ABSTRACT

ObjectiveTo evaluate the effect of Astragali Radix (AR)-Angelicae Sinensis Radix (ASR) drug pair on supplementing Qi and activating blood circulation in rats with Qi deficiency and blood stasis and provide a theoretical basis for clinical rational medication and identification and quality control of compound pharmacodynamic substances from the three aspects of characteristic map, identification of pharmacodynamic substances, and comparison of blood components. MethodHigh-performance liquid chromatography (HPLC) was employed to establish the fingerprint of AR∶ASR (3∶1), and ultra-high performance liquid chromatography-Q-Exactive Orbitrap-mass spectrometry (UPLC-Q-Exactive Orbitrap-MS) was employed to analyze the ingredients of the decoction. Adult male Wistar rats with SPF grades were selected and randomly divided into a blank group, a model group, a 3∶1 group, and a 5∶1 group. The rat model of Qi deficiency and blood stasis syndrome was prepared by controlling food intake and swimming in cold water every day. In parallel, each group was given medicine (or water) once a day. The dose of drug groups was 10.2 g∙kg-1, and the model group and blank group were given the same amount of distilled water for 15 d. Animal behavior, body weight, whole blood and plasma viscosity, thymus index, spleen index, the levels of adenosine triphosphate (ATP), adenosine diphosphate(ADP), von willebrand factor (vWF), and ATP/ADP value in serum of rats were recorded. The morphology of vascular endothelium was observed by hematoxylin-eosin (HE) staining and scanning electron microscopy. UPLC-Q-Exactive Orbitrap-MS was used to analyze prototype and metabolic components in serum. ResultThe fingerprint of AR-ASR drug pair (AR-ASR 3∶1) was established. UPLC-Q-Exactive Orbitrap MS identified 49 chemical components in vitro and preliminarily identified 11 prototype components absorbed into blood in vivo. As compared with the blank group, the body mass decreased significantly (P<0.01), the whole blood (high shear, middle shear, and low shear) viscosity and plasma viscosity were significantly increased (P<0.05, P<0.01), the thymus index and spleen index decreased significantly (P<0.05, P<0.01), serum ATP content decreased significantly (P<0.01), ADP content increased significantly (P<0.01), ATP/ADP value decreased significantly (P<0.01), and vWF content increased significantly (P<0.01). The results of HE staining and scanning electron microscopy showed that the vessels were partially damaged, showing the structural disorder of the intima, the bulge, defect, and roughness of the endothelium, and the obvious cell adhesion and migration in the model group. As compared with the model group, the body mass also increased significantly (P<0.01). The results of whole blood and plasma viscosity showed that the whole blood low shear viscosity was significantly decreased in the 3∶1 group (P<0.05). The results of thymus index and spleen index showed that 5∶1 group significantly increased the thymus index of rats (P<0.05). The results of serum ATP and ADP levels showed that the 5∶1 group had more significant effects on ATP and ADP levels (P<0.05), and both groups significantly reduced ATP/ADP values (P<0.01). The results of serum vWF level showed that the vWF content in the 3∶1 group decreased significantly (P<0.05). The results of HE staining and scanning electronic microscopy showed that the damage of vascular endothelium was improved in the treatment group and the structure of intima was neat. ConclusionAR-ASR drug pair can improve the macro and micro indexes of rats with qi deficiency and blood stasis in the 3∶1 and 5∶1 groups. Overall, the 5∶1 ratio has a better effect on supplementing Qi but 3∶1 ratio has a better effect on promoting blood circulation.

Animal medicine, as an important part of natural medicines, has the characteristics of wide resources, complex chemical components, and broad pharmacological effects. It has been extensively used in the field of anti-osteoporosis. This article summarizes the pharmacological effects and applications of several major animal medicines for osteoporosis, and discusses the existing problems, aiming to provide a reference for the development of animal drugs against osteoporosis.

ABSTRACT

ObjectiveTo explore the distribution and characteristics of traditional Chinese medicine (TCM) syndromes in patients with endometriosis (EMS). MethodA cross-sectional survey was conducted to analyze the characteristics of TCM syndromes in 1 895 cases of EMS in hospitals of 19 provinces, autonomous regions, and municipalities. ResultAmong the 1 895 patients, Qistagnation and blood stasis syndrome accounted for the highest proportion, followed by Qi deficiency and blood stasis syndrome, cold coagulation and blood stasis syndrome, and kidney deficiency and blood stasis syndrome. The distribution of TCM syndrome types of patients with EMS in different regions, different treatment stages, and different disease types and with different therapeutic goals was different, and the differences were statistically significant. However, under different conditions, the Qi stagnation and blood stasis syndrome accounted for the highest proportion. Under the stratification of different regions, the cold coagulation and blood stasis syndrome in north China was relatively high, the kidney deficiency and blood stasis syndrome in south China was relatively high, and the combined phlegm and stasis syndrome was relatively high in southwest China. Different diagnosis and treatment goals corresponded to different clinical syndromes. With pelvic pain as the main manifestation, the proportion of cold coagulation and blood stasis syndrome was higher. The proportion of kidney deficiency and blood stasis syndrome was higher in married patients with infertility. Patients with the main diagnosis and treatment goals of controlling mass and inhibiting recurrence had a higher proportion of Qi deficiency and blood stasis syndrome. In different treatment stages, the proportion of Qi deficiency and blood stasis syndrome in postoperative patients and those with recurrent EMS was higher. Among different disease types, the Qi deficiency and blood stasis syndrome accounted for a higher proportion in patients with ovarian endometriosis (OEM). The cold coagulation and blood stasis syndrome accounted for a higher proportion in patients with deep invasive endometriosis (DIE). The kidney deficiency and blood stasis syndrome accounted for a higher proportion in patients with peritoneal EMS. There were significant differences in age, body mass index (BMI), and course of disease among patients with different syndromes. Patients with Qi deficiency and blood stasis syndrome was relatively older, and their course of the disease was relatively long. Patients with combined phlegm and stasis syndrome had relatively high BMI. There was no significant difference in CA125 and CA199 levels among all syndrome types. ConclusionThe distribution of TCM syndromes of EMS has a certain regularity, and there are differences in regional distribution, therapeutic goals, treatment stages, and disease types(P<0.05). However, the Qi stagnation and blood stasis syndrome accounts for a large proportion under different conditions, suggesting that Qi stagnation is the key link of EMS. In the early stage, the team took relieving depression and activating blood as the primary treatment, and created Huoxue Xiaoyi prescription, which was the core prescription for the treatment of EMS with Qi stagnation and blood stasis syndrome, achieving good clinical effect. At the same time, it is emphasized that EMS treatment should be integrated into the concept of chronic disease management and combined with health management. Through psychological counseling, cognitive behavior intervention, popular science lectures, and other methods, it is advised to adjust the emotion of patients with EMS, thereby increasing the curative effect. This study is expected to provide references for the clinical treatment of EMS.

ABSTRACT

OBJECTIVE: To evaluate the prognosis effect of Chinese herbal medicines (CHMs) for benefiting qi and activating blood circulation adjunctive to conventional treatment in patients with acute coronary syndrome (ACS) after percutaneous coronary intervention (PCI). METHODS: A total of 702 patients with ACS who underwent PCI were enrolled and randomly assigned to receive conventional treatment plus CHMs for benefiting qi and activating blood circulation (treatment group, 351 cases) or conventional treatment alone (control group, 351 cases) for 6 months. Six months later, all patients received conventional treatment alone. Follow-ups were scheduled at 6th, 12th, 18th, 24th month after enrollment in April 2008, and the final follow-up visit was during September 2011 and November 2011. The primary endpoint was the composite of cardiac death, nonfatal myocardial infarction or revascularization (PCI or coronary artery bypass grafting); and the secondary endpoint was the composite of re-admission for ACS, congestive heart failure, nonfatal stroke or other thrombus events. RESULTS: A total of 621 (88.59%) patients completed 35.4±3.8 months follow-up, while 80 (11.41%) patients withdrew from the trial (41 in the treatment group and 39 in the control group). The incidence of primary endpoint was 5.7% (20 patients) in the treatment group versus 10.86% (38 patients) in the control group [relative risk (RR): 0.53; 95% confidence interval (CI): 0.30, 0.88; P=0.013; absolute risk reduction (ARR):-0.052, 95% CI: -0.06, 0.01]. The incidence of secondary endpoint was 5.98% (21 patients) in the treatment group versus 10.28% (36 patients) in control group (RR: 0.58, 95% CI: 0.33, 0.97, P=0.037; ARR: -0.043, 95% CI: 0.06, 0.01). Most of the primary and secondary endpoints were occurred in 18 months (84.50% in the treatment group versus 78.10% in the control group). CONCLUSION: CHMs for benefiting qi and activating blood circulation adjunctive to conventional treatment improved clinical outcomes for patients with ACS after PCI in long-term follow-up.

ABSTRACT

The biological potency assay and chemical fingerprint chromatogram were applied to quality evaluation of rhubarb. Using the biological potency as indicators, we evaluated the differences in quality of multiple batches of rhubarbs and related products. Using the platelet aggregation analyzer, we determined platelet aggregation rate in the different rhubarbs preparations, and calculated the biological potency based on the simplified probit principle. UPLC was adopted to establish the fingerprint spectra for rhubarbs. The spectral efficiency correlation analysis between chromatograms and biological potencies were conducted using the double variables of SPSS 22.0 software. We used three chemical composition to verify the potency. The biological potency results suggest that Rheum palmatum has a more potent activity than Rheum tanguticum, and wine-treated rhubarb had a higher potentcy than charred. We identified 10 elements in the Fingerprint Spectrum. The relevant elements including rhein-8-O-β-D-glucoside, emodin-8-O-β-D-glucoside and rhein have the strongest activity in the inhibition of platelet aggregation. In conclusion, this study provides a analytical method for rhubarb biological potency based on determination of the maximum antagonism rate model. The rhein may be the effective substance. It may serve as a reference in the quality control of wine processed rhubarb products.

ABSTRACT

Pain is an unpleasant sensory and emotional experience associated with various diseases. Extensive research has been conducted to find appropriate methods of relieving pain and improving the quality of life. However, the most commonly used pain-relieving agents such as opioid therapeutics are often associated with harmful side effects; moreover, users are prone to become addicted to these agents and may develop tolerance. Often, nonopioid therapeutics is only marginally effective, thus leading to a significant unmet medical need. Scientists have studied herbal medicines, finding more than 800 kinds of traditional Chinese medicine (TCM) to be effective in relieving pain while also creating several monomeric compounds to develop novel analgesic drugs. In this review, we summarize the representative TCM currently available for the treatment and modulation of pain. Ten different natural products, mainly herbs, used in Chinese medicine to relieve pain are discussed in light of the theories of TCM and modern pharmacology. We hope that this review will provide valuable information for future studies on the potential of TCM in alleviating pain.

Objective] To explore the theoretical origin, clinical application method, effect and practical significance of the treatment of ectopic pregnancy with blood stasis. [Method] From blood stasis on the treatment by promoting blood circulation to remove blood stasis, Yiqi Yangxue Huayu Shapei, expelling blood circulation, method to eliminate the disease, stage of treatment, a reasonable choice of promoting blood circulation and removing blood stasis drugs in the treatment of ectopic pregnancy. ABSTRACT

Objective] With the method of removing wind, activating blood, removing phlegm and relieving cough, apply Bee-silkworm Zhike Decoction, observe its cure effect on chronic cough. [Method] For 32 cases of chronic cough that had no marked cure effect with WM anti-infection and relieving cough to remove phlegm, apply Bee-silkworm Zhike Decoction to them, for 2w as a course, make evaluation after successive 2 courses. [Result] The average onset time was (9.1 ±4.6)d,the cough average disappearance period (20.3 ±9.1)d,28 cases were cured, 4 had effect, the total effective rate was 100%. [Conclusion] Bee-silkworm Zhike Decoction treating chronic cough has characteristics of quick onset, short course and marked cure effect, worth further study.

On this base, professor forms Zhang Peiying's treatment principle focusing on relieving both the secondary and primary symptoms, such as replenishing Yang-Qi,and warming up the heart yang and dispel ing cold ,removing blood stasis.Taking an example for the therapy to prove it.[Conclusions]Professor Zhang Peiying's scholar thought and clinical experience in diagnosing and treating coronary heart disease is effective and is trusted and welcomed by patients. His thought and clinical expe-rience are worth to extend and apply.

ABSTRACT

[Objective]To observe the cure effect of Mifepristone(RU486) joined with TCD on tubal pregnancy. [Method] Randomly divide the said 134 patients into 2 groups, the control group 66 cases take MTX 50mg/㎡ for one-time muscular injection; the study group 68 Mifepristone 100mg/time, 2times/d, for successive 3 days, meanwhile added with TCD. Periodically measure both groups’ ?-HCG, test the mass reduction with B ultrasonic exam. [Result] For the study group, the cure rate was 91.2%; 83.3% for the control one, without statistical meaning; the ?-HCG negative rate and mass reduction rate were better in study group than the control one, with marked difference; moreover, the study group had less side effect, less in-hospital time. So, Mifepristone combined with TCD is an effective method for preserved treatment of tubal pregnancy.

Objective To observe the effect of Yiqi Fuzhi Granule on activating blood circulation and removing blood stasis in rat models of multiple infarctional dementia.Methods Internal carotid artery injection method was used to establish rat models of multiple infarctional dementia.The changes of blood rheology,blood platelet aggregation,thromboplastin time,plasmozyme time and kaolin partial thromboplastin time before and after operation were observed.Results Yiqi Fuzhi Granule could markedly improve blood rheology in rat models of multiple infarctional dementia and reduce blood viscosity and blood platelet aggregati on rate,the effect being obvious before operation.Conclusion Yiqi Fuzhi Granule has a good action of activating blood circulation and removing blood stasis.

Objective To investigate the mechanism of Bushen Huoxue Recipe (Recipe to reinforce the kidney and activate blood) in the treatment of glomerulosclerosis of the rats with mesangial proliferative glomerulonephritis (MsPGN).Methods Fifty-four SD male rats were randomized into normal group, model group, and treated group with 18 rats in each. The MsPGN models were made by injection of anti-rat-thymocyte antiserum (ATS) through the tail vein. After modeling, the treated group was administered Bushen Huoxue Recipe with a dosage of 5ml/kg, once a day. Six rats from each group were selected in the second, fourth, and eighth week respectively to determine the content of urine protein (Upro), blood urea nitrogen (BUN), and serum creatinine (SCr), and the kidney was cut for pathological observation and immunohistochemical detection of the kidney tissues, the expression of Ⅳ-type collagen (Col-Ⅳ) and laminin (LN).Results The pathological determination showed that, at the same time points, the hyperplasia of mesangial cells in the treated group was significantly relieved and mesangial matrix reduced as compared with the model group.
