ABSTRACT

OBJECTIVE: This study describes the frequency of obstetric anal sphincter injuries (OASIS) in patients after instrumental delivery according to the type of forceps used. METHODS: A retrospective comparative cohort study was conducted on patients who underwent instrumental delivery from January 2017 to April 2022. The primary outcome was the presence of OASIS following delivery. Patients were categorized into Cohort A if only rotation forceps were used, Cohort B for only traction forceps, and Cohort C if both types were used sequentially. Statistical analysis was performed with SPSS (IBM, New York, NY) with χ2, Fisher's exact, and analysis of variance testing. A P-value <0.05 was considered significant. RESULTS: OASIS occurred in 45 of 328 instrumental deliveries. OASIS after rotation forceps occurred in 12.9% (n = 8) of cases, after traction forceps in 13.2% (n = 34), and after sequential use of rotation and traction forceps in 37.5% (n = 3) of cases (p = 0.141). An odds ratio (OR) of 0.91 (95% confidence interval [CI] 0.40-2.08) for OASIS was obtained with the use of rotation forceps, 0.81 (95% CI 0.38-1.70) for traction forceps, and 3.97 (95% CI 0.91-17.2) for the sequential use of rotation and traction forceps. CONCLUSION: There were no significant differences in the presence of OASIS comparing traction and rotation forceps. A non-significant trend of higher OASIS following the sequential use of traction and rotation forceps was observed.

ABSTRACT

Introducción: El vacuum es el instrumento de elección para asistir el parto instrumental en países desarrollados. Objetivo: El objetivo del estudio es exponer los resultados maternos y fetales del uso del vacuum en población chilena y promover su disponibilidad y uso en nuestro país y Latinoamérica. Método: El estudio se realizó entre octubre de 2017 y diciembre de 2021, incluyendo a pacientes que tuvieron parto instrumental con vacuum en el Hospital San Juan de Dios de Chile. Se compararon los partos exitosos con vacuum y los partos en los que se requirió el uso de otro instrumento o una cesárea posterior al uso del instrumento. Resultados: Se realizaron 153 vacuum, correspondientes al 28,59% del total de partos instrumentales. El 79,74% tuvo éxito. El 5,88% no recibió anestesia o se aplicó anestesia local. Solo el 3,27% presentó desgarro perineal con compromiso del esfínter anal. El 91,5% de los recién nacidos no tuvieron complicaciones fetales. Un 20,3% requirió reanimación neonatal. Conclusiones: El estudio muestra que el vacuum es una opción segura y efectiva de parto instrumental en nuestra población. La correcta capacitación y protocolos adecuados son necesarios para garantizar resultados exitosos.


Introduction: Vacuum extraction is the instrument of choice for assisting instrumental delivery in developed countries. Objective: The aim of the study is to present the maternal and fetal outcomes of vacuum use in the Chilean population and promote its availability and usage in our country and Latin America. Method: The study was performed between October 2017 and December 2021, including patients who underwent instrumental delivery with a vacuum extraction at the Hospital San Juan de Dios in Chile. Successful vacuum deliveries were compared with deliveries where another instrument was required, or a cesarean section was performed after instrument use. Results: A total of 153 vacuum deliveries were carried out, accounting for 28.59% of all instrumental deliveries. 79.74% were successful. 5.88% did not receive anesthesia or received local anesthesia. Only 3.27% experienced perineal tear with involvement of the anal sphincter. 91.5% of newborns had no fetal complications. 20.3% required neonatal resuscitation. Conclusions: The study demonstrates that the vacuum is a safe and effective option for instrumental delivery in our population. Proper training and appropriate protocols are necessary to ensure successful outcomes.

ABSTRACT

OBJECTIVE: To describe the feasibility of an ultrasound-guided repositioning technique for partially expelled intrauterine devices (IUDs) without use of sedation. METHODS: This was a descriptive feasibility study of patients with a partially expelled IUD managed in our outpatient clinic from January 2016 to February 2020. The partially expelled IUDs (vertical arm extending partially or entirely through the cervical canal) were repositioned at the uterine fundus using Hartmann alligator forceps under ultrasound guidance. Paracervical or intracervical anesthesia and prophylactic antibiotics were not used. Data related to the procedure and 6-month follow-up were extracted from patient medical records. The primary outcome was the success rate of the repositioning procedure, defined as ultrasound confirmation of the entire IUD located above the internal os. Secondary outcomes included the retention and expulsion rates of the repositioned IUD at 6 months after the procedure and description of complications. RESULTS: We included data from 55 women with a partially expelled IUD (35 levonorgestrel IUDs and 20 copper IUDs) referred for repositioning. Ultrasound-guided repositioning of the IUD was successful in 51 (92.7%) cases, while the procedure was not completed in four patients due to pain. Of the 55 procedures, 48 (87.3%) were performed by obstetrics and gynecology trainees under the supervision of a senior specialist. Among the 51 successfully repositioned IUDs, nine (17.6%) were expelled within 6 months after the procedure and six patients were lost to follow-up. No uterine perforation or infection-related complications occurred within 6 months of the procedure. CONCLUSION: The ultrasound-guided repositioning technique appears to be a safe and feasible approach for partially expelled IUDs. © 2022 International Society of Ultrasound in Obstetrics and Gynecology.

Resumen OBJETIVO: Diseñar y validar un instrumento para evaluar el nivel de conocimiento en la aplicación del fórceps Salinas en médicos residentes de Ginecología y Obstetricia. MATERIALES Y MÉTODOS: Estudio descriptivo, transversal, tipo validación de un instrumento. El primer constructo se desarrolló con base en el marco teórico; el cuestionario fue objeto de análisis por parte de un grupo de expertos que se basaron en la metodología Delphi. Se obtuvo un constructo de 20 preguntas y se emprendió un estudio para evaluar la pertinencia del instrumento. La confiabilidad se evaluó con el coeficiente de Kuder-Richardson, prueba Kaiser-Meyer-Olkin y de esfericidad de Bartlett. Los datos se procesaron en el programa SPSS versión 21. RESULTADOS: El instrumento se aplicó a 66 médicos residentes con media de edad de 28.5 ± 0.7 años. El coeficiente de Kuder-Richardson fue 0.608. Las pruebas de Kaiser-Meyer-Olkin y Bartlett mostraron que se trató de un instrumento multidimensional. CONCLUSIONES: Se obtuvo un instrumento válido, confiable y multidimensional para determinar el nivel de conocimientos en relación con la aplicación del fórceps Salinas en médicos residentes de Ginecología y Obstetricia.


Abstract OBJECTIVE: To design and validate an instrument to assess the level of knowledge of Salinas forceps application in Gynecology and Obstetrics residents. MATERIALS AND METHODS: Descriptive, cross-sectional, validation-type study of an instrument. The first construct was developed based on the theoretical framework; the questionnaire was analyzed by a group of experts based on the Delphi methodology. A construct of 20 questions was obtained and a study was undertaken to assess the relevance of the instrument. Reliability was evaluated with the Kuder-Richardson coefficient, Kaiser-Meyer-Olkin test and Bartlett's sphericity test. The data were processed in SPSS version 21. RESULTS: The instrument was applied to 66 resident physicians with a mean age of 28.5 ± 0.7 years. The Kuder-Richardson coefficient was 0.608. The Kaiser-Meyer-Olkin and Bartlett tests showed that it was a multidimensional instrument. CONCLUSIONS: A valid, reliable and multidimensional instrument was obtained to determine the level of knowledge in relation to the application of Salinas forceps in Gynecology and Obstetrics residents.

Objetivos: evaluar la frecuencia del parto instrumentado en Colombia y por regiones entre el 2015 y 2019. Materiales y métodos: estudio de corte transversal a partir de registros poblacionales. Se incluyeron los registros de mujeres con edad gestacional mayor a 28 semanas y parto vaginal. Muestreo consecutivo. La información se obtuvo a partir del certificado de nacido vivo del Departamento Administrativo Nacional de Estadística (DANE). Se realizó la descripción de las variables sociodemográficas y clínicas. La frecuencia del parto instrumentado fue calculada y descrita por año y departamento. Resultados: se incluyeron 3.224.218 registros de recién nacidos vivos. De estos, 1.719.405 (53,33 %) correspondieron a partos vaginales, y 1.468.726 (45,55 %) a partos por vía cesárea. La frecuencia nacional del parto instrumentado entre el 2015 y 2019 fue de 36.087 nacimientos (1,11 %); Antioquia y Bogotá, D.C. fueron los lugares con mayor ocurrencia, 16.201 (4,5 %) y 13.686 (2,52 %), respectivamente. Conclusiones: el parto vaginal instrumentado es la vía de parto con menor ocurrencia en Colombia con tendencia a la disminución. La formación en este recurso para el uso durante el trabajo de parto no debe perderse en la formación del profesional de salud, especialmente en los programas de Ginecología y Obstetricia. Se debe evaluar si el aumento en el adecuado uso de esta técnica puede disminuir la tasa de cesáreas observadas actualmente, así como describir los escenarios clínicos donde su uso es seguro para la madre y el feto. Se requieren estudios prospectivos, pues pueden permitir establecer las causas del descenso en el uso de esta herramienta obstétrica, los resultados maternos y perinatales en términos de su riesgo/beneficio.

Prospective studies are required to identify the causes leading to the lower use of this obstetric tool as well as the risks and benefits in terms of maternal and perinatal outcomes.

BACKGROUND: Operative vaginal birth represents an alternative to address problems during the second stage of labor. Obstetricians have access to three different instruments: forceps, vacuum, and spatulas. OBJECTIVE: To evaluate the safety and effectiveness of the use of spatulas for operative vaginal birth. SEARCH STRATEGY: MEDLINE/PubMed, Embase, CENTRAL, LILACS, SciELO, ClinicalTrials.gov, and OpenGrey. SELECTION CRITERIA: Randomized controlled studies and non-randomized controlled studies (NRS) were included. DATA COLLECTION AND ANALYSIS: Studies were independently assessed for inclusion, data extraction, and risk of bias. MAIN RESULTS: Nine NRS (n = 16  497 women) compared the use of spatulas versus forceps. Low-certainty evidence suggests that spatulas were not different in terms of failed operative vaginal delivery rate (odds ratio [OR] 0.89, 95% confidence interval [CI] 0.53-1.52), but may decrease the incidence of birth canal trauma (OR 0.70, 95% CI 0.54-0.91), birth canal laceration (OR 0.50, 95% CI 0.28-0.91), and neonatal soft-tissue injury (OR 0.19, 95% CI 0.13-0.29). Six NRS (n = 2992 women) compared the use of spatulas versus vacuum. Low-quality evidence suggests that spatulas may decrease failed operative vaginal delivery rate (OR 0.10, 95% CI 0.04-0.26). There were no apparent differences in other maternal and neonatal outcomes. CONCLUSION: Low-certainty evidence suggests that spatulas could be a safe and effective alternative for operative vaginal birth.

Introduction: Transbronchial lung cryobiopsy (TBCB) has emerged as a diagnostic alternative to surgical lung biopsy mostly in interstitial lung disease. Despite its less invasive nature and reported higher diagnostic yield, some associated complications have been described, such as pneumothorax. Moreover, a comparison between TBCB and transbronchial forceps biopsy is seldomly made. The aim of the present study is to evaluate the incidence of pneumothorax following TBFB and TBCB and the need for pleural drainage. Methods: Retrospective study of patients who underwent transbronchial forceps biopsy or transbronchial lung cryobiopsy, specifically those who developed postoperative pneumothorax. Results: A total of 181 transbronchial lung biopsies were performed. Sixty three (35%) were TBFB and 118 (65%) were TBCB. Three patients in the TBFB group (5%) presented postoperative pneumothorax, while 16 patients (14%) presented pneumothorax in the TBCB group (p 0,051). The univariate analysis revealed a statistically significant association between the preoperative diagnosis of fibrosis and a higher risk of postoperative pneumothorax following TBCB (p 0.027), while other variables did not yield a statistical significance. Conclusion: Even though more high-volume comparative studies are needed, this paper highlights the relevance of pneumothorax following TBCB. This derives in a strong need for clearly standardized procedure protocols for TBCB and careful evaluation of its complications vs. its definitive diagnostic yields.


Introducción: La criobiopsia pulmonar transbronquial (TBCB) se ha convertido en una alternativa diagnóstica a la biopsia pulmonar quirúrgica, principalmente en la enfermedad pulmonar intersticial. A pesar de su naturaleza menos invasiva y de un mayor rendimiento diagnóstico, se han descrito algunas complicaciones asociadas, como el neumotórax. En pocas oportunidades se ha comparado la TBCB y la biopsia transbronquial con fórceps (TBFB). El objetivo del presente estudio es evaluar la incidencia de neumotórax después de TBFB y TBCB y la necesidad de drenaje pleural. Métodos: Estudio retrospectivo de pacientes que se sometieron a TBCB y TBFB, específicamente aquellos que desarrollaron neumotórax postoperatorio. Resultados: Se realizaron un total de 181 biopsias pulmonares transbronquiales. Sesenta y tres (35%) fueron TBFB y 118 (65%) fueron TBCB. Tres pacientes en el grupo TBFB (5%) presentaron neumotórax postoperatorio, mientras que 16 pacientes (14%) presentaron neumotórax en el grupo TBCB (p 0,051). El análisis univariado reveló una asociación estadísticamente significativa entre el diagnóstico preoperatorio de fibrosis y un mayor riesgo de neumotórax postoperatorio después de TBCB (p 0.027), mientras que otras variables no arrojaron un resultado significativo. Conclusión: Aunque se necesitan más estudios comparativos de alto volumen, este documento destaca la relevancia del neumotórax después de la TBCB. Esto deriva en una fuerte necesidad de protocolos de procedimientos claramente estandarizados para TBCB y una evaluación cuidadosa de sus complicaciones versus su rendimiento diagnóstico

OBJECTIVES: To determine the association between cesarean delivery and childhood infections up to 13 years of age. STUDY DESIGN: We conducted a longitudinal cohort study of 731 803 children born between 2006 and 2016 at all hospitals in the province of Quebec, Canada. We followed children born by cesarean, operative vaginal, and nonoperative vaginal delivery up to 13 years of age. Outcomes included hospitalization for otitis media, respiratory, infectious enteritis, and other infections. We estimated hazard ratios with 95% CIs for the association between mode of delivery and childhood infections, adjusted for patient characteristics. RESULTS: At age 3-4 years, cesarean delivery was associated with a 1.07-fold greater risk of otitis media (95% CI, 1.03-1.11), a 1.15-fold greater risk of respiratory infection (95% CI, 1.09-1.22), and a 1.13-fold greater risk of infectious enteritis (95% CI, 1.03-1.25) compared with nonoperative vaginal delivery. However, operative vaginal delivery was associated with these same outcomes. Both cesarean and operative vaginal delivery were more strongly associated with infection hospitalization before age 1 year, but associations disappeared after 5 years. CONCLUSIONS: Cesarean delivery is associated with infection hospitalization before but not after age 5 years. However, associations were also present for operative vaginal delivery, which suggests that mechanisms other than exposure to maternal vaginal flora explain the relationship.

Objetivo: No decorrer dos séculos, o parto migrou do ambiente domiciliar para o hospitalar, sendo então repleto de procedimentos que, embora tenham o intuito de ajudar, nem sempre são adequados ou baseados em evidências. Por isso, o objetivo deste estudo é identificar os procedimentos realizados com parturientes durante o parto em uma maternidade do Tocantins, além de caracterizar o perfil dessas pacientes. Método: Foi aplicado um questionário a 70 puérperas de parto normal, maiores e menores de idade, durante quatro meses. O questionário de referência foi o utilizado do estudo Nascer no Brasil. Resultados: A maioria das pacientes se autodeclarou parda, tinha ao menos o ensino médio completo e era maior de 18 anos. Mais da metade delas tiveram alguma alteração no períneo, 25% sofreram manobra de Kristeller e 88% submeteram-se à litotomia. Além disso, a grande maioria avaliou o serviço da maternidade como bom/ótimo/excelente. Conclusão: A assistência ao parto no Tocantins ainda se divide em práticas adequadas e técnicas ultrapassadas. Trata-se de um estudo original e um dos primeiros nesse sentido realizado no estado mais novo do Brasil.(AU)


Objective: Throughout the centuries, childbirth has migrated from the home environment to the hospital, being then full of procedures that although they are intended to help, are not always adequate or based on evidence. Therefore, the objective of this study is to identify the procedures performed with parturients during childbirth in a maternity hospital in Tocantins, in addition to characterizing the profile of these patients. Method: A questionnaire was applied to 70 mothers of normal birth, older and younger, for four months. The reference questionnaire was used in the Nascer no Brasil study. Results: Most patients declared themselves to be brown, had at least completed high school, and were over 18 years old. More than half of them had some alteration in the perineum, 25% underwent a Kristeller maneuver and 88% underwent lithotomy. In addition, the vast majority rated the maternity service as good/excellent/excellent. Conclusion: Assistance to childbirth in Tocantins is still divided into outdated technical and appropriate practices. This is an original study and one of the first in this sense carried out in the newest state of Brazil.(AU)

ABSTRACT

Introducción: Los obstetras siempre han tratado de sujetar la cabeza del feto de la manera más segura posible para acelerar el parto y reducir el difícil trabajo de la mujer. La historia del uso de instrumentos obstétricos para facilitar el parto (ya sean fórceps o espátulas) es una parte pintoresca y particular de la historia de la medicina. Objetivos: Identificar la experiencia adquirida por los residentes en la instrumentación obstétrica. Métodos: Para cumplir los objetivos propuestos se revisó el plan temático de la residencia de Ginecobstetricia y posteriormente fueron seleccionados los aspectos referidos al tema en estudio. Resultados: Los resultados encontrados a partir de las encuestas realizadas y los datos aportados por los tutores de formación y la revisión de los libros de partos aportaron el nivel de aprendizaje de los residentes. Conclusiones: Se aprecia en los estudiantes demora en la aprehensión de los conocimientos relacionado con la instrumentación obstétrica(AU)


Introduction: Obstetricians have always tried to hold the fetus head as safely as possible to accelerate delivery and reduce the women's difficult work. The history of usage of obstetric instruments to facilitate labor (whether forceps or spatulas) is a colorful and particular passage in the history of medicine. Objectives: To identify the residents' acquired experience regarding the obstetric instrumentation, for which we decided to determine their received practical theoretical training. Methods: In order to meet the proposed objectives, we reviewed the system of contents for the Gynecology residence and, thereafter, we selected the aspects regarding the subject under study. Results: The results obtained from the surveys conducted and the data provided by the training professors and the review of delivery books contributed to the residents' level of learning. Conclusions: The students show delay in the apprehension of knowledge regarding obstetric instrumentation(AU)

Resumen OBJETIVO: Analizar los factores de riesgo asociados con lesión obstétrica del esfínter anal en una población donde la aplicación de fórceps es bien aceptada. MATERIALES Y MÉTODOS: Estudio de cohorte simple, retrospectivo, al que que se incluyeron pacientes que finalizaron el embarazo por parto, entre enero de 2013 y diciembre de 2017, en el Instituto Nacional de Perinatología Isidro Espinosa de los Reyes. Criterios de inclusión: embarazo mayor de 35 semanas, con feto único y en presentación cefálica. La variable dependiente de lesión obstétrica del esfínter anal se clasificó en tercer y cuarto grado. Para el análisis estadístico se utilizó el programa SPSS Statistics 23; la prueba de χ2 y t de Student para la comparación entre grupos, y razón de momios con intervalo de confianza de 95%. RESULTADOS: Se incluyeron 6199 pacientes. La edad promedio fue de 26.5 años; el peso al nacimiento de 2745 g y se colocaron fórceps en 6.9% (n = 428) casos. La frecuencia de lesión obstétrica del esfínter anal fue de 1.03% (n = 65). Entre los factores de riesgo asociados con lesión del esfínter anal se encontraron: aplicación de fórceps (RM 22.4; IC95% 13.3-36.7, p < 0.001) y primiparidad (RM 2.8; IC95% 1.68-4.93, p < 0.001). CONCLUSIONES: La edad materna de 20 a 30 años, el peso fetal mayor de 3500 g, el parto con fórceps y la primiparidad son factores de riesgo asociados con lesión obstétrica del esfínter anal.


Abstract OBJECTIVE: To analyze the associated risk factors of obstetric anal sphincter injury in a population in which the use of forceps is well accepted. MATERIALS AND METHODS: A simple retrospective cohort was performed where all women with vaginal delivery were identified in a period of 5 years (January 2013 to December 2017). Pregnancies older than 35 weeks were included, unique and in cephalic presentation. The dependent variable of obstetric lesion of the anal sphincter was classified in 3rd and 4th grade. The SPSS 23 program was used for the statistical analysis. χ2 test and Student's t test were used to compare the groups, as well as Odds radio (OR) with a 95% confidence interval (95%CI). RESULTS: A total of 6199 women were included. The average age was 26.5 years, birth weight of 2745 grams on average, forceps were placed in 6.9% (n = 428). The frequency of obstetric lesion of the anal sphincter was 1.03% (n = 65) in the study period. The following risk factors were found to be associated with anal sphincter injury: use of forceps OR 22.4 (95% CI 13.3-36.7, p <0.001), primiparity OR 2.8 (95% CI 1.68-4.93 p < 0.001. CONCLUSIONS: Maternal age of 20 to 30 years, fetal weight greater than 3500 grams, births with forceps and primiparity are risk factors for obstetric anal sphincter injuries.

Resumen Introducción: tradicionalmente, los pólipos colónicos se miden empíricamente por estimación visual y con las pinzas de biopsia, aunque dichos métodos son inexactos. Diferentes métodos han sido investigados, pero no tienen la exactitud esperada. Por lo anterior, se realizó este trabajo para construir un algoritmo que permitiera medir los pólipos a partir de fotografías tomadas con el colonoscopio. Materiales y métodos: el trabajo se realizó en tres fases. En la primera, se construyó un algoritmo en el programa MATLAB. Se capturaron fotos en formato JPG con el colonoscopio. En la segunda fase, con el algoritmo se midieron imágenes de objetos con tamaños conocidos para verificar la exactitud del algoritmo. Después de verificar la exactitud, fue sometida al algoritmo la fotografía de los pólipos del colon. En la tercera fase, se utilizaron imágenes de pólipos previamente enviadas a tres expertos. Todas las fotografías fueron tomadas con el colonoscopio Olympus Exera II. Resultados: en los objetos menores de 5 mm, el algoritmo sobreestimó el tamaño entre 0,11 y 0,08 mm; en los mayores de 5 mm, sobreestimó el tamaño entre 0,25 mm y 1,76 mm en los de 22 mm. Los expertos sobrestimaron los tamaños de manera importante. En los pólipos de 7, 8 y 9 mm, los expertos dijeron que medían 12, 15 y 18 mm, respectivamente. Conclusión: el algoritmo desarrollado tiene adecuada exactitud para medir pólipos colónicos. Por su fácil consecución y utilización, podría ser una herramienta para solucionar la dificultad de medir pólipos durante una colonoscopia.


Abstract Introduction: Traditionally, colon polyps' measurements have been empirically estimated visually and with biopsy forceps, but neither method is inaccurate. Other methods have been studied but have not had the accuracy expected. The research reported here was undertaken to address this issue by building an algorithm for measure polyps from photographs taken through a colonoscope. Materials and methods: The study was done in three phases. First, an algorithm was built in MATLAB, and photos taken with a colonoscope were stored in the JPG format. In the second phase, images of objects with known sizes were checked against the algorithm to verify its accuracy. After verification of the algorithm's accuracy, photographs of colon polyps were measured using the algorithm. In phase 3, images of polyps previously sent to three experts were measured with the algorithm. All photographs were taken with an Olympus Exera II Colonoscope. Results: For objects smaller than 5 mm, the algorithm overestimated sizes by 0.11 to 0.08 mm. For those greater than 5 mm, it overestimated sizes by 0.25 mm to 1.76 mm in those of 22 mm. The experts seriously overestimated sizes. They estimated that 7 mm polyps measured 12 mm, 8 mm polyps measured 15 mm, and 9 mm polyps measured 18mm. Conclusion: The algorithm developed is sufficiently accurate for measuring colon polyps and is easy to obtain and relatively easy to use. It could become a tool for overcoming the difficulty of measuring polyps during a colonoscopy.

The recognition of inferior vena cava filter related complications has motivated increased attentiveness in clinical follow-up of patients with inferior vena cava filters and has led to development of multiple approaches for retrieving filters that are challenging or impossible to remove using conventional techniques. Endobronchial forceps and excimer lasers are tools for designed to aid in complex inferior vena cava filter removals. This article discusses endobronchial forceps-assisted and excimer laser-assisted inferior vena cava filter retrievals.

OBJECTIVES: To establish the prevalence of external (EAS) and internal (IAS) anal sphincter defects present 15-24 years after childbirth according to mode of delivery, and their association with development of fecal incontinence (FI). The study additionally aimed to compare the proportion of women with obstetric anal sphincter injuries (OASIS) reported at delivery with the proportion of women with sphincter defect detected on ultrasound 15-24 years later. METHODS: This was a cross-sectional study including 563 women who delivered their first child between 1990 and 1997. Women responded to a validated questionnaire (Pelvic Floor Distress Inventory) in 2013-2014, from which the proportion of women with FI was recorded. Information about OASIS was obtained from the National Birth Registry. Study participants underwent four-dimensional transperineal ultrasound examination. Defect of EAS or IAS of ≥ 30° in at least four of six slices on tomographic ultrasound was considered a significant defect and was recorded. Four study groups were defined based on mode of delivery of the first child. Women who had delivered only by Cesarean section (CS) constituted the CS group. Women in the normal vaginal delivery (NVD) group had NVD of their first child and subsequent deliveries could be NVD or CS. The forceps delivery (FD) group included women who had FD, NVD or CS after FD of their first born. The vacuum delivery (VD) group included women who had VD, NVD or CS after VD of their first born. Multiple logistic regression was used to calculate adjusted odds ratios (aORs) for comparison of prevalence of an EAS defect following different modes of delivery and to test its association with FI. Fisher's exact test was used to calculate crude odds ratios (ORs) for IAS defects. RESULTS: Defects of EAS and IAS were found after NVD (n = 201) in 10% and 1% of cases, respectively, after FD (n = 144) in 32% and 7% of cases and after VD (n = 120) in 15% and 4% of cases. No defects were found after CS (n = 98). FD was associated with increased risk of EAS defect compared with NVD (aOR = 3.6; 95% CI, 2.0-6.6) and VD (aOR = 3.0; 95% CI, 1.6-5.6) and with increased risk of IAS defect compared with NVD (OR = 7.4; 95% CI, 1.5-70.5). The difference between VD and NVD was not significant for EAS or IAS. FI was reported in 18% of women with an EAS defect, in 29% with an IAS defect and in 8% without a sphincter defect. EAS and IAS defects were associated with increased risk of FI (aOR = 2.5 (95% CI, 1.3-4.9) and OR = 4.2 (95% CI, 1.1-13.5), respectively). Of the ultrasonographic sphincter defects, 80% were not reported as OASIS at first or subsequent deliveries. CONCLUSIONS: Anal sphincter defects visualized on transperineal ultrasound 15-24 years after first delivery were associated with FD and development of FI. OBJECTIVE: We aimed to determine whether a threshold number of forceps deliveries in residency predicts use of forceps in independent practice. STUDY DESIGN: We surveyed obstetrics and gynecology residency graduates of 2 academic programs from 2008 through 2012 regarding the use of operative vaginal delivery in practice. At these programs, residents are trained in both forceps and vacuums. Individual case log data were obtained with the number of forceps deliveries performed by each respondent during residency. Respondents were grouped as currently using any forceps or vacuums alone. A logistic regression model estimated the probability of forceps use, predicted by the number of residency forceps deliveries. From the resulting receiver-operating characteristic curve, we assessed sensitivity, specificity, positive predictive value, and area under the curve. RESULTS: The response rate was 85% (n = 58) and 90% (n = 52) practice obstetrics. Seventy-nine percent (n = 41) use forceps in practice. The mean number of forceps performed during residency was 22.3 ± 1.3 (mean ± SE) in the any-forceps group and 18.5 ± 2.1 in the vacuums-only group (P = .14). Although the model performed only moderately (area under the curve, 0.61, 95% confidence interval [CI], 0.42-0.81), more than 13 residency forceps deliveries corresponded to a 95% sensitivity (95% CI, 84-99) and a positive predictive value of 83% (95% CI, 69-92) for using forceps in practice. The specificity of this threshold is 27% (95% CI, 6-61). CONCLUSION: Although exceeding 13 forceps deliveries made it highly likely that obstetricians would use them in practice, further study is necessary to set goals for a number of resident forceps deliveries that translate into use in practice.

Los trastornos del piso pélvico son una condición que afecta al 30% de las mujeres de todas las edades, e incluyen un abanico de patologías anatómicas y funcionales que, aunque no alteran la supervivencia de las pacientes, sí afectan en gran medida su calidad de vida. Dentro de estas patologías encontramos la incontinencia urinaria y fecal, el prolapso de órganos pélvicos y la disfunción sexual. La importancia del tema radica en que los trastornos del piso pélvico son más frecuentes después del embarazo y el parto por los cambios inherentes al embarazo mismo y por el trauma obstétrico que son factores de riesgo conocidos y que pueden estar asociados con secuelas graves del piso pélvico. Es por ello que se deben prever posibles complicaciones en el trabajo de parto que puedan aumentar el riesgo de trastornos del piso pélvico e implementar ciertas estrategias de prevención y de intervenciones seguras, que minimicen los daños y las secuelas que se producen en el largo plazo. A continuación haremos una revisión de la literatura sobre los cambios fisiológicos del embarazo en el piso pélvico, los trastornos disfuncionales más frecuentes, el papel del parto intervenido en la aparición de estos trastornos y las estrategias de prevención que pueden implementarse para disminuir los efectos sobre el piso pélvico.


Pelvic floor disorders are a medical condition that affects 30% of women of all ages, and include a plethora of anatomical and functional pathologies that, though they do not alter patient survival, do affect their well-being to a large extent. Urinary and fecal incontinence, pelvic organ prolapse and sexual dysfunction are among these pathologies. The importance of the subject rests on the fact that pelvic floor disorders are more frequent after pregnancy and birth as a result of inherent changes to pregnancy itself and obstetric injuries that are widely known risk factors, which may be associated to severe damage to the pelvic floor. Therefore, possible labor complications that may increase the risk of pelvic floor disorders must be prevented, along with the implementation of certain preventive strategies and safe interventions that reduce damage and possible long-term consequences. Here, we present a literature review about the physiological changes in the pelvic floor during pregnancy, the most frequent dysfunctional disorders, the role of forceps delivery in the appearance of different conditions, and preventive strategies that may be implemented to reduce the effects on the pelvic floor.


Os transtornos do piso pélvico são uma condição que afeta a 30% das mulheres de todas as idades, e incluem um leque de patologias anatómicas e funcionais que, embora não alteram a supervivência das pacientes, sim afetam em grande medida sua qualidade de vida. Dentro destas patologias encontramos a incontinência urinaria e fecal, o prolapso de órgãos pélvicos e a disfunção sexual. A importância do tema radica em que os transtornos do piso pélvico são mais frequentes depois da gravidez e do parto pelas mudanças inerentes à gravidez em si e pelo trauma obstétrico que são fatores de risco conhecidos e que podem estar associados com sequelas graves do piso pélvico. É por isso que se devem prever possíveis complicações no trabalho de parto que possam aumentar o risco de transtornos do piso pélvico e implementar certas estratégias de prevenção e de intervenções seguras, que minimizem os danos e as sequelas que se produzem em longo prazo. A continuação faremos uma revisão da literatura sobre as mudanças fisiológicas da gravidez no piso pélvico, os transtornos disfuncionais mais frequentes, o papel do parto intervindo na aparição destes transtornos e as estratégias de prevenção que podem implementar-se para diminuir os efeitos sobre o piso pélvico.

CONTEXT AND OBJECTIVE: Despite all the medical care provided during delivery labor, perineal injury is still prevalent and may lead to diverse pelvic floor disorders. The aim here was to investigate the prevalence of obstetric and anal sphincter injuries (OASIS) in healthy pregnant women after vaginal delivery. DESIGN AND SETTING: Cross-sectional study involving 3,034 patients with singletons in a secondary hospital for low-risk cases. METHODS: A standardized questionnaire was prepared and applied to medical files that had been completely filled out (classification of the Royal College of Obstetricians and Gynecologists, RCOG) in order to identify OASIS and analyze risk factors associated with mild and severe perineal lacerations. RESULTS: The women's mean age was 25 years; more than half (54.4%) were primiparae. Almost 38% of the participants had perineal lacerations; these were severe in 0.9% of the cases. Previous vaginal delivery (odds ratio, OR: 1.64 [1.33-2.04]) and forceps delivery (OR: 2.04 [1.39-2.97]) were risk factors associated with mild perineal injuries (1st and 2nd OASIS classifications). Only remaining standing for prolonged periods during professional activity (OR: 2.85 [1.34-6.09]) was associated with severe perineal injuries. CONCLUSION: The prevalence of severe perineal injuries was concordant with data in the literature. The variable of standing position was considered to be a risk factor for severe perineal injury and should be further investigated. .


CONTEXTO E OBJETIVOS: Apesar do cuidado médico executado durante o trabalho de parto, os traumas perineais ainda são prevalentes e podem levar a várias desordens do assoalho pélvico. O objetivo foi investigar a prevalência de injúrias obstétricas e do esfíncter anal em mulheres saudáveis após parto vaginal. DESENHO E LOCAL DE ESTUDO: Estudo transversal envolvendo 3.034 pacientes com recém-natos únicos de um hospital secundário de baixo risco. MÉTODOS: Um questionário padronizado foi preparado e aplicado aos prontuários completamente preenchidos (classificação do Royal College of Obstetricians and Gynecologists) para identificar as lesões obstétricas e do esfíncter anal e analisar fatores de risco associados com lacerações perineais leves e graves. RESULTADOS: A média de idade das mulheres era 25 anos; mais da metade (54,4%) era primípara. Quase 38% das participantes tiveram lacerações perineais; estas foram graves em 0,9% dos casos. A presença de parto vaginal prévio (odds ratio, OR, 1,64 [1,33-2,04]) e o parto fórceps (OR 2,04 [1,39-2,97]) foram fatores de risco associados às lesões perineais leves (primeira e segunda classificações de lesão esfíncter e anal). Somente a posição em pé prolongada durante a atividade profissional (OR 2,85 [1,34-6,09]) estava associada com lesões perineais graves. CONCLUSÃO: A prevalência de trauma perineal grave concordou com dados da literatura. A variável posição em pé foi considerada fator de risco para trauma perineal grave e necessita ser investigada. .

ABSTRACT

Ankylosis of the temporomandibular joint following forceps delivery is a rare anomaly. The aetiopathogenesis involves wrongful application of the forceps or forceful closure of the forceps handle against the condyle with haemathrosis, organisation and subsequent ankylosis. Because of the lack of epidemiological data, there is little information about the true incidence and the management of this rare anomaly. The purpose ofthis presentation is to report the challenges encountered in the airway management of a six-year old female with right temporomandibular joint ankylosis following forceps delivery in a private hospital setting.


La anquilosis de la articulacion temporomandibular tras el uso de forceps siguientes es una rara anomalia. La etiopatogenesis implica la aplicacion indebida de los forceps o el cierre forzoso del mango de los forceps contra el condilo con hemartrosis, organizacion y subsiguiente anquilosis. Debido a lafalta de datos epidemiologicos, hay poca informacion acerca de la verdadera incidencia y el tratamiento de esta rara anomalia. El objetivo de esta presentacion es informar los problemas encontrados en el tratamiento de las vias respiratorias de una niha de seis ahos con anquilosis de la articulacion temporomandibular derecha tras de un parto con forceps en el contexto de un hospital privado.

Objective: The objective of this study was to compare the effects of the modes of delivery on the health of newborns in a private maternity hospital in the city of São Paulo. Methods: Between January 1995 and December 1998, all patients consecutively admitted for deliveries were included in this cross-sectional retrospective study. A total of 8,457 medical records were analyzed, being excluded of the sample 460 multiple pregnancies and 517 pregnant women with obstetric and/or clinical disorders. The incidence of neonatal birth injury, respiratory distress and anoxia was analyzed, as well as birth weight, type of delivery and gestational age (according to Näegele and Capurro). Results: The final sample consisted of 7,480 neonates, and 69.6% were born by cesarean section, 24% vaginally and 6.4% through the vagina with the aid of forceps. A significant association was found between anoxia and the three types of delivery (p < 0.001). Respiratory distress was more frequent in cesarean delivery in newborns with gestational age superior to 37 weeks and in newborns weighing more than or equal to 2,500 g. Respiratory distress was significantly associated with cesarean delivery and/or forceps delivery, as compared with vaginal delivery, in the entire sample. Neonatal birth injury was associated with the use of forceps. In neonates born by cesarean section, anoxia was associated with lower gestational age estimated by the Capurro method and with lower weight. Conclusions: The abdominal approach is associated with greater morbidity of fetuses due to respiratory distress. Vaginal delivery is safer in newborns with more than 37 weeks of gestation and in those weighing more than 2,500 g.


Objetivo: Analisar comparativamente os efeitos dos tipos de parto sobre a saúde do recém-nascido em maternidade privada da cidade de São Paulo. Métodos: Entre janeiro de 1995 e dezembro de 1998, incluíram-se no estudo transversal retrospectivo todas as pacientes consecutivamente internadas para dar à luz. Foram analisados 8.457 prontuários, excluídas 460 gestações múltiplas e 517 gestantes com afecções obstétricas e/ou clínicas. Foram analisadas as incidências de tocotraumatismo, desconforto respiratório e anoxia, avaliando-se o peso ao nascer, o tipo de parto e a idade gestacional (conforme Näegele e Capurro). Resultados: A amostra final constituiu-se de 7.480 recém-nascidos, sendo que 69,6% nasceram por cesárea, 24% por parto normal e 6,4% com auxílio de fórcipe. Houve associação significativa entre anoxia e os três tipos de parto (p < 0,001). O desconforto respiratório foi mais frequente no parto tipo cesárea em recém-nascidos com mais de 37 semanas de idade gestacional e em recém-nascidos com peso maior ou igual a 2.500 g. Essa complicação teve associação significativa com parto cesariano e/ou fórcipe, mas não com parto normal na amostra total. O tocotraumatismo associou-se ao uso do fórcipe. Em recém-nascidos por cesárea, a anoxia associou-se à menor idade gestacional estimada pelo método de Capurro e ao menor peso. Conclusões: A via abdominal incorre em maior morbidade para o feto em termos de desconforto respiratório. O parto vaginal mostrou-se mais seguro para recém-nascidos com idade gestacional superior a 37 semanas e em recém-nascidos com peso maior que 2.500 g.
