ABSTRACT

Este artigo se baseia em um estudo feito com o objetivo de analisar indicadores sobre a testagem da sífilis na gestação no Programa de Qualificação das Ações de Vigilância em Saúde (PQAVS) e no Programa Previne Brasil no estado da Paraíba, e também de levantar aspectos do tratamento terapêutico para sífilis gestacional. Trata-se de uma pesquisa descritiva-exploratória, na qual foram sistematizados dados do indicador 11, testes por gestantes, do PQAVS e do indicador de desempenho da Atenção Primária à Saúde (APS), com base na proporção de gestantes que realizaram exames de sífilis e HIV durante o pré-natal em 2020; também foi feita a sistematização do webquestionário direcionado a profissionais da APS (médicos/enfermeiros) e autoaplicado sobre a atuação e tratamento terapêutico para sífilis gestacional. Dos 223 municípios da Paraíba, apenas 12% atingiram a meta do PQAVS e 39% a do Previne Brasil em 2020. Em relação ao webquestionário, houve a participação de 142 profissionais, dos quais 85% realizam o tratamento terapêutico preconizado pelo Ministério da Saúde para a APS. Desse modo, deve ser ressaltada a importância da ampliação da oferta de testes para sífilis, dos insumos para o tratamento adequado e da qualificação dos profissionais e da informação em saúde.


This article is based on a study to analyze indicators on syphilis testing during pregnancy in the PQAVS - Programa de Qualificação das Ações de Vigilância em Saúde (Health Surveillance Actions Qualification Programme) and in the Programa Previne Brasil (Previne Brasil Programme) in the state of Paraíba, Brazil, and also to survey aspects of the therapeutic management for gestational syphilis. It is a descriptive-exploratory research, in which data from indicator 11, tests for pregnant women, from the PQAVS and from the Primary Health Care (PHC) performance indicator, based on the proportion of pregnant women with syphilis and HIV tests during prenatal care in 2020 were systematised; in addition to this systematization, a self-administered webquestionnaire on the performance and therapeutic management for gestational syphilis by professionals (doctors/nurses) from the PHC was also systematised. Taking into account the 223 municipalities in Paraíba, only 12% reached the PQAVS goal and 39% reached the Previne Brasil goal in 2020. Regarding the webquestionnaire, 85% of the 142 professionals who answered it, carry out the therapeutic management recommended by the Ministry of Health for the PHC. Thus, it is fundamental to emphasise the importance of expanding the supply of tests for syphilis, supplies for adequate treatment, and the qualification of health professionals and information.


El presente artículo se basa en un estudio efectuado con el objetivo de analizar indicadores sobre la prueba de sífilis durante el embarazo en el PQAVS - Programa de Qualificação das Ações de Vigilância em Saúde (Programa de Calificación para Acciones de Vigilancia en Salud) y en el Programa Previne Brasil en el estado de Paraíba, Brasil, y de resaltar aspectos del tratamiento terapéutico de la sífilis gestacional. Se trata de una investigación descriptiva-exploratoria, en la que se sistematizaron datos del indicador 11, pruebas realizadas por embarazadas, del PQAVS y del indicador de desempeño de la Atención Primaria de Salud (APS), a partir de la proporción de gestantes que se sometieron a pruebas de sífilis y de HIV durante la atención prenatal en 2020; también se sistematizóel cuestionario web dirigido a profesionales de la APS (médicos/enfermeros) y autoadministrado sobre el desempeño y el tratamiento terapéutico de la sífilis gestacional. De los 223 municipios de Paraíba, apenas 12% alcanzaron la meta del PQAVS y 39% lograron la meta del Previne Brasil en 2020. En relación al cuestionario web, participaron 142 profesionales, de los cuales 85% realizan el tratamiento terapéutico recomendado por el Ministerio de Salud para la APS. Así, es fundamental la importancia de ampliar la oferta de pruebas para la sífilis, de los medicamentos para el tratamiento adecuado, la calificación de los profesionales e la información relacionada a la salud.

ABSTRACT

Chikungunya, arbovirose que provoca febre e artralgia debilitante, demonstra potencial crônico e incapacitante por longos períodos, não havendo vacinas ou terapias específicas. Recentemente, a doença evoluiu da condição negligenciada para uma ameaça à saúde pública, impactando milhões de pessoas em regiões tropicais e subtropicais. Este estudo analisa a produção do conhecimento sobre a Chikungunya, na perspectiva da Vigilância em Saúde. Trata-se de metodologia exploratória-descritiva, com análise bibliométrica. Realizou-se a coleta nas bases Scopus e Web of Science para artigos de 2008 a 2022. A análise revelou uma série temporal de produção destacando a contribuição de Estados Unidos, Brasil e França. Identificaram-se como áreas mais produtivas: doenças transmissíveis, medicina tropical e parasitologia, fundamentais à Vigilância em Saúde e ao planejamento de políticas públicas. Quanto aos pesquisadores, Weaver, Scott C., Failloux, Anna-Bella e De Lamballerie, Xavier foram relevantes no cenário global, indicando a importância da colaboração e da abordagem interdisciplinar.


Chikungunya, an arbovirus that causes fever and debilitating arthralgia, has potential to be chronic and incapacitating for long periods, and there are no vaccines or therapies available for it. Recently, the disease has evolved from a neglected condition to public health threat, impacting millions in tropical and subtropical regions. This study analyzed the knowledge production about chikungunya, from the perspective of Health Surveillance, using an exploratory-descriptive methodology and bibliometric analysis. Articles from 2008 to 2022 were collected from Scopus and Web of Science databases. The analysis showed a production time series, highlighting the contribution of United States, Brazil, and France. The most productive areas were identified as: communicable diseases, tropical medicine and parasitology, which are fundamental to Health Surveillance and public policy planning. Regarding researchers, Weaver, Scott C., Failloux, Anna-Bella, and De Lamballerie, Xavier were relevant in the global scenario, indicating the importance of collaboration and of the interdisciplinary approach.


El chikungunya, arbovirus que causa fiebre y artralgia debilitante, tiene potencial de ser crónico e incapacitante por largos periodos, todavía no hay vacunas ni terapias. Recientemente, la enfermedad ha pasado de afección desatendida a amenaza para la salud pública, afectando a millones en regiones tropicales y subtropicales. Este estudio analiza la producción de conocimientos sobre chikungunya, desde la perspectiva de la Vigilancia Sanitaria. Se utilizó una metodología exploratoria-descriptiva con análisis bibliométrico. Se recompilaron artículos de Scopus y Web of Science, 2008-2022. El análisis reveló una serie temporal de producción, destacando la contribución de EEUU, Brasil y Francia. Se identificaron como áreas más productivas: enfermedades transmisibles, medicina tropical y parasitología, fundamentales para la Vigilancia Sanitaria y la planificación de políticas públicas. En cuanto a los investigadores, Weaver, Scott C., Failloux, Anna-Bella y De Lamballeire, Xavier fueron relevantes en el escenario global, indicando la importancia de la colaboración y del enfoque interdisciplinario.

ABSTRACT

Human listeriosis is an infectious disease caused by Listeria monocytogenes. The invasive form of this disease leads to a high rate of hospitalizations and fatality. The main mode of transmission is through contaminated ready-to-eat foods such as dairy, vegetables and meat products. The knowledge of the diversity and population dynamics of isolates collected from human and food sources is essential for the detection of clusters and the identification of common sites of infection. The aim of this study was the molecular characterization of L. monocytogenes isolates in Argentina. We sequenced a total of 63 isolates, 35 from human and 28 from food sources, collected between 2018 and 2023. Our genomic study divided the isolates into two lineages, four serogroups, 17 sequence types and 15 clonal complexes (CCs). The hypervirulent clone CC1 (lineage I; serogroup IVb) predominated in human and food samples. The phylogenomic analysis showed a high and possible epidemiological relationship between isolates from human and/or food sources, suggesting the presence of transmission chains in our country. These findings highlight the need to strengthen genomic surveillance of L. monocytogenes in Argentina. The identification of geographic distribution and characteristics of predominant and emerging clones from human and food sources might help to focus action plans and public health policies better directed at the control and prevention of listeriosis.

ABSTRACT

RESUMO Objetivou-se investigar as condições de trabalho e a saúde física e mental de profissionais de saúde atuantes na linha de frente da covid-19 em serviços de urgência, emergência e terapia intensiva no Brasil, no segundo ano da pandemia. Estudo transversal, com uso de questionário eletrônico, por meio do qual coletaram-se dados sobre condições de trabalho, saúde física e mental, além do instrumento Depression, Anxiety and Stress Scale (DASS-21). A amostra (n=209) incluiu enfermeiros (28,7%), técnicos de enfermagem (30,1%), fisioterapeutas (33%) e médicos (8,2%). Os profissionais possuíam idade média de 34,6 anos e relataram uma carga horária média de 53,5 horas/semana. Verificou-se aumento das horas trabalhadas (62%) e da quantidade de pacientes (84%). A maioria relatou bom relacionamento com o chefe (89%) e satisfação com o trabalho (87%). A prevalência de sintomas de estresse, ansiedade e depressão foi superior a 45%, com predomínio de sintomas graves ou extremamente graves. A prevalência de dor musculoesquelética e fadiga foi de 84,7% e 83,3%, respectivamente. Os profissionais de saúde apresentaram aumento de volume de trabalho e de exigência durante a pandemia de covid-19. Observou-se, ainda, intenso prejuízo à saúde física e mental desses trabalhadores.


ABSTRACT The objective was to investigate the working conditions and physical and mental health of health professionals working on the front lines of COVID-19 in urgent, emergency, and intensive care services in Brazil, in the second year of the pandemic. Cross-sectional study, using an electronic questionnaire, through which data on working conditions, and physical and mental health were collected, in addition to the Depression, Anxiety and Stress Scale (DASS-21) instrument. The sample (n=209) included nurses (28.7%), nursing technicians (30.1%), physiotherapists (33%), and physicians (8.2%). The professionals had an average age of 34.6 years and reported an average workload of 53.5 hours/week. There was an increase in hours worked (62%) and in the number of patients (84%). Most reported a good relationship with their boss (89%) and job satisfaction (87%). The prevalence of symptoms of stress, anxiety, and depression was greater than 45%, with a predominance of severe or extremely severe symptoms. The prevalence of musculoskeletal pain and fatigue was 84.7% and 83.3%, respectively. Health professionals showed an increase in workload and demand during the COVID-19 pandemic. There was also intense damage to the physical and mental health of these workers.

ABSTRACT

Resumo Paulo Freire pensava a Educação Popular (EP) como um processo emancipatório, por meio da problematização e agir crítico como dimensões da existência humana. A partir de suas contribuições aos campos da Educação e da Saúde, este texto tem como objetivo realizar uma revisão narrativa de literatura sobre as articulações das ideias de Paulo Freire com as ações de Vigilância Popular em Saúde (VPS). O texto apresenta como a contribuição de Paulo Freire e da Educação Popular em Saúde inspirou a construção de uma Vigilância Popular em Saúde, que busca promover a transformação da realidade local frente às situações de violações de direitos e na defesa da vida. Dessa maneira, experiências dos territórios e das populações em situação de vulnerabilidade, na maioria das vezes, lançam mão de estratégias pedagógicas da Educação Popular para se constituírem como práticas de VPS. É no território que a Educação Popular em Saúde se torna fundamental para o estímulo à transformação da percepção dos indivíduos, problematizando sua realidade. A promoção de uma práxis acerca da "situação limite" no cotidiano dos atores sociais permite um diagnóstico da realidade, baseado na informação científica em diálogo com a cultura e a organização popular, como possibilidade de construção de "inéditos viáveis".


Abstract Paulo Freire considered Popular Education (PE) as an emancipatory process, through debate and critical action, as a dimension of human existence. This text aims to conduct a narrative literature review on the articulations of Paulo Freire's ideas with Popular Health Surveillance (PHS) actions based on his contributions to Education and Health. The text presents how the contributions of Paulo Freire and Popular Health Education inspired the construction of Popular Health Surveillance, which seeks to promote the transformation of local reality in the face of rights violations and advocate for life. Thus, experiences from vulnerable territories and populations often use Popular Education pedagogical strategies to establish PHS practices. Popular Health Education becomes crucial in the territory to stimulate the transformation of individual perception and discuss their reality. Promoting a praxis about the "critical situation" in the daily lives of social stakeholders allows diagnosing reality based on scientific information in dialogue with culture and popular organization as a possibility of building the "viable unprecedented".

ABSTRACT

Preparation and response to Public Health emergencies involve efforts in developing systems for early detection, alert and response. Models for dealing with notification delay and diversification of data sources are some of the commonly used strategies for faster information and action. In this paper, we present the strategy implemented in Rio de Janeiro municipality, where data from urgency and emergency visits were acquired and modeled, in order to detect trend shifts and generate alerts. From the ICD-10 field in electronic records, time series representing events of interest were created. A GAM model was fitted for smoothing, slope determination in each point, and alert generation. The results obtained are displayed in a dashboard, monitored daily. From 2023, multiple events of interest were identified through the dashboard, some of which lead to coordinated communication and actions in the territory. We draw attention to the potentials in the use of these type of data on identifying events of interest in a timely manner, approaching the concepts of a modern surveillance.


A preparação e resposta às emergências em Saúde Pública envolve o investimento em sistemas de detecção precoce, alerta e resposta. Modelos de correção de atraso de notificação e a diversificação de fontes de dados utilizadas são algumas abordagens comumente utilizadas para geração de informação e ação mais oportunos. Neste artigo é apresentada a estratégia implementada no município do Rio de Janeiro de utilização de dados de atendimentos de urgência e emergência unida à aplicação de modelos de detecção de tendências para geração automatizada de alertas. A partir de CIDs marcados nos prontuários eletrônicos de atendimentos, monitoram-se séries temporais de eventos de interesse no município. Um modelo GAM é ajustado às séries para suavização, determinação da inclinação e geração dos alertas. Os resultados são exibidos em painel e monitorados diariamente. Desde 2023, múltiplos eventos de interesse foram identificados através do painel e resultaram em comunicação coordenada e ações no território. Os resultados exaltam a potencialidade no uso desses dados na identificação de eventos de interesse em tempo oportuno, alinhando-se a conceitos de uma vigilância moderna.

ABSTRACT

Desde el año 1996 el subtipo de gripe aviar de alta patogenicidad A(H5N1) ha estado casi de forma ininterrumpida causando brotes en aves salvajes y domésticas, además de casos en seres humanos con una mortalidad cercana al 50%. Sin embargo, los años de mayor circulación han sido precisamente los años posteriores a la pandemia de COVID-19, en los que se han registrado diversos casos en humanos en lugares donde nunca antes habían aparecido, además de múltiples casos en mamíferos salvajes, domésticos y peri domésticos, que entrañan cierta preocupación por el riesgo que puede suponer para el salto del virus al ser humano través de cadenas de transmisión de mayor o menor extensión. El brote actual de A(H5N1) nos muestra que el concepto One-Health debe estar más vivo que nunca para aunar esfuerzos entre profesionales de diferentes sectores de la sanidad humana, animal y medio ambiental para evitar o minimizar estos riesgos, de tal forma que los laboratorios de referencia como los Centros Nacionales de Gripe dispongan de los medios humanos y materiales para ofrecerinformación rápida y relevante en el menor tiempo posible antes emergencias de este tipo. Las herramientas de diagnóstico y seguimiento que se deben utilizar en estos casos deben estar disponibles para cualquier eventualidad, y llegar más allá de los datos básicos debe ser una premisa indispensable para poder hacer un seguimiento pormenorizado que sirva para acotar brotes, limitar la difusión de la enfermedad, y ayudar al diseño de futuras vacunas pandémicas frente a virus aviares. Since 1996, the highly pathogenic avian influenza subtype A(H5N1) has been causing almost uninterrupted outbreaks in wild and domestic birds, as well as cases in humans with a mortality rate close to 50%. However, the years of greatest circulation have been precisely the years following the COVID-19 pandemic, in which several cases have been recorded in humans in places where they had never appeared before, in addition to multiple cases in wild, domestic and peri-domestic mammals, which raise some concern about the risk that the virus may jump to humans through chains of transmission of greater or lesser extent. The current outbreak of A(H5N1) shows us that the One-Health concept should be more alive than ever to join efforts between professionals from different sectors of human, animal and environmental health to avoid or minimize these risks, so that reference laboratories such as the National Influenza Centers have the human and material resources to provide rapid and relevant information in the shortest possible time before emergencies of this type. The diagnostic and monitoring tools to be used in these cases must be available for any eventuality, and going beyond the basic data must be an indispensable premise to be able to carry out a detailed monitoring that serves to limit outbreaks, limit the spread of the disease, and help in the design of future pandemic vaccines against avian viruses. ABSTRACT

Antecedentes: El Registro Español de Investigación en Dermatitis de Contacto y Alergia Cutánea tiene entre sus objetivos la vigilancia epidemiológica de la dermatitis de contacto. Para ello es importante conocer si se producen alteraciones en el tiempo de las prevalencias de las positividades a los distintos alérgenos. Objetivos: Describir las variaciones en las tendencias temporales en positividades a alérgenos en la serie estándar del GEIDAC en el periodo comprendido entre 2018 y el 31 de diciembre de 2022. Métodos: Estudio observacional multicéntrico de pacientes estudiados consecutivamente mediante pruebas epicutáneas dentro del estudio de un posible eczema alérgico de contacto recogidos de forma prospectiva en el seno del Registro Español de Investigación en Dermatitis de Contacto y Alergia Cutánea. Se analizaron los datos mediante 2 pruebas estadísticas: una de homogeneidad (para ver si hay cambios en los diferentes años) y otra de tendencia (para ver si los cambios siguen una tendencia lineal). Resultados: Se incluyeron un total de 11.327 pacientes en el periodo de estudio. Los alérgenos en los que de forma global se detectó una sensibilización mayor fueron sulfato de níquel, metilisotiazolinona, cloruro de cobalto, metilcloroisotiazolinona/metilisotiazolinona y mezcla de fragancias i. Se detectó una disminución estadísticamente significativa en el porcentaje de positividades de metilisotiazolinona a lo largo de años de estudio con una tendencia ordenada. Conclusiones: Si bien se pueden apreciar diferentes cambios en las tendencias a sensibilizaciones a varios de los alérgenos de la batería estándar, se observa que persiste una alta sensibilización al níquel, a la metilcloroisotiazolinona/metilisotiazolinona y a la mezcla de fragancias i. Solo se aprecia una tendencia a disminuir de forma significativa en el caso de la metilisotiazolinona.(AU)


Background: The epidemiological surveillance of contact dermatitis is one of the objectives of the Spanish Registry of Research in Contact Dermatitis and Cutaneous Allergy. Knowing whether the prevalence of positive tests to the different allergens changes over time is important for this monitoring process. Objectives: To describe the various temporary trends in allergen positivity in the GEIDAC standard series from 2018 through December 31, 2022. Methods: This was a multicenter, observational trial of consecutive patients analyzed via patch tests as part of the study of possible allergic contact dermatitises collected prospectively within the Spanish Registry of Research in Contact Dermatitis and Cutaneous Allergy. The data was analyzed using 2 statistical tests: one homogeneity test (to describe the changes seen over time) and one trend test (to see whether the changes described followed a linear trend). Results: A total of 11327 patients were included in the study. Overall, the allergens associated with a highest sensitization were nickel sulfate, methylisothiazolinone, cobalt chloride, methylchloroisothiazolinone/methylisothiazolinone, and fragrance mix i. A statistically significant decrease was found in the percentage of methylisothiazolinone positive tests across the study years with an orderly trend. Conclusions: Although various changes were seen in the sensitizations trends to several allergens of the standard testing, it became obvious that a high sensitization to nickel, methylchloroisothiazolinone/methylisothiazolinone and fragrances mix i remained. Only a significant downward trend was seen for methylisothiazolinone.(AU)

Objetivos: Describir las variaciones en las tendencias temporales en positividades a alérgenos en la serie estándar del GEIDAC en el periodo comprendido entre 2018 y el 31 de diciembre de 2022. Métodos: Estudio observacional multicéntrico de pacientes estudiados consecutivamente mediante pruebas epicutáneas dentro del estudio de un posible eczema alérgico de contacto recogidos de forma prospectiva en el seno del Registro Español de Investigación en Dermatitis de Contacto y Alergia Cutánea. Se analizaron los datos mediante 2 pruebas estadísticas: una de homogeneidad (para ver si hay cambios en los diferentes años) y otra de tendencia (para ver si los cambios siguen una tendencia lineal). Resultados: Se incluyeron un total de 11.327 pacientes en el periodo de estudio. Los alérgenos en los que de forma global se detectó una sensibilización mayor fueron sulfato de níquel, metilisotiazolinona, cloruro de cobalto, metilcloroisotiazolinona/metilisotiazolinona y mezcla de fragancias i. Se detectó una disminución estadísticamente significativa en el porcentaje de positividades de metilisotiazolinona a lo largo de años de estudio con una tendencia ordenada. Conclusiones: Si bien se pueden apreciar diferentes cambios en las tendencias a sensibilizaciones a varios de los alérgenos de la batería estándar, se observa que persiste una alta sensibilización al níquel, a la metilcloroisotiazolinona/metilisotiazolinona y a la mezcla de fragancias i. Solo se aprecia una tendencia a disminuir de forma significativa en el caso de la metilisotiazolinona.(AU)

Methods: This was a multicenter, observational trial of consecutive patients analyzed via patch tests as part of the study of possible allergic contact dermatitises collected prospectively within the Spanish Registry of Research in Contact Dermatitis and Cutaneous Allergy. The data was analyzed using 2 statistical tests: one homogeneity test (to describe the changes seen over time) and one trend test (to see whether the changes described followed a linear trend). Results: A total of 11327 patients were included in the study. Overall, the allergens associated with a highest sensitization were nickel sulfate, methylisothiazolinone, cobalt chloride, methylchloroisothiazolinone/methylisothiazolinone, and fragrance mix i. A statistically significant decrease was found in the percentage of methylisothiazolinone positive tests across the study years with an orderly trend. Conclusions: Although various changes were seen in the sensitizations trends to several allergens of the standard testing, it became obvious that a high sensitization to nickel, methylchloroisothiazolinone/methylisothiazolinone and fragrances mix i remained. Only a significant downward trend was seen for methylisothiazolinone.(AU)

ABSTRACT

O câncer colorretal é desafiador devido à necessidade de atenção especializada nos serviços de saúde. Assim, objetivou-se analisar os fatores associados ao estadiamento do câncer colorretal (CCR) no estado do Maranhão, no Brasil. Trata-se de um estudo transversal, com abordagem quantitativa. Os dados foram coletados a partir do Sistema de Informação de Registro Hospitalar de Câncer, com a subsequente utilização do teste de qui-quadrado, considerando o nível de significância (p<0,05). Dos 421 casos de CCR analisados, houve predomínio de mulheres (57,8%), acima dos 60 anos (31,3%), de cor parda (68,2%), baixa escolaridade (43%) e múltiplas profissões. O adenocarcinoma foi o mais recorrente dos casos (73,5%) e, após tratamento com cirurgia e quimioterapia, houve remissão da doença em 27,2% e 37,9% dos casos, respectivamente. Houve associação do estadiamento tardio com tipo histológico adenocarcinoma (p<0,001), e os tratamentos recebidos nos casos tardios foram associados à cirurgia e quimioterapia (p<0,001). Os pacientes de casos precoces tiveram maiores possibilidades da remissão completa (p<0,000), sendo o tempo para o tratamento maior que 60 dias em casos tardios. As variáveis analisadas refletem principalmente a demora do diagnóstico, resultando na necessidade de tratamentos conjugados para efetiva possibilidade do desaparecimento da doença. Os casos analisados tiveram estadiamento tardio, o que reflete casos com diagnóstico e tratamento tardio, devido ao caráter silencioso da doença, à dificuldade de acesso dos pacientes aos serviços de saúde e à ainda recente estruturação dos fluxos de atendimento, medida que visa a reduzir a espera, identificar os casos precoces e favorecer melhores condições de tratamento para remissão da doença na população alvo.


Colorectal cancer is a challenge due to the need for specialized care in health services. Thus we aimed to analyze factors associated with colorectal cancer (CRC) staging in the state of Maranhão, Brazil. This is a cross-sectional study, with a quantitative approach. Data were collected from System Information of Hospital Cancer Registry, with subsequent use of the chi-square test, considering the significance level (p<0.05). Of the 421 cases of CRC analyzed, there was a predominance of women (57.8%), over 60 years (31.3%), mixed race (68.2%), low education (43%), and multiple professions. Adenocarcinoma was the most frequent case (73.5%) and, after treatment were treated with surgery and chemotherapy 27.2% and 37.9% of cases showed disease remission, respectively. There was an association between late staging and histological type of adenocarcinoma (p<0.001), and the treatments received in late cases were associated with surgery and chemotherapy (p<0.001). The patients with early cases had greater chances of complete remission (p<0.000), and the time to treatment was greater than 60 days in late cases. The analyzed variables mainly reflect the delay in diagnosis, requiring combined treatments for the effective possibility of healing the disease. The analyzed cases had late staging, which reflects in cases with late diagnosis and treatment, due to the silent nature of the disease, the difficulty of the patient's access to health services, and the recent structuring of service flow, a measure that aims to reduce waiting times, identify early cases and favor better treatment conditions for disease remission in the target population.


El cáncer colorrectal es un reto por la necesidad de atención especializada en los servicios sanitarios. Este trabajo buscó analizar los factores asociados con la estadificación del cáncer colorrectal (CCR) en el estado de Maranhão, Brasil. Se trata de un estudio transversal con enfoque cuantitativo. Los datos se recolectaron del Sistema de Información de Registro Hospitalario de Cáncer, en el que se utilizó la prueba de chi-cuadrado y el nivel de significancia (p<0,05). De los 421 casos de CCR evaluados, hubo predominio de mujeres (57,8%), mayores de 60 años (31,3%), de color pardo (68,2%), bajo nivel educativo (43%) y de múltiples profesiones. El adenocarcinoma fue el más común entre los casos (73,5%) y, tras el tratamiento con cirugía y quimioterapia, se produjo una remisión de la enfermedad en el 27,2% y el 37,9% de los casos. Hubo asociación entre la estadificación tardía con adenocarcinoma de tipo histológico (p<0,001), y los tratamientos que recibieron los casos tardíos se asociaron con cirugía y quimioterapia (p<0,001). Los pacientes con casos tempranos tuvieron mayores posibilidades de remisión completa (p<0,000), y el tiempo hasta el tratamiento fue superior a 60 días en los casos tardíos. Las variables analizadas reflejan principalmente el retraso en el diagnóstico, requiriendo tratamientos combinados para la posibilidad efectiva de desaparición de la enfermedad. Los casos analizados tuvieron estadificación tardía, lo que refleja casos con diagnóstico y tratamiento tardío debido al carácter silencioso de la enfermedad, el acceso de los pacientes a los servicios de salud y la reciente estructuración de los flujos de atención que apuntan a reducir la espera, identificar casos tempranos y favorecer un mejor tratamiento para la remisión de la enfermedad en la población objetivo.

ABSTRACT

The first detection of the tiger mosquito, Aedes (Stegomyia) albopictus (Skuse, 1894), in the autonomous community of Galicia (Spain) is reported. The finding has been possible thanks to the collaboration between citizens, the citizen science application Mosquito Alert and the Rede Galega de Vixilancia de Vectores (ReGaViVec). At the beginning of August 2023, a same person submitted through the app several reports consistent with the tiger mosquito in the municipality of Moaña, in Pontevedra. The ReGaViVec entomological team confirmed the species and conducted vector surveillance in the area by placing traps (11 ovitraps and 3 BG-Sentinel 2 with BG-Lure attractant) with a weekly collection frequency. This finding represents the most northwestern detection of the tiger mosquito in the Iberian Peninsula and shows the crucial role of citizen science in vector surveillance.

Fundamentos: Durante la pandemia de la COVID-19 se implementaron medidas de prevención con el propósito de reducir su transmisión comunitaria. El grado de cumplimiento con estas medidas estuvo influenciado por diversos factores sociodemográficos y ambientales. Sin embargo, existe escasa literatura científica que aborde el cumplimiento de las medidas preventivas en la población general. El objetivo de este estudio fue analizar la asociación entre factores sociodemográficos y ambientales, así como la adhesión al uso de mascarillas en contactos estrechos. Métodos: Se realizó un estudio transversal con 1.778 individuos, identificados mediante el rastreo de contactos estrechos de personas con SARS-CoV-2 por la Central de Coordinación de la COVID-19 de Mallorca, entre febrero y junio de 2021. Se realizó un análisis descriptivo y se utilizó un modelo de regresión logística para determinar los factores asociados al incumplimiento del uso de mascarillas.Resultados: La edad media de los participantes fue de 42,8±17,4 años (53,6% de mujeres). El 60,8% (IC 95%: 57,8-62,3) de los contactos estrechos no utilizó mascarilla durante su contacto. No se observaron diferencias estadísticamente significativas en función del sexo o grupo etario (p=0,497 y p=0,536, respectivamente). Las situaciones de mayor incumplimiento con el uso de mascarillas se dieron en el entorno domiciliario, espacios cerrados sin ventilación y al mantener distancias cortas (p<0,001). Conclusiones: Nuestros hallazgos indican una menor adhesión al uso de mascarillas en escenarios de mayor riesgo. Frente a futuras situaciones de crisis sanitaria, se deberían diseñar intervenciones que realcen la conciencia sobre los riesgos y que promuevan una mayor adhesión a medidas de prevención y control.(AU)


Background: During COVID-19 pandemic, prevention measures were implemented to mitigate the community transmission of SARS-CoV-2. Compliance with these measures was influenced by several sociodemographic and environmental factors. However, literature addressing compliance with these prevention measures among the general population remains limited. The study aimed to assess the association of sociodemographic and environmental factors and mask usage during close contact situations. Methods: A cross-sectional study was conducted with a sample of 1,778 individuals identified through close contact tracing of indivi-duals diagnosed with SARS-CoV-2 by the COVID-19 Coordination Center of Mallorca, from February to June 2021. A descriptive analysis was conducted, and a logistic regression model was utilized to evaluate factors associated with mask non-compliance. Results: The mean age of the participants was 42.8±17.4 years, with 53.6% being female. Among close contacts, 60.8% (95% CI: 57.8-62.3) did not use masks during their contact. No significant differences were observed between genders or across age groups (p=0.497 and p=0.536, respectively). Factors linked to mask non-compliance included the home setting, indoor spaces without venti-lation, and closer physical distances (p<0.001). Conclusions: Our findings indicate that mask adherence was notably lower among close contacts exposed to higher risk. In future public health crises, interventions should be developed to raise awareness about risks and promote adherence to preventive and control measures.(AU)

BACKGROUND: The indicators of the pandemic have been based on the total number of diagnosed cases of COVID-19, the number of people hospitalized or in intensive care units, and deaths from the infection. The aim of this study is to describe the available data on diagnostic tests, health service used for the diagnosis of COVID-19, case detection and monitoring. METHOD: Descriptive study with review of official data available on the websites of the Spanish health councils corresponding to 17 Autonomous Communities, 2 Autonomous cities and the Ministry of Health. The variables collected refer to contact tracing, technics for diagnosis, use of health services and follow-up. RESULTS: All regions of Spain show data on diagnosed cases of COVID-19 and deaths. Hospitalized cases and intensive care admissions are shown in all regions except the Balearic Islands. Diagnostic tests for COVID-19 have been registered in all regions except Madrid region and Extremadura, with scarcely information on what type of test has been performed (present in 7 CCAA), requesting service and study of contacts. CONCLUSIONS: The information available on the official websites of the Health Departments of the different regions of Spain are heterogeneous. Data from the use of health service or workload in Primary Care, Emergency department or Out of hours services are almost non-existent.

The first part of the book "Health Situation Analysis" in Tocantins provides a comprehensive overview of various aspects of public health in the state. ABSTRACT Objective: This study aimed to assess the consistency of data regarding the provision of fluoridation in Brazilian municipalities with water supply systems. Methods: Official data from the National Basic Sanitation Survey and the National Information System on Sanitation for 2017 were compared. Results: Out of 5,570 municipalities in Brazil, 4,546 (81.6%) had water supply systems. The agreement between data sources was 84%, with a Kappa of 0.668, indicating substantial agreement. However, the estimates of fluoridation provision exhibited an average discrepancy of 8.1 percentage points, ranging from 1.2 points in the Central-West region to 21.4 points in the Northeast region. Conclusion: To address these inconsistencies, it is essential to enhance information sources, ensuring more reliable data for health, sanitation authorities, and society at large.


RESUMO Objetivo: Avaliar a consistência dos dados sobre a provisão de fluoretação em municípios brasileiros com sistemas de abastecimento de água. Métodos: Foram comparados os dados da Pesquisa Nacional de Saneamento Básico e do Sistema Nacional de Informações sobre Saneamento referentes a 2017. Resultados: De 5.570 municípios no Brasil, 4.546 (81,6%) possuíam sistemas de água. A concordância entre as fontes foi de 84%, com Kappa de 0,668, indicando substancial concordância. Entretanto, as estimativas de provisão divergiram em média 8,1 pontos percentuais, variando de 1,2 ponto na região Centro-Oeste a 21,4 pontos na região Nordeste. Conclusão: Para superar essa inconsistência, é fundamental aprimorar as fontes de informação, garantindo dados mais confiáveis para as autoridades de saúde, saneamento e para a sociedade em geral.
