ABSTRACT

PURPOSE: The effects of episiotomy and second-degree tears on postpartum sexual function are key areas of enquiry in women's health research. Episiotomy and second-degree tears are common procedures and injuries that occur during childbirth. Understanding their impact on post-childbirth sexuality is crucial to women's overall well-being. This study aimed to examine the relationship between episiotomy, second-degree tears, and post childbirth sexuality. METHODS: A cross-sectional design was employed, including 83 women who gave birth to Cáceres in 2017. Participants were evaluated based on sociodemographic and sexual health factors. RESULTS: No significant differences were found in dyspareunia or sexual function between women who underwent episiotomies and those with second-degree tears. However, women who underwent episiotomies waited longer before resuming sexual activity after childbirth. Factors such as age, number of previous births, employment status, educational level, and breastfeeding status affected the timing and frequency of postpartum sexual activity. CONCLUSION: Dyspareunia negatively affects various aspects of sexual function. When comparing episiotomy and second-degree tears, their impacts on postpartum sexual function were similar. However, episiotomy delays the resumption of sexual activity. Sociodemographic factors significantly influence postpartum sexual health. These findings highlight the importance of individualised interventions and support for new mothers during the postpartum period to address potential sexual health concerns.

ABSTRACT

BACKGROUND: No widely accepted, validated instrument currently exists to measure clinical outcomes in patients who undergo dacryocystorhinostomy (DCR) for treatment of epiphora. OBJECTIVE: To develop a patient-reported outcome measure applicable to this population. METHODS: Psychometric evaluations, consultation with experts, and review of the literature informed item generation of a 12-question questionnaire to incorporate the most relevant symptoms experienced by patients with nasolacrimal duct obstruction. This questionnaire, known as the Tearing Assessment and Rating Scale-12 (TEARS-12), was administered to 32 patients before and after intervention, in the form of endoscopic DCR. Statistical analysis was performed to measure internal consistency, responsiveness, and test-retest reliability. RESULTS: Pre-operative and post-operative TEARS-12 scores (28.2 [standard error (SE) 3.19] vs 11.8 [SE 3.25], respectively, P = 0.001) demonstrated improved patient outcome within 6 weeks following endoscopic DCR. Cronbach's alpha for the questionnaire was 0.90, indicating high overall reliability. Additionally, each question demonstrated internal reliability, with a corrected item-total correlation greater than 0.30. The intraclass correlation between the two pre-operative scores was 0.858 (P < 0.001), indicating high test-retest reliability. CONCLUSION: TEARS-12 is a statistically valid, easy-to-administer instrument to measure clinical outcomes in patients who undergo endoscopic DCR.

ABSTRACT

PURPOSE: Assess aqueous tear production when measured with the dogs' eyelids open or closed. METHODS: Thirty healthy dogs (15 Shih Tzus, 15 Labrador retrievers) were recruited. With the order of testing randomized for each dog, two sessions (separated by 30 min) of STT-1 testing were performed with the dogs' eyelids closed or open. Schirmer strip wetness (every 10 s for 60 s) and number of time(s) the strip dislodged during testing were recorded in each eye. Preferred STT-1 method was surveyed via a global Listserv of the veterinary ophthalmology community. RESULTS: STT-1 values were significantly higher in closed versus open eyes in Shih Tzus (18.6 ± 2.7 mm/min vs. 16.3 ± 2.5 mm/min; p = .002) and Labrador retrievers (21.6 ± 2.9 mm/min vs. 17.8 ± 3.2 mm/min, p < .001), findings that were also significant at times <60 s for either breed (p ≤ .004). Schirmer strips dislodged from six dogs with open eyelids and no dogs with closed eyelids. Maximal STT-1 difference with closed versus open eyelids was 13 mm/min in Labrador retrievers and 7 mm/min in Shih Tzus. Survey results from 275 veterinarians showed STT-1 performed with "closed eyelids" (38.5%), "open eyelids" (26.9%), or "never paid attention, sometimes closed, sometimes open" (34.6%). CONCLUSIONS: Eyelids status (closed or open) during STT-1 testing had a significant impact on aqueous tear secretion in brachycephalic and nonbrachycephalic dogs, highlighting the importance of consistency when repeating STT-1 in a canine patient. STT-1 differences are likely due to sustained reflex tearing throughout the test duration when the dogs' eyelids are closed.

BACKGROUND: Short-lasting unilateral neuralgiform headache attacks (SUNHA) have the features of both short-lasting unilateral neuralgiform pain, such as trigeminal neuralgia or stabbing headache, and associated trigeminal autonomic symptoms, such as paroxysmal hemicrania or cluster headache. Recognizing and adequately treating SUNHA is essential but current treatment methods are ineffective in treating SUNHA. METHODS: We reviewed the changes in the concept of short-lasting unilateral neuralgiform headache attacks and provide a narrative review of the current medical and surgical treatment options, from the first choice of treatment for patients to treatments for selective intractable cases. RESULTS: Unlike the initial impression of an intractable primary headache disorder affecting older men, SUNHA affects both sexes throughout their lifespan. One striking feature of SUNHA is that the attacks are triggered by cutaneous or intraoral stimulation. The efficacy of conventional treatments is disappointing and challenging, and preventive therapy is the mainstay of treatment because of highly frequent attacks of a very brief duration. Amongst them, lamotrigine is effective in approximately two-third of the patients with SUNHA, and intravenous lidocaine is essential for the management of acute exacerbation of intractable pain. Topiramate, oxcarbazepine and gabapentin are considered good secondary options for SUNHA, and botulinum toxin can be used in selective cases. Neurovascular compression is commonly observed in SUNHA, and surgical approaches, such as neurovascular compression, have been reported to be effective for intractable cases. CONCLUSIONS: Recent advances in the understanding of SUNHA have improved the recognition and treatment approaches for this unique condition.

ABSTRACT

Introduction Eyelash extensions have become a trendy cosmetic procedure, especially among young female students at various universities. This study assessed the relationship between the usage pattern of eyelash extensions and the ocular side effects arising from eyelash extension use. Methods The study was conducted using 400 female undergraduate students at the University of Benin with an age range of 15-30 years and a mean age of 22.50 ± 3.79 years. This observational cross-sectional study was conducted by evaluating the usage pattern of eyelash extensions, ocular side effects, and the relationship between them. Data obtained from the participants were processed using the Pearson correlation analysis in the Statistical Product and Service Solutions (SPSS) (version 22.0; IBM SPSS Statistics for Windows, Armonk, NY) software. Results The results showed that only 16% (n=64) of the participants had any side effects before fixing eyelash extensions, while 54% (n=216) of the participants had experienced one or more side effects during/after artificial eyelash wear. The most common side effects experienced were itching (n=152, 38%), lashes pulling out (n=144, 36%), heavy eyelids (n=136, 34%), and red eyes (n=136, 34%). Other ocular side effects were grittiness/foreign body sensation (n=128, 32%), tearing (n=96, 24%), burning sensation (n=96, 24%), pain on the eyelids (n=88, 22%), misdirected lashes (n=40,10%), eyelids swelling (n=24, 6%), a boil on eyelids (n=16, 4%), and discharge (n=8, 2%). Moreover, the result showed statistically significant correlation between the usage pattern and the ocular side effects such as pain (r=0.22), itching (r=0.23), tearing (r=0.21), burning sensation (r=0.17), a boil on the eyelids (r=0.21), heavy eyelids (r=0.16), misdirected lashes (r=0.22), red eyes (r=0.15), lashes pulling out (r=0.12), eyelids swelling (r=0.18), grittiness (r=0.10), and discharge (r=0.10) (p<0.05). Conclusions It is concluded that there is a relationship between the usage pattern of eyelash extensions and the ocular side effects. Adequate and proper attention should be paid to this ever-growing cosmetic procedure's potential visual health risks.

BACKGROUND: Although double-row suture-anchored (DRSA) techniques for Achilles insertional tendinosis has proven successful, a reoccurring failure mode not yet addressed is suture tearing through the tendon. This study aims to address suture tearing by incorporating a rip-stop element. Authors hypothesized that the Rip-Stop group would demonstrate increased strength compared with more traditional techniques. METHODS: 12 paired cadaveric feet were used in this study (n = 24). One sample from each pair was assigned to receive the standard double-row (SDR) Achilles repair with 4.75-mm knotless anchors (n = 12). The control's matched sides were divided between 2 DRSA bridge groups: modified double-row (MDR) bridge with 3.9-mm anchors or rip-stop double-row (RS-DR) bridge repair with soft proximal anchors and 3.9-mm anchored distal row. In neutral position, specimens underwent 1000 cycles (20-100 N) followed by load to failure. Displacements, stiffness, ultimate load, and failure mode were recorded. RESULTS: RS-DR had the lowest initial displacement values followed by SDR and MDR (1.3 ± 0.4, 2.7 ± 1.4, and 3.2 ± 1.3 mm, respectively). Significance was detected when comparing initial displacement of RS-DR to MDR (P = .038). Cyclic displacement was lowest for RS-DR, followed by MDR and SDR (1.6 ± 0.9, 2.2 ± 1.1, and 4.5 ± 3.2 mm, respectively). Cyclic stiffness was similar for RS-DR and MDR (89.1 ± 24.6 and 81.9 ± 5.6 N/mm, respectively). RS-DR ultimate load (1116.8 ± 405.7 N) was statistically greater than SDR (465.6 ± 352.7, P = .003). CONCLUSION: RS-DR-repaired specimens demonstrated a decrease in displacement values and increased ultimate load and stiffness when compared to other groups. Results of this cadaveric model suggest that the addition of a rip-stop to DRSA Achilles repair is more impactful than anchor size. Limitations include that this was a time-zero biomechanical study, which cannot simulate the performance of the repairs during postoperative healing and recovery. CLINICAL RELEVANCE: A rip-stop technique for Achilles repair effectively improves dynamic mechanical characteristics and may mitigate suture tearing through tendon in a patient cohort.

ABSTRACT

The two-dimensional to three-dimensional configuration transition through self-tearing promises the engineering and promising applications of graphene. However, it is challenging to control the tearing path on demand through common thermal and interfacial treatments. In this manuscript, a defect-guided self-tearing technique is proposed to generate wider, longer, and even curved and serrated configurations, which is impossible for defect-free graphene. The underlying tearing mechanisms regarding the advancing displacement are disclosed through molecular dynamics simulations and theoretical model. This study provides a useful guidance to the implementation of complex and functional three-dimensional graphene structures.

ABSTRACT

Decellularized calfskins are a well-established skin substitute that retains the dermal tissue's spatial structure, facilitating skin regeneration, and is already available in the market. However, their mechanical properties can change with degradation, leading to tearing at the suture. Moreover, decellularized calfskins do not possess inherent antimicrobial abilities, which can lead to wound infection and further injury during the healing process. With the objectives of supporting the clinical use of decellularized calfskins, minimizing the probability of decellularized calfskin fracture and damage during usage, and improving their anti-infective properties, this study utilized a post-loading method to load gentamicin sulfate onto the decellularized calfskin to functionalize it for antimicrobial purposes. In addition, the mechanical and physicochemical properties of the drug-carrying film were investigated to see if they could meet the clinical requirements. The results revealed that vancomycin sulfate could be loaded onto the decellularized calfskin without affecting collagen. The tensile strength of the drug-loaded membrane was determined to be in the range of 5.53-29.25 MPa, meeting the clinical requirements. Thermal analysis and pH analysis experiments demonstrated that the drug-loaded membrane did not undergo thermal denaturation or decomposition during skin repair and remained within the normal pH range of the skin, avoiding significant fluctuations in wound pH.

ABSTRACT

In this paper, a new technology for on-orbit cold forming of space truss rods is proposed. For the cold roll forming process of asymmetric cross sections of thin strips, the effects of roll gap and roll spacing on the forming of asymmetric cross sections of strips were investigated using ABAQUS simulation + experiments. The study shows the following. When forming a strip with a specific asymmetric cross section, the stresses are mainly concentrated in corners 2/4/6, with the largest strain value in corner 2. With increasing forming passes, when the roll gap is 0.3 mm, the maximum equivalent strain values are 0.09, 0.24, 0.64 sequentially. Roll gaps of 0.4 mm and 0.5 mm equivalent strain change amplitude are relatively similar, and their maximum equivalent strain values are approximately 0.07,0.15, 0.44. From the analysis of the stress-strain history of the characteristic nodes in corners 2/4/6, it can be seen that the stress and strain changes in the deformation process mainly occur at the moment of interaction between the upper and lower rollers, where the stress type of node 55786 shows two tensile types and one compressive type, the stress type of nodes 48594 and 15928 shows two compressive and one tensile type, and the strain of the three nodes is in accordance with the characteristics of plane strain. When the roll gap is about 0.4 mm, the forming of the strip is relatively good. With increased roll spacing, the strip in the longitudinal stress peak through the rollers shows a small incremental trend, but the peak stresses are 380 Mpa or so. When the roll spacing is 120 mm, the longitudinal strain fluctuation of the strip is the most serious, followed by the roll spacing at 100 mm, and the minimum at 140 mm. Combined with the fluctuation in strip edges under different roll spacings, manufacturing cost and volume and other factors, a roll spacing of 100 mm is more reasonable. It is experimentally verified that when the roll gap is 0.4 mm and the roll spacing is 100 mm, the strip is successfully prepared in accordance with the cross-section requirements. When the rolling gap is 0.3 mm, due to stress-strain concentration, the strip is prone to edge waves in forming. The top of corner 2 of the flange triangular region is susceptible to intermittent tear defects, and the crack extension mechanism is mainly based on the cleavage fracture + ductile fracture.

Objetivo: Identificar el número de casos con posible diagnóstico de cefalea neuralgiforme unilateral de corta duración con inyección conjuntival y lagrimeo (SUNCT) o cefalea neuralgiforme unilateral de corta duración con síntomas autonómicos craneales (SUNA) en pacientes con un previo diagnóstico de neuralgia del trigémino (NT) en el servicio de neurología del Centro Médico Nacional 20 de Noviembre, comprobando así que estas cefaleas trigeminoautonómicas deben ser descartadas y consideradas como diagnósticos diferenciales de la NT. Pacientes y métodos: Estudio transversal y retrospectivo. Se evaluaron los expedientes clínicos electrónicos completos de 100 pacientes con diagnóstico de NT durante el período de abril de 2010 a mayo de 2020. Intencionalmente se buscaron síntomas autonómicos en éstos y se compararon con los criterios diagnósticos de SUNCT y SUNA de la Clasificación Internacional de las Cefaleas, tercera edición. Se realizaron pruebas de chi cuadrado y posteriormente de regresión bivariada para determinar la asociación entre las variables. Resultados: Se incluyó a 100 pacientes con diagnóstico de NT. Tras la revisión de las manifestaciones clínicas, se encontró a 12 pacientes con síntomas autonómicos y se compararon con los criterios diagnósticos de SUNCT y SUNA. Estos no cumplieron los criterios absolutos para ser diagnosticados con las enfermedades previamente mencionadas; sin embargo, cumplieron las características del espectro de cefaleas trigeminoautonómicas. Conclusión: La NT es una entidad dolorosa y frecuente que puede presentar síntomas autonómicos, y es importante pensar en diagnósticos diferenciales, como la SUNCT y la SUNA, para la identificación y el tratamiento correctos.(AU)


Objective: Identify the number of cases with a possible diagnosis of short-lasting unilateral neuralgiform headache attacks with conjunctival injection and tearing (SUNCT) or short-lasting unilateral neuralgiform headache attacks with cranial autonomic symptoms (SUNA) in patients with a previous diagnosis of Trigeminal Neuralgia (TN) at the Neurology Service of the National Medical Center 20 de Noviembre. This will confirm that these trigeminal-autonomic cephalalgias should be ruled out and considered as differential diagnoses of trigeminal neuralgia. Patients and methods: Cross-sectional and retrospective study. The complete electronic medical records of 100 patients with a diagnosis of TN were evaluated during the period from April 2010 to May 2020. Autonomic symptoms were intentionally searched for in these patients and compared with the diagnostic criteria of SUNCT and SUNA of the 3rd edition of the International Classification of Headache Disorders. Chi-square tests and subsequent bivariate regression were performed to determine the association between variables. Results: One hundred patients with a diagnosis of TN were included. After reviewing the clinical manifestations, 12 patients with autonomic symptoms were found and compared with the diagnostic criteria of SUNCT and SUNA. However, they did not meet the absolute criteria to be diagnosed with the previously mentioned diseases, nor to be ruled out. Conclusions: TN is a painful and frequent entity that can present with autonomic symptoms, therefore making it important to identify SUNCT and SUNA as differential diagnoses, to recognize them and treat them appropriately.(AU)

The behaviour of geosynthetics can be affected by many agents, both in the short and long term. Mechanical damage caused by repeated loading or abrasion are examples of agents that may induce undesirable changes in the properties of geosynthetics. The research conducted in this work complemented previous studies and consisted of submitting a geocomposite, isolated and successively, to two degradation tests: mechanical damage under repeated loading and abrasion. The geocomposite (a nonwoven geotextile reinforced with polyethylene terephthalate filaments) was tested on both sides (with or without filaments) and directions (machine and cross-machine). The impact of the degradation tests on the geocomposite was quantified by monitoring changes in its tensile and tearing behaviour. The results showed that, in most cases, the degradation tests caused the deterioration of the tensile and tearing behaviour of the geocomposite, affecting its reinforcement function. The decline in tensile strength correlated reasonably well with the decline in tearing strength. Changing the side and direction tested influenced, in some cases (those involving abrasion), the degradation experienced by the geocomposite. The reduction factors (referring to tensile and tearing strength) for the combined effect of the degradation agents tended to be lower when determined by using the common method (compared to those resulting directly from the successive exposure to both agents).

The role of TRPA1 in the thermosensitivity of the corneal cold thermoreceptor nerve endings was studied in young and aged mice. The contribution of the TRPA1-dependent activity to basal tearing and thermally-evoked blink was also explored. The corneal cold thermoreceptors' activity was recorded extracellularly in young (5-month-old) and aged (18-month-old) C57BL/6WT (WT) and TRPA1-/- knockout (TRPA1-KO) mice at basal temperature (34 °C) and during cooling (15 °C) and heating (45 °C) ramps. The blink response to cold and heat stimulation of the ocular surface and the basal tearing rate were also measured in young animals using orbicularis oculi muscle electromyography (OOemg) and phenol red threads, respectively. The background activity at 34 °C and the cooling- and heating-evoked responses of the cold thermoreceptors were similar in WT and TRPA1-KO animals, no matter the age. Similar to the aged WT mice, in the young and aged TRPA1-KO mice, most of the cold thermoreceptors presented low frequency background activity, a low cooling threshold, and a sluggish response to heating. The amplitude and duration of the OOemg signals correlated with the magnitude of the induced thermal change in the WT but not in the TRPA1-KO mice. The basal tearing was similar in the TRPA1-KO and WT mice. The electrophysiological data suggest that the TRPA1-dependent nerve activity, which declines with age, contributes to detecting the warming of the ocular surface and also to integrating the thermally-evoked reflex blink.

Background: Several classifications have been proposed for subscapularis tendon tearing (SCTs); however, there remains a poor agreement between orthopedic surgeons regarding the diagnosis and management of these lesions. Distinguishing the various tear patterns and classifying them with some prognostic significance may aid the operating surgeon in planning appropriate treatment. Purpose: The purpose of this study was to outline the current literature regarding SCT classification and treatment and conduct a survey among shoulder and elbow surgeons to identify the approaches regarding surgical decision-making for these injuries. Methods: In this systematic review, we analyzed 12 articles regarding the subscapularis tendon tear classification and implications regarding treatment plans and outcomes. In addition, 4 international experts in subscapularis repair surgery participated in the development of a questionnaire form that was distributed to 1161 ASES members. One hundred sixty five surgeons participated and chose whether they agree, disagree, or abstain for each of the 32 statements in 4 parts including indications/contraindications, treatment plan, and the factors affecting outcomes in the survey. Results: Classification criteria were extremely variable with differing recommendations and descriptions of tear morphology; most were based on tear size, associated shoulder pathology, or lesser tuberosity footprint exposure. Considering the multiple classification systems and the overall poor agreement regarding SCT management, our study found that the most widely agreed upon (more than 80%) statements included early surgery is advised for traumatic SCT, chronic degenerative SCT (without fatty infiltration) associated with acute supraspinatus tear is a candidate for repair, and rotator cuff arthropathy is a contraindication for SCT repair. Conclusion: Our study was able to identify both patient and tear characteristics that are well agreed upon among surgeons in the treatment of these injuries. Lafosse classification is generally widely accepted; however, it needs to be improved by some additions. Continued collaboration among surgeons is needed to establish an acceptable and broadly applicable classification system for the management of these injuries.

ABSTRACT

In situ fenestration of endovascular stent-grafts has become a mainstream bailout technique to treat complex emergent aneurysms while maintaining native anatomical visceral and aortic arch blood supplies. Fabric tearing from creating the in situ fenestration using balloon angioplasty may extend beyond the intended diameter over time. Further tearing may result from the physiologic pulsatile motion at the branching site. A resultant endoleak at the fenestrated sites in stent-grafts could ultimately lead to re-pressurization of the aortic sac and, eventually, rupture. In an attempt to address this challenge, plain woven fabrics were designed. They hold a specific corona surrounding a square-shaped cluster with a plain weave fabric structure, a 2/2 twill, or a honeycomb. The corona was designed to stop potential further tearing of the fabric caused by the initial balloon angioplasty and stent or later post-implantation motion. The cluster within the corona was designed with relatively loose fabric structures (plain weave, 2/2 twill weave, and honeycomb) to facilitate the laser fenestration. Two commercial devices, Anaconda (Vascutek, Terumo Aortic) and Zenith TX2 (Cook), were selected as controls for comparison against this new design. All the specimens were characterized by morphology, thickness, and water permeability. The results demonstrated that all specimens with a low thickness and water permeability satisfied the requirements for a stent graft material that would be low profile and resistant to endoleaks. The in situ fenestrations were performed on all fabrics utilizing an Excimer laser followed by balloon angioplasty. The fabrics were further observed by light microscopy and scanning electron microscopy. The dimension of the fenestrated apertures was smaller than the balloon's diameter. The tearing was effectively confined within the corona. The clinical acceptability of this concept deserves additional bench testing and animal experimentation.

ABSTRACT

Purpose: Understanding demographic and MRI parameters with determinant role in rotator cuff tearing (RCT) is required to predict outcomes and select more appropriate treatment in patients with non-traumatic shoulder pain. This study aimed to survey the association of rotator cuff tears with these factors. Methods: We performed this cross-sectional study on patients referred to Kashani Hospital with non-traumatic shoulder pain suspected of rotator cuff injuries from April 2020 to February 2021. We obtained their demographic data and MRI from electronic hospital records. After that, two radiologists reviewed MRI and recorded data. We employed the SPSS 23 software to organize the obtained results and statistical evaluations. Results: 371 subjects, with the age of 48.61 ± 14.89 years, were included in our study, and 50% of them were male. The flat and down-sloping acromion and partial tearing of the supra and infraspinatus tendons were more frequent than the others. Gender could not consider as a determinant factor for the severity of tendon injuries, acromion types and variants. The relation of age to acromion morphology and the tendinopathy severity was significant. Remarkably, down-sloping acromion was higher at all the acromion shapes; however, it makes no difference in the level of supra and infraspinatus involvement. Moreover, we found that partial tearing was higher in all acromion types, except concave. This distribution was significant only for supraspinatus tears. Conclusion: The clinical decision is the most important step in patients with RCT and MRI prognostic factors, which are helpful to improve the management of the cases. RCT in older patients and down-slope acromion was more severe than the others. According to the previous studies, acromion morphology (as an important determinant factor) in RCT could be influenced by age, as well.

ABSTRACT

PURPOSE: To investigate the effect of order of examination on the results of noninvasive keratograph tear film evaluation using Keratograph5M in dry eye patients. METHODS: One hundred and four patients with dry eye symptoms were analyzed retrospectively. All patients underwent bilateral noninvasive tear film evaluation using measurements of tear meniscus height (TMH) and noninvasive keratograph break-up time (NIKBUT) obtained using Keratograph5M. Measurements were performed sequentially in the order of right TMH, left TMH, right NIKBUT, and left NIKBUT. RESULTS: There was no statistically significant difference in TMH values between the right and left eyes (0.24 ± 0.08 and 0.23 ± 0.08 mm, respectively). Mean NIKBUT-first (time at first tear film break-up) and mean NIKBUT-average (the mean of all tear film break-up time over the entire cornea) were 6.17 ± 3.28 and 10.00 ± 3.97 seconds, respectively, for right, and 7.43 ± 3.86 and 11.57 ± 4.34 seconds, respectively, for left eyes. In addition, mean NIKBUT-first between right and left eyes, and mean NIKBUT-average between them were statistically significant (p = 0.013 and p = 0.007, respectively). Mean NIKBUT and mean TMH differences were not significantly influenced by right or left eyes, age, or sex (all p > 0.050). Spearman correlation analyses of TMH, NIKBUT-first, and NIKBUT-average results showed moderate positive correlations between right and left eyes (r = 0.470, r = 0.322, and r = 0.576, respectively; p < 0.001). CONCLUSIONS: TMH evaluation was not affected by test order; however, NIKBUT measurement was affected by test order, because of reflex tearing due to forced eye opening during the examination. Therefore, TMH should be evaluated before NIKBUT, and sufficient time interval and caution should be needed between NIKBUT measurements on both eyes.

Hot tearing is the most common and serious casting defect that restricts the light weight and integration of magnesium alloy components. In the present study, trace Ca (0-1.0 wt.%) was added to improve the resistance of AZ91 alloy to hot tearing. The hot tearing susceptivity (HTS) of alloys was experimentally measured by a constraint rod casting method. The results indicate that the HTS presents a ν-shaped tendency with the increase in Ca content, and reaches its minimum value in AZ91-0.1Ca alloy. Ca is well dissolved into α-Mg matrix and Mg17Al12 phase at an addition not exceeding 0.1 wt.%. The solid-solution behavior of Ca increases eutectic content and its corresponding liquid film thickness, improves the strength of dendrites at high temperature, and thereby promotes the hot tearing resistance of the alloy. Al2Ca phases appear and aggregate at dendrite boundaries with further increases in Ca above 0.1 wt.%. The coarsened Al2Ca phase hinders the feeding channel and causes stress concentration during the solidification shrinkage, thereby deteriorating the hot tearing resistance of the alloy. These findings were further verified by fracture morphology observations and microscopic strain analysis near the fracture surface based on kernel average misorientation (KAM).

ABSTRACT

Bovine pericardium has been commonly used as leaflets in cardiac valve prosthesis replacement for decades because of its good short-term hemocompatibility and hemodynamic performance. However, fatigue, abrasion, permanent deformation, calcification, and many other failure modes have been reported as well. The degradation of the performance will have a serious impact on the function of valve prostheses, posing a risk to the patient's health. This study aimed to introduce a flexible fabric composite with better mechanical performance such that it can be employed as a substitute material for bioprosthetic valve leaflets. This composite has a multilayered thin film structure made of ultrahigh molecular weight polyethylene (UHMWPE) fabric and thermoplastic polyurethane (TPU) membranes. The mechanical properties of three specifications with different design parameters were tested. The tensile strength, shear behavior, tear resistance, and bending stiffness of the composites were characterized and compared to those of bovine pericardium. A constitutive model was also established to describe the composites' mechanical behaviors and predict their strength. According to the results of the tests, the composite could maintain a flexible bending stiffness with high in-plane tensile strength and tear strength. Therefore, bioprosthetic valve made of this substitute material can withstand harsher loads in the blood flow environment than those made of bovine pericardium. Moreover, all these test results and constitutive models can be used in future research to evaluate hemodynamic performance and clinical applications of fabric composite valve prostheses.

ABSTRACT

PURPOSE: A systematic review of the literature on diagnostic and therapeutic indications, techniques, and complications of dacryoendoscopy (DE) was performed. METHODS: The authors performed a PubMed search of articles published in English on DE. Data were collected and classified according to the categories of the disease. The clinical outcomes and limitations were particularly analyzed. RESULTS: The lacrimal drainage system from the canaliculus to the inferior meatus could be examined based on the specific anatomical features by DE. The canalicular mucosa is smooth and brightly colored, the lacrimal sac shows covering mucosa with good vascularization and the nasolacrimal duct is lined with bright tubular mucosal folds. DE allows direct visualization of the detailed internal condition of the lacrimal disorders, to directly diagnose the site of obstruction with accuracy and address the causes and recanalize the lacrimal drainage system using assisted micro lacrimal surgical instruments in the tearing patients. CONCLUSIONS: Better visualization of the lacrimal canal with DE improves the understanding of physiology and precise identification of the obstructing lesions, both of which are the key to a comprehensive management for the tearing patients.
