ABSTRACT

Introducción La pandemia derivada de la infección por SARS-CoV-2 propició cambios en los cuidados tanto a familiares como a pacientes de cuidados intensivos durante las diferentes olas de incidencia del virus. La línea de humanización seguida por la mayoría de los hospitales se vio gravemente afectada por las restricciones aplicadas. Como objetivo, planteamos conocer las modificaciones experimentadas durante las diferentes olas de la pandemia por SARS-CoV-2 en España respecto a la política de visitas a los pacientes en UCI, el acompañamiento al final de la vida, y el uso de las nuevas tecnologías de la comunicación entre familiares, pacientes y profesionales. Métodos Estudio descriptivo transversal multicéntrico mediante encuesta a las UCI españolas desde febrero a abril de 2022. Se realizaron métodos de análisis estadísticos a los resultados según lo apropiado. El estudio fue avalado por la Sociedad Española de Enfermería Intensiva y Unidades Coronarias. Resultados Respondieron un 29% de las unidades contactadas. Los minutos de visita diarios de los familiares se redujeron drásticamente de 135 (87,5-255) a 45 (25-60) en el 21,2% de las unidades que permitían su acceso, mejorando levemente con el paso de las olas. En el caso de duelo, la permisividad fue mayor, aumentando el uso de las nuevas tecnologías para la comunicación paciente-familia en el caso del 96,5% de las unidades. Conclusiones Las familias de los pacientes ingresados en UCI durante las diferentes olas de la pandemia por COVID-19 han experimentado restricciones en las visitas y cambio de la presencialidad por técnicas virtuales de comunicación. Los tiempos de acceso se redujeron a niveles mínimos durante la primera ola, recuperándose con el avance de la pandemia pero sin llegar nunca a los niveles iniciales... (AU)

The line of humanization followed by the majority of the hospitals was seriously affected by the restrictions applied. As an objective, we propose to know the modifications suffered during the different waves of the SARS-CoV-2 pandemic in Spain regarding the policy of visits to patients in the ICU, monitoring at the end of life, and the use of new technologies of communication between family members, patients and professionals. Methods Multicenter cross-sectional descriptive study through a survey of Spanish ICUs from February to April 2022. Statistical analysis methods were performed on the results as appropriate. The study was endorsed by the Spanish Society of Intensive Nursing and Coronary Units. Results Twenty-nine percent of the units contacted responded. The daily visiting minutes of relatives dropped drastically from 135 (87.5-255) to 45 (25-60) in the 21.2% of units that allowed their access, improving slightly with the passing of the waves. In the case of bereavement, the permissiveness was greater, increasing the use of new technologies for patient-family communication in the case of 96.5% of the units. Conclusions The family of patients admitted to the ICU during the different waves of the COVID-19 pandemic have suffered restrictions on visits and a change from face-to-face to virtual communication techniques. Access times were reduced to minimum levels during the first wave, recovering with the advance of the pandemic but never reaching initial levels. Despite the implemented solutions and virtual communication, efforts should be directed towards improving the protocols for the humanization of healthcare that allow caring for families and patients whatever the healthcare context. ABSTRACT

Objetivo: presentar un plan de decisiones y acciones de enfermería basado en un enfoque transcultural a través del Modelo del Sol Naciente propuesto por Madeleine Leininger. Síntesis de contenido: investigación cualitativa tipo reporte de caso, en la que participó una familia residente del área rural del municipio de Villamaría (Caldas, Colombia) entre noviembre de 2017 y septiembre de 2018. Se realizaron entrevistas semiestructuradas sobre condiciones de cuidado individual-familiar, determinantes sociales y riesgo familiar, y se aplicaron instrumentos de valoración de la familia. La información se analizó mediante la técnica de Bardin, previa sistematización en el software ATLAS.ti. Se presenta un plan de decisiones y acciones de enfermería a la luz de una valoración coherente con la cultura, resultado de la conjunción de los sistemas populares y profesionales. Se valoraron siete factores a partir del Modelo del Sol Naciente (tecnológicos, religiosos, sociales, culturales, políticos, económicos y educativos) y se seleccionó el fenómeno "sobrecarga del cuidador familiar del adulto mayor". Conclusiones: la propuesta de abordaje otorga herramientas de análisis al profesional de enfermería para brindar cuidado según necesidades, valores, creencias y modos de vida de las personas.

Realizaram-se entrevistas semiestruturadas sobre condições de cuidado individual-familiar, determinantes sociais e risco familiar, utilizaram-se também instrumentos de valoração da familia. A informação se analisou pela técnica de Bardin, com prévia sistematização no software ATLAS.ti. Apresenta-se um plano de decisões e ações de enfermagem à luz de uma valoração consistente com a cultura, resultado da conjunção dos sistemas populares e profissionais. Avaliaram-se sete fatores a partir do Modelo do Sol Nascente (tecnológicos, religiosos, sociais, culturais, políticos, econômicos, educativos) e se selecionou o fenômeno "sobrecarga do cuidador familiar do idoso". Conclusões: a proposta de abordagem outorga ao profissional de enfermagem ferramentas de análise para oferecer cuidado segundo necessidades, valores, crenças e modos de vida das pessoas.


Objective: To introduce a nursing decision-and-action plan based on a cross-cultural approach through the Rising Sun Model created by Madeleine Leininger. Content synthesis: Qualitative research under the case report method that involved the participation of a rural family from the municipality of Villamaría (Caldas, Colombia), from November 2017 to September 2018. Semi-structured interviews were conducted on individual-family care conditions, social determinants, and family risk. Family assessment instruments were also applied. The information was analyzed using the Bardin technique, after its systematization in ATLAS.ti software. A plan for nursing decisions and actions, resulting from the conjunction of popular and professional systems, is presented in the light of an assessment consistent with culture. Seven factors were evaluated based on the Rising Sun Model (technological, religious, social, cultural, political, economic, educational), selecting the phenomenon known as "overload of elderly family caregivers." Conclusions: The suggested approach gives nursing professionals analytical tools in order to provide care according to people's needs, values, beliefs, and lifestyles.

ABSTRACT

A visita domiciliar constitui o principal instrumento de trabalho dos Agentes Comunitários de Saúde (ACS) na Estratégia Saúde da Família. O presente estudo aborda a temática capacitação dos agentes comunitários de saúde para atuarem na visita domiciliar no Programa Saúde da Família Casa Branca em Brumadinho. Utiliza-se como metodologia a revisão de literatura que subsidiou a elaboração de uma proposta de intervenção por meio do método de Planejamento Estratégico Situacional. Seu objetivo foi elaborar uma proposta de qualificação da visita domiciliar para o ACS. Acredita-se que com essa medida será possível otimizar, qualitativamente a realização da atividade.

ABSTRACT

A visita domiciliar é uma tecnologia disponibilizada como recurso para a Estratégia de Saúde de Família (ESF) dentro das Unidades Básicas de Saúde, como metodologia para alcançar e conhecer bem os usuários em seu entorno ambiental, familiar, e pessoal, e com isso fornecer e garantir a saúde como um direito universal consolidado. A população do bairro Tupi - Lajedo permaneceu sem atendimento medico por quase um ano, associam-se a este quadro as dificuldades geográficas e limitações físicas dos usuários e acamados, que apresentam severas dificuldades para atingir o acesso aos serviços básicos de saúde. Com a ampliação do acesso através do Programa Mais Médicos, iniciou-se com um reforço significativo o número de visitas domiciliares, com atendimentos e o seguimento evolutivo intradomiciliar dos usuários e suas doenças, fornecendo o devido acompanhamento à aqueles que tiveram ou foram considerados ter, uma condição de saúde mais vulnerável. Previamente realizou-se uma análise situacional, a criação da estratégia da otimização das visitas domiciliares e o plano de ação do trabalho, além do planejamento para a abordagem e posterior avaliação dos casos clínicos, incluindo o processo educativo tanto para usuários, quanto para familiares cuidadores e Agentes Comunitários de Saúde (ACS) pertencentes a ESF.
