ABSTRACT

A child's illness and hospitalization have repercussions for parents and siblings. Caregivers have a duty not only to care for their young patient, but also to welcome the family, whose dynamics are disrupted. Beyond the somatic and technical aspects, the challenge lies in caring for each member of the family, particularly the parents. The complexities of supporting parents when dealing with a child in an emergency situation will be explored through the testimony of a nursery nurse.

ABSTRACT

BACKGROUND: The perinatal period is a psychic risk for fathers because of multiple overhauls. Fathers' place in perinatal medicine has evolved these last few years but continues to be poorly limited. These psychic difficulties are barely investigated and diagnosed in everyday medical practice. The prevalence of depressive episodes by new fathers was high in the most recent research studies. This thus is a public health problem with consequences on the family system both in the short and the long-term. REVIEW: In the mother and baby unit, the father's psychiatric care is often secondary. But with societal modifications, there is the question of impact of separation between the father and the mother and the baby. With the family based approach, care by the father is indispensable for the care of the mother, baby, and the family's as a whole. OBJECTIVE: In our mother and baby unit located in Paris, fathers were also hospitalized as patients. Therefore difficulties in the familial dynamic, between each member of the triad and mental health difficulties of fathers were able to be treated. CONCLUSION: Following the hospitalization of a few triads with a positive evolution, a reflection process is now in progress.

Abstract Objective To evaluate the functional results of patients submitted to a surgical approach for the treatment of the terrible triad of the elbow, analyzing the treatment methods used and associated epidemiological variables. Methods Patients who underwent surgical treatment for the terrible triad of the elbow from February 2018 to June 2020 at our service were evaluated. The identified sample consisted of 17 patients, but of these, only 13 completed all stages of the study and, therefore, were considered as the universe to be considered. Epidemiological information of interest was collected: age, sex, hand of dominance, affected side, characteristics and classification of injuries, trauma mechanism, time to surgery, type of procedure performed and range of motion. The Mason classification was used for radial head fractures and the Regan and Morrey classification for the coronoid process. In order to perform a functional analysis, the DASH and BRUCE questionnaires were applied. Results About 77% of the patients were male, 92% of the fracture mechanisms were due to high-energy trauma. Contrary to this, the predominance of the non-dominant side was observed as the most affected. Evaluating the results according to the time to start the treatment, the patients operated within 14 days had statistically better functional results. Conclusion Surgical treatment of TTIE generates acceptable functional results in most cases. The success of the treatment is related to the time interval between the trauma and the first surgery, in addition to the severity of the injuries.


Resumo Objetivo Avaliar os resultados funcionais dos pacientes submetidos a abordagem cirúrgica para o tratamento da tríade terrível do cotovelo, analisando os métodos de tratamento utilizados e variáveis epidemiológicas associadas. Métodos Foram avaliados pacientes submetidos aotratamento cirúrgico de tríade terrível do cotovelo de fevereiro de 2018 a junho de 2020 em nosso serviço. A amostra identificada foi de 17 pacientes, mas destes apenas 13 concluíram todas as etapas das pesquisas e por isso foram considerados como o universo a ser levado em consideração. Coletou-se informações epidemiológicas de interesse: idade, sexo, dominância, lado acometido, características e classificações das lesões, mecanismo do trauma, tempo para cirurgia, tipo de procedimento realizado e o arco de movimento. Foi utilizada a classificação de Mason para a fratura de cabeça do rádio e a de Regan e Morrey, para o processo coronoide. A fim de realizar uma análise funcional, aplicou-se os questionários de DASH e BRUCE. Resultados Cerca de 77% dos pacientes foram do sexo masculino, 92% dos mecanismos de fratura foram por trauma de alta energia. Contrariamente a esta, observou-se a predominância do lado não dominante como o mais afetado. Avaliando os resultados de acordo com o tempo para início do tratamento, os pacientes operados em até 14 dias obtiveram resultados funcionais estatisticamente melhores. Conclusão O tratamento cirúrgico da TTC gera resultados funcionais aceitáveis na maioria dos casos. O sucesso do tratamento está relacionado ao intervalo de tempo entre o trauma e a primeira cirurgia, além de se relacionar com a gravidade das lesões.

ABSTRACT

This literature review focuses on tactile interactions between parents and their infants. Research on the dyad has explored both cultural differences in touch and the relationship between touch style and hormonal secretion in both parents. The few studies that have examined this communicative modality within the triad have investigated the frequency and type of parent-infant touch, as well as the effect of skin-to-skin contact on tactile interactions at three months postpartum.

Diet plays a role in the pathophysiology and treatment of women with hyperandrogenic menstrual disturbances; however, limited research exists examining components of dietary intake in women with subclinical menstrual disturbances. The aim of this investigation was to evaluate the relationship between diet quality and hormonal status in exercising women with menstrual disturbances. Eighty exercising women with ovulatory menstrual cycles (OV; n = 32), women with oligo/amenorrhea without evidence of hyperandrogenism (Oligo/Amen-LowFAI; n = 28), and women with oligo/amenorrhea and evidence of subclinical hyperandrogenism (Oligo/Amen-HighFAI; n = 32) participated in the cross-sectional observational study (Clinical Trial Number: NCT00392873). Self-reported menstrual history, resting energy expenditure, body composition, hormonal and metabolic hormone concentrations determined reproductive and metabolic status. Serum androgens and calculated free androgen index (FAI) determined androgen status. The Diet Quality Index International (DQI-I) and the Dietary Inflammatory Index (DII) evaluated quality of diet. Oligo/Amen-HighFAI group had the highest androgen concentrations (P < 0.05) and lower DQI-I score compared to OV group and Oligo/Amen-LowFAI (P < 0.05). The Oligo/Amen-HighFAI group consumed less of vitamin A, B2, B6, B12, magnesium, and potassium compared to the Oligo/Amen-LowFAI group (all P < 0.05). In the women with menstrual disturbances with subclinically elevated androgens, poor diet quality is related to altered hormonal parameters which may have implications for future nutritional treatment strategies.

OBJECTIVES: Though relatively unexplored in clinical populations, aversive personality traits have been shown to impact the expression, the management, and the outcomes of psychotic disorders. This paper seeks to gather and organize existing evidence of the complex interplay linking social ethics, personality and experiences of psychosis through the lens of the so-called Dark Triad personality model, comprising the three multidimensional constructs Machiavellianism, narcissism and psychopathy. METHODS: A semi-systematic review of major literature databases was conducted; search terms aimed at capturing each of the Dark Triad dimensions and their known relations with both clinical (with single-symptom approach) and non-clinical psychotic experiences. RESULTS: Reviewed studies suggest that all the components of the Dark Triad model present significant clinical implications in the management of psychotic disorders. CONCLUSIONS: Aversive personality traits interact with and modulate the experiences of psychosis. They can be shown to influence the clinical and functional outcomes of psychotic patients. Therefore, further research on this theme seems justified in that it may inform rehabilitative efforts.

Las sociedades están comprometidas a combatir la pobreza del mundo, elevar el desarrollo y productividad de sus naciones, por ese motivo el objetivo del presente estudio es analizar el aporte de la psicología aplicada al desarrollo socioeconómico de un país considerando el capital humano y su función en los estamentos del desarrollo socioeconómico que es la triada productiva: Gobierno (funcionario público y líderes), empresa (emprendedores, empleados y líderes) y academia o universidad (docente, estudiantes y líder). El presente estudio es analítico-reflexivo de tipo empírico propositivo, con relación a los resultados se ha considerado dos aportes por parte de la psicología en el desarrollo socioeconómico estos son: formación integral del capital humano (valores, habilidades y conocimientos) y salud mental (Bienestar social, emocional y psicológico). Asimismo, se sugieren acciones dentro de la labor del psicólogo como ser: enfatizar en Educación en Salud Sexual y Reproductiva Integral, educación al emprendimiento, desarrollar investigación en diferentes áreas de la psicología, formar recursos humanos de calidad y proponer políticas públicas.


Societies are committed to combating poverty in the world, raising the development and productivity of their nations, for this reason the objective of this study is to analyze the contribution of applied psychology to the socioeconomic development of a country considering human capital and its role in the levels of socioeconomic development that is the productive triad: Government (civil servant and leaders), company (entrepreneurs, employees and leaders) and academy or university (teacher, students). The present study is analytical-reflexive of an empirical propositive type, in relation to the results, two contributions have been considered by psychology in socioeconomic development, these are: integral formation of human capital (values, skills and knowledge) and mental health (Social, emotional and psychological well-being). Likewise, actions were suggested within the work of the psychologist, such as: emphasizing Comprehensive Sexual and Reproductive Health Education, entrepreneurship education, developing research in different areas of psychology, training quality human resources and proposing public policies.

ABSTRACT

Introdução: com a pandemia da COVID-19, os hospitais precisaram se reorganizar para atender a nova demanda assistencial; essa pesquisa trata de como os enfermeiros gestores atuaram na transformação de um hospital para referência no tratamento exclusivo de pacientes com COVID-19. Objetivos: analisar a atuação de enfermeiros gestores na transformação de uma instituição hospitalar no contexto da pandemia pela COVID-19; relatar as ações desenvolvidas e as experiências adquiridas pelos enfermeiros gestores na reorganização da estrutura de uma instituição hospitalar no contexto da pandemia pela COVID-19; descrever a gestão dos processos de trabalho de enfermagem nesse contexto; e discutir as experiências vivenciadas pelos enfermeiros gestores na gestão de risco de emergências e na avaliação dos resultados. Método: pesquisa qualitativa, descritiva, exploratória. Dez enfermeiros gestores de um hospital público no Rio de Janeiro foram entrevistados, no período entre janeiro e maio de 2021. Aplicou-se a análise de conteúdo na modalidade temática. Foram respeitados os aspectos éticos. Resultados: emergiram três categorias, a saber: Reorganização da estrutura hospitalar para o atendimento exclusivo de pacientes com COVID-19, que abordou a coordenação da infraestrutura, a gestão de insumos e de recursos humanos, incluindo a contratação de novos profissionais de enfermagem e recepção dos que vieram remanejados de outras unidades; Reestruturação dos processos de trabalho da enfermagem em resposta ao novo hospital e ao avanço da pandemia, que descreveu a gestão de pessoas, do conhecimento e dos processos de trabalho, como a implantação do Time de Resposta Rápida e de fluxos de comunicação entre equipe, paciente e família; e Experiência dos enfermeiros gestores na transformação hospitalar para pandemia: relatos de resultados alcançados, categoria que expos a importância do trabalho em equipe, a relação entre resolutividade, qualidade e segurança, com base no desenvolvimento de competências e habilidades na gestão de risco de emergências. Considerações finais: o trabalho dos enfermeiros gestores na transformação de um hospital referência para COVID-19 no segundo maior município brasileiro em densidade populacional foi essencial. Os enfermeiros gestores atuaram no gerenciamento da infraestrutura, no desenvolvimento de estratégias para enfrentar desafios, e no trabalho em equipe, com destaque para a liderança transformacional. Enfrentaram desafios, como a falta de tempo, a gestão de pessoas em proporção muito maior que anterior, elevado turnover, impactos emocionais da pandemia, novos cargos, setores e conhecimentos. Os enfermeiros gestores valorizaram a experiência, a resiliência, o senso de coletividade e de compromisso para com a equipe de enfermagem e população. Implicações para Prática: destacou-se a importância do trabalho interprofissional, da educação permanente, da gestão do conhecimento, da liderança transformacional, e da aplicação dos princípios da tríade estrutura ­ processo ­ resultado na gestão em enfermagem e saúde. Tem-se a urgência em tratar o tópico da gestão de risco de emergência na educação do profissional da saúde.


Introduction: with the COVID-19 pandemic, hospitals had to reorganize themselves to meet the new care demand; this research deals with how nurse managers acted in the transformation of a hospital to be a reference in the exclusive treatment of patients with COVID-19. Objectives: to analyze the role of nurse managers in the transformation of a hospital institution in the context of the COVID-19 pandemic; report the actions developed and the experiences acquired by nurse managers in the reorganization of the structure of a hospital institution in the context of the pandemic caused by COVID-19; describe the management of nursing work processes in this context; and to discuss the experiences lived by nurse managers in the management of risk of emergencies and in the evaluation of the results. Method: qualitative, descriptive, exploratory research. Ten nurse managers of a public hospital in Rio de Janeiro were interviewed between January and May 2021. Content analysis was applied in the thematic modality. Ethical aspects were respected. Results: three categories emerged, namely: Reorganization of the hospital structure for the exclusive care of patients with COVID-19, which addressed the coordination of infrastructure, management of supplies and human resources, including the hiring of new nursing and reception professionals of those who came relocated from other units; Restructuring of nursing work processes in response to the new hospital and the advance of the pandemic, which described the management of people, knowledge and work processes, such as the implementation of the Rapid Response Team and communication flows between staff, patient and family; and Experience of nurse managers in the hospital transformation to a pandemic: reports of results achieved, a category that exposes the importance of teamwork, the relationship between resolution, quality and safety, based on the development of skills and abilities in emergency risk management. Final considerations: the work of nurse managers in transforming a reference hospital for COVID-19 into the second largest Brazilian municipality in population density was essential. Nurse managers acted in infrastructure management, in the development of strategies to face challenges, and in teamwork, with emphasis on transformational leadership. They faced challenges, such as lack of time, managing people in a much greater proportion than before, high turnover, emotional impacts of the pandemic, new positions, sectors and knowledge. Nurse managers valued experience, resilience, a sense of collectivity and commitment to the nursing team and population. Implications for Practice: the importance of interprofessional work, continuing education, knowledge management, transformational leadership, and the application of the principles of the structure ­ process ­ result triad in nursing and health management were highlighted. There is an urgency to address the topic of emergency risk management in health professional education.

ABSTRACT

Abstract Recurrent stress fractures rarely affect the same athlete. We present the case of a female triathlete who suffered multiple stress fractures in both tibias, the right fibula, and the left femoral neck. Conservative treatment was instituted in all episodes, with rest, reduced training load, and physical therapy rehabilitation. The relative energy deficiency in sport syndrome, along with an eating disorder, training overload, and osteopenia, was identified as a risk factor. Although rare, multiple stress fractures can occur in female triathletes. These patients must be screened for risk factors associated with biomechanics, nutrition, and training to develop an effective prevention and treatment program.


Resumo Fraturas por estresse recorrentes num mesmo atleta são raras. Apresentamos o caso de uma triatleta que sofreu múltiplas fraturas por estresse na tíbia bilateral, na fíbula direita e no colo femoral esquerdo. O tratamento conservador foi instituído em todos os episódios, com repouso, redução da carga de treino e reabilitação fisioterápica. Foi identificada como fator de risco a síndrome da deficiência energética relativa no esporte, com distúrbio alimentar, sobrecarga de treino e osteopenia. Mesmo que seja raro, múltiplas fraturas por estresse podem ocorrer em mulheres triatletas, nas quais é importante avaliar os fatores de risco associados à biomecânica, nutrição e ao treinamento para arquitetar um programa de prevenção e tratamento efetivos.

ABSTRACT Introduction: Low energy availability, amenorrhea and osteoporosis make up the Female Athlete Triad observed in physically active females and athletes. The Low Energy Availability in Females Questionnaire (LEAF-Q) was created with the purpose of identifying female athletes at risk for the Female Athlete Triad. Objective: To translate and culturally adapt the LEAF-Q for Brazilian Portuguese and validate it in a group of Brazilian athletes. Methods: The first stage of the study consisted of translation, cultural adaptation and content validation of the questionnaire in 20 athletes. In the second stage, for the test-retest reliability analysis and the construct validation, the final adapted version was applied in a sample of 127 athletes from various sports disciplines, 54 of whom responded to the questionnaire on a second occasion. For the test-retest reliability analysis, the intraclass correlation coefficient (ICC) was calculated and the paired t-test, McNemar's test and Bland-Altman plot were carried out. The construct validation modeled by known or contrasted groups was carried out by comparing the mean LEAF-Q scores of group 1 (athletes who practiced weight-sensitive sports) with those of group 2 (athletes who practiced team sports) using the Student's t test. Results: The Brazilian version of the LEAF-Q showed excellent test-retest reliability, with an ICC of 0.92. The construct validity by known or contrasted groups was confirmed after demonstrating that athletes who practiced weight-sensitive sports had a higher LEAF-Q mean score than athletes who practiced team sports (p≤0.05). Conclusion: The Brazilian version of the LEAF-Q is an important tool, which presented textual and cultural adequacy, proved to be reliable in terms of test-retest reliability, and presents evidence of validity to investigate the risk of the triad. RESUMEN Introducción: La baja energía disponible, amenorrea y osteoporosis conforman la Tríada de la mujer deportista, y afecta a mujeres físicamente activas y atletas. El cuestionario Low Energy Availability in Females Questionnaire (LEAF-Q) fue creado con la finalidad de identificar a atletas del sexo femenino en riesgo de Tríada de la mujer deportista. Objetivo: Traducir y adaptar culturalmente al portugués de Brasil y validar el LEAF-Q en un grupo de atletas brasileñas. Métodos: La primera etapa del estudio consistió en traducción, adaptación cultural y validación de contenido del instrumento en 20 atletas. En la segunda etapa, para el análisis de la confiabilidad test-retest y validación de constructo, la versión final adaptada del cuestionario fue aplicada a una muestra de 127 atletas de diversas modalidades y, entre ellas, 54 respondieron el cuestionario en una segunda ocasión. Para el análisis de la confiabilidad test-retest fue calculado el coeficiente de correlación intraclase (ICC), así como la realización de los tests t pareado, de McNemar y del gráfico de Bland-Altman. La validación de constructo modelada por grupos conocidos o contrastados fue realizada a partir de la comparación de las puntuaciones promedio del LEAF-Q del grupo 1 (atletas de modalidades sensibles al peso) y del grupo 2 (atletas de deportes colectivos), empleando el test t de Student. ABSTRACT

Low energy availability (EA) suppresses many physiological processes, including ovarian function in female athletes. Low EA could also predispose athletes to develop a state of overreaching. This study compared the changes in ad libitum energy intake (EI), exercise energy expenditure (ExEE), and EA among runners completing a training overload (TO) phase. We tested the hypothesis that runners becoming overreached would show decreased EA, suppressed ovarian function and plasma leptin, compared with well-adapted (WA) runners. After 1 menstrual cycle (baseline), 16 eumenorrheic runners performed 4 weeks of TO followed by a 2-week recovery (131 ± 3% and 63 ± 6% of baseline running volume, respectively). Seven-day ExEE, EI, running performance (RUNperf) and plasma leptin concentration were assessed for each phase. Salivary estradiol concentration was measured daily. Urinary luteinizing hormone concentration tests confirmed ovulation. Nine runners adapted positively to TO (WA, ΔRUNperf: +4 ± 2%); 7 were non-functionally overreached (NFOR; ΔRUNperf: -9 ± 2%) as RUNperf remained suppressed after the recovery period. WA increased EI during TO, maintaining their baseline EA despite a large increase in ExEE (ΔEA = +1.9 ± 1.3 kcal·kg fat free mass (FFM)-1·d-1, P = 0.17). By contrast, NFOR showed no change in EI, leading to decreased EA (ΔEA = -5.6 ± 2.1 kcal·kg FFM-1·d-1, P = 0.04). Plasma leptin concentration mid-cycle and luteal salivary estradiol concentration decreased in NFOR only. Contrasting with WA, NFOR failed to maintain baseline EA during TO, resulting in poor performance outcomes and suppressed ovarian function. ClinicalTrials.gov no. NCT02224976. Novelty: Runners adapting positively to training overload (TO) increased ad libitum energy intake, maintaining baseline EA and ovarian function through TO. By contrast, NFOR runners failed to increase energy intake, showing suppressed EA and ovarian function during TO.

ABSTRACT

"Tríade da mulher atleta" e "deficiência relativa de energia no esporte" são afecções comuns encontradas em esportistas. Tendo como fisiopatologia a disponibilidade energética negativa, essas síndromes têm impacto negativo na saúde das atletas. Apesar de serem frequentemente discutidas entre especialistas vinculados ao mundo desportivo, ainda são pouco conhecidas entre outras especialidades. Essa revisão da literatura foi proposta com o intuito de expor o problema ao ginecologista e obstetra, considerando esses profissionais importantes aliados na prevenção e diagnóstico precoce. Da mesma maneira, a intervenção terapêutica correta minimiza os diversos prejuízos à saúde e melhora o desempenho esportivo.(AU)


"Female athlete triad" and "relative energy deficiency in sport" are conditions relatively common among women participating in sports. Its pathophysiology based on negative energy availability, these syndromes have a negative impact on the athlete's health. Although they are frequently discussed among specialists linked to the sports all over the world, a little has been known among other physicians. This literature review was proposed in order to expose the problem to the gynecologist and obstetrician, considering these professionals as important allies in prevention and early diagnosis. In the same way, the correct therapeutic intervention allows to minimizes the numerous damages to athlete's health and to improve their sports performance.(AU)

ABSTRACT

We (i) identified alternative scoring strategies for the Female Athlete Triad Coalition cumulative risk assessment (CRA) tool to be utilized when particular risk factors (bone mineral density (BMD), oligomenorrhea/amenorrhea) cannot be determined; (ii) objectively defined dietary restriction for use in the CRA tool; and (iii) explored proxy measures of energy deficiency. This cross-sectional investigation of exercising women (n = 166) utilized an existing database derived from multiple studies designed to assess health, exercise, and menstrual function. Data from the screening/baseline period of each study included: anthropometrics, dual-energy X-ray absorptiometry, disordered eating questionnaires, descriptive data, and proxy measures of energy deficiency (total triiodothyronine (TT3) and ratio of measured-to-predicted resting metabolic rate (mRMR/pRMR)). Substituting delayed menarche for BMD was the best-fit replacement resulting in 15 (9%) participants being categorized in different clearance categories. When menstrual status cannot be assessed, such as during hormonal contraceptive use, low energy availability (EA) determined using self-report and disordered eating questionnaires was the best substitution, resulting in 34 (20%) participants being categorized in different clearance categories. Based on original clearance categorizations, the provisional group had lower TT3 (78.3 ± 2.2 ng/dL; 92.7 ± 2.7 ng/dL) and Harris-Benedict mRMR/pRMR (0.85 ± 0.01; 0.90 ± 0.01) than the full group. Until an updated risk assessment tool is developed, delayed menarche can substitute for low BMD and low EA for oligomenorrhea/amenorrhea. Novelty This investigation addresses previous limitations of the Triad CRA tool. Disordered eating questionnaires can be used to objectively identify dietary restriction for the low EA risk factor. When a risk factor cannot be assessed, delayed menarche can substitute for low BMD and low EA for oligomenorrhea/amenorrhea.

ABSTRACT

Gastrointestinal stromal tumors (GIST) are the most common non-epithelial tumors of the gastrointestinal tract. Wild-type GISTs (WT-GIST) consist of a rare heterogeneous group characterized by the lack of activating mutations in the tyrosine kinase receptor (Kit) and/or platelet derived growth factor receptor A (PDGFRA). However, WT-GIST is characterized by other genomic alterations, including dehydrogenase succinate (SDH) deficiency or mutations in the Ras pathway. Recent studies have reported many mutations in others genes that may be incriminated in the development of WT-GISTs. Moreover, WT-GIST is frequently associated with hereditary cancer syndromes such as the Carney Triad and Type 1 Neurofibromatosis (NF1). WT-GIST affects usually young and pediatric patients. Most WT-GIST subtypes are insensitive to imatinib; therefore, their therapeutic management is somewhat different from usual GISTs. This review resumes the molecular and therapeutic features of this rare entity.

ABSTRACT

The mechanism underlying oligo/amenorrhea in exercising women is often presumed as hypothalamic inhibition secondary to energy deficiency; however, hyperandrogenism may provide an alternative mechanism in some exercising women. Our purpose was to compare reproductive, metabolic, and androgen profiles of exercising women with eumenorrheic, ovulatory menstrual cycles (n = 91), oligo/amenorrhea without evidence of hyperandrogenism (Oligo/Amen; n = 83), and oligo/amenorrhea with evidence of hyperandrogenism (Oligo/Amen-HA; n = 17), and determine the prevalence of oligo/amenorrhea with evidence of hyperandrogenism in exercising women. Self-reported menstrual history and quantification of daily estrogen and progesterone urinary metabolites determined reproductive status. Resting energy expenditure, body composition, and metabolic hormone concentrations determined metabolic status. Serum androgens and calculated free androgen index (FAI) determined androgen status. Groups were similar in age (22.4 ± 0.3 years), height (165.1 ± 0.5 cm), resting energy expenditure (1198.4 ± 12.0 kcal/day), and total triiodothyronine (85.0 ± 1.5 ng/dL) concentration. Oligo/Amen-HA had greater weight (60.0 ± 1.6, 56.1 ± 0.7 kg), body mass index (22.3 ± 0.4, 20.6 ± 0.2 kg/m2), percentage body fat (27.3% ± 1.4%, 24.4% ± 0.6%), fat mass (16.2 ± 1.0, 13.8 ± 0.4 kg), insulin (5.8 ± 0.7, 4.2 ± 0.3 µIU/mL), leptin (12.2 ± 2.3, 6.6 ± 0.7 ng/mL), FAI (6.1 ± 0.3, 1.7 ± 0.1), and luteinizing hormone/follicle-stimulating hormone (1.9 ± 0.3, 1.3 ± 0.2) compared with Oligo/Amen, respectively. In our sample, 17% of those with oligo/amenorrhea had concurrent hyperandrogenism. This study supports that oligo/amenorrhea in some exercising women is related to hyperandrogenism. Novelty Caution must be utilized when discriminating hypothalamic oligo/amenorrhea from hyperandrogenic oligo/amenorrhea. In our sample, 17% of those with presumed hypothalamic oligo/amenorrhea had concurrent hyperandrogenism. Exercise and/or mild energy deficiency may be protective against developing severe hyperandrogenic symptoms.

A tríade sombria ­ maquiavelismo, narcisismo e psicopatia ­ tem sido tradicionalmente avaliada via inventários de autorrelato. Entretanto, instrumentos de autorrelato podem estar suscetíveis à aquiescência ou tendência a concordar com itens mesmo quando possuem conteúdo antagônico. O presente estudo teve por objetivo investigar a dimensionalidade de dois populares instrumentos de autorrelato da tríade, o Dirty Dozen e o Short Dark Triad, verificando a interferência do viés da aquiescência na estrutura fatorial de cada medida. Participaram do estudo 449 universitários (média de idades= 23,47; DP = 6,76; 64% mulheres). O controle da aquiescência produziu soluções fatoriais mais interpretáveis do que a tradicional modelagem bi-factor, amplamente utilizada na área. Os achados também revelam fragilidades na composição teórica de ambos os instrumentos. Argumenta-se que a economia das medidas breves da "tríade sombria" não deve vir desencontrada de uma estrutura fatorial que reflita o conhecimento atual sobre a multidimensionalidade desses traços


The Dark Triad ­ Machiavellianism, narcissism and psychopathy ­ has been traditionally assessed via self-report inventories. Self-report instruments might be susceptible to acquiescent responding or the tendency to manifest agreement with items even when they are semantically opposed. We investigated the dimensionality of two popular self-report instruments of the Dark Triad, the Dirty Dozen and the Short Dark Triad, and inspected the interference of the acquiescence bias in the factor structure of each measure. Participants were 449 students (mean age of 23.47 years; SD = 6.76; 64% females. Controlling for acquiescence produced factor solutions that were more theoretically interpretable than the standard bi-factor modeling, largely used in the field. Findings also raise questions about the theoretical underpinnings of both instruments. We argue that, despite valuable, brief measures of the Dark Triad traits should not come at the expense of a structure adequately reflecting the current knowledge about the multidimensionality of these traits


La Tríada Oscura (maquiavelismo, narcisismo y psicopatía) se ha evaluado a través de inventarios de autoinformes. El instrumento de autoinforme puede ser susceptible de responder o de la tendencia a manifestar acuerdo con los elementos, incluso cuando se oponen semánticamente.Investigamos la dimensionalidad de dos instrumentos populares de autoinforme de la Tríada, Dirty Dozen y Short Dark Triad, e inspeccionamos la interferencia del sesgo de aquiescencia en la estructura de factores de cada medida. Los participantes fueron 449 estudiantes (edad promedio de 23.47 años; SD = 6.76; 64% mujeres. El control de la aquiescencia produjo soluciones factoriales que eran teóricamente más fáciles de interpretar que el modelo bifactor estándar, ampliamente utilizado en el campo. Los hallazgos también plantean preguntas sobre los fundamentos teóricos de ambos instrumentos. Los rasgos de la Tríada, no deben realizarse a expensas de una estructura que refleje adecuadamente el conocimiento actual sobre la multidimensionalidad de estos rasgos

ABSTRACT

Austrian syndrome is a rare triad of meningitis, pneumonia, and endocarditis caused by Streptococcus pneumoniae. We present a case of the Austrian syndrome in the oldest patient in the reviewed literature, with no other classically described risk factors. She had an unusual initial presentation and microorganism portal of entry. Her hospital course was complicated by the diagnosis of monoclonal gammopathy, septic knee joint, septic brain emboli and respiratory failure. We also provide an extensive review of available literature of this commonly unrecognized entity.

A Tríade da Atleta (TA) é uma síndrome que se manifesta na mulher por meio da baixa disponibilidade de energia (com ou sem distúrbios alimentares), disfunção menstrual e baixa densidade mineral óssea (DMO). Geralmente, acomete pessoas envolvidas com a prática de atividades físicas que enfatizam o emagrecimento.(1,2,3) O diagnóstico pode ser feito por meio da análise do Índice de Massa Corporal (IMC) da paciente; pelo cálculo da disponibilidade de energia; exames de densitometria óssea e radiografia da coluna vertebral, associados à presença de menstruações infrequentes ou amenorreia no ciclo menstrual da paciente. A incidência da TA vem aumentando na atualidade devido à maior divulgação dos benefícios da prática esportiva e à preocupação excessiva com a estética corporal imposta pela sociedade em que vivemos. Os profissionais da saúde devem estar aptos a detectar precocemente os sinais da TA para dar início ao tratamento, uma vez que este é o fator mais importante para diminuir as severas consequências ao organismo.(3) O presente artigo tem como objetivo informar sobre a existência dessa síndrome, com destaque para os métodos diagnósticos e o tratamento.(AU)


The Athlete Triad (AT) is a syndrome that is manifested in women through low energy availability with or without eating disorders, menstrual dysfunction and low bone mineral density (BMD). Usually affects people involved in physical activities that emphasize weight loss. (1,2,3) The diagnosis can be made through the patient's Body Mass Index (BMI) analysis; by calculating the availability of energy; examination of bone densitometry and radiography of the spine associated with infrequent periods or amenorrhea in the patient's menstrual cycle. The incidence of AT is increasing today because the dissemination of sports benefits and excessive concern with body imposed by society. Health professionals must be able to detect early signs of AT to start the treatment, because is the most important factor to reduce the severe consequences on the body.(3) This article has the objective to inform health professionals about the existence of this syndrome with emphasis on diagnostic methods and treatment tools.(AU)

ABSTRACT

Resumo O estudo propõe reflexões que tangenciam as práticas de atenção e educação em saúde. Essas práticas, em muitos aspectos, são permeadas por uma base dualista, que enfraquece a potência educativa e a efetividade na promoção da saúde. Aposta-se na possibilidade de que o gesto de timpanizar, proposto por Jacques Derrida, oferece uma possibilidade de aproximação metodológica com o tema, exercício que é feito a partir da leitura de textos produzidos por estudantes de Medicina no contexto de uma aula e de uma carta escrita por Antonin Artaud, considerando sua biografia e suas vivências de saúde-doença. Timpanizar inclui três procedimentos: tatear escombros, disseminar sentidos e criar cadeias de suplementos. Este método cria possibilidades de novos sentidos para conhecimentos e práticas de atenção e educação em saúde.


Abstract The study proposes reflections about care practices and health education. These practices, in many respects, are permeated by a dualistic base, which weakens the educational power and effectiveness in health promotion. There is the hypothesis that the tympanizing gesture proposed by Jacques Derrida, offers a possibility of a methodological approach to the subject, exercise that is done through an Antonin Artaud's letter, considering his biography and his health and illness experiences. Tympanizing includes three procedures: groping the wreckage; disseminating meanings and invention of supplements chains. This method can open different meanings for knowledge and practices of attention and health education.
