ABSTRACT

BACKGROUND: Functional somatic disorder (FSD) is a prevalent disorder that can be severely disabling for the patient and is associated with major health costs. There are few formalized care programs for these disorders in the country, and their management encounters various difficulties, both in the diagnosis and its announcement and the treatment. Cognitive-behavioural and emotional therapy (CBT) is the standard on psychiatry care, and its efficacy has been demonstrated through several randomised controlled studies on the intensity of physical and psychological symptoms. INTERVENTION: At Lille's University Hospital Center, we have established the "ACSEPT" care pathway for TSF management which consists of a psychiatric consultation followed by an individual referral, including a treatment using repeated transcranial magnetic stimulation (rTMS) or the integration of a CBT-based psychoeducation group. This group had the particularity of caring for all patients presenting FSD regardless of the associated physical symptoms. Educating medical professionals and conducting FSD research were other goals of ACSEPT. DISCUSSION/CONCLUSION: ACSEPT allows an improvement in the care offered to these patients with a defined orientation and interdisciplinary, early, organized cares that are repeatable. Our goal is to study the effectiveness of these different interventions in subsequent studies, to continue the development of ACSEPT and to be able to distribute this intervention at the regional level to establish a clear care program allowing early management of these patients.

INTRODUCTION: The COVID-19 pandemic and accompanying containment measures created major changes to everyone's daily life that had an impact on mental health and psychological burden. METHODS: In five surveys of the COPSY study more than 1,500 parents of 7- to 17-year-olds and their children between the ages of 11 and 17 were interviewed using established mental health assessment tools and a self-developed item on the experience of mental burden. Pre-pandemic comparative data were drawn from the representative BELLA study (BEfragung zum seeLischen Wohbefinden und VerhAlten). RESULTS: The majority of children and adolescents as well as their parents in Germany felt stressed by the pandemic between 2020 and 2022. There was an increase in both mental health problems and symptoms of anxiety and depression at the beginning of the COVID-19 pandemic and during the second nationwide lockdown. A slight decrease in the subjective experience of psychological burden and mental health problems among children and their parents was observed in the fall of 2022. However, the prevalence of mental health problems and anxiety remained above pre-pandemic levels. DISCUSSION: The results illustrate the difficult situation of many children, adolescents and parents, even after 3 years of pandemic. Low-threshold counseling and support services as well as more therapy places and professional support services should be made available.

ABSTRACT

The Importance of Self-Regulation in the Development of Internalizing Symptoms During Middle Childhood Current research discusses low self-regulation abilities during childhood as risk factors for the development of internalizing symptoms. However, longitudinal studies investigatingmultiple self-regulation facets simultaneously are scarce. We examined whether impairments in various self-regulation facets (emotional reactivity, inhibition, inhibitory control, planning behavior) in middle childhood predict internalizing symptoms two years later and whether they make an incremental contribution when established risk factors (gender, family adversity) are considered. Furthermore, we investigated whether self-regulation facets predict later internalizing symptoms under consideration of internalizing symptoms at baseline.The sample consisted of 1,617 children (t1:Mage = 9.1, t2:Mage = 11.1 years), assessed at two measurement points. Internalizing symptoms were rated by parents on the Emotional Problems Scale of the Strengths and Difficulties Questionnaire. Self-regulation facets were measured at the first measurement point using various methods and informants. A stepwise regression analysis revealed that increased emotional reactivity, reduced inhibitory control, and reduced planning behavior significantly predicted later internalizing symptoms, explaining 14.8 % of the variance. Adding risk factors increased the explained variance by 2.5 %. Under consideration of baseline internalizing symptoms, reduced inhibition and inhibitory control significantly predicted later internalizing symptoms, while other self-regulation facets and risk factors did not. Results of this study may inform prevention and intervention measures.

Caring for people with Alzheimer's disease and related disorders is a complex process, that of a chronic illness. Psychological and behavioral symptoms associated to dementia can appear in all neuro-degenerative diseases to varying degrees, and depend on numerous factors that need to be understood in order to take appropriate action. We propose a systemic approach to psychological and behavioral symptoms, with a view to preventing their onset or reducing their severity.

Na esteira das lógicas da economia psíquica e da economia política desenvolvidas por Freud e Lacan, tomamos como cerne um tipo de defesa que aparece em muitos casos da clínica contemporânea - o desmentido da privação - para colocar em evidência sua relação com o discurso capitalista pensado por Lacan. O desmentido da privação é um modo de defesa subjetivo que aparece como um índice do fracasso do pai privador na passagem do segundo para o terceiro tempo do complexo de Édipo. Trata-se de uma forma de se defender da castração buscando satisfação pulsional sem mediação simbólica ­ busca essa que é fracassada e extrapola o princípio de prazer. Conclui-se que esse desmentido contemporâneo é um sintoma do próprio discurso capitalista. Se a for aclusão da castração no discurso capitalista é uma promessa igualmente fracassada, o desmentido da privação aparece na subjetividade tentando solucionar esse fracasso. Ele é um sinal de que o discurso capitalista não cumpre sua promessa - e isso por sua própria engrenagem lógica.


Dans le sillage des logiques d'économie psychique et d'économie politique développées par Freud et Lacan, nous nous concentrons sur un type de défense qui apparaît dans de nombreux cas de pratique clinique contemporaine - le déni de la privation - pour mettre en évidence sa relation avec le discours capitaliste d' accord avec ce qui fut pensé par Lacan. Le déni de la privation est une défense subjective qui apparaît comme un indice de l'échec du père qui prive dans le passage du deuxième au troisième stade du complexe d'Œdipe. C'est une manière de se défendre de la castration en recherchant une satisfaction pulsionnelle sans médiation symbolique ­ une recherche qui échoue et dépasse le principe du plaisir. Nous concluons que ce déni contemporain est un symptôme du discours capitaliste lui-même. Si la for clusion de la castration dans le discours capitaliste est une promesse également ratée, le déni de la privation apparaît dans la subjectivité qui tente de résoudre cet échec. C'est le signe que le discours capitalistene tient pas ses promesses - et cela par sa propre logique.


Following the logics of psychic economy and political economy developed by Freud and Lacan, we target a type of defense that appears in many cases of contemporary clinical practice - the denial of deprivation - to highlight its relationship with the capitalist discourse as thought by Lacan. The denial of deprivation is a subjective defense that appears as an index of the failure of the depriving father in the transition from the second to the third stage of the Oedipus complex. It is a way of defending oneself from castration by seeking instinctual satisfaction without symbolic mediation ­ a search that fails and goes beyond the pleasure principle. The conclusion is that this contemporary denial is a symptom of capitalist discourse itself. If the foreclosure of castration in capitalist discourse is an equally failed promise, the denial of deprivation appears in subjectivity trying to resolve this failure. It is a sign that capitalist discourse does not fulfill its promise - and this by its own logical perspective.

ABSTRACT

INTRODUCTION: Choanal atresia is a rare congenital anomaly in humans and animals, characterized by the absence of communication of one or both nasal cavities with the nasopharynx. The severity of clinical signs depends on the presence of unilateral versus bilateral stenosis as well as comorbidities. With bilateral atresia, respiration may be severely compromised particularly during sleep, as airflow can only occur when breathing through the open mouth. Various therapeutic modalities have been described in people and adopted for animals. All treatments may be associated with complications, the most important being post-therapeutic scar formation with re-stenosis. This report describes a 10-month-old British Shorthair cat with chronic unilateral serosal nasal discharge that changed to mucopurulent discharge. When acute neurological signs developed, the cat was presented to the veterinary hospital. A diagnosis of primary, membranous right sided choanal atresia was achieved via computed tomography (CT) and nasopharyngeal (posterior) rhinoscopy. Secondary changes included destructive rhinitis with progression to the CNS with a subdural empyema and meningoencephalitis. Retinal changes and aspiration bronchopneumonia were suspected additional complications. After recovery from the secondary infections, the membranous obstruction was perforated and dilated using a valvuloplasty balloon by an orthograde transnasal approach under endoscopic guidance from a retroflexed nasopharyngeal view. To prevent re-stenosis, a foley catheter was placed as a transient stent for 6 days. The cat recovered uneventfully and was asymptomatic after the stent removal. Endoscopic re-examination after 5 months confirmed a persistent opening and patency of the generated right choanal passage. The cat remains asymptomatic 10 months after the procedure. Transnasal endoscopic balloon dilation and transient stenting of choanal atresia is a minimally invasive and relatively simple procedure with potentially sustained success.


INTRODUCTION: L'atrésie des choanes est une anomalie congénitale rare chez l'homme et l'animal, caractérisée par l'absence de communication d'une ou des deux cavités nasales avec le nasopharynx. La gravité des signes cliniques dépend de la présence d'une sténose unilatérale ou bilatérale, ainsi que des comorbidités. En cas d'atrésie bilatérale, la respiration peut être gravement compromise, en particulier pendant le sommeil, car l'air ne peut circuler que par la bouche ouverte. Diverses modalités thérapeutiques ont été décrites chez l'homme et adaptées pour les animaux. Tous les traitements peuvent être associés à des complications, la plus importante étant la formation de cicatrices post-thérapeutiques avec resténose. Ce rapport décrit un chat British Shorthair de 10 mois présentant un écoulement nasal séreux unilatéral chronique qui s'est finalement transformé en un écoulement muco-purulent. Lorsque des signes neurologiques aigus sont apparus, le chat a été présenté à l'hôpital vétérinaire. La tomodensitométrie (CT) et la rhinoscopie nasopharyngée (postérieure) ont permis de diagnostiquer une atrésie choanale primaire membraneuse du côté droit. Les altératiins secondaires comprenaient une rhinite destructrice avec une progression vers le SNC avec empyème sous-dural et méningo-encéphalite. Des altérations de la rétine et une bronchopneumonie par aspiration étaient des complications supplémentaires présumées. Après guérison des infections secondaires, l'obstruction membraneuse a été perforée et dilatée à l'aide d'un ballonnet de valvuloplastie par une approche transnasale orthograde sous guidage endoscopique à partir d'une vue nasopharyngée rétrofléchie. Pour éviter une nouvelle sténose, une sonde de Foley a été placée comme stent transitoire pendant 6 jours. Le chat s'est rétabli sans incident et était asymptomatique après le retrait du stent. Le réexamen endoscopique effectué 5 mois plus tard a confirmé la persistance de l'ouverture et de la perméabilité de la voie choanale droite générée. Le chat reste asymptomatique 10 mois après l'intervention. La dilatation endoscopique transnasale par ballonnet et la pose d'une endoprothèse transitoire dans le cas d'une atrésie des choanes est une procédure peu invasive et relativement simple dont le succès peut être durable.

Between November 2020 and June 2021, twelve children were treated at a university hospital in western France for pediatric multisystem inflammatory syndrome (PIMS). While the clinical presentation may have been reminiscent of Kawasaki disease, PIMS, a new nosological entity, was mentioned in the media in the context of the Covid-19 pandemic. In 2023, research into this syndrome will continue in France and Europe.

ABSTRACT

When we talk about the clinic of delusional symptoms, we are first and foremost affirming that delusions exist. Professionals working in psychiatry know that delusions can be listened to, that they give meaning to suffering and are part of the history of the subject. Taking the time to reflect on this type of symptom allows us to invest in the clinical field specific to psychiatry, and only makes sense if the clinical approach is based on work referring to the patient's individual history. Maintaining and developing these attitudes to care is essential to the survival of psychiatry as a discipline. Focusing on the clinic of delusion is neither a luxury nor a waste of time.

ABSTRACT

Prevalence of neuropsychiatric symptoms of people with dementia in long-term care units: A secondary analysis Abstract: Background: In a progress of dementia, most people develop neuropsychiatric symptoms. However, there is little knowledge about the prevalence of these symptoms and their specific characteristics in long-term care. Aims: A differentiated investigation of the prevalence and characteristics of neuropsychiatric symptoms in people with dementia in a long-term care setting. Methods: The prevalence of neuropsychiatric symptoms of people with dementia in a long-term care setting was examined using a secondary analysis of cross-sectional data from the research projects LebenQD I and II and FallDem. The data were collected using the neuropsychiatric inventory - nursing home version. The analysis included data from 699 people with dementia from a total of 21 long-term care facilities in North Rhine-Westphalia. Results: The symptoms agitation/aggression (36%), depression/dysphoria (33%), apathy/indifference (33%), irritability/lability (30%) and aberrant motor behaviour show the highest prevalence. The symptoms hallucinations (9%) and euphoria/elation (6%) have the lowest prevalence. Conclusions: The high prevalence of specific neuropsychiatric symptoms and their characteristics in people with dementia illustrates the need for care-related or psychosocial interventions to counteract the reasons for the occurrence of the symptoms.

Partindo-se do pressuposto de que, com Descartes, a questão que subjaz na constituição da modernidade é o que seria o sujeito, este texto se propõe a explorar desdobramentos sobre essa questão tendo como referência teorizações de Alain Badiou e algumas concepções básicas que aprendemos a ler na psicanálise. Ele procura demonstrar os impasses que encontramos ao tentarmos estabelecer formalizações sobre o sujeito, impasses esses que estariam na constituição do homem moderno. Trabalha também interseções e implicações dessas formalizações paradoxais com conceitos igualmente caros à psicanálise, como os de sintoma, corpo e verdade, assim como ressonâncias que poderíamos desdobrar entre a tipologia formal do sujeito proposta por Badiou e formalizações sobre esse tema que poderíamos depreender da leitura das obras de Freud e Lacan.


Based on the assumption that with Descartes the question that underlies the constitution of modernity is what the subject would be, this text proposes to explore developments on this issue with Alain Badiou's theorizations as a reference and some basic concepts that we learn to read in psychoanalysis. He tries to demonstrate the impasses that we find when we try to establish formalizations about the subject, impasses that would be in the constitution of modern humans. It also works on the intersections and implications of these paradoxical formalizations with concepts equally dear to psychoanalysis such as symptom, body, and truth, as well as resonances that we could unfold between the formal typology of the subject proposed by Badiou and formalizations on this theme that we could infer from reading the works by Freud and Lacan.


Sur la base de l'hypothèse que, avec Descartes, la question qui sous-tend la constitution de la modernité est ce qui serait le sujet, ce texte propose d'explorer les développements sur cette question ayant pour référence des théorisations d'Alain Badiou et quelques concepts de base que nous apprenons à lire en psychanalyse. Il essaie de démontrer les impasses que nous trouvons lorsque nous essayons d'établir des formalisations sur le sujet, des impasses qui seraient dans la constitution de l'homme moderne. Il travaille également sur les intersections et implications de ces formalisations paradoxales avec des concepts tout aussi chers à la psychanalyse, tels que les symptômes, le corps et la vérité, ainsi que les résonances que nous pourrions déployer entre la typologie formelle du sujet proposée par Badiou et les formalisations sur ce thème que nous pourrions déduire de la lecture des travaux de Freud et de Lacan.

A questão que introduz este artigo diz respeito à constituição do sintoma da criança e como este é atravessado pelo discurso dos pais. Na bagagem adquirida na clínica foi possível observar como as manifestações do inconsciente aparecem nos detalhes no discurso do indivíduo. Tem-se como intuito marcar a importância das funções paterna e materna na constituição da criança e, quando essas não operam, a desordem que podem causar no psiquismo infantil. Busca-se sensibilizar a noção de que a desorganização psíquica dos pais tem repercussão no processo de subjetivação dos filhos. Nota-se a impossibilidade de separar a criança dos pais no processo analítico, uma vez que ela ainda não se diferenciou enquanto sujeito. Convém não desconsiderar a resposta da criança. Há algo que é dela, que diz respeito à sua singularidade e que não podemos deixar de lado. A função parental tem como responsabilidade a imposição de limites, a orientação para a lógica da hierarquia, e não do igualitarismo, em que pai e filho têm os mesmos direitos e deveres. Entretanto, lidamos hoje com uma sociedade que desacredita da potência desse aparelho regulador e, por consequência, podemos observar um excesso de gozo. O que se colhe disso são crianças que têm que lidar com a falta da falta que a castração impõe a todos


La question qui introduit cet article concerrne la constitution du symptôme de l'enfant et comme elle est traversée par le discours des parents. Avec l'expérience acquise en clinique, cela a été possible d'observer comment les manifestations de l'inconscient apparaissent dans les détails du discours de l'individu. Il a l'intention de souligner l'importance des fonctions paternelle et maternelle dans la constitution de l'enfant et quand celles-ci n'opèrent pas, elles peuvent causer le désordre dans le psychisme infantile. Nous cherchons à sensibiliser la notion que la désorganisation psychique des parents a une répercussion dans le procès de subjectivation des fils. On peut noter l'impossibilité de séparer l'enfant des parents dans le procès analytique, une fois qu'il ne s'est pas différencié en tant que sujet. Il convient de déconsidérer que la réponse de l'enfant. Il y a quelque chose qui est à lui, qui concerne sa singularité et que nous pouvons laisser de côté. Cependant, nous faisons face aujourd'hui à une société qui ne croit pas à la puissance de cet appareil régulateur et, par conséquence, nous pouvons observer un excès de jouissance. Ce que nous en recueillons ce sont des enfants qui doivent gérer avec le manque du manque que la castration impose à tous.


The question that introduces this article concerns the constitution of the child's symptom and how it is traversed by the discourse of the parents. In the baggage acquired through clinical practice, it was possible to observe how manifestations of the unconscious appear in the details of the individual's speech. The aim is to emphasize the importance of both paternal and maternal functions in the constitution of the child, and when these functions do not operate, the disorder they can cause in the child's psyche. The intention is to raise awareness of the notion that the psychic disorganization of the parents has repercussions on the process of subjectivation of the children. It is noted the impossibility to separate the child from the parents in the analytic process, as the child has not yet differentiated itself as a subject. It is worth not disregarding the child's response. There is something that belongs to the child, related to their uniqueness, that we cannot overlook. The parental function bears the responsibility of imposing limits, guiding towards the logic of hierarchy, rather than egalitarianism, where the parent and the child have the same rights and duties. However, today we deal with a society that disbelieves in the potency of this regulatory apparatus, and consequently, we can observe an excess of jouissance. What is gathered from this is that we have children who must cope with the lack of absence that castration imposeson everyone

Relapsing-remitting multiple sclerosis progresses by relapse. It is therefore necessary to know how to identify this phenomenon in order to be able to provide the best possible support to patients. The term "relapse" is used to characterize the period of a few days to a few weeks during which an attack of inflammation of the myelin occurs. Depending on the area affected, the symptoms will be different. To qualify as a relapse, the patient must have had new, permanent symptoms for at least 24 hours without fever or signs of infection and within 30 days of the last relapse.

The step of going to a therapist is the fruit of a contingency of life, a painful reality, sometimes hardly noticeable and which becomes repetitive or unbearable. The therapist takes support on this adventure which begins in order to reveal the object which is nestled in the patient's speech. To orientate this work, the transference, the symptom and the part of jouissance are crossed. The adventure of speech thus takes the risk of going towards the intimate, present in what makes suffering. ABSTRACT

The agony, this pivotal moment, which is more or less long, before death, is very anxiety-provoking. When the person and his or her loved ones want this last stage of life to take place at home, healthcare professionals play an important role in providing clinical support to the patient and creating the conditions for relative emotional security for everyone. This requires clinical knowledge and interpersonal skills to explain what is happening to loved ones, to reassure them and to accompany the life that is passing away. A nurse referent in palliative care sheds light on the challenges of this specific multi-professional practice at home.

ABSTRACT

A Rare Cause for B Symptoms Abstract. The 78-year-old patient was referred by her general practitioner for a tumor search after a 3-month deterioration of her general condition, B-symptomatology, persistently elevated inflammation values, progressive anemia and renal insufficiency. Given the previously discovered pulmonary nodules and microhematuria, we suspected vasculitis proven by elevated MPO-ANCA levels and a kidney biopsy. Therapy with steroids and cyclophosphamide effected remission.

Marie Rose Moro, a child psychiatrist and specialist in cross-cultural issues, spoke on this topic in December 2021 at the symposium Repenser la place du corps en psychiatrie, de l'enfance à l'âge adulte. No fundamento do que o filósofo Dany-Robert Dufour chama de dessimbolização está o esvaecimento do grande Sujeito, o desaparecimento do Outro. Este, um conceito eminentemente psicanalítico, considerável no ensino do psicanalista Jacques-Marie Émile Lacan, do qual o filósofo francês se serve. Se foi possível a Dufour indicar um processo de dessimbolização, não é porque a questão do Outro permaneceu intacta. O fato é que, na psicanálise lacaniana, também não. Esta pesquisa bibliográfica toma o singular paradoxo apresentado por Dufour, que aventa um regozijo frente ao sofrimento imiscuído na dessimbolização, mas contornando-o sobretudo desde o último Lacan, isto é, a partir do Seminário 20, e de seus aliados, a fim de fornecer outros operadores que possam problematizar essa leitura. Os desdobramentos e resultados alcançam a inconsistência do Outro frente ao protagonismo do modo de gozar do parlêtre


En el fundamento de lo que el filósofo Dany-Robert Dufour llama desimbolización está el desvanecimiento del gran Sujeto, la desaparición del Otro. Se trata de um concepto eminentemente psicoanalítico, considerable en la enseñanza del psicoanalista Jacques-Marie Émile Lacan, del que hace uso el filósofo francés. Si fue posible para Dufour señalar un proceso de desimbolización, no es porque la cuestión del Otro permaneciera intacta. El caso es que, en el psicoanálisis lacaniano, tampoco. Esta investigación bibliográfica toma la singular paradoja presentada por Dufour, quien sugiere una alegría ante el sufrimiento que implica la desimbolización, pero sorteándola principalmente desde el último Lacan, es decir, del Seminario 20, y sus aliados, en con el fin de proporcionar otros operadores que puedan problematizar esta lectura. Los desarrollos y resultados alcanzan la inconsistencia del Otro frente al protagonismo del modo de gozar del parlêtre


At the foundation of what the philosopher Dany-Robert Dufour calls desymbolization is the fading of the great Subject, the disappearance of the Other.
