ABSTRACT

OBJECTIVE: To analyze the characteristics of acupoint catgut embedding methods, tools, catgut types, and the treatment cycles in the clinical research in recent ten years both at home and abroad, so as to summarize its regularities and to provide technical references for further studies. METHODS: Articles about clinical researches on catgut embedding therapy published in recent ten years (from January 1, 2010 to December 31,2020) were retrieved from the databases of CNKI, Wanfang, VIP, and PubMed by using key words of "acupoint embedding" "acupoint catgut embedding" and "catgut implantation at acupoint". According to the inclusion and exclusion criteria, a new database was established for analyzing the data mentioned above. RESULTS: 1) A total of 1 196 articles were collected, including 15 English articles and 1 181 Chinese articles, presenting a fluctuating increasing trend in recent ten years. 2) The commonly used acupoint embedding methods included disposable catgut embedment needle method (399 times, 38.89%) and disposable syringe needle catgut embedding method (347 times, 33.82%), for which two or multiple methods were mentioned in the same one article. 3) The most frequently used top two tools for catgut embedding were the dispo-sable catgut embedment needle (463 times, 43.03%) and disposable syringe needle (406 times,37.73%), with a significant increase in the application of disposable syringe needle. The most commonly used size of tools included No. 7 (283 times, 39.86%), No. 9 (196 times, 27.61%) and No. 8 (109 times, 15.35%). 4) The most frequently implanted surgical suture was still the common catgut (671 times, 58.15%) despite of a reduction in clinical application year by year, and the types of the implanted suture materials were gradually enriched since 2018, such as the absorbable surgical suture, polyethylprolactide(PGLA), collagen protein thread, polydioxanone(PPDO), etc. The commonly used implanted catgut size was 3-0 (227 times, 30.15%), 2-0 (176 times, 23.37%), 4-0 (131 times, 17.40%), 0 (103 times, 13.68%), with the commonly used catgut length being 1 cm (332 times, 35.55%), 1.5 cm (103 times, 11.03%), 1-2 cm (92 times, 9.85%) and 2 cm (92 times, 9.85%). 5) The intervals of the catgut implantation were 7 days (313 times, 28.95%), 14 days (262 times, 24.24%), 10 days (174 times, 16.10%), and 15 days (162 times, 14.99%). CONCLUSION: In recent ten years, clinical research on acupoint catgut embedding is growing rapidly, and the embedding methods, tools, implanted sutures, and embedding intervals are various, which may provide certain technical references for the future researches and suggest an urgent need of formulation of the standardized and unified standards in this field.

ABSTRACT

OBJECTIVE: To compare the clinical effect on the joint attention and social communication in children with autism spectrum disorder (ASD) between the combined treatment of acupoint catgut embedding therapy with rehabilitation training and the simple application of rehabilitation training. METHODS: A total of 60 children with ASD were randomized into an observation group and a control group, 30 cases in each one. In the control group, the routine ASD rehabilitation training was adopted, including conductive education, speech training and music therapy, once a day. In the observation group, on the base of the treatment as the control group, the catgut embedding therapy was applied to Shenting (GV 24), Baihui (GV 20), Shenmen (HT 7) and the optic area (the point of scalp acupuncture), once a week. In the two groups, the treatment for 4 weeks was as one course, at the interval of 1 week between the courses. A total of 3 courses of treatment were required. The social domain of the autism treatment evaluation checklist (ATEC) and the autism behavior checklist (ABC) were adopted to assess the therapeutic effect of the two groups. RESULTS: After treatment, the scores of each item of the social domain in ATEC and the scores of ABC (feeling, communication, physical movement, language and healthy behavior) were all lower than those before treatment in the two groups (P<0.01). The scores of each item in the observation group were lower than those in the control group after treatment (P<0.05). CONCLUSION: The combined treatment of acupoint catgut embedding therapy with the rehabilitation training effectively improves in the joint attention and social communication. The therapeutic effect of this combined treatment is better than the simple application of rehabilitation training.

ABSTRACT

Polylactic-co-glycolic acid (PLGA) thread is frequently used for acupoint catgut embedding therapy (ACET), but the poor hydrophilicity and biocompatibility largely limited its wider applications. The aim of this study is to functionalize the PLGA braided thread and improve its cell adhesion property. The PLGA strands are first processed into threads on a circular braiding machine, and then, antibacterial treatment was introduced with and without oxygen plasma treatments. Afterward, functional characterizations such as antibacterial activity (Staphylococcus aureus and Escherichia coli), cytotoxicity, cell attachment and cell morphology, histological observation, and biodegradation experiments of threads were measured. Moreover, tensile properties and flexibility of the threads were determined to evaluate their mechanical properties. The modified threads showed rougher surfaces than those of the unmodified ones from SEM observations, and the weights and fiber diameters of the threads increased correspondingly, together with the improved surface hydrophilicity. All coated sutures showed durable antimicrobial function and slow drug releasing features for more than 5 days and good cell viability (more than 75%), according to the standard of ISO 10993-5:2009. Besides, cell attachment, tissue growth, and collagen regeneration of plasma-treated samples were greatly improved compared to those of without the plasma treatment. The threads presented slow degradation behavior after the antibacterial treatment. The threads with only plasma-treated revealed a promising prospect for clinical applications.

ABSTRACT

OBJECTIVE: To observe the differences in the clinical therapeutic effects on insomnia of spleen and stomach disharmony pattern in comparison of the combination of acupoint catgut-embedding therapy and auricular point pressure with the simple application of auricular point pressure. METHODS: One hundred and eighty patients of insomnia of spleen and stomach disharmony pattern were randomized into an observation group (93 cases) and a control group (87 cases). In the observation group, the combination of acupoint catgut-embedding therapy and auricular point pressure was adopted. The acupoint catgut-embedding therapy was applied at Zhongwan (CV 12), Anmian (Extra), Xinshu (BL 15), Pishu (BL 20), Weishu (BL 21), Zusanli (ST 36), Yinlingquan (SP 9) and Sanyinjiao (SP 6), once every two weeks. The auricular point pressure was applied to Shenmen (TF4), Pizhixia (AT4), Xin (CO15), Pi (CO13), Wei (CO4) on both sides alternatively, twice a week. The duration of treatment was 2 months for the two groups. In the control group, the auricular point pressure was simply used with the same points, manipulation and treatment duration as the observation group. After treatment, the clinical therapeutic effects were observed in the patients of the two groups. The Pittsburgh sleep quality index (PSQI) score was compared before and after treatment in the two groups. The recurrence was followed up in 3 months after the end of treatment. RESULTS: It was observed that 2 cases were dropped out in the observation group, 4 cases in the control group. The dropped out cases were considered to be ineffective. After treatment, the total effective rate was 92.5% (86/93) in the observation group and was 80.5% (70/87) in the control group. The result in the observation group was better than that in the control group (P<0.05). After treatment, PSQI scores were all reduced apparently in the two groups as compared with those before treatment (both P<0.05). The score after treatment and the different value before and after treatment in the observation group were lower than those in the control group (both P<0.05). In 3-month follow-up after treatment, the recurrence rate was 3.2% (3/93) in the observation group, which was lower than 13.8% (12/87) in the control group (P<0.05). CONCLUSION: In the treatment of insomnia of spleen and stomach disharmony pattern, the acupoint catgut-embedding therapy combined with auricular point pressure are superior to the simple application of auricular point pressure in the clinical therapeutic effects, improving sleep quality and reducing the recurrence rate.

ABSTRACT

This paper aims to review the research status of the effect mechanism of catgut-embedding therapy. The literature was retrieved from China Academic Journal Network Publishing Database with the keywords as "catgut-point embedding" "embedding", dated from the database establishment to December 2015. Totally, over 5 000 papers had been collected. It was summarized that TCM effect mechanism of catgut-point embedding therapy includes the effects of needle retaining and embedding, harmonizing yin and yang, balancing zangfu organs, promoting meridian qi, regulating qi and blood, tonifying for the deficiency and reducing for the excess, strengthening the antipathogenic qi and eliminating pathogens. From the point of view of western medicine, the effect mechanism of catgut-embedding therapy refers to recovering nerve function, regulating neural reflex, increasing human immunity, improving local circulation, inhibiting the release of inflammatory factors, reducing apoptosis, regulating cellular factor and improving body metabolism. It was found that the effects of catgut-embedding therapy were not only related to the effects induced by common acupuncture, but also to the persistent treatment through prolonged stimulation duration, especially in the treatment of chronic diseases with many systems involved. This therapy reduces or complements to the use of western medicine to certain extent. However, the longitude theoretic study on the catgut-embedding therapy is limited; the specific statement is less predominant and the clinical controlled trial is insufficient at present. Hence, further studies are requested.

ABSTRACT

Objective To compare the clinical efficacy of acupoint catgut-embedding therapy plus fundamental treatment versus fundamental treatment alone for the prevention and treatment of bone marrow depression in breast cancer patients induced by FEC(fluorouracil, epirubicin and cyclophosphamide) chemotherapy. Methods A total of 46 post-operation breast cancer patients accepting the first cycle of FEC chemotherapy were randomly divided into treatment group and control group, 23 cases in each group. The control group was given fundamental treatment including oral use of Chinese medicine of modified Xiangsha Liujunzi Decoction for regulating stomach and arresting vomiting, modified Guipi Decoction and Gui Lu Erxian Decoction for nourishing blood and generating blood, and western medicine of Batilol Tablets and Vitamin B4 Tablets. The treatment group was given acupoint catgut-embedding therapy on bilateral Zusanli(ST36) and Shenshu(BL23) plus fundamental treatment. The effect on bone marrow depression in the two groups was observed after treatment. Results(1) On post-chemotherapy day 7 and 8, the count of white blood cells (WBC) and neutrophils (NE) in the treatment group was higher than that in the control group, the difference being significant(P < 0.05 or P < 0.01).(2) The utilization of granulocyte colony-stimulating factor(G-CSF) in the treatment group was less than that in the control group, the difference being significant (P < 0.05). (3) On post-chemotherapy day 10, the effect on improving bone marrow depression in the treatment group was superior to that in the control group, the difference being significant (P<0.01) . Conclusion The acupoint catgut-embedding therapy plus fundamental treatment is more effective and safe for the prevention and treatment of bone marrow depression in breast cancer patients induced by FEC chemotherapy than fundamental treatment alone.

ABSTRACT

Objective To observe the therapeutic effect of catgut-embedding (CE) therapy based on respiration-induced reinforcing and reducing and electro-acupuncture (EA) therapy in treating simple obesity with spleen deficiency and dampness retention. Methods Sixty simlpe obesity patients with spleen deficiency and dampness retention were randomized into CE group and EA group, 30 cases in each group. The acupoints selected for the two groups were the same, and the points were Zhongwan, Shuifen, Qihai, Guanyuan, Tianshu, Liangmen, Daheng, Fujie, Quchi, Xuehai, Yinlingquan, Fenglong, and Ashi. CE group was given CE therapy with the needling for CE therapy referred to the respiration-induced reinforcing and reducing method, and EA group was given EA therapy for 2 continuous treatment courses, 4 weeks constituting one course. Body mass and body mass index (BMI) of the two groups before and after treatment were observed, and the clinical efficacy was also evaluated after treatment. Results (1) After treatment for 2 courses, body mass and BMI of the two groups were obviously decreased(P 0.05). (2) The total effective rate of CE group was 90.0% and that of EA group was 86.7%, and the difference between the two groups was insignificant (P > 0.05). Conclusion The therapeutic effect of CE therapy based on respiration-induced reinforcing and reducing in treating simple obesity with spleen deficiency and dampness retention is similar to that of EA therapy, and the patients can choose anyone of them for loosing body weight according to the preference.

ABSTRACT

Objective] To investigate the effect and safety of acupoint catgut embedding therapy on treating rheumatoid arthritis. [Method] Using the randomized controlled method, 87 RA patients were randomly divided into treatment group and control group, the treatment group 45 cases, control group 42 cases. Treatment group with acupuncture point bury line therapy, basic acupoints:Shenshu(BL23), Ganshu(BL18), Dazhu(BL 11),Pishu(BL 20),Geshu(BL 17);With holes according to the patients with lesion to find acupoints. Control group to methotrexate 15 mg per week, folic acid 0.4 mg/qd, prednisone acetate tablets 10 mg/qd, a total of eight weeks after treatment in both groups, evaluation of two groups of patients after treatment of joint disease activity score(DAS-28), long-term efficacy and safety. [Results] At the end of the two groups of patients, treatment after the end of March DAS-28 score was improved significantly, the difference was statistically significant(P0.05 or higher). In six months after the treatment, the treatment way of DAS-28 score<2.6 points in 29 cases, 19 cases in the control group patients DAS-28 score<2.6 points, the long-term clinical curative effect comparison of two groups of patients, the difference was statistically significant(χ2=4.53,P=0.033<0.05), long curative effect in clinical treatment group was better than the control group. The treatment group adverse reactions occurred in 2 cases, control group for 9 cases, two groups of comparisons difference statistically significant(χ2=5.49,P=0.019<0.05), treatment group security was better than the control group. [Conclusion] Acupoint catgut embedding therapy can improve clinical symptoms in patients with RA, and the forward curative effect and adverse reactions were superior to drug therapy, providing more choices for clinical treatment of RA.

ABSTRACT

Objective To observe expression levels of N-methyl-D-aspartate (NMDA) receptor and cholecystokinin (CCK) in the hippocampus and spinal cord in morphine withdrawal or tolerance mice treated by electroacupuncture or catgut embedding and explore the difference between the regulating effects of electroacupuncture and catgut embedding on morphine withdrawal and tolerance.Methods Fifty-six male C57BL/6J mice were randomly allocated to withdrawal control, withdrawal model, withdrawal catgut embedding and withdrawal electroacupuncture groups, and tolerance control, tolerance model, tolerance catgut embedding and tolerance electroacupuncture groups, 7 mice in each group. A model of morphine withdrawal was made by subcutaneous injection of morphine hydrochloride using 7-day increasing addiction method. The withdrawal control group was injected with an equal volume of normal saline at the same time points. In the withdrawal electroacupuncture group, electroacupuncture at bilateral points Shenshu was performed using a Han’s acupoint nerve stimulation device (HANS-200) at 15 min after an injection of morphine hydrochloride. In the withdrawal catgut embedding group, 0.5 cm chromic catgut was embedded in bilateral points Shenshu at 15 min after an injection of morphine hydrochloride. Addiction was promoted by intraperitoneal injection of naloxone 4 mg/kg at 10 o’clock on the seventh day’s morning and Withdrawal reactions were observed in the mice. The score was recorded using the Ryuta Tomoji opioid withdrawal symptoms evaluation scale. NMDA receptor and CCK contents in the hippocampus and spinal cord were measured by enzyme-linked immunosorbent assay (ELISA). A model of morphine tolerance was made by subcutaneous injection of morphine 10 mg/kg. The tolerance control group was injected with tolerance normal saline 10 ml/kg at the same time. In the tolerance catgut embedding group, catgut was embedded in point Shenshu at the first day after model making. In the tolerance electroacupuncture group, point Shenshu was given electroacupuncture at the first day after model making. After seven days of treatment, NMDA receptor and CCK contents in the hippocampus and spinal cord were measured by ELISA.Results There were statistically significant differences in hippocampal NR2B and CCK expressions between the withdrawal model and withdrawal control groups (P<0.05). There was a statistically significant difference in hippocampal NR2B expression between the withdrawal electroacupuncture and withdrawal model groups (P<0.05). There was a statistically significant difference in hippocampal CCK expression between the withdrawal catgut embedding or withdrawal electroacupuncture group and the withdrawal model group (P<0.05). There were statistically significant differences in spinal cord NR2A, NR2B and CCK expressions between the withdrawal model and withdrawal control groups (P<0.05). There were statistically significant differences in spinal cord NR2A and NR2B expressions between the withdrawal electroacupuncture and withdrawal model groups (P<0.05). There were statistically significant differences in hippocampal NR2A, NR2B and CCK expressions between the tolerance model and tolerance control groups (P<0.05). There was a statistically significant difference in hippocampal CCK expression between the tolerance catgut embedding and tolerance model groups (P<0.05). There was a statistically significant difference in hippocampal NR1 expression between the tolerance electroacupuncture group and the tolerance model or tolerance catgut embedding group (P<0.05). There was a statistically significant difference in spinal cord CCK expression between the tolerance catgut embedding or withdrawal electroacupuncture group and the tolerance model group (P<0.05).Conclusions Both catgut embedding and electroacupuncture at point Shenshu have a reducing effect on morphine tolerance and withdrawal. The therapeutic effect of electroacupuncture is better than that of catgut embedding.

ABSTRACT

Objective To evaluate the clinical efficacy of facial acupoint catgut embedding therapy in the treatment of skin aging.Methods The sixty patients with facial skin aging were randomly divided into catgut embedding group and sham embedding group,30 cases each.Catgut embedding group received catgut embedding therapy; the control group also received acupuncture but not the filling line.Two groups were operated 2 times,once every 21 days,a total of 42 days.The facial skin aging were evaluated and analyzed using VISIA and CK complexion analysis system.Results In catgut embedding group,wrinkles,pores,texture,moisture and elasticity were improved signficantly (P<0.05).Ultraviolet,pigmentation,telangiectasia and melasma were not obviously improved.There was no significant change in the sham embedding group.Conclusions Facial acupoint catgut embedding therapy is an effective method for the treatment of skin aging with no significant side effects.

ABSTRACT

Objective: To observe the effect of acupoint catgut embedding therapy in treating postpartum obesity. Methods: According to the clinical standard, observing the effect of reducing weight through 120 postpartum obesity patients by the therapy of acupoint catgut embedding. Results: The follow-up observation showed that the result was satisfied. The total effective rate was 91.67%. Conclusion: The acupoint catgut embedding therapy played a positive role in reducing weight of the postpartum obesity patients. This therapy can achieve reducing weight well by enhancing the physical body, regulating endocrine, restraining appetite, and accelerating fat metabolism.
