ABSTRACT

With advances in sample preparation, small-volume liquid dispensing technologies, high-resolution MS/MS instrumentation, and data acquisition methodologies, it has become increasingly possible to confidently investigate the heterogeneous proteome found within individual cells. In this chapter, we present an automated high-throughput sample preparation workflow based on the Tecan Uno instrument for quantitative single-cell mass spectrometry-based proteomics. Cells are analyzed by the Single-Cell Proteome Analysis platform (SCREEN), which was introduced earlier and provides deeper proteome coverage across single cells.

ABSTRACT

Background: The primary objective is to address the specific needs of plants at different growth stages by delivering precise nutrient concentrations tailored to their developmental requirements. Challenges such as uneven nutrient distribution, fluctuations in pH and electrical conductivity, and inadequate nutrient delivery pose potential hindrances to achieving optimal plant health and yield in hydroponic systems. By overcoming these challenges, the hydroponic farming community aims to enhance the accuracy of nutrient dosing, streamline automation processes, and minimize resource wastage. Hydroponics, a cultivation technique without soil, facilitates the growth of organic vegetation while concurrently minimizing water use and eliminating the necessity for pesticides. In order to achieve effective cultivation of hydroponic plants, it is essential to maintain a controlled environment that encompasses essential factors such as temperature, carbon dioxide (CO2) levels, oxygen availability, and appropriate lighting conditions. Additionally, it is crucial to ensure the provision of vital nutrients to maximize output and productivity. Due to the demanding nature of a hydroponic farmer's schedule, it is necessary to minimize the amount of time dedicated to nutrient management, as well as pH and EC adjustments. Methods: In order to determine and deliver the proper amount of vital nutrients, such as nitrogen, phosphorus, and potassium, based on the plant growth stage, we presented an automatic hydroponic nutrient estimator in this system. We noticed that the plant's nutrient consumption varies depending on its stage of growth according to plant psychology. Four peristaltic pumps with the necessary sensors are controlled by an Arduino board in the suggested system. Both filling and draining the water are done using each pump. To identify the plant stage, we apply the Plant Growth Stage Identification algorithm to encompass the seedling, vegetative, flowers, and fruit stages. Results: The experimental results reveal that the Growth Stage Identification algorithm obtains 97.5% accuracy for the first 5 weeks with 1,715 ppm of nutrition ingestion, identifying the vegetative state. The flowering stage is identified with 97.5% accuracy in the 6-9th week with 2,380 ppm of nutrition consumption, and the fruiting location is determined with 99.4% accuracy in the last 10-15th week with 2,730 ppm of nutrition consumption.

ABSTRACT

This review article describes a historical perspective of elucidation of the nature of the chemical bonds of the high-valent transition metal oxo (M=O) and peroxo (M-O-O) compounds in chemistry and biology. The basic concepts and theoretical backgrounds of the broken-symmetry (BS) method are revisited to explain orbital symmetry conservation and orbital symmetry breaking for the theoretical characterization of four different mechanisms of chemical reactions. Beyond BS methods using the natural orbitals (UNO) of the BS solutions, such as UNO CI (CC), are also revisited for the elucidation of the scope and applicability of the BS methods. Several chemical indices have been derived as the conceptual bridges between the BS and beyond BS methods. The BS molecular orbital models have been employed to explain the metal oxyl-radical character of the M=O and M-O-O bonds, which respond to their radical reactivity. The isolobal and isospin analogy between carbonyl oxide R2C-O-O and metal peroxide LFe-O-O has been applied to understand and explain the chameleonic chemical reactivity of these compounds. The isolobal and isospin analogy among Fe=O, O=O, and O have also provided the triplet atomic oxygen (3O) model for non-heme Fe(IV)=O species with strong radical reactivity. The chameleonic reactivity of the compounds I (Cpd I) and II (Cpd II) is also explained by this analogy. The early proposals obtained by these theoretical models have been examined based on recent computational results by hybrid DFT (UHDFT), DLPNO CCSD(T0), CASPT2, and UNO CI (CC) methods and quantum computing (QC).

Precision agriculture is crucial for ensuring food security in a growing global population. Nutrients, their presence, concentration, and effectiveness, are key components in data-driven agriculture. Assessing macro and micro-nutrients, as well as factors such as water and pH, helps determine soil fertility, which is vital for supporting healthy plant growth and high crop yields. Insufficient soil nutrient assessment during continuous cropping can threaten long-term agricultural viability. Soil nutrients need to be measured and replenished after each harvest for optimal yield. However, existing soil testing procedures are expensive and time-consuming. The proposed research aims to assess soil nutrient levels, specifically nitrogen and phosphorus concentrations, to provide critical information and guidance on restoring optimal soil fertility. In this research, a novel chip-level colorimeter is fabricated to detect the N and P elements of soil onto a handheld colorimeter or spectrophotometer. Chemical reaction with soil solution generates color in the presence of nutrients, which are then quantitatively measured using sensors. The test samples are collected from various farmlands, and the results are validated with laboratory analysis of samples using spectrophotometers used in laboratories. ANOVA test has been performed in which F value > 1 in our study indicates statistically significant differences between the group means. The alternate hypothesis, which proposes the presence of significant differences between the groups, is supported by the data. The device created in this paper has crucial potential in terms of environmental and biological applications.

ABSTRACT

Shelf life estimation is crucial in ensuring the quality of food products. However, traditional methods are time-consuming and inefficient. Therefore, there is an urgent need for simple, efficient and rapid techniques for quality assessments. An electronic nose (E-nose) serves as a solution by rapidly and accurately detecting release of volatile organic compounds (VOCs) during food deterioration. This study aims to develop Arduino-Uno R3 microprocessor based E-nose, equipped with MQ4, MQ5, MQ9 and MQ135 sensors for evaluating shelf life of different edible seeds over the storage period of 150 days. Sensor values were recorded, revealing a significant increase (p-value ≤ 0.05) in MQ5 sensor readings for Nigella seeds from 349 to 480. Sensor values were positively correlated with physical, microbiological and Fourier-transform infrared (FTIR) spectroscopy parameters. Maximum peak shifts were observed from 3000 cm-1 to 2800 cm-1 and 1500 cm-1 to 1000 cm-1 wavenumbers. Hence, this study provides successful E-nose system to determine shelf life of seeds.

ABSTRACT

This work describes the frequency stabilization of a dual longitudinal mode, red (632.8 nm) He-Ne laser, implemented using an open source low-cost microcontroller (Arduino Uno) and its performance characterization using a simple interferometric method. Our studies demonstrate that frequency stability up to 0.42 MHz (3σ, 17 h) can be achieved using this setup. This simple and low-cost system can serve as an excellent part per billion level frequency reference for high-resolution spectroscopy based applications.

ABSTRACT

Temperature transducers are commonly used to monitor process parameters that are controlled by various types of industrial controllers. The purpose of this study is to design and model a simple microcontroller-based acoustic temperature transducer based on the variations of resonance conditions in a cylindrical resonance tube. The transducer's operation is based on the generation of an acoustic standing wave in the free resonance mode of generation within a cylindrical resonance tube which is converted into a train of pulses using Schmitt trigger circuit. The frequency of the generated standing wave (i.e., the train of pulses) is measured by the Arduino Uno microcontroller, where a digital pin is used to acquire pulses that are counted using a build-in software function in an Arduino IDE environment. Experimental results are performed for three sizes of diameters to investigate the effect of the diameter of resonance tube on the obtained results. The maximum nonlinearity error according to Full-Scale Deflection (FSD) is about 2.3 percent, and the relative error of the transducer is evaluated using experimental findings and the regression model. The circuit simplicity and design of the suggested transducer, as well as the linearity of its measurements, are notable.

ABSTRACT

Over a billion people around the world are disabled, among whom 253 million are visually impaired or blind, and this number is greatly increasing due to ageing, chronic diseases, and poor environments and health. Despite many proposals, the current devices and systems lack maturity and do not completely fulfill user requirements and satisfaction. Increased research activity in this field is required in order to encourage the development, commercialization, and widespread acceptance of low-cost and affordable assistive technologies for visual impairment and other disabilities. This paper proposes a novel approach using a LiDAR with a servo motor and an ultrasonic sensor to collect data and predict objects using deep learning for environment perception and navigation. We adopted this approach using a pair of smart glasses, called LidSonic V2.0, to enable the identification of obstacles for the visually impaired. The LidSonic system consists of an Arduino Uno edge computing device integrated into the smart glasses and a smartphone app that transmits data via Bluetooth. Arduino gathers data, operates the sensors on the smart glasses, detects obstacles using simple data processing, and provides buzzer feedback to visually impaired users. The smartphone application collects data from Arduino, detects and classifies items in the spatial environment, and gives spoken feedback to the user on the detected objects. In comparison to image-processing-based glasses, LidSonic uses far less processing time and energy to classify obstacles using simple LiDAR data, according to several integer measurements. We comprehensively describe the proposed system's hardware and software design, having constructed their prototype implementations and tested them in real-world environments. Using the open platforms, WEKA and TensorFlow, the entire LidSonic system is built with affordable off-the-shelf sensors and a microcontroller board costing less than USD 80. Essentially, we provide designs of an inexpensive, miniature green device that can be built into, or mounted on, any pair of glasses or even a wheelchair to help the visually impaired. Our approach enables faster inference and decision-making using relatively low energy with smaller data sizes, as well as faster communications for edge, fog, and cloud computing.

Se realizó una revisión bibliográfica con el objetivo de obtener información actualizada acerca de las características, diagnóstico y tratamiento del síndrome del uno y medio. Se emplearon principalmente las bases de datos disponibles en Infomed, Google Scholar y Pubmed. El síndrome del uno y medio es una entidad infrecuente, que se caracteriza por parálisis de la mirada conjugada horizontal y alteración del fascículo longitudinal medial ipsilateral secundario a diversas etiologías, entre las que se incluyen la enfermedad cerebrovascular y la esclerosis múltiple. Clínicamente se presenta con exotropía y nistagmo a la abducción. Su diagnóstico puede ser establecido en la exploración por las alteraciones típicas de los movimientos oculares, mientras que la imagen de resonancia magnética cerebral resulta indispensable para el diagnóstico diferencial y etiológico(AU)


One and a half syndrome is an infrequent condition characterized by conjugate horizontal gaze palsy and ipsilateral medial longitudinal fasciculus alteration secondary to various etiologies, including cerebrovascular disease and multiple sclerosis. Clinically, it presents with exotropia and abduction nystagmus. Its diagnosis may be established during exploration, due to the typical eye movement alterations, whereas brain magnetic resonance imaging is indispensable for differential and etiological diagnosis. A bibliographic review was conducted to obtain updated information about the characteristics, diagnosis and treatment of one and a half syndrome. Use was made of the databases available in Infomed, Google Scholar and Pubmed(AU)

ABSTRACT

Molecular target therapies have markedly improved the survival of non-small cell lung cancer (NSCLC) patients, especially those with epidermal growth factor receptor (EGFR) mutations. A positive EGFR mutation is even more critical when the chronicity of spinal metastasis is considered. However, most prognostic models that estimate the life expectancy of spinal metastasis patients do not include these biological factors. We retrospectively reviewed 85 consecutive NSCLC patients who underwent palliative surgical treatment for spinal metastases to evaluate the following: (1) the prognostic value of positive EGFR mutation and the chronicity of spinal metastasis, and (2) the clinical significance of adding these two factors to an existing prognostic model, namely the New England Spinal Metastasis Score (NESMS). Among 85 patients, 38 (44.7%) were EGFR mutation-positive. Spinal metastasis presented as the initial manifestation of malignancy in 58 (68.2%) patients. The multivariate Cox proportional hazard model showed that the chronicity of spinal metastasis (hazard ratio (HR) = 1.88, p = 0.015) and EGFR mutation positivity (HR = 2.10, p = 0.002) were significantly associated with postoperative survival. The Uno's C-index and time-dependent AUC 6 months following surgery significantly increased when these factors were added to NESMS (p = 0.004 and p = 0.022, respectively). In conclusion, biological factors provide an additional prognostic value for NSCLC patients with spinal metastasis.

ABSTRACT

Due to the spread of COVID-19 across the world and the increased need for non-contact thermometers to prevent the spread of disease, a new electronic thermometer has been designed and implemented for measuring human body temperature from a distance. This device is currently in use at building entrances to measure the body temperatures of employees, students, and customers. This system is designed using low-cost easy-to-assemble open-source electronic components. The system consists of seven main parts: an Arduino UNO microcontroller, an infrared (IR) thermometer for non-contact temperature measurements (GY-906 MLX90614ESF module), an IR motion sensor (TCRT 5000) for the purpose of contactless initiation of the system, a graphic LCD to display results, a DS3231 clock module for a real-time clock and calendar, and a micro-SD storage board to store device audio instructions.

RESUMO Objetivo Conhecer o processo de trabalho da equipe de enfermagem em Centros de Atenção Psicossocial Infanto-Juvenil de um município do estado de São Paulo. Método Estudo qualitativo, descritivo e exploratório, embasado na abordagem do materialismo histórico e dialético, realizado com a equipe de enfermagem por meio de observação participante e entrevistas semiestruturadas. Resultados Participaram 11 sujeitos da equipe de enfermagem. O processo de trabalho da equipe de enfermagem emerge da tensão dialética de duas categorias: o não saber, pautado na alienação do processo de trabalho de enfermagem e na subordinação ao saber da equipe multiprofissional, e o saber pautado na clínica de enfermagem delineada entre as perspectivas biológica e relacional. Conclusão O processo de trabalho da equipe de enfermagem acontece quando pautado em um não saber pela alienação ao saber do outro, e em um saber por meio da implementação do Processo de Enfermagem pela Sistematização da Assistência de Enfermagem na perspectiva biológica, considerando aspectos relacionais e administrativos, o que se configura como uma contradição.


RESUMEN Objetivo: Conocer el proceso de trabajo del equipo de enfermería en Centros de Acompañamiento Psicosocial Infantil y Juvenil de un municipio de la provincia de São Paulo. Método: Estudio cualitativo; descriptivo y exploratorio; basado en el abordaje del materialismo histórico y dialéctico; realizado con el equipo de enfermería por medio de observación participante y entrevistas semiestructuradas. Resultados: Participaron 11 miembros del equipo de enfermería. Su proceso de trabajo surge de la tensión dialéctica de dos clases: el no saber; basado en la alienación del proceso de trabajo de enfermería y la subordinación al saber del equipo multiprofesional; y al saber basado en la clínica de enfermería diseñada entre la perspectiva biológica y relacional. Conclusión: El proceso de trabajo del equipo de enfermería ocurre cuando basado en un "no saber" por la alienación al saber de uno; y en un saber a través de la implementación del Proceso de Enfermería por la Sistematización del Acompañamiento de Enfermería en la perspectiva biológica; considerando aspectos de relación y administración; lo que se define como una contradicción


ABSTRACT Objective To know the nursing team work process in Children and Adolescents Psychosocial Care Centers in a city of the state of São Paulo. Method This is a qualitative, descriptive, and exploratory study, based on the approach of historical and dialectical materialism, carried out with the nursing team through participant observation and semi-structured interviews. Results Eleven subjects from the nursing team participated in the study. The nursing team work process emerges from the dialectical tension of two categories: the non-knowledge, based on the alienation of the nursing work process and subordination to the knowledge of the multidisciplinary team, and the knowledge based on the nursing clinic outlined within the biological and relational perspectives. Conclusion The nursing team work process takes place when guided by a non-knowledge resulting from alienation due to the knowledge of the other, and by knowledge through the implementation of the Nursing Process through the Nursing Care Systematization based on a biological perspective, considering relational and administrative aspects, which emerges as a contradiction.

ABSTRACT

Neste trabalho a autora procura pensar a experiência clínica com dois pacientes, com os quais surgiram vivências estranhas e ameaçadoras, que ela nomeia de início como assombrações. Suas reflexões sugerem que as transformações que o analista pode fazer dessas assombrações - suas intuições, seu pensamento onírico e funcionamento alfa - podem trazer desenvolvimentos úteis à dupla analítica, como o surgimento de novos vértices, a ampliação do espaço mental e a possibilidade de digerir e elaborar fortes emoções, movimentos mais ligados à noção de assombro, pensado como propiciador do conhecer e do ser. O trabalho aborda também as transformações vividas pelo analista em sua formação, as transformações propiciadas pela escrita analítica, bem como as transformações existenciais durante a vida do analista, que podem dificultar sua função analítica, necessitando constantes cuidados e reanálises.


In this article the author seeks to reflect on the clinical experience with two patients who lived through strange and threatening episodes, which, right from the very start, the author named as hauntings. Her considerations suggest that these hauntings may be transformed - through the analyst's intuition, oneiric thought and alpha function - and the result of these transformations may bring useful unfoldings to the analytical pair, such as the springing of new vertex, the amplification of the mental space and the possibility to digest and elaborate strong emotions, movements closer to the notion of haunt regarded as a sponsor for knowledge and for being. This article also approaches the transformations experienced by the analyst in their formation, the transformations the analytical writing may provide, as well as existential transformations in the analyst's own life, which may hamper the analytical activity, demanding constant care and reanalyses.


En el presente trabajo, la autora plantea la experiencia clínica con dos pacientes, en la que surgen experiencias extrañas y amenazantes, que inicialmente denomina apariciones. Sus reflexiones sugieren que las transformaciones de estas experiencias que el analista puede llevar a cabo (sus intuiciones, su pensamiento onírico y su funcionamiento alfa) pueden aportar desarrollos útiles al dúo analítico, como la aparición de nuevos vértices, la expansión del espacio mental y la posibilidad de digerir y elaborar emociones fuertes, movimientos más vinculados a la noción de asombro, pensado como propiciador del saber y del ser. El trabajo también aborda las transformaciones experimentadas por el analista en su formación, las transformaciones proporcionadas por la escritura analítica, así como las transformaciones existenciales durante la vida del analista, que pueden obstaculizar su función analítica, lo que requiere un cuidado constante y reanálisis.


Dans ce travail, l'auteur essaie de réfléchir l'expérience clinique avec deux patients, où des expériences étranges et menaçantes ont émergé, qu'elle nomme d'abord comme des hantises. Ses réflexions suggèrent que les transformations que l'analyste peut faire de ces hantises - ses intuitions, sa pensée onirique et son fonctionnement alpha - peuvent apporter des développements utiles au duo analytique, tels que l'émergence de nouveaux sommets, l'expansion de l'espace mental et la possibilité de digérer et élaborer des émotions fortes, des mouvements plus liés à la notion d'étonnement, pensé comme apportant de la connaissance et de l'être. Le travail aborde également les transformations vécues par l'analyste dans sa formation, les transformations apportées par l'écriture analytique, ainsi que les transformations existentielles au cours de la vie de l'analyste, qui peuvent entraver sa fonction analytique, nécessitant des soins constants et réanalyses.

INTRODUCTION: Objective: this study aims to investigate the physical stability of standard formulations for parenteral nutrition, with and without lipids, in one bag for preterm babies. Method: standard formulations for first-day and for second-day parenteral nutrition of preterm babies weighing 1,000 grams were prepared in triplicate. Standard all-in-one formulas for first-day and for second-day parenteral nutrition were compared with equivalent standard lipid-free formulations. The standard formulas contain glucose, lipids, calcium gluconate, potassium chloride, sodium chloride, and vitamins. Stability was evaluated using visual inspection, particle size analysis, and pH measurement. The physical instability of the all-in one parenteral nutrition formulas was reported as creaming, coalescence, or cracking, whereas the instability of the lipid-free parenteral nutrition formulas was described as turbidity, precipitation, gas formation, or colour changes. Two independent evaluators assessed the visual changes under light and against a dark-light background, as well as using the Tyndall beam effect. Particle size was measured using a particle size analyzer. Chemical compatibility was checked using a pH-meter. Result: the result showed that the all-in-one (AIO) parenteral nutrition formulas develop reversible creaming on day three, while the lipid-free ones remain clear. As regards pH and particle size, none of the four AIO and lipid-free formulas developed significant changes (ΔpH < 0.05 and particle size < 400 nm) until after seven days. Conclusion: all four formulas are stable following examination with visual inspection, a pH-meter, and a particle size analyzer.


INTRODUCCIÓN: Objetivo: el objetivo del estudio es investigar la estabilidad de las formulaciones estandarizadas de nutrición parenteral, con y sin lípidos, para prematuros. Métodos: se prepararon por triplicado las formulaciones estandarizadas del 1º y 2º día para prematuros de menos de 1000 gramos. Se compararon las soluciones preparadas "todo-en-uno" con las soluciones estandarizadas equivalentes que no contenían lípidos. Las soluciones estandarizadas contenían glucosa, aminoácidos, lípidos, gluconato cálcico, cloruro potásico, cloruro sódico y vitaminas. La estabilidad se evaluó mediante inspección visual, medición del tamaño de las partículas, y medición del pH. Se interpretó como inestabilidad física de las soluciones ternarias la presencia de separación de fases, coalescencia o la formación de una capa grasa, mientras que en las preparaciones sin lípidos se describió como turbidez, precipitación, formación de gas o cambios de coloración. Dos evaluadores independientes comprobaron los cambios visuales bajo luz directa o en contraste con un fondo oscuro, así como mediante el uso del efecto Tyndall. El tamaño de las partículas se midió mediante un analizador de partículas. La compatibilidad química se comprobó con el Phmetro. Resultados: todas las nutriciones parenterales todo-en-uno (AIO) desarrollaron una capa grasa (creaming) al tercer día, mientras que las mezclas sin lípidos permanecieron transparentes. Con respecto al pH y el tamaño de las partículas, ninguna de las cuatro emulsiones AIO y nutrición parenteral sin lípidos mostraron cambios significativos (incremento de pH < 0,03 y tamaño de las partículas < 400 nm) en los siete primeros días. Conclusión: las cuatro formulaciones fueros estables tras inspección visual, medición del pH y análisis del tamaño de las partículas.

ABSTRACT

The electrical equivalent circuit for a neuron is composed of common electrical components in a configuration that replicates the passive electrical properties and behaviors of the neural membrane. It is a powerful tool used to derive such fundamental neurophysiological equations as the Hodgkin-Huxley equations, and it is also the basis for well-known exercises that help students to model the passive (Ohmic) properties of the neuronal membrane. Unfortunately, as these exercises require basic knowledge of electronics, they are generally not physically conducted in biomedical courses, but remain merely conceptual exercises in a book or simulations on a computer. In such manifestations, they lack the "hands-on" appeal for students and teachers afforded by laboratory experimentations. Here, we propose a new approach to these experiments in which a desktop 3D printer and conductive paint are used to build the circuit and the popular programmable microcontroller, the Arduino UNO, is used as a graphical oscilloscope when connected to a standard computer. This set-up has the advantage of being very easy to build and less clumsy than a circuit in a prototyping board or connected with alligator clips, with the added benefit of being conveniently portable for classroom demonstrations. Most importantly, this method allows the monitoring of real-time changes in the current flowing through the circuit by means of a graphical display (by way of the Arduino) at a fraction of the cost of commercially available oscilloscopes.

ABSTRACT

The one and a half Fisher syndrome is produced by an unilateral lesion in the dorsal and inferior part of the pons located specifically in the pontine tegmentum. This is an area that contains the medial longitudinal fasciculus (LMF), the paramedian pontine reticular formation (PPRF) and the nucleus of the sixth cranial nerve, responsible for the horizontal movements of the gaze. Clinically it manifests with conjugate gaze palsy with internuclear ophthalmoplegia. The case is reported on a 60 year-old patient, who presented with binocular diplopia, ophthalmoplegia and paralysis of the gaze conjugated to the left. Imaging studies were performed that showed a hemi-protuberant haematoma secondary to a cavernoma. As expected, he presented with several episodes of re-bleeding with clinical worsening: headache, diplopia, tetraparesis, and hypoaesthesia in the right side of the face, as such that the microsurgical exeresis of the protuberant cavernoma was considered.

RESUMEN INTRODUCCIÓN: Los pacientes con trastorno afectivo bipolar pueden presentar alteraciones cognoscitivas que en algunos casos tienen un curso progresivo, por lo cual se ha cuestionado si la evolución de esta enfermedad se asocia a demencia, particularmente aquellas pertenecientes al espectro de la degeneración lobar frontotemporal. En este contexto, discriminar si un paciente presenta una demencia secundaria a la enfermedad psiquiátrica de base o si cursa una enfermedad neurodegenerativa además del trastorno afectivo bipolar, es un desafío para el diagnóstico diferencial. OBJETIVO: Comparar los desempeños cognoscitivos en pacientes con trastorno afectivo bipolar, con veinte años o más de evolución de la enfermedad y pacientes con demencia frontotemporal variante conductual. MATERIALES Y MÉTODOS: Estudio exploratorio, descriptivo y transversal en una cohorte seleccionada de casos por método no probabilístico. Los datos se analizan por medio de estadísticos no paramétricos. RESULTADOS: Eespecto al grupo control (N:27), los pacientes con demencia frontotemporal (N:24) presentan desempeños significativamente bajos en memoria verbal, funciones ejecutivas, praxias visoconstruccionales y atención (p <0,01). El grupo de trastorno bipolar (N:17) tiene bajos desempeños en estos procesos, pero no presenta fenómenos patológicos significativos asociados a intrusiones y perseveraciones. Entre los grupos clínicos no se identifican diferencias significativas. CONCLUSIÓN: Aunque los grupos clínicos comparten el compromiso en los procesos cognoscitivos evaluados, los desempeños son más bajos en el grupo de demencia frontotemporal, lo que sugiere que en una enfermedad degenerativa de menor tiempo de evolución y aparición en etapa presenil el déficit cognitivo es mayor que en una enfermedad psiquiátrica crónica.


SUMMARY INTRODUCTION: Patients with Bipolar Disorder may present cognitive alterations that in some cases have a progressive course, whereby it has been questioned if the evolution of this disease is associated with dementia, in particular those that belong to the spectrum of frontotemporal lobar degeneration. Thereby, discriminate if a patient has a dementia secondary to the underlying psychiatric illness or if the patient presents a neurode-generative disease besides the bipolar disorder is a challenge for the differential diagnosis. OBJECTIVE: To compare the cognitive performance in a sample of patients with Bipolar Disorder and twenty years or more of disease progression, and patients with behavioral variant of frontotemporal dementia. MATERIALS AND METHODS: Exploratory, descriptive and transversal study in a cohort of cases selected with a non probabilistic method. Dates are compared through non parametric statistics. RESULTS: Relative to Control group (N:27), Frontotemporal Dementia Patients (N:24) have significantly lower performances in verbal memory, executive functions, visoconstructional praxis and attention tasks (p <0,01). Bipolar Disorder group (N:17) has lower performances in this processes but don't present pathological markers such as intrusions and perseverative responses. There are no significant differences when comparing between clinical groups. CONCLUSION: Although clinical groups share the compromise in most of the cognitive process evaluated, the performances are lower in Frontotemporal dementia group, which suggests that in a degenerative disease of less evolution time and onset in presenile stage, the cognitive deficit is greater than in a chronic psychiatric illness.

ABSTRACT

El síndrome del uno y medio es una entidad que se caracteriza por parálisis de la mirada conjugada horizontal y alteración del fascículo longitudinal medial (FLM) ipsilateral secundario a diversas etiologías, entre las que se incluye el ataque cerebrovascular (ACV). Se describen dos casos de pacientes que consultaron por hemiparesia, incapacidad para realizar movimientos horizontales en uno de los ojos y abducción forzada del ojo contralateral e imposibilidad para la aducción. En ambos casos los estudios demostraron la presencia de un ACV isquémico vertebrobasilar como origen del síndrome; el reconocimiento de la fisiología de la mirada conjugada es fundamental en el abordaje del paciente y permite realizar un diagnóstico exacto a partir de una historia clínica detallada y una evaluación física minuciosa.


The One and a Half Syndrome is a condition characterized by paralysis of the horizontal conjugate gaze and a ipsilateral internuclear ophthalmoplegia secondary to other processes such as a stroke. We describe two cases of patients that had hemiparesis, inability to perform horizontal movements in one eye and forced abduction of the contralateral eye with inability to adduct it. With the corresponding study of both cases it was possible to diagnose a vertebrobasilar ischemic stroke as the origin of the syndrome; the recognition and understanding of the physiology of the conjugate gaze is crucial in the approach of the patients and allow an accurate diagnosis using a detailed history and a thorough physical evaluation.
