or postoperative complications were found. CONCLUSIONS: In our experience, LASIK and surface ablation did not produce significant clinical complications in selected patients taking amiodarone. The absolute exclusion of certain systemic medications should be reconsidered.

ABSTRACT

Objetivo: mostrar y evaluar los resultados clínicos de un procedimiento de cirugía plástica periodontal, efectuada para cubrir una recesión radicular de clase III de Miller, con una técnica bilaminar. Además, analizar el aumento de la banda de encía y el cambio de biotipo gingival de la pieza dentaria. Caso clínico: paciente femenino de 30 años de edad, con una recesión gingival de Clase III de Miller por vestibular de la pieza 43 que no sobrepasa la línea mucogingival, con pérdida interproximal de tejidos duros y blandos. El tratamiento consiste en un colgajo de doble papila a espesor parcial, con injerto libre subepitelial tomado del paladar, con seguimiento a 1 año. Conclusiones: la técnica bilaminar es una solución viable en casos de recubrimiento radicular poco predecibles, como la recesión de clase III de Miller. El biotipo gingival se vio engrosado y la encía queratinizada no sufrió variaciones.

ABSTRACT

O objetivo desse trabalho foi apresentar o relato de caso de uma paciente com queixa estética e funcional de uma prótese sobre implante osseointegrado, que estava instalado em posição tridimensional equivocada, na região do incisivo central superior esquerdo. Essa situação provocou extensas perdas de tecido ósseo e tecido mole peri-implantar. O tratamento iniciou-se pela remoção da prótese e do implante instalado, seguido sequencialmente de enxerto de tecido conjuntivo removido do palato, instalação de novo implante na posição adequada concomitantemente à regeneração óssea guiada e novo enxerto de conjuntivo no momento da reabertura. Tais procedimentos promoveram um ganho tecidual que, aliado à cirurgia plástica periodontal e à finalização protética, permitiram uma condição reabilitadora estética e funcional mais condizente com os anseios da paciente. O acompanhamento clínico e tomográfico de 2 anos permitiu uma avaliação das estabilidades teciduais e da previsibilidade dos procedimentos (AU).


The aim of this study was to present the case report of a patient with aesthetic and functional complaint of prosthesis on dental implant, which was installed in wrong three-dimensional position in the left maxillary central incisor region. This resulted in extensive bone and peri-implant soft tissue loss. Treatment was initiated removing the prosthesis and implant installed, followed sequentially by connective tissue graft removed from the palate, new implant installation in the proper position concurrently with guided bone regeneration and new connective tissue graft in the moment of reopening were performed. Such procedures promoted a tissue gain that, combined with periodontal plastic surgery and prosthetic completion, allowed an aesthetic and functional rehabilitative condition more consistent with the patient's wish. Two years of clinical and tomographic follow-up allowed an assessment of tissue stability and procedures predictability (AU).

ABSTRACT

Se presenta un caso clínico donde se evaluó si la agregación del derivado de la matriz del esmalte (DME) al procedimiento del colgajo de avance coronal con injerto de tejido conectivo subepitelial (CDC + ITCS) mejoraría la cantidad de cobertura radicular en recesiones gingivales clase I y II de Miller comparados con el mismo procedimiento solo, en un paciente con recesiones gingivales múltiples a seis meses. Se incluyeron 12 recesiones gingivales, seis tratadas con (CAC + ITCSE + DME) y seis con (CAC + ITCSE) en diferentes cuadrantes. Al inicio y a los seis meses se midieron los parámetros clínicos tales como profundidad de la recesión gingival (PR), profundidad al sondeo (PS), nivel de inserción clínica (NIC), y ancho de tejido queratinizado en dirección apico-coronal (TQ). Un valor p < 0.05 se consideró significativo. Los resultados mostraron que a los seis meses ambos procedimientos, CAC + ITCSE + DME y CAC + ITCSE produjeron una significativa cobertura radicular en promedio 2.83 ± 1.16 mm (p = 0.001) y 2.50 ± 0.83 mm (p = .002), respectivamente. Todas las recesiones gingivales tratadas con el DME tuvieron el 100% de cobertura radicular y sólo el 65.3% de cobertura para los sitios tratados con CAC + ITCSE. Al comparar ambos procedimientos a los seis meses, se observaron mejores resultados con CAC + ITCSE + DME en cuanto al nivel de inserción clínica (p = .02) y la cobertura radicular (p = .06); sin embargo, ni la diferencia del nivel de inserción clínico ni la ganancia en la cobertura radicular mostraron ser significativos. Por otro lado, no se observaron diferencias significativas en la PS y TQ. Conclusión: El presente caso clínico no mostró beneficio adicional cuando se agregó el DME al procedimiento de CAC + ITCSE para la cobertura de recesiones gingivales múltiples clase I y II de Miller.


The present article described a clinical case where it was assessed whether aggregation of enamel matrix derivative (EMD) to the procedure of coronary-advanced flap with sub-epithelial connective tissue graft (CAF + SCTG) would improve the amount of root coverage in Miller's class I and II gingival recessions when compared to the same isolated procedure in a patient suffering multiple gingival recessions, in a 6 month time-span. Twelve gingival recessions were included in the study: six treated with (CAF + SCTG + EMD) and six treated with (CAF + SCTG) in different quadrants. At beginning of procedure as well as six months later, the following clinical parameters were measured: gingival recession depth (RD), depth to probing (PD), clinical insertion level (CIL) and width of keratinized tissue (KT) in apex-coronary direction. A p < 0.05 was considered statistically significant. Results established that after a six month procedure CAF + SCTG + EMD and CAF + SCTG produced significant root coverage, respective averages were 2.83 ± 1.16 mm (p = 0.001) and 2.50 ± 0.83 mm (p = .002). All gingival recessions treated with EMD experienced 100% root coverage, sites treated with CAF + SCTG + EMD exhibited coverage of only 65.3%. When comparing results at six months, better results were observed with CAF + SCTG + EMD with respect to clinical insertion level (p = .02) and root coverage (p = .06). Nevertheless, neither the difference of clinical level insertion nor the gain in root coverage resulted significant. Additionally, no significant differences were observed between PD and KT. Conclusion: The present clinical case did not show additional benefits when EMD were aggregated to the CAF + SCTG in the coverage of multiple Miller's class I and class II gingival recessions.

ABSTRACT

Objetivo: presentar la resolución quirúrgica de un caso clínico de recesiones gingivales tratadas con técnica de túnel y la evaluación de los resultados a los 180 días. Caso clínico: un paciente de sexo masculino, de 21 años de edad, concurrió a la consulta con recesiones de clase I de Miller, abfracciones e hipersensibilidad en las piezas 1.4 y 1.5. El procedimiento quirúrgico elegido fue la técnica de túnel. Se efectuó el seguimiento de la cicatrización y de la estabilidad de la cobertura radicular a los 180 días. Conclusión: el recubrimiento radicular de recesiones de clase I de Miller es altamente predecible, ya que permite anticipar una cobertura del 100 por ciento, siempre y cuando el enfoque quirúrgico esté acompañado del tratamiento de los factores etiológicos, así como támbién de la selección de una técnica adecuada

ABSTRACT

Las lesiones o tumores subepiteliales (TSE) son raros. Se considera que en 1 de cada 300 endoscopias puede encontrarse un TSE, lo que corresponde al 0,36% de las endoscopias. De estos, solo el 10% se ubican en el duodeno, lo que proporciona una idea de lo infrecuente que es este hallazgo. Al realizar la endoscopia y detectar un TSE en el duodeno, inicialmente se describirá su tamaño, forma, color, movilidad, pulsación y finalmente la consistencia, la cual puede evaluarse con la pinza de biopsia cerrada. Esto permitirá detectar si es quístico, firme o si tiene el “signo de la almohada”, que es altamente sugestivo de lipoma. En caso contrario, está indicada la realización de una ecoendoscopia, especialmente si el tumor es mayor de 1 cm.


Subepithelial lesions (SE) are rare. They are found in only one out of every 300 endoscopies (0.36%). Of these, only 10% are located in the duodenum. When endoscopy does detect a subepithelial lesion in the duodenum it is important to make an initial description of its size, shape, color, mobility, and consistency to touch. Next, a closed biopsy sample must be taken with pincers and analyzed to detect whether it is a cyst and whether it has a pillow sign which is highly suggestive of lipoma. When the lesion is neither a cyst nor a lipoma, endoscopic ultrasound is indicated, especially if the lesion is larger than one centimeter.

Introdução: A ultrassonografia endoscópica é importante ferramenta para o diagnóstico diferencial dos abaulamentos subepiteliais do trato gastrointestinal por permitir delinear as diferentes camadas da parede do tubo digestivo. Neste estudo, analisamos o papel do método na avaliação das lesões intramurais subepiteliais e das compressões extrínsecas do trato gastrointestinal. Métodos: Uma análise retrospectiva do papel da ultrassonografia endoscópica para pacientes encaminhados com abaulamentos subepiteliais do esôfago, estômago, duodeno e retossigmoide foi conduzida em um centro de referência pelo mesmo examinador (CVL). Os achados ecográficos analisados foram a natureza da lesão, sua camada de origem, tamanho, bordos, ecogenicidade, homogeneidade e diagnóstico presuntivo. Resultados: De setembro/2009 a março/2013, 126 pacientes (63,5% mulheres; idade média: 51,6 anos) com 128 abaulamentos subepiteliais à endoscopia foram analisados. Das 128 lesões, 104 (81,2%) eram lesões intramurais. Outros 24 (18,8%) abaulamentos eram compressões extrínsecas, na maioria dos casos (n = 21), por órgãos e estruturas normais. Para a detecção de lesões intramurais pela imagem da ultrassonografia endoscópica, a sensibilidade, especificidade, valores preditivos positivo e negativo, e a acurácia do método foram de, respectivamente, 98,1%, 92%, 98,1%, 92% e 96,9%. Por sua vez, para a detecção de compressões extrínsecas pela ultrassonografia endoscópica, os mesmos parâmetros foram de, respectivamente, 92%, 100%, 100%, 98,1% e 98,5%. Quando da punção ecoguiada para lesões intramurais, agulhas calibrosas e lesões maiores de 2cm apresentaram melhores resultados quanto ao diagnóstico diferencial. Conclusões: A ultrassonografia endoscópica é método de grande acurácia para o diagnóstico diferencial entre lesões subepiteliais intramurais e compressões extrínsecas do trato gastrointestinal (AU)


Introduction: Endoscopic ultrasonography is an important tool for the differential diagnosis of gastrointestinal subepithelial bulges because it allows to outline the different layers of the wall of the digestive tract. In this study, we analyzed the role of the method in the evaluation of subepithelial intramural lesions and extrinsic compressions of the gastrointestinal tract. Methods: A retrospective analysis of the role of endoscopic ultrasound for patients with subepithelial bulges of the esophagus, stomach, duodenum and rectosigmoid was conducted in a reference center by the same examiner (CVL). The sonographic findings analyzed were the nature of the injury, its layer of origin, size, borders, echogenicity, homogeneity, and presumptive diagnosis. Results: From September 2009 to March 2013, 126 patients (63.5% women, mean age: 51.6 years) with 128 subepithelial bulges at endoscopy were analyzed. Of the 128 lesions, 104 (81.2%) were intramural lesions. Other 24 (18.8%) bulges were extrinsic compressions, in most cases (n = 21) by normal organs and structures. For the detection of intramural lesions by endoscopic ultrasound image, the sensitivity, specificity, positive and negative predictive values, and accuracy of the method were, respectively, 98.1%, 92%, 98.1%, 92% and 96.9%. In turn, for the detection of extrinsic compression by endoscopic ultrasound, the same parameters were, respectively, 92%, 100%, 100%, 98.1% and 98.5%. For ultrasonographic-guided puncture for intramural lesions, thicker needles and lesions larger than 2 cm showed better outcomes concerning differential diagnosis. Conclusions: Endoscopic ultrasound is a highly accurate method for the differential diagnosis between subepithelial intramural lesions and extrinsic compressions of the gastrointestinal tract (AU)
