ABSTRACT

The combination of medical care and nursing is an important measure of health care for the aged, and it is also an important part of aging work. Under the background of increasing aging, and under the background of changing from disease-centered and patient-centered medical model to health-centered "big medicine" model, medical humanities are particularly important for how to make the elderly spend their old age in peace and make the road of combining medical care with nursing care wider. Based on the practice of combination of medical care and nursing, under the guidance of the concept of great health, this paper discussed the establishment of a "Five-sphere" all-round humanistic care concept integrating medical care, nutrition care, sports care, mental care and other convalescent health care factors, and provided a series of comprehensive multi-angle humanistic care service measures from physiology to psychology for elderly patients with chronic diseases, which has achieved good results.

Introduction: The level of nurses' readiness and knowledge can significantly influence on the quality of treatment and patients' health. Nurses should have a high level of knowledge and understanding of clinical care and its aspects. According to this issue, our study has been performed with the aim to compare the readiness of different wards of the hospital in evaluating important indicators of clinical care. Methods: This descriptive cross-sectional study was performed on 99 medical staff working in different wards of the hospital - during July 2020 to December 2021. Nurses in different wards of the hospital were compared in terms of care indicators. The tool used was a checklist for the effectiveness and evaluation of clinical care. The data were analyzed by SPSS statistical software. Findings: About 16.3% of the nurses were male, and 85.7% were female. Nurses in different wards were significantly different in terms of catheter and ligament care, blood transfusion and products, serum therapy and IV therapy, communication and training skills, pharmaceutical processes, and working with ventilators (p < 0.05). Emergency and general wards' nurses showed better readiness for pharmaceutical processes and emergency, and neonatal intensive care unit (NICU) ward nurses showed the best readiness in relation to catheter care and absorption and excretion processes and blood transfusion and products (p < 0.05). Conclusion: Despite an appropriate readiness in many wards, care readiness was placed on a lower extent in some wards. There were also limitations, strengths, and weaknesses in some aspects, including medication, care and standards, and infection control, which, because of this issue, can help improve nurses' performance in the future.

ABSTRACT

BACKGROUND: One of the results of burn injuries is the appearance of scars and deformities in various organs of the body, which can cause many physical and psychological challenges to burn patients. Lack of proper communication between nurse and patient leads to inaccurate identification of psychological and social needs of these patients and thus affects the quality of care. AIM: This study aimed to investigate the effect of seeing the patient's pre-burn face photo on the quality of care and empathy of nurses with patients admitted to the burn intensive care unit. METHODS: The present study is a quasi-experimental interventional study conducted on 26 Burn Intensive Care Unit (BICU) nurses who were selected by census sampling method in 2018. Data were collected through QUALPAC Nursing Care Quality Questionnaire, Lumonica Empathy Questionnaire and Demographic Questionnaire, which were completed by nurses before and after the intervention. Pre-burn photos of patients with facial burns were displayed at the nursing station for one month and the data were compared before and after the intervention. Descriptive statistics and paired t-test and independent t-test, Mann-Whitney and Pearson correlation tests were used to analyze the data. FINDINGS: The results of present study showed that there was a statistically significant difference between the quality of nursing care (p = 0.001) and its psychological dimension (p < 0.001) before and after the intervention in the group of nurses. However, no significant relationship was found between nurses' empathy with patients before (p = 0.901) and after intervention (p = 0.001). CONCLUSION: Seeing the photo of the patient's pre-burn face and establishing a relationship between the nurse and the patient had an effect on the quality of nursing care and especially its psychological dimension. However, in order to generalize the results and implement this low-cost, convenient and low-complication intervention for all patients admitted to the intensive care unit, more studies are needed in this field.

ABSTRACT

BACKGROUND: Self-rated health (SRH) is a health measure used in studies of older adults. The objective of this study is to analyze SRH as a predictor of mortality in the institutionalized older population and the characteristics of those who do not provide information about their SRH on health questionnaires. METHODS: This is a 15-year follow-up study of older adult residents in nursing or care homes in of Madrid, Spain. SRH was measured on a 5-point Likert type scale. The association between answering the SRH question and socio-demographic and health characteristics was evaluated through prevalence ratio (PR), estimated by Poisson regression models. Survival rates associated with SRH were studied through a multivariate Cox regression. RESULTS: The sample has a mean age of 83.4 (standard deviation, SD = 7.3), with 75.7% women. Twelve percent did not answer the SRH item. Those who did not answer showed a higher probability of disability (Barthel index, PR = 0.76, 95% confidence interval = 0.67-0.86) and/or dementia (PR = 8.03, 3.38-19.03). A trend for higher mortality was observed in those persons who did not respond (adjusted hazard ratio HR = 1.26, 0.75-2.11). The mortality rate was 32% higher for those who declared poor SRH in comparison with those who reported good SRH (adjusted HR = 1.32, 1.08-1.6). CONCLUSIONS: There is an elevated number of people who do not respond to the SRH item, mainly those with disabilities and cognitive deterioration. Lack of response to SRH is a good indicator of 15-year mortality for persons institutionalized in care or nursing homes.

Introducción. El análisis de las situaciones de salud, en el acto del cuidado de la enfermería, requiere de conocimientos, pues es el marco conceptual y la guía de esta labor. Aunque el cuidado integral de la salud, según Watson, depende del compromiso moral del enfermero de preservar la dignidad humana; son frecuentes los informes que muestran una mala percepción de la prestación de servicios de salud. En los informes, se indica que el 36% de los encuestados tienen una mala o muy mala percepción de la calidad de la atención médica. El objetivo es reflexionar sobre las circunstancias que influyen en la calidad de la atención médica percibida por los colombianos. Temas de reflexión. Se abordará el tema desde los patrones de conocimiento de la enfermería: el empírico, el estético, el ético, el sociocultural y el personal. Discusión. Las condiciones que influyen en la percepción de la calidad de la atención médica se agrupan en dos conceptos del metaparadigma de la enfermería. El primero es el entorno, en el que existen barreras de oferta y demanda, como la tecnificación, la infraestructura y los recursos del sistema de salud. El segundo es el cuidado, en el que la condición laboral tiene incidencia, ya que los profesionales de enfermería sienten culpabilidad, agotamiento y estrés, y comprometen la buena percepción del cuidado médico y el sentido de trascendencia profesional. Conclusiones. El acto del cuidado de la enfermería está rodeado de situaciones poco favorables, dadas las barreras en el sistema de salud colombiano, que comprometen la labor del enfermero. Por lo tanto, es necesario reflexionar y desarrollar conceptos relacionados con los patrones de conocimiento de la disciplina, que inciden en la formación de los nuevos profesionales. Introduction. The analysis of health situations, in terms of nursing care, requires knowledge, since it is the conceptual framework and guide for this work. Although comprehensive healthcare, according to Watson, depends on the nurse's moral commitment to preserve human dignity, there are frequent reports that show a poor perception of the provision of healthcare services. The reports indicate that 36% of those surveyed had a poor or very poor perception of the quality of healthcare. The objective is to reflect on the circumstances that influence the quality of healthcare perceived by the people in Colombia. Topics of reflection. The topic will be addressed through the patterns of knowledge of nursing: empirical, aesthetic, ethical, sociocultural and personal. Discussion. The conditions that influence the perception of healthcare quality are grouped into two nursing metaparadigm concepts. The first is the environment, in which there are supply and demand barriers, such as technification, infrastructure and resources of the healthcare system. The second is the care, on which the employment status has an influence, since nursing professionals feel guilt, exhaustion and stress, and compromise the good perception of healthcare and the sense of professional importance. Conclusions. The act of caring in nursing is surrounded by unfavorable situations, given the barriers in the Colombian healthcare system that compromise the work of nurses. Therefore, it is necessary to reflect and develop concepts related to the patterns of knowledge of this discipline, which have an effect on the training of new professionals. ABSTRACT

A survey was developed to map provision, knowledge, attitudes and views towards videoconferencing in care homes in Yorkshire and The Humber. The survey was sent to 859 care homes, with a 14% response rate. Twelve homes reported using videoconferencing. Non-users appeared skeptical, managers using the system reported improvements in outcomes.

In this study, using the unexpected intervention overturning the interaction amount of the field and the mental model, an interaction of a robot system that enables sustained nonverbal communication with the mildly demented elderly was proposed and its effectiveness was shown in the group home of the mildly demented elderly.

ABSTRACT

A assistência e os cuidados neonatais associados ao Método Canguru reforçam seu valor para a sobrevida e a redução da morbidade dos neonatos. Nesse ambiente, acreditamos que a observância das questões individuais que possam facilitar ou atender às necessidades singulares de cada mãe são elementos primordiais para a autodeterminação e autonomia, importantes para a sua demanda como cidadã. Tendo como objeto o significado para as mães da sua vivência no Método Canguru, passamos a nos questionar: Que significado tem para a mãe a sua vivência no Método Canguru? Para responder a esse questionamento, este estudo tem o objetivo de analisar o significado da vivência para mães no Método Canguru. Partimos do pressuposto de que, diante da construção sociocultural direcionada ao amor materno, as mães mesmo sentindo dificuldade para permanecerem durante as 24h, na enfermaria, por longo período de tempo, atendendo aos objetivos previstos para o Método Canguru se mantêm, na maioria das vezes, sem reclamar. Trata-se de uma pesquisa qualitativa e de caráter exploratório e descritivo, desenvolvida em duas Instituições públicas da cidade de Salvador­Bahia. Participaram desta pesquisa 19 mães, sendo que 10 na Instituição A e 09 na Instituição B, respeitando os aspectos éticos e legais da Resolução 466/2012. A produção do material empírico deu-se em três momentos: mediante a realização de observação participante, oficinas de reflexão e entrevistas individuais. Para operacionalizar a técnica e tratar as informações coletadas, optamos por utilizar a análise de conteúdo, modalidade temática, norteada por Bardin. Nesse contexto, o significado da vivência para mães nesse método configura-se em quatro categorias: Significado da vivência no Método Canguru; aprendizagem para a continuidade do cuidado no domicílio; dificuldades para vivência e convivência no MC; atendendo às construções sociais na vivência do MC. Os depoimentos confirmaram a importância desse método para o preparo das mães na aquisição de autonomia e experiências no cuidado do(a) RN e relataram que enfrentaram dificuldades em se ausentarem dos outros filhos e do marido para permanecerem na Unidade Canguru 24 horas ininterruptamente; deixando explícitas as questões de gênero relacionadas ao papel de mãe e dona de casa. Ressaltamos a importância que tem a equipe de saúde ao proporcionar um ambiente o mais acolhedor possível; o entendimento singular de cada necessidade verbalizada, assim como os encaminhamentos necessários para possível solução das intercorrências com vistas à promoção do bem-estar da mulher e o cumprimento dos objetivos do método. Nesse processo, os(as) profissionais de saúde e gestores institucionais poderão contribuir no atendimento aos objetivos do método mediante a reflexão e busca de estratégias que atendam aos objetivos desse método e reduzam a angústia e o sofrimento de mães internadas com seus(as) filhos(as). Consideramos ser importante a implementação de mudanças com vistas ao acolhimento e empoderamento das mulheres em relação aos seus direitos como cidadã, preservando a humanização do cuidado sem se afastar dos objetivos do Método Canguru.
