ABSTRACT

Introducción: El test UD Interferencia evalúa la velocidad de procesamiento y la atención, y está basado en el concepto de interferencia del test de Stroop. El objetivo principal de este estudio es proporcionar una versión alternativa del test de Stroop que evite algunas de las limitaciones de versiones anteriores, relacionadas con el daltonismo y las dificultades de lectura en personas mayores, y obtener datos normativos y estandarizados para este test, adaptados a población española. Métodos: El estudio se enmarca dentro del proyecto Normacog, para el cual se evaluó a 905 participantes (18-93 años). Se analizó la fiabilidad y la validez concurrente y de constructo del test. Se analizaron el efecto de la edad, nivel educativo y sexo sobre el rendimiento en UD Interferencia y se crearon percentiles y puntuaciones escalares ajustadas por edad y nivel educativo. Resultados: El test muestra buena fiabilidad (α = 0,875) y validez concurrente (r de 0,443 a 0,725; p < 0,001) y de constructo (r de 0,472 a 0,737; p < 0,001). Se observó un efecto significativo de la edad y el nivel educativo sobre el rendimiento en UD Interferencia, explicando del 12 al 40% de la varianza. El sexo únicamente presentó un efecto significativo en la variable índice de resistencia a la interferencia. Conclusiones: Se presenta una versión alternativa del test de Stroop que presenta algunas ventajas sobre anteriores versiones. Se aportan baremos estandarizados y normalizados para población española que permiten corregir el test teniendo en cuenta la edad y educación de la persona evaluada.(AU)

ABSTRACT

Resumen La prosopagnosia es un tipo de agnosia visual caracterizada por la incapacidad de reconocer los rostros de las personas. Existen básicamente dos variantes, aperceptivas y asociativas. El "efecto Tortoni" es un fenómeno descripto por Bekinschtein y col. hace unos años en mozos de café en Buenos Aires, quienes utilizaban esta herramienta para recordar los pedidos de cada integrante de una mesa. Presentamos un caso de prosopagnosia asociada a lesión temporo-occipital bilateral secundaria a traumatismo encefalocra neano, manifestada en forma inicial por la falta de reconocimiento de rostros, con la utilización de una estra tegia asociativa similar a la descripta en el efecto "Tortoni" como compensación. Mujer de 62 años que sufrió un traumatismo encefalocraneano grave. Pocos meses después del evento, presentó dificultad para reconocer personas conocidas, hecho evidenciado por sus allegados cuando en una mesa los integrantes cambiaron su asiento, permanecieron callados por unos instantes, y posteriormente la paciente continuó nombrándolos por su ubicación previa. En la resonancia magnética de cerebro se objetivaron lesiones contusas de aspecto secuelar en región temporo-occipital bilateral. La prosopagnosia adquirida secundaria a lesiones focales en la región temporo-occipital generalmente bilateral, derecha, y raramente izquierda, es un cuadro poco frecuente. La es trategia utilizada en el "efecto Tortoni" fue en nuestra paciente una de las manifestaciones iniciales del cuadro. La realización de un test neuropsicológico ecológico que considere esta estrategia podría ser de utilidad en el rastreo y detección precoz de esta entidad.


Abstract Proposapnosia is a type of visual agnosia characterized by the inability to recognize people's faces. There are basically two variants, apperceptive and associative. The "Tortoni effect" is a phenomenon described by Bekinschtein et al a few years ago in waiters from Buenos Aires, who used this tool to remember the orders of each member of a table. We present a case of prosopagnosia associated with bilateral temporo-occipital injury secondary to head trauma, initially manifested by the lack of face recognition with the use of an associative strategy similar to that described in the "Tortoni effect" as compensation, in a 62-year-old female who suffered a severe head injury. A few months after this event, the patient had difficulty in recognizing familiar people, a fact evidenced by her relatives when at a restaurant table, they changed their seats, remained silent momentarily, and right after the patient kept naming them by their previous location. The magnetic resonance imaging of the brain revealed blunt sequelae lesions in the bilateral temporo-occipital region. Acquired prosopagnosia due to focal lesions in the temporo-occipital region, generally bilateral and right, and less frequently left, is a rare condition. The strategy used in the "Tortoni effect" was one of the initial manifestations of the condition in our patient. Carrying out an ecological neuropsychological test that considers this strategy could be useful in the screening and early detection of this entity.

Antecedentes: El Frontal Assessment Battery (FAB) evalúa disfunciones ejecutivas; sin embargo, no existe una versión culturalmente adaptada del instrumento a la población española. Este estudio tuvo como objetivo traducir y adaptar culturalmente al español la versión original del FAB y realizar un estudio piloto de esta versión.MétodosSe utilizaron procedimientos estandarizados para la adaptación cultural. La versión final del FAB (FAB-E) fue testada con 19 sujetos sanos españoles. La puntuación total y de las subescalas del FAB-E se analizaron para comprobar su comp,rensión y verificar que los ítems evaluaban lo que tenían que evaluar.ResultadosLos ítems/instrucciones del FAB-E fueron adecuados culturalmente y escritos con claridad. En el estudio piloto la mayoría de los participantes tuvieron puntuaciones entre 2 y 3 en todos los ítems. La media del FAB-E total fue 158 (DE=1.3) y los valores oscilaron entre 13 y 18. La edad y el nivel educativo parecieron indicar diferencias en las puntuaciones del FAB-E.ConclusiónNuestros resultados avalan que el FAB-E puede ser utilizado para examinar disfunciones ejecutivas en la población hispanohablante. ABSTRACT

There is no consensus in the literature as to neuropsychological functioning, the severity of obsessive-compulsive symptoms (OCS) and the definitions of the OCS dimensions. We conducted a cross-sectional study investigating the relationship between executive function and OCS severity in the various dimensions, according to the Dimensional Yale-Brown Obsessive-Compulsive Scale criteria. We evaluated 28 patients with OCS, using eight neuropsychological instruments to evaluate executive function. We found that OCS severity in the contamination/cleaning dimension correlates negatively with executive function, inhibitory control and attentional control. Severity in the hoarding dimension correlated positively with cognitive flexibility, visual processing and logical reasoning, whereas it correlated negatively with the capacity to develop efficient complex problem-solving strategies. There was also a positive correlation between severity in the symmetry/ordering dimension and attentional control. Our findings suggest that the profile of executive function in OCD is defined by the severity of the various OCS dimensions.


Não há consenso na literatura quanto ao funcionamento neuropsicológico, a gravidade dos sintomas obsessivo-compulsivos (SOC) e as definições de suas dimensões. Realizou-se um estudo transversal para investigar a relação entre as funções executivas e a gravidade dos SOC nas várias dimensões, de acordo com os critérios dimensionais da Escala de Sintomas Obsessivo-compulsivos de Yale-Brown. Foram avaliados 28 pacientes com SOC, por meio de oito instrumentos de avaliação das funções executivas. A gravidade dos SOC, na dimensão contaminação/limpeza, correlacionou-se negativamente com as funções executivas controle inibitório e atenção. A gravidade da dimensão colecionismo foi positivamente correlacionada com a flexibilidade cognitiva, o processamento visual e o raciocínio lógico. Houve uma correlação negativa com a capacidade de desenvolver estratégias eficientes de resolução de problemas. Houve também uma correlação positiva entre a gravidade na dimensão simetria/ordenação e controle da atenção. Resultados sugerem que o perfil de funções executivas no TOC é definido pela gravidade das dimensões dos SOC.

Los déficits inhibitorios se observan en vários cuadros neurológicos y psiquiátricos, lo que puede aumentar la frecuencia de las respuestas perseverativas. No existe uma normalización de criterios para evaluar la perseveración. Lo Estudio 1 tiene por objetivo obtener datos preliminares de hasta cuantas casillas la repetición de un número debe ser considerada una perseveración en la tarea Generación Aleatoria de Números (GAN). El potencial de discriminación del puntaje de corte será examinado mediante la comparación de TDAH y saludables (Estudio 2). Se utilizaron los instrumentos: Cuestionario de datos socio-demográficos y de salud, Cuestionario abreviado de Conners, Matrices progresivas de Raven e lo GAN. Participaron niños de 6 a 12 años de edad (Estudio 1: n=60; Estudio 2: Trastornopor déficit de atención con hiperactividad (TDAH), n=9 e controles, n=18). Los niños tuvieron una media de 4,97 (dp=1,78) casillas para repetir un número. El criterio que considera la perseveración como la repetición del número hasta cinco casillas después de su última evocación ha distinguido TDAH y controles. Por lo tanto, esto parece ser el criterio más sensible para evaluar la perseveración.


Observam-se déficits inibitórios em vários quadros neurológicos e psiquiátricos, podendo aumentar a frequência de respostas perseverativas. Não há uma padronização dos critérios para avaliar a perseveração. O estudo 1 visou a obter dados preliminares de até quantas casas a repetição de um número pode ser considerada uma perseveração na tarefa Geração Aleatória de Números (GAN). O potencial discriminativo dos pontos de corte foi investigado pela comparação entre TDAH e saudáveis (Estudo 2). Os instrumentos foram: questionário de dados sociodemográficos e de saúde, Questionário Abreviado de Conners, Matrizes Progressivas Coloridas de Raven e o GAN. Participaram crianças com idades de 6 a 12 anos (Estudo 1: n= 60; Estudo 2: transtorno de déficit de atenção/hiperatividade (TDAH), n=9 e controles, n=18). As crianças levaram em média 4,97 (dp=1,78) caselas para repetir um número. O critério que considera perseveração como a repetição de um número até cinco caselas após a sua última evocação diferenciou TDAH de controles. Portanto, esse parece ser o critério mais sensível para avaliar a perseveração.


Inhibitory deficits are observed in several neurological and psychiatric disorders, which may increase the frequency of perseverative responses. There are no standard criteria to assess perseveration. The first study aimed to obtain preliminary data on how many blanks until the repetition of a number can be considered a perseveration in Random Number Generation (RNG) task. The discriminative potential of cutoff points was investigated by comparing children with Attention Deficit Hiperactivity Disorder (ADHD) and healthy children (Study 2). The instruments were: sociodemographic and health questionnaire, Conners Abbreviated Rating Scale, Raven Coloured Progressive Matrices and RNG. The sample was composed of children aged 6 to 12 years (Study 1: n= 60; Study 2: ADHD, n=9 e Controls, n=18). The children took on average 4.97 (sd=1.78) blanks to repeat a number. The criteria that considers perseveration as the repetition of a number up to five blanks after their last recall discriminated ADHD from controls. Therefore, this seems to be the most sensitive criteria to assess perseveration.

ABSTRACT

El trastorno neurocognitivo asociado a VIH (TNAV) es una complicación poco conocida pero de elevada prevalencia e impacto en los pacientes con VIH. El término TNAV agrupa un espectro de complicaciones progresivas del sistema nervioso central (SNC), desde un compromiso neurocognitivo asintomático y un trastorno neurocognitivo leve hasta una demencia asociada a VIH (DAV). Mientras que la incidencia de DAV ha disminuido significativamente con el tratamiento antirretroviral de gran actividad (TARGA), las formas más leves de TNAV se han incrementado. En esta revisión, describimos la nomenclatura actualizada y definiciones de caso para orientar el diagnóstico del TNAV. También, resumimos las manifestaciones clínicas, diagnóstico y recomendaciones para el tratamiento. Finalmente, mientras que el deterioro cognitivo es típicamente diagnosticado con una evaluación neuropsicológica completa, la interpretación de los resultados se basa en la comparación entre el rendimiento del paciente con valores normalizados en poblaciones ajustados culturalmente y según edad; así, los resultados de estas pruebas son válidos solo si existen datos representativos normalizados adecuadamente para un paciente. Por lo tanto, proponemos un grupo de pruebas neuropsicológicas breves validadas en nuestra población peruana, que pueden ser utilizadas para la detección temprana del TNAV no solo por los neurólogos sino también por el médico tratante del paciente con VIH o en centros de escasos recursos...


HIV-associated neurocognitive disorder (HAND) is not a well recognized entity but has high prevalence and impact in individuals infected with HIV. The term HAND encompasses a spectrum of progressive central nervous system (CNS) involvement, ranging from asymptomatic neurocognitive impairment and minor neurocognitive disorder through to the most severe form of HIV-associated dementia (HAD). While the incidence of HAD has declined significantly with highly active antiretroviral treatment (HAART), the milder forms of HAND have increased. In this review, updated nomenclature and research case definitions to guide HAND diagnosis are described. Clinical manifestations, diagnosis and treatment recommendations are also outlined. Finally, while neurocognitive impairment is typically diagnosed with full neuropsychological evaluation, the interpretation of test results is based on comparing the patientÆs performance to age and culturally adjusted population based normal values; thus, test results are valid only if adequate representative normative data exist for a given patient. Therefore, we propose a group of brief neuropsychological tests validated in Peruvian population that could be used to detect HAND opportunely not only by neurologists but also by a primary HIV-care provider or in resource-poor settings...

BACKGROUND: HIV-associated cognitive impairment occurs even in the early stages of infection. Short-term memory, psychomotor speed, attention, and executive functioning are the main capacities affected. Controversy exists regarding whether highly active antiretroviral therapy (HAART) is helpful in combating this process. The objective of the present study is to determine the association between cognitive impairment and HAART in HIV-infected patients from Hospital Regional de Huacho. METHODS: Prospective study of HIV patients meeting criteria to start HAART. Twenty-one HIV-positive patients were recruited between April and July 2011. Researchers administered a standardised neuropsychological test battery before and 4 weeks after onset of HAART. Psychomotor speed, executive function, short term memory (visual and verbal), attention, and visuospatial performance were evaluated. RESULTS: Nineteen patients completed the study (14 males and 5 females). In the pre-HAART evaluation, most patients scored below average on the executive function and psychomotor speed subtests. Psychomotor speed and immediate visual memory improved significantly after four months of treatment with HAART. CONCLUSIONS: Some degree of cognitive decline may present even in the early and asymptomatic stages of HIV infection. The benefits of antiretroviral treatment for cognitive performance can be detected after only a few weeks of follow-up.

Introducción: Diversos estudios han comunicado la presencia de deterioro cognitivo asociado al virus de inmunodeficiencia humana (VIH) tipo 1, aún en estadios tempranos de la infección. Las principales funciones comprometidas son la memoria a corto plazo, velocidad psicomotriz, atención y función ejecutiva. Objetivos: Describir el perfil cognitivo en pacientes con VIH neurológicamente asintomáticos, antes de iniciar terapia antirretroviral de gran actividad (TARGA), y determinar posibles factores asociados. Diseño: Estudio transversal-analítico. Lugar: Hospital Regional de Huacho. Participantes: Pacientes con VIH y con criterios para ingresar a TARGA. Intervenciones: Veintiún pacientes con VIH, que fueron atendidos desde de abril a julio de 2011, fueron evaluados clínicamente y se les administró una batería neuropsicológica estandarizada. Principales medidas de resultados: Puntajes alcanzados en las pruebas de velocidad psicomotriz, función ejecutiva, memoria inmediata, atención y habilidades visuoespaciales. Resultados: El 47,6% de pacientes presentó un deterioro cognitivo significativo. Los pacientes fallaron principalmente en las pruebas de velocidad psicomotriz y función ejecutiva. Con excepción de un paciente, todos fracasaron en al menos un test. La edad, tiempo de enfermedad, grado de instrucción y depresión no influyeron significativamente sobre el perfil cognitivo. Conclusiones: Estos hallazgos sugieren que el compromiso cognitivo en algún grado puede presentarse desde las fases tempranas y asintomáticas de la infección por el VIH; estos hallazgos coinciden con la literatura.


Background: Several studies have reported cognitive impairment in HIV-1 infected individuals even at early stages of the infection. Short-term memory, psychomotor speed, attention and executive functions are mainly affected. Objectives: To determine the cognitive profile in neurologically asymptomatic HIV-infected patients before starting highly active antiretroviral therapy (HAART), and possible associated factors. Design: Analytic and cross-sectional study. Setting: Hospital Regional de Huacho, Peru. Participants: HIV patients with criteria to start HAART. Interventions: Twenty-one asymptomatic HIV patients attended from April to July 2011 had a detailed clinical history and a battery of standardized neuropsychological tests. Main outcomes measures: Scores in psychomotor speed, executive function, immediate memory, attention, and visuospatial performance evaluations. Results: Significant cognitive impairment was determined in 47.6% of patients. Patients performed worse in psychomotor speed and executive function tests. All but one patient failed at least in one test. Age, level of instruction, time of disease, and depression were not statistically associated. Conclusions: In accordance with the literature, these findings suggest cognitive decline may occur in early asymptomatic stages of HIV infection.

Introducción: Durante los últimos años ha existido un envejecimiento creciente y progresivo de la población a nivel mundial que ha traído como consecuencia altas tasas de demencias y de deterioro cognitivo leve, en las cuales la memoria resulta notablemente afectada. En nuestro país no contamos con datos normativos que nos permitan determinar el déficit en el rendimiento de la memoria para la población de adultos mayores. Se realizó una investigación con el objetivo de evaluar la validez y la confiabilidad del test de memoria Aprendizaje Verbal de Rey, obtener valores normativos en adultos mayores cubanos y evaluar el efecto de la edad y la escolaridad sobre el rendimiento en el mencionado test. Métodos: La muestra la conformaron 270 adultos mayores divididos en 9 grupos (30 sujetos en cada uno), según la edad y la escolaridad, que cumplieran con los criterios de inclusión en el estudio. Para el análisis y procesamiento de la información se utilizaron métodos de estadística descriptiva (media y desviación estándar) e inferencial (prueba de correlación de Pearson y ANOVA ONE WAY). Resultados: Se obtuvieron los valores normativos para el test de Aprendizaje Verbal de Rey; se detectó un efecto de la edad y la escolaridad sobre el rendimiento de la memoria. Conclusiones: Se demostró que el test de Aprendizaje Verbal de Rey constituye una medida válida y confiable para la evaluación de la memoria en adultos mayores(AU)


Introduction: In recent years there has been a progressive and increasing aging of world population that has caused high rates of dementia and mild cognitive damage, in which memory results notoriously affected. In our country we do not have normative data to allow us to determine the deficit in memory performance for the elderly population. In this investigation a correlational and descriptive study of transversal cut was made whit the objective of evaluating the validity and reliability of memory test Verbal Learning, of Rey, obtaining normative values for Cuban elderly adults and evaluating the effects of age and scholarship in the test performance. Methods: For this a sample of 270 was taken, and the subjects were divided into 9 groups of 30 each, according to age and scholarship, provided that they satisfied the inclusion criteria of the study. For the analysis and processing of information descriptive statistic methods (media and standard desviation) were used as well as inferential (Pearson correlation test and ANOVA ONE WAY). Results: As a result, normative values for Verbal Learning test of Rey were obtained, and it was detected and effect of age and scholarship on memory performance, Conclusions: It was proved to be a reliable and valid measuring tool for the evaluation of memory in elderly adults(AU)
