ABSTRACT

A co-delivery system based on multilayer microparticles was developed and characterized for the sequential release of phenolic compounds (PCs) using different encapsulation processes (spray drying: SD and drying-chilling spray: SDC) and wall materials to improve the stability and bioavailability of PCs. Samples were characterized in terms of process yield (PY%), phenolic retention efficiency (PRE%), chemical structure and crystallinity (NMR, FTIR, DXR), thermal stability (DSC and FT-IR), anti-radical capacity (ORAC and ABTS) and in vitro digestion. PRE% of samples by SD were higher (p < 0.05) than SDC due to the formation of PCs from CRF (cará-roxo flour). NMR, FTIR, DXR confirmed the presence of key components and interactions for the formation of the advanced co-delivery system. The SDC particles showed crystalline regions by XRD and were stable at ∼47 °C. All samples showed good release of PC in the intestinal phase, and antiradical capacity that reached 23.66 µmol TE g-1.

ABSTRACT

PURPOSE: The purpose of this field report is to describe an evaluation of the fidelity with which the comprehensive addiction and recovery act (CARA) policy has been implemented in New Mexico. DESCRIPTION: The CARA program in New Mexico focuses on providing nonpunitive supportive care for pregnant people affected by substance use and on coordinating services for parents, caregivers, and family members of newborns affected by substance exposure. The evaluation used information from program reports, a family follow-up survey, the plan of safe care database, and a data linkage between CARA participant records with Medicaid claims data. RESULTS: Follow-up survey data substantiated the program reports. Both sources showed that families were not engaged consistently in developing or receiving information about plans of safe care. The survey answers also indicated that the time-period immediately after the delivery of a baby is not the best time to communicate the contents of the plan of safe care to families. Additionally, the survey found that respondents believed that medical staff judged them for using substances during pregnancy. The Medicaid data linkage showed that 40.3% of families of infants exposed to substances in-utero did not receive a plan of safe care. Program reports revealed that limited resources existed for implementing CARA. CONCLUSION: Program funding, limited system capacity, lack of systematic screening for prenatal substance use, regional differences in access to care, and provider biases toward pregnant people using substances affected health-care workers' ability to identify at-risk families and develop plans of safe care. To support CARA implementation, healthcare systems must implement universal prenatal substance use screening, increase the level of anti-bias training pertaining to substance use, increase hospital supports, and improve data management systems.


What is already known on this subject? Stigma towards pregnant people for maternal substance use reduces access to prenatal care due to fear of involvement from child protective services. Punitive approaches to perinatal substance use result in lower healthcare utilization and opportunities to treat substance use disorder. What this study adds? This field report evaluates the implementation of a non-punitive state level policy meant to increase services for pregnant people using substances and reduce reports to child protective services for abuse neglect due to infants exposed to substances prenatally.

RESUMEN Objetivo : Determinar las diferencias en la percepción estética del canteamiento del plano incisal (CPI), de la desviación de la línea media (DLM) y de la angulación de la línea media (ALM) dentaria maxilar entre personas naturales, estudiantes de pregrado y de posgrado de una universidad peruana. Material y métodos : Se empleó la fotografía de un sujeto, la cual fue digitalmente manipulada para crear un modelo facial simétrico (MFS). A partir de este modelo se hicieron modificaciones a nivel del plano incisal y de la línea media dentaria, realizando desviaciones y angulaciones con diferentes valores. Se recolectó la información mediante una encuesta virtual a 60 personas naturales, 30 estudiantes de pregrado y 30 de posgrado, sobre su preferencia estética. Resultados : Se encontró que las imágenes calificadas por la mayoría de cada uno de los tres grupos como «muy poco estético¼ fueron la DLM de 3mm, el CPI de 3°, la ALM de 7°, y la imagen con dos variables combinadas de DLM 3mm y ALM 7°. Conclusión : El aumento de CPI, DLM y ALM disminuye el atractivo de la sonrisa.


ABSTRACT Objective : The aim of this study is to identify differences in aesthetic perception regarding incisal plane edging (IPC), midline deviation (MLD), and maxillary dental midline angulation (MLA) among natural individuals, undergraduate students, and graduate students at a Peruvian university. Material and methods : A photograph of a subject was digitally manipulated to generate a symmetrical facial model (SFM). Subsequently, alterations were introduced to the incisal plane and dental midline, incorporating variations in deviations and angulations with distinct values. A virtual survey was administered to 60 natural individuals, 30 undergraduate students, and 30 graduate students, gathering information regarding their aesthetic preferences. Results : The analysis revealed that images which were consistently rated by the majority within each group as "very unaesthetic" included the following: dental midline deviation (DLM) of 3 mm, midline deviation (MLD) of 3°, and maxillary dental midline angulation (ALM) of 7°. Similarly, the combined image involving a DLM of 3 mm and an ALM of 7° was also perceived as very unaesthetic. Conclusion : The study demonstrates that an increase in CPI, DLM, and ALM leads to a reduction in the perceived attractiveness of a smile.


RESUMO Objetivo : Determinar as diferenças na percepção estética da borda do plano incisal (CPI), do desvio da linha média (DLM) e da angulação da linha média dentária maxilar (ALM) entre pessoas com dentição natural, estudantes de graduação e pós-graduação em uma universidade peruana. Material e métodos : Foi utilizada uma fotografia de um indivíduo, que foi digitalmente manipulada para criar um modelo facial simétrico (MFS). Com base nesse modelo, foram realizadas modificações nos níveis do plano incisal e na linha média dentária, introduzindo desvios e angulações com diferentes valores. Os dados foram coletados por meio de um questionário virtual respondido por 60 pessoas com dentição natural, 30 estudantes de graduação e 30 estudantes de pós-graduação, sobre suas preferências estéticas. Resultados : Foi constatado que as imagens classificadas pela maioria dos três grupos como "muito inestéticas" foram aquelas com um DLM de 3 mm, um CPI de 3°, uma ALM de 7° e a imagem contendo duas variáveis combinadas, ou seja, um DLM de 3 mm e uma ALM de 7°. Conclusão : O aumento do CPI, DLM e ALM diminui a atratividade do sorriso, conforme percebido pelos participantes do estudo, que incluíram pessoas com dentição natural, estudantes de graduação e pós-graduação.

ABSTRACT

Periodontal disease in ruminants is common and occurs in farmed and wild animals. Periodontal lesions can result from the secretion of endotoxins by pathogenic bacteria and as consequences of immune system activity. Three main types of periodontitis have been described. The first is chronic inflammation involving mainly premolars and molars-periodontitis (PD). The second type is an acute inflammatory reaction occurring with calcification of the periosteum of the jawbone and swelling of the surrounding soft tissues (Cara inchada, CI-"swollen face"). Finally, a third type, similar to the first but located in the incisor area, is called "broken mouth" (BM). Etiological variation between the different types of periodontitis is indicated. This particularly manifests in the composition of the microbiome, which is characteristic of the different forms of periodontitis. The widespread detection of lesions has drawn attention to the current nature of the problem.

ABSTRACT

Single parent adoption (SPA) is a relatively new construct worldwide and in India. The Ministry of Women and Child Development, Government of India, has laid down criteria for adoption in general and SPA in particular, in conjunction with the Juvenile Justice Act (Care and Protection of Children), 2015. There is scant literature on this topic of SPA, more so in India, that looks into the various psychological nuances of SPA from a mental health professional's (MHP) perspective. This review paper aims to assess SPA from the perspective of a MHP that will focus on its various legal nuances as well as the psychological connotations attached to it. For this, a search strategy was employed that included a thorough literature search from two databases (PubMed and Google Scholar) with relevant keywords related to the topic. The various legal issues pertaining to SPA in the current scenario, the psychological issues and challenges faced by single parents, the behavioral outcomes of adoptees who are adopted by single parents, and ways to deal with the various obstacles of SPA are discussed.

Introdução:Mulheres têm sido subjugadas pela violência em prol da identidade machista e domínio viril do homem. Objetivo: analisar os atendimentos de emergência em mulheres vítimas de violência na região de cabeça e pescoço. Metodologia:Análise de dados do inquérito de Vigilância e Acidentes em 25 capitais e no Distrito Federal no ano de 2011 por meio de amostra de 351 atendimentos de mulheres entre 15 e 49 anos em relação à ocorrência de violência na região de cabeça e pescoço. Resultados:A ocorrência da violência foi predominante em mulheres pretas (70,8%),jovens (68,3% 20-29 anos)e com pouca escolaridade (< 8 anos de estudo), sendo mais frequentes no domicílio (45,5%), cometida de forma intencional (86,9%) por um agressor do sexo masculino (77,4%), identificado como parceiro ou ex-parceiro íntimo (44,5%), sendo aface (87,0%) a região de cabeça/pescoço mais atingida. Conclusões:A violência contra a mulher perpassa a discussão das desigualdades de gênero, interseccionalidade e capital social. O importante não é o gênero em si, mas a junção do gênero, raça e classe socioeconômica. Isto requer políticas públicas intersetoriais e mobilização social em prol do capital social. Para tanto, o modelo de Redes de Atenção, com a consequente multiprofissionalidade, intersetorialidade e transdisciplinaridadeé fundamental. Isto pressupõe, inclusive, a maior inserção do cirurgião-dentista na identificação e acolhimento das vítimas para posteriores encaminhamentos (AU).


Introduction:Women have been subjugated by violence in favor of the male identity and virile dominance of men. Objective:to analyze emergency care for women victims of violence in the head and neck region. Methodology:Analysis of data from the Surveillance and Accident Survey in 25 state capitals and the Federal District in 2011 througha sample of 351 visits to women between 15 and 49 years old regarding the occurrence of violence in the head and neck region. Results:The occurrence of violence was predominant in black women (70.8%), young (68.3% 20-29 years) and with little schooling (< 8 years of study), being more frequent at home (45.5 %), committed intentionally (86.9%) by a male aggressor (77.4%), identified as a partner or former intimate partner (44.5%), with the face (87.0%) the most affected head/neck region. Conclusions:Violence against women permeates the discussion of gender inequalities, intersectionality and social capital. What is important is not gender per se, but the combination of gender, race and socioeconomic class. This requires intersectoral public policies and social mobilization in favor of social capital. Therefore, the model of Care Networks, with the consequent multiprofessionality, intersectoriality and transdisciplinarity is fundamental. This presupposes, including, greater involvement of the dental surgeon in the identification and reception of victims for subsequent referrals (AU).

Metodología:Análisis de datos de la Encuesta de Vigilancia y Accidentalidad en 25 capitales de estado y Distrito Federal en 2011 a través de una muestra de 351 visitas a mujeres entre 15 y 49 años sobre la ocurrencia de violencia en la región de cabeza y cuello. Resultados: Predominó la ocurrencia de violencia en mujeres negras (70,8 %), jóvenes (68,3 % 20-29 años) y con poca educación(< 8 años de estudio), siendo más frecuente en el hogar (45,5 %), cometida intencionalmente (86,9%) por un agresor masculino (77,4%), identificado como pareja o ex pareja íntima (44,5%), siendo el rostro (87,0%) la región de cabeza/cuello más afectada. Conclusiones:La violencia contra las mujeres permea la discusión sobre las desigualdades de género, la interseccionalidad y el capital social. Lo importante no es el género per se, sino la combinación de género, raza y clase socioeconómica. Esto requiere políticas públicas intersectoriales y movilización social a favor del capital social. Por ello, es fundamental el modelo de Redes de Atención, con la consiguiente multiprofesionalidad, intersectorialidad y transdisciplinariedad. Esto presupone incluso una mayor inserción del cirujano dentista en la identificación y recepción de las víctimas para su posterior derivación (AU).

SUMMARY: During the COVID-19 pandemic, a questionnaire was administered to the students who took anatomy courses through distance education in the Faculties of Medicine and Dentistry. Through the questionnaire, the aim was to figure out whether the infrastructure of Firat University was ready and adequate for distance education, how efficient the distance theoretical and practical anatomy classes were, and in what proportions the students would prefer to take face-to-face and distance anatomy classes when the pandemic is over. A questionnaire of 35 questions was applied to 555 students studying at the Medicine and Dentistry Faculties of Firat University. The students widely accepted the opinion that Firat University successfully implemented the distance education system, and distance education offered the opportunity to receive the lessons repeatedly regardless of time and place. In addition, it has been determined as a common opinion that anatomy classes given via distance education were equally beneficial as face-to-face education in terms of duration, content, and efficiency. Despite these advantages, the lack of face-to-face interaction and weak information permanence were reported as the negative aspects of distance education. The students emphasized that applied anatomy classes and especially cadaver studies should be conducted face to face. Anatomy education is quite substantial in the acquisition and development of professional skills. It is considered that distance anatomy education will be inadequate to provide this gain. The high demand for face-to-face practical classes by students also backs this up.


Durante la pandemia de COVID-19, se administró un cuestionario a los estudiantes que cursaban asignaturas de anatomía a distancia en las Facultades de Medicina y Odontología. A través del cuestionario, se pretendía conocer si la infraestructura de la Universidad de Firat estaba preparada y era adecuada para la educación a distancia, ¿qué tan eficientes eran las clases teóricas y prácticas de anatomía a distancia? y ¿en qué proporciones los estudiantes preferirían tomar clases presenciales? y clases de anatomía a distancia cuando termine la pandemia. Se aplicó un cuestionario de 35 preguntas a 555 estudiantes de las Facultades de Medicina y Odontología de la Universidad de Firat. Los estudiantes aceptaron ampliamente la opinión de que la Universidad de Firat implementó con éxito el sistema de educación a distancia, y la esta manera de enseñar ofreció la oportunidad de recibir lecciones repetidamente, independientemente de la hora y el lugar. Además, se ha determinado como opinión común que las clases de anatomía impartidas a distancia fueron igualmente beneficiosas que las presenciales en términos de duración, contenido y eficiencia. A pesar de estas ventajas, la falta de interacción cara a cara y la débil permanencia de la información fueron reportadas como los aspectos negativos de la educación a distancia. Los estudiantes enfatizaron que las clases de anatomía aplicada y en especial los estudios de cadáver deben ser presenciales. La educación en anatomía es bastante sustancial en la adquisición y desarrollo de habilidades profesionales. Se considera que la educación anatómica a distancia será inadecuada para proporcionar esta ganancia. La alta demanda de clases prácticas presenciales por parte de los estudiantes también lo avala.

ABSTRACT

We consider theoretical and practical issues for innovatively using a large number of covariates in clinical trials to achieve various design objectives without model misspecification. Specifically, we propose a new family of semiparametric covariate-adjusted response-adaptive randomization (CARA) designs and we use the target maximum likelihood estimation (TMLE) to analyze the correlated data from CARA designs. Our approach can flexibly achieve multiple objectives and correctly incorporate the effect of a large number of covariates on the responses without model misspecification. We also obtain the consistency and asymptotic normality of the target parameters, allocation probabilities, and allocation proportions. Numerical studies demonstrate that our approach has advantages over existing approaches, even when the data-generating distribution is complicated.

ABSTRACT

El carcinoma baso escamoso (CBE) o metatípico, es considerado un subtipo de carcinoma basocelular (CBC). Con frecuencia es infradiagnosticado ya que no posee características clínicas ni histológicas definidas, por lo que se suele diagnosticar erróneamente como un carcinoma basocelular o espinocelular (CEC). En este artículo se presenta el caso de un paciente masculino de 86 años con un CBE que se expresó en un colgajo cérvico-facial previo. Se ha descrito un alto riesgo de recurrencia y metástasis, lo que supone un pronóstico desfavorable para los pacientes. No existen protocolos terapéuticos estandarizados para esta entidad, sin embargo, el manejo es fundamentalmente quirúrgico. No se encontraron en la literatura reportes previos de carcinoma metatípico en injertos autólogos de piel(AU)


Metatypical or basosquamous carcinoma (BSC) is considered a subtype of basal cell carcinoma (BCC). It is often underdiagnosed due to the lack of defined clinical and histological characteristics, leading to misdiagnosis as either BCC or squamous cell carcinoma (SCC). This article presents a case of an 86-year-old male patient with metatypical carcinoma that manifested in a previous cervicofacial flap. There is a high risk of recurrence and metastasis associated with this condition, leading to an unfavorable prognosis for patients. Although there are no standardized therapeutic protocols for this entity, surgical management is the primary approach. No previous reports of metatypical carcinoma in autologous skin grafts were found in the literature(AU)

ABSTRACT

Since the 1990s, big strides have been made in plastic and reconstructive surgery, thanks to the implementation of new techniques and resources for its execution. In cases where restoration is sought for small defects, biosurgery, involving the utilization of stem cells, biomaterials, chemical engineering, and tissue engineering, can be employed. The potential to stimulate the innate regenerative capacity of tissue and generate a persistent response over time, minimizing surgical trauma and re-interventions, lies in stem cell transplantation. However, the evidence published on this matter is very scarce, leading to divergence of opinions, suggestions, and recommendations. The aim of this review is to analyze the most recent evidence concerning the outcomes of using fat grafts enriched with stem cells in facial reconstruction, as compared to autologous fat grafts. The review of the literature demonstrates a marked trend suggesting that fat grafts enriched with stem cells may be superior to autologous grafts in facial reconstruction, with potential benefits in medium-term volume retention and faster attainment of results. La cirugía plástica y reconstructiva, ha dado pasos agigantados desde la década de los años 90, con la implementación de nuevas técnicas y recursos para su realización. En los casos de defectos pequeños donde se busca la restauración, se puede hacer uso de la biocirugía, que involucra el uso de células madre, biomateriales, ingeniería química y de tejidos. El trasplante de células madre, tiene el potencial de estimular la capacidad innata regenerativa del tejido, así como de generar una respuesta persistente a lo largo del tiempo, minimizando el trauma quirúrgico y la reintervención. A pesar, de la ventaja plausible, la calidad y cantidad de la evidencia publicada al respecto es muy escasa, ocasionando divergencia de opiniones, sugerencias y recomendaciones. Con base en lo anterior, el objetivo de esta revisión consiste en realizar un análisis de la evidencia más reciente sobre los resultados en el uso de injertos de grasa enriquecidos con células madre en la reconstrucción facial, comparado a los injertos de grasa autólogos. A través de la revisión de la literatura, se pudo evidenciar que, aunque la evidencia es heterogénea a la fecha, existe una tendencia marcada en que el uso de injertos de grasa enriquecidos con células madre, podría ser superior a los injertos autólogos en la reconstrucción facial, con potenciales beneficios en la retención de volumen a mediano plazo y mayor celeridad en la obtención de resultados. Introducción y objetivo: La evolución de las técnicas quirúrgicas para rejuvenecimiento facial y cervical ha sido significativa en los últimos años. La elección de la técnica debe personalizarse según las características anatómicas del paciente. El presente trabajo tiene como objetivo presentar nuestra evaluación preoperatoria sistemática de pacientes que se someten a rejuvenecimiento quirúrgico de cara y cuello y nuestro diagrama de toma de decisiones. Material y método: Presentamos nuestra lista de verificación preoperatoria (subdividida en 9 pasos) para identificar variantes anatómicas y deformidades individuales. Mediante exploración física y valoración ecográfica de cara y cuello, predecimos algunas variables que pueden hacer que la cirugía sea más previsible y estandarizada. Para ello creamos 7 pasos para el Diagrama de Toma de Decisiones Preoperatorias en Cirugía de Rejuvenecimiento de Cara y Cuello. Resultados: Cincuenta y nueve pacientes se sometieron a técnicas de estiramiento profundo de cara y cuello entre enero de 2019 y junio de 2021 utilizando nuestro diagrama de toma de decisiones. Presentamos 2 casos clínicos demostrativos.Conclusiones: En este artículo explicamos por qué se deben ofrecer diferentes técnicas quirúrgicas a diferentes pacientes teniendo en cuenta sus necesidades individuales. Con esta lista de comprobación y diagrama de toma de decisiones, el cirujano plástico podrá tomar decisiones personalizadas antes de la cirugía, como la orientación de la movilización del colgajo SMAS, el tipo de tratamiento de la porción más anterior del platisma o la necesidad de abordaje y tratamiento de las estructuras cervicales subplatismales, a fin de reducir la consecución de resultados menos satisfactorios, los errores quirúrgicos y agilizar la cirugía. Background and objective: The evolution of surgical techniques for facial and neck rejuvenation has been significant over the last years. The choice of technique must be personalized according to the patient's ana- tomical details and specificities to provide individual and personalized results. This work aims to expose our systematic preoperative evaluation of the patients that will be submitted to a face and neck surgical rejuvenation and our decision-making diagram. Methods: We present our preoperative evaluation checklist (subdivided into 9 steps) to recognize the individual anatomical variants and deformities. Using the physical exam and ultrasound evaluation of the face and neck we predict some variables that can make surgery more predictable and standardized. To do so we apply our 7 steps of pre-operative Decision Making Diagram of Face and Neck Rejuvenation Surgery. Results: Fifty-nine patients were submitted to face and neck lift between January 2019 and June 2021 using deep plane cervico-facial techniques according to our decision-making diagram. We demonstrate 2 clinical cases. Conclusions: In this paper, we explain why different surgical techniques should be offered to different patients considering their individual needs. Using this checklist and decision-making diagram the surgeon will be able to make individualized decisions before surgery, such as SMAS flap lift vector direction, the type of the platysma treatment or the need for deep platysmal structures approach and treatment. This work aims to reduce the likelihood of obtaining less satisfactory results by decreasing surgical errors and making the surgery faster. (AU)

ABSTRACT

Aim: This study aimed to evaluate the correlation between vertical dimension of occlusion (VDO) and various facial measurements in a sample of Sudanese adults. Material and Methods: A total of 113 dental students (33 males and 80 females) with a mean age of 21.7±1.26 years were enrolled in this study. Different facial measurements including (Eye-Mouth, Eye-Eye, Eye-Ear, and Ear Height) were compared with two different measurements of VDO: N-Gn (from the tip of the nose to the tip of the chin), and Sn-Me (from the base of the nose to the bottom of the chin). Pearson's correlation coefficient test was utilized for the correlation between the measured parameters. A p-value of less than 0.05 was considered significant for all analyses. Results: A significant positive correlation was shown between all measured facial distances and both measured VDO distances. Though, the strongest correlation was seen for the eye-mouth distance (r= 0.725, p<0.001), while the weakest was for ear height (r= 0.254, p= 0.007). A paired t-test revealed a significant longer N-Gn distance than Sn-Me distance. Also, it has been shown that there were no significant differences between right and left sides of the face. Conclusion: The distance measured from the outer canthus of the eye to the angle of the mouth can be used to predict Subnasale-Menton (Sn-Me) distance.


Objetivo: Este estudio tuvo como objetivo evaluar la correlación entre dimensión vertical oclusal (DVO) y varias medidas faciales en una muestra de adultos sudaneses. Material y Métodos: Un total de 113 estudiantes de odontología (33 hombres y 80 mujeres) con una edad media de 21,7 ± 1,26 años se inscribieron en este estudio. Se compararon diferentes medidas faciales que incluyen (ojo- boca, ojo-ojo, ojo-oído y altura de la oreja) con dos medidas diferentes de DVO: N-Gn (desde la punta de la nariz hasta la punta del mentón) y Sn -Yo (desde la base de la nariz hasta la parte inferior del mentón). Se utilizó la prueba del coeficiente de correlación de Pearson para la correlación entre los parámetros medidos. Un valor de p inferior a 0,05 se consideró significativo para todos los análisis. Resultados: Se mostró una correlación positiva significativa entre todas las distancias faciales medidas y ambas distancias DVO medidas. Sin embargo, la correlación más fuerte se observó para la distancia ojo-boca (r=0,725, p<0,001), mientras que la más débil fue para la altura de las orejas (r=0,254, p=0,007). Una prueba de t pareada reveló una distancia N-Gn significativamente más larga que la distancia subnasal-mentón. Además, se ha demostrado que no hubo diferencias significativas entre los lados derecho e izquierdo de la cara. Conclusión: La distancia medida desde el canto externo del ojo hasta el ángulo de la boca puede utilizarse para predecir la distancia subnasal-mentón.

RESUMEN Introducción: El síndrome de Freeman-Sheldon es un síndrome hereditario raro, de severidad variable que afecta principalmente la cara, manos y pies, sin preferencia de género, étnica o geográfica. Objetivo: Caracterizar clínicamente a un paciente con síndrome Freeman-Sheldon. Presentación del caso: Niña ecuatoriana de 6 años de edad, hija de madre de 43 años y padre de 42 años, la cuarta de 6 hermanos, todos sanos, no historia de consanguinidad. La cual presenta cara parecida a una máscara, ojos hundidos, puente nasal ancho, boca pequeña con apariencia de silbador, hoyuelo cutáneo en mentón en forma de H, defecto en las manos, contractura de los dedos con desviación cubital y pies equinovaro, dificultad para la marcha y baja talla. Conclusiones: El síndrome de Freeman-Sheldon es un síndrome raro que afecta principalmente la cara y las extremidades de los pacientes, cuyo diagnóstico clínico es posible luego de un examen físico exhaustivo.


ABSTRACT Introduction: Freeman-Sheldon syndrome is a rare hereditary syndrome of varying severity that mainly affects the face, hands and feet, without gender, ethnic or geographical preference. Objective: Clinically characterize a patient with Freeman-Sheldon syndrome. Presentation of the case: Ecuadorian girl, 6 years old, daughter of mother of 43 years and father of 42 years, the fourth of 6 brothers, all healthy, not history of consanguinity. She presents mask-like face, sunken eyes, wide nasal bridge, small mouth with the appearance of a whistler, skin dimple on the chin in the shape of an H, defect in the hands, contracture of the fingers with ulnar deviation and clubfoot, also walking difficulty and short height. Conclusions: Freeman-Sheldon syndrome is a rare syndrome that mainly affects the face and limbs of patients, whose clinical diagnosis is possible after a thorough physical examination.

ABSTRACT

The COVID-19 pandemic has highlighted the need for a proper risk assessment of respiratory pathogens in indoor settings. This paper documents the COVID Airborne Risk Assessment methodology, to assess the potential exposure of airborne SARS-CoV-2 viruses, with an emphasis on virological and immunological factors in the quantification of the risk. The model results from a multidisciplinary approach linking physical, mechanical and biological domains, enabling decision makers or facility managers to assess their indoor setting. The model was benchmarked against clinical data, as well as two real-life outbreaks, showing good agreement. A probability of infection is computed in several everyday-life settings and with various mitigation measures. The importance of super-emitters in airborne transmission is confirmed: 20% of infected hosts can emit approximately two orders of magnitude more viral-containing particles. The use of masks provides a fivefold reduction in viral emissions. Natural ventilation strategies are very effective to decrease the concentration of virions, although periodic venting strategies are not ideal in certain settings. Although vaccination is an effective measure against hospitalization, their effectiveness against transmission is not optimal, hence non-pharmaceutical interventions (ventilation, masks) should be actively supported. We also propose a critical threshold to define an acceptable risk level.

La cirugía de correctiva del envejecimiento facial tiene más de un siglo de historia documentada. Los cambios culturales han aumentado el número de operaciones. En Latinoamérica también ha aumentado, y la región ha sido activa en realizar aportes en los conceptos, técnicas y procedimientos complementarios. Además del crecimiento cuantitativo, la comprensión de los planos anatómicos faciales, cambios con la edad y los modos de corregirlos han sido de gran utilidad. Nuestra técnica del lifting cérvico-facial presentan ventajas como seguridad en el plano de disección, ausencia de tracción cicatricial y respeto capilar, proporcionando un resultado final natural de acuerdo a la solicitud de los pacientes


Corrective surgery for facial aging has more than a century of documented history. Cultural changes have increased the number of operations. In Latin America it has also increased, and the region has been active in making contributions in complementary concepts, techniques, and procedures. In addition to quantitative growth, the understanding of facial anatomical planes, changes with age and the ways to correct them have been very useful. Our cervical-facial lifting technique presents advantages such as safety in the dissection plane, absence of scar traction and capillary respect, providing a natural final result according to the request of the patients

ABSTRACT

We report the impact of a Gentle Touch Stimulation (GTS) program. Forty-three mothers provided daily 10-min GTS with C-tactile (CT) afferent optimal stroking touch, for 4 weeks to their 3-12 weeks old infants. CT-afferents are cutaneous unmyelinated, low-threshold mechanosensitive nerves hypothesized to underly the regulatory impact of affective touch. We compared physiological and behavioral responses during a no-touch-baseline (BL), static-touch-baseline (BL-T), intervention/control (GTS/CTRL), Still Face (SF) and Reunion (RU) condition for GTS-infants versus a control-group (CTRL) at the start (T1) and end of (T2) of the program. We collected mother-infant ECG, respiration, cortisol, video-recordings, and diary-reports. At T1, physiological arousal significantly increased during SF in both groups, that is, decreased respiratory sinus arrhythmia (RSA) and R-R interval (RRI). At T2, GTS-infants showed significantly increased RSA, RRI, decreased respiration during GTS, buffering SF-arousal and allowing complete recovery during RU; CTRL-infants showed higher SF-arousal and small recovery, under initial BL-levels. Maternal cardio-respiratory showed a metabolic investment during RU. Cortisol and behavioral analyses showed higher arousal in CTRL-infants than GTS-infants at T2. We suggest that the combination of phasic short-term and tonic long-term responses to CT-optimal stroking touch, delivered in a structured daily manner, contribute to the building of infant stress regulation and resilience.


Reportamos el impacto del programa de Estimulación Táctil Suave (GTS). Cuarenta y tres madres les dieron a sus infantes de 3-12 semanas 10 minutos de GTS con un toque táctil-C (CT) aferente óptimo de caricias por 4 semanas. Los aferentes CT son nervios cutáneos amielínicos mecano-sensibles de bajo impacto que según hipótesis rigen el impacto regulatorio del toque afectivo. Comparamos las respuestas fisiológicas y de comportamiento durante un momento de referencia no táctil (BL), un momento de referencia táctil estático (BL-T), intervención/control (GTS-CTRL), la Cara Seria (SF) y la condición de Reunión (RU) para infantes GTS versus un grupo de control (CTRL) al principio (T1) y al final (T2) del programa. Recogimos información ECG de madre-infante, respiración, cortisol, grabaciones en vídeo, y reportes de un diario. Al T1, la estimulación fisiológica significativamente aumentó durante SF en ambos grupos, v.g., una baja en la arritmia respiratoria paranasal (RSA) y el intervalo R-R (RRI). Al T2, los infantes GTS significativamente mostraron un aumento de RSA, RRI, una baja en la respiración durante GTS, amortiguando la estimulación SF y permitiendo un completo recobro durante RU; los infantes de CTRL mostraron una más alta estimulación SF y un pequeño recobro, bajo los niveles BL iniciales. El aspecto cardio-respiratorio materno mostró inversión metabólica durante RU. Los análisis de cortisol y de comportamiento mostraron más altas estimulaciones de infantes de CTRL que en infantes GTS al T2. Proponemos que la combinación de las respuestas fásicas a corto plazo y tónicas a largo plazo a las caricias táctiles óptimas CT, ofrecidas de una manera estructural diaria, contribuyen a moldear la regulación del estrés y capacidad de resistencia del infante.


Cet article fait état de l'impact du programme Stimulation Toucher en Douceur (an anglais Gentle Touch Stimulation, que nous abrégeons ici GTS). Quarante-trois mères ont procédé chaque jour à un GTS de 10 minutes avec un toucher par caresse optimale C-tactile (CT) afférent, pendant 4 semaines à leurs nourrissons de 3-12 semaines. Les afférents CD sont des nerfs cutanés amyéliniques, à faible seuil et mécano-sensibles, dont on pense qu'ils sous-tendent l'impact régulatoire du toucher affectif. Nous comparons les réponses physiologiques et les réponses comportementales durant la condition à un niveau de référence NR sans toucher, un niveau de référence toucher statique (NR-T), un contrôle/intervention (GTS/CTRL), Visage Impassible (Still Face, soit SF) et la Réunion (RU) pour les bébés GTS par rapport aux bébés du groupe de contrôle (CTRL) au commencement (T1) et à la fin (T2) du programme. Nous avons recueilli l'électrocardiogramme mère-bébé, la respiration, le niveau de cortisol et les rapports-journaux. Au T1 la stimulation physiologique a augmenté de façon importante durant le SF pour les deux groupes, c'est-à-dire une arythmie des sinus respiratoires (RSA) décrue et un intervalle R-R décru (RRI). Au T2 les bébés GTS ont fait preuve d'une RSA et d'un RRI bien plus élevés, une respiration décrue durant le GTS, un éveil SF tampon et ont fait preuve d'un rétablissement complet durant la RU. Les bébés contrôle ont fait preuve de plus d'éveil SF et d'un petit rétablissement, moins élevés que les niveaux BL initiaux. La cardio-respiration maternelle a fait preuve un investissement métabolique durant la RU. Les analyses de cortisol et les analyses comportementales ont montré un éveil plus élevé chez les bébés-CTRL que chez les bébés GTS au T2. Nous suggérons que la combinaison de réactions phasiques à court terme et de réponses toniques à long terme au toucher caresse CT-optimal, donné chaque jour de manière structurée, contribue à la construction de la régulation du stress du bébé et de sa résilience.

Reactive oxygen species (ROS) play a crucial role in the cellular defense against S. aureus, as evidenced by the importance of this pathogen in patients lacking the ROS-generating phagocyte NADPH oxidase NOX2. ROS concentrations required to kill S. aureus in vitro are much higher than those found in the phagosome. We therefore hypothesized that sublethal ROS concentrations may play a role in S. aureus gene dysregulation and investigated the in vitro transcriptomic response of S. aureus to sublethal concentrations of hydrogen peroxide (H2O2). A striking observation of these experiments was a coordinated and massive downregulation of genes involved in pyrimidine metabolism. Using transposon insertion mutants, we demonstrated that deletion of carA, a gene involved in pyrimidine synthesis, led to a significant growth defect and to an increased sensitivity of S. aureus to added H2O2. The phenotype of the carA mutant could be reversed through supplementation with the pyrimidine precursor uracil, or with a multicopy vector encoding carA. As opposed to the impact of ROS on extracellular survival, carA deletion did not affect the intracellular survival in neutrophils. Our results raise the possibility that ROS-dependent downregulation of pyrimidine metabolism might be a survival strategy of S. aureus, allowing colonization through intracellular survival, while decreasing the risk of killing the host through dampened extracellular growth.

ABSTRACT

SUMMARY: The human skull is comprised of many flat and irregular bones, divided into two groups: cranial and facial bones. The size, shape and growth of the human skull are the product of many interrelated internal and external factor interactions. The purpose of the present study was to explore the systematic differences in cephalofacial size and shape classification between two measured groups of different ages and different period of done measurements. Five cephalofacial variables and four cephalofacial indexes were measured on 795 entities of the Kosovo Albanian population in two different timelines; 401 male entities aged 20-30 years old were measured during the timeline 1997-1999, while 394 male entities aged 18 years old were measured during the timeline 2016-2018. The gained data were analyzed in terms of basic descriptive statistical parameters and the percentage of distribution of results. The most pronounced differences between the age groups of 18 year-olds and the age group of 20-30 year-olds, are mainly emphasized in the morphological height of the face, total facial index and transverse cephalic index. While in other dimensions, the distribution of results is very similar. Differences between groups can be justified by the age at which the growth of the cephalofacial bones ends and the influence of external factors such as nutritional and socio-economic factors.


RESUMEN: El cráneo humano está compuesto por huesos planos e irregulares, divididos en dos grupos: huesos craneales y faciales. El tamaño, la forma y el crecimiento del cráneo humano son el producto de muchas interacciones de factores internos y externos interrelacionados. El propósito del presente estudio fue explorar las diferencias sistemáticas en la clasificación de tamaño y forma cefalofacial entre dos grupos de diferentes edades y en diferentes períodos de mediciones. Se midieron cinco variables cefalofaciales y cuatro índices cefalofaciales en 795 entidades de la población albanesa de Kosovo en dos líneas de tiempo diferentes. Se midieron 401 entidades masculinas de 20 a 30 años. Durante el periodo 1997-1999 se medieron 401 entidades masculinas de 20 a 30 años, mientras que 394 entidades masculinas de 18 años se se midieron durante el periodo 2016-2018. Los datos obtenidos se analizaron en términos de parámetros estadísticos descriptivos básicos y el porcentaje de distribución de los resultados. Las diferencias más pronunciadas entre los grupos de edad de 18 años y el grupo de 20-30 años, se enfatizan principalmente en la altura morfológica de la cara, índice facial total e índice cefálico transversal. Mientras que en otras dimensiones, la distribución de resultados es muy similar. Las diferencias entre grupos pueden estar asociadas a la edad en la que finaliza el crecimiento de los huesos cefalofaciales y la influencia de factores externos, tal como aspectos nutricionales y socioeconómicos.

ABSTRACT

The Nerve Growth Factor (NGF) neurotrophin acts in the maintenance and growth of neuronal populations. Despite the detailed knowledge of NGF's role in neuron physiology, the structural and mechanistic determinants of NGF bioactivity modulated by essential endogenous ligands are still lacking. We present the results of an integrated structural and advanced computational approach to characterize the extracellular ATP-NGF interaction. We mapped by NMR the interacting surface and ATP orientation on NGF and revealed the functional role of this interaction in the binding to TrkA and p75NTR receptors by SPR. The role of divalent ions was explored in conjunction with ATP. Our results pinpoint ATP as a likely transient molecular modulator of NGF signaling, in health and disease states.
