Resumo Introdução: A Cultura de Segurança do Paciente é considerada um importante componente estrutural dos serviços, que favorece a implantação de práticas seguras e a diminuição da ocorrência de eventos adversos. Objetivo: Identificar os fatores associados à cultura de segurança do paciente nas unidades de terapia intensiva adulto em hospitais de grande porte da região Sudeste do Brasil. Método: Estudo transversal do tipo survey e multicêntrico. Participaram 168 profissionais de saúde de quatro unidades (A, B, C e D) de terapia intensiva adulto. Foi utilizado o questionário "Hospital Survey on Patient Safety Culture". Considerou-se como variável dependente o nível de cultura de segurança do paciente e variáveis independentes aspectos sociodemográficos e laborais. Foram usadas estatísticas descritivas e para a análise dos fatores associados foi elaborado um modelo de regressão logística múltipla. Resultados: Identificou-se associação entre tipo de hospital com onze dimensões da cultura de segurança, quanto à função a categoria profissional médico, técnico de enfermagem e enfermeiro foram relacionadas com três dimensões; o gênero com duas dimensões e tempo de atuação no setor com uma dimensão. Conclusão: Evidenciou-se que o tipo de hospital, categoria profissional, tempo de atuação no setor e gênero foram associados às dimensões de cultura de segurança do paciente.


Resumen Introducción: La cultura de seguridad del paciente se considera un componente estructural importante de los servicios, que favorece la aplicación de prácticas seguras y la reducción de la aparición de acontecimientos adversos. Objetivo: Identificar los factores asociados a la cultura de seguridad del paciente en unidades de terapia intensiva adulto en hospitales de la región Sudeste del Brasil. Metodología: Estudio transversal de tipo encuesta y multicéntrico. Participaron 168 profesionales de salud de cuatro unidades (A, B, C y D) de terapia intensiva adulto. Se utilizó el cuestionario "Hospital Survey on Patient Safety Culture". Se consideró como variable dependiente el nivel de cultura de seguridad del paciente y variables independientes los aspectos sociodemográficos y laborales. Fueron usadas estadísticas descriptivas y, para analizar los factores asociados, fue elaborado un modelo de regresión logística múltiple. Resultados: Se identificó asociación entre tipo de hospital con once dimensiones de cultura de seguridad del paciente. En relación a la función, personal médico, técnicos de enfermería y personal de enfermería fueron asociados con tres dimensiones, el género con dos dimensiones y tiempo de actuación con una dimensión en el modelo de regresión. Conclusión: Se evidenció que el tipo de hospital, función, tiempo de actuación en el sector y género fueron asociados a las dimensiones de la cultura de seguridad del paciente.


Abstract Introduction: Patient safety culture is considered an important structural component of the services, which promotes the implementation of safe practices and the reduction of adverse events. Objective: To identify the factors associated with patient safety culture in adult intensive care units in large hospitals in Belo Horizonte. Method: Cross-sectional survey and multicenter study. A total of 168 health professionals from four units (A, B, C and D) of adult intensive care participated. The questionnaire "Hospital Survey on Patient Safety Culture" was used. The patient's level of safety culture was considered as a dependent variable, and sociodemographic and labor aspects were the independent variables. Descriptive statistics were used and a multiple logistic regression model was developed to analyze the associated factors. Results: An association was identified between the type of hospital and eleven dimensions of the safety culture. In terms of function, the doctors, nursing technicians, and nurse were related to three dimensions; gender with two dimensions, and time working in the sector with one dimension. Conclusion: It was evidenced that the type of hospital, function, time working in the sector, and gender were associated with the dimensions of patient safety culture.

Fundamentos: La diabetes mellitus es una enfermedad crónica con alta morbimortalidad que afecta a 537 millones de adultos en el mundo. España es el segundo país europeo en prevalencia, con un 14,8% en población de veinte-setenta y nueve años, con 11,6 casos por cada 1.000 personas/año. La retinopatía diabética (RD) es la quinta causa de pérdida de visión a nivel mundial y la séptima causa de ceguera/discapacidad visual entre afiliados a la Organización Nacional de Ciegos de España (ONCE). La detección precoz de RD previene la ceguera en diabéticos y está condicionada por la hemoglobina glicosilada. El objetivo de este trabajo fue analizar el manejo de los pacientes diabéticos en la comarca del Aljarafe (Sevilla) e identificar oportunidades de mejora en la coordinación de su seguimiento entre el médico de Atención Primaria y el médico oftalmólogo. Métodos: Se realizó un estudio observacional retrospectivo (2016-2019) con los pacientes registrados en el censo de diabéticos de los veintiocho municipios del Aljarafe. Se consultó la historia de salud de Atención Primaria y Hospital, así como el programa de Telemedicina. En cuanto al análisis estadístico, para variables cualitativas se calcularon totales y porcentajes; para variables cuantitativas, media y distribución estándar (si distribución normal), y la mediana y cuartiles (distribución no normal). Resultados: Se registraron 17.175 diabéticos en el Aljarafe (5,7% de población); 14.440 pacientes (84,1%) tenían alguna determinación de hemoglobina durante el periodo, 9.228 (63,9%) las tenían todas en rango adecuado. Tenían control fundoscópico 12.040 diabéticos (70,1%), y de los que no, 346 (10,6%) tenían todas fuera de rango. Hubo 1.878 (10,9%) pacientes sin control fundoscópico ni metabólico, 1.019 (54,3%) eran mujeres, 1.219 (64,9%) menores de sesenta y cinco años, 1.019 (54,3%) con comorbilidad grave...(AU)

ABSTRACT

BACKGROUND: Key performance indicators (KPIs) are quantifiable measures used to monitor the quality of health services. Implementation guidelines for clinical pharmacy services (CPS) do not specify KPIs. AIM: To assess the quality of the studies that have developed KPIs for CPS in inpatient hospital settings. METHOD: A systematic review was conducted by searching in Web of Science, Scopus, and PubMed, supplemented with citation analyses and grey literature searches, to retrieve studies addressing the development of KPIs in CPS for hospital inpatients. Exclusions comprised drug- or disease-specific studies and those not written in English, French, Portuguese, or Spanish. The Appraisal of Indicators through Research and Evaluation (AIRE) instrument assessed methodological quality. Domain scores and an overall score were calculated using an equal-weight principle. KPIs were classified into structure, process, and outcome categories. The protocol is available at https://doi.org/10.17605/OSF.IO/KS2G3 . RESULTS: We included thirteen studies that collectively developed 225 KPIs. Merely five studies scored over 50% on the AIRE instrument, with domains #3 (scientific evidence) and #4 (formulation and usage) displaying low scores. Among the KPIs, 8.4% were classified as structure, 85.8% as process, and 5.8% as outcome indicators. The overall methodological quality did not exhibit a clear association with a major focus on outcomes. None of the studies provided benchmarking reference values. CONCLUSION: The KPIs formulated for evaluating CPS in hospital settings primarily comprised process measures, predominantly suggested by pharmacists, with inadequate evidence support, lacked piloting or validation, and consequently, were devoid of benchmarking reference values.

Diabetic retinopathy (DR) is the fifth cause of vision loss worldwide and the seventh cause of blindness/visual impairment among members of the National Organization of the Blind in Spain (ONCE). Early detection of DR prevents blindness in diabetics and is conditioned by glycosylated hemoglobin. However, a significant percentage with no glycated hemoglobin within range lack fundoscopic control, and another smaller group lack fundoscopic or metabolic control, with inter-municipal variability. We propose to improve communication channels between levels.

ABSTRACT

Este trabalho teve como objetivo avaliar a percepção de curadores sobre a qualidade dos serviços de Fisioterapia neuropediátrica ofertados em Centros Especializados em reabilitação (CER) no Brasil. Trata-se de um estudo transversal, quantitativo, de caráter avaliativo, cujo público-alvo foi representado por cuidadores das crianças em reabilitação. O instrumento AQSF-Neuroped foi utilizado para avaliar a qualidade dos serviços de Fisioterapia neuropediátrica em três CER, com base na percepção de 320 cuidadores. Utilizou-se o teste de hipótese para uma proporção a partir de cada item e dimensão do AQSF-Neuroped. Os modelos Weight of Evidence e Análise de Classes Latentes foram usados para avaliar o comportamento de variáveis sobre a percepção de qualidade. Obteve-se um escore médio total de qualidade de 133±14 pontos, implicando uma percepção positiva de qualidade; porém, há aspectos relacionados à estrutura física (dimensão interpessoal) e ao acesso (dimensão organizacional II) que precisam ser aprimorados (p-valores<0,05) para melhor atender às expectativas dos cuidadores. O CER B foi melhor avaliado dentre os demais serviços (IV<0,10). A ACL permitiu identificar, por meio da formação de grupo, características do cuidador que determinaram uma perspectiva positiva ou negativa da qualidade. Assim, disponibilizou-se os resultados para subsidiar as melhores práticas nos serviços.


This study evaluates the perception of caregivers of the quality of neuropediatric physiotherapy services offered at specialized rehabilitation centers (SRC) in Brazil. This is a cross-sectional, quantitative, evaluative study whose target population was caregivers of children in rehabilitation. The AQSF-Neuroped instrument was used to evaluate the quality of three SRC based on the perception of 320 caregivers. The one-proportion hypothesis test was used based on each AQSF-Neuroped item and dimension. The Weight of Evidence and Latent Class Analysis models were used to evaluate the influence of variables on the perception of quality. A mean overall quality score of 133±14 points indicated a positive perception of quality; however, there were aspects related to physical structure (Interpersonal Dimension) and access (Organizational Dimension II) that needed to be improved (p-values<0.05) to better meet the expectations of caregivers. By group formation, the LCA enabled identification of the caregivers' characteristics that determined either a positive or negative perspective of the quality. SRC B was best evaluated among all SRC (IV<0.10). Finally, managers are provided with results to support improvements in services.

ABSTRACT

Objetivo: Analizar la prevalencia de factores de complejidad de cuidados en los pacientes atendidos en el servicio de urgencias y determinar su relación con las reconsultas durante los 30 días posteriores a la vista inicial. Método: Estudio observacional transversal correlacional. Se incluyeron de forma consecutiva todos aquellos pacientes adultos que consultaron al servicio de urgencias de un hospital de tercer nivel durante un periodo de 6 meses. Las variables principales del estudio fueron la reconsulta a los 30 días y 26 factores individuales de complejidad de cuidados categorizados en 5 fuentes (psicoemocional, mental-cognitiva, sociocultural, evolutiva, comorbilidades-complicaciones). Los datos fueron recogidos de la historia clínica electrónica. Resultados: Se incluyeron un total de 15.556 episodios de pacientes. El 82,4% (12.811) presentó algún factor de complejidad de cuidados y el 11,9% (1.088) de los pacientes dados de alta reconsultaron durante los 30 días posteriores. La presencia de mayor número de factores de complejidad de cuidados se asoció a la reconsulta a los 30 días (OR: 1,26; IC 95%: 1,11-1,43; p < 0,05), y los siguientes factores se asociaron con reconsulta: incontinencia, inestabilidad hemodinámica, riesgo de hemorragia, extremo de edad, ansiedad y temor, deterioro de funciones cognitivas y analfabetismo (p < 0,05). Conclusiones: La prevalencia de factores de complejidad de cuidados en pacientes que consultan en el servicio de urgencias es elevada. Los pacientes que reconsultaron a los 30 días presentaron mayor número de factores de complejidad, por lo que su identificación precoz podría ayudar a estratificar los pacientes y diseñar estrategias preventivas para disminuir la incidencia de reconsultas. ABSTRACT

A multiple dependent state sampling plan (MDSSP) is designed when the lifetimes of the variables follow New Lomax Rayleigh Distribution (NLRD). The decision to accept or reject a lot in the proposed methodology is based on the quality of the given present or previous lots. A binomial model-based operating characteristic curve (OC curve) for continuous lots of variables under similar settings in healthcare is used in finding the probability of acceptance, acceptance number, rejection number, and the number of preceding (succeeding) lots to consider. Time truncated life test based on the specified median of the NLRD is used in designing the current acceptance sampling plan. For specified values of the parameters of NLRD, quantile ratios, consumer's risk and producer's risk, average sampling number (ASN) and probability of acceptance of a lot are reported in tables. Real data on worldwide suicide rates of 15-19 years in the year 2019 from the World Health Organization (WHO) website is considered to illustrate this methodology. The minimum sample size required from the selected data to comment on worldwide suicide rates in late adolescents is explained with MDSSP. The results of the proposed acceptance sampling method are compared with the single-stage sampling plan.

ABSTRACT

Since the publication of study results on adverse events to health care in OECD countries, the importance of the national quality improvement strategies has been recognised. To examine how these strategies have been shaped in different jurisdictions, we carried out this study. We conducted a web-based comparative study of international practices. We first defined seven key health care and services quality management functions. We then drew on the experience of authors to make a reasoned selection of 13 countries or states across the world. We determined the distance that separates each of these functions from a country's Ministry of Health (MoH); and examined whether these functions are concentrated in a single organisation or dispersed across several organisations. Afterwards, we correlated our results with the quality level of these countries based on the OECD's health care indicators. Overall, Netherlands, Québec (Canada), Korea, Germany, England (UK), and the United States had at least 50% of their quality management functions controlled by self-regulated organisations. The Market Concentration Index ranged from 937 for the United States to 6800 for Russia. Graphical representation has shown us two health system models. Our results also clearly showed that countries had a better quality of care most often when they belong to model 1 of our taxonomy. These findings will help countries design and implement large-scale health care and services quality strategies for better and safer health care and services.

Las variables principales del estudio fueron la reconsulta a los 30 días y 26 factores individuales de complejidad de cuidados categorizados en 5 fuentes (psicoemocional, mental-cognitiva, sociocultural, evolutiva, comorbilidades-complicaciones). Los datos fueron recogidos de la historia clínica electrónica. RESULTADOS: Se incluyeron un total de 15.556 episodios de pacientes. El 82,4% (12.811) presentó algún factor de complejidad de cuidados y el 11,9% (1.088) de los pacientes dados de alta reconsultaron durante los 30 días posteriores. La presencia de mayor número de factores de complejidad de cuidados se asoció a la reconsulta a los 30 días (OR: 1,26; IC 95%: 1,11-1,43; p 0,05), y los siguientes factores se asociaron con reconsulta: incontinencia, inestabilidad hemodinámica, riesgo de hemorragia, extremo de edad, ansiedad y temor, deterioro de funciones cognitivas y analfabetismo (p 0,05). CONCLUSIONES: La prevalencia de factores de complejidad de cuidados en pacientes que consultan en el servicio de urgencias es elevada. Los pacientes que reconsultaron a los 30 días presentaron mayor número de factores de complejidad, por lo que su identificación precoz podría ayudar a estratificar los pacientes y diseñar estrategias preventivas para disminuir la incidencia de reconsultas.

ABSTRACT

BACKGROUND: The association between cardiovascular risk factors and nonalcoholic fatty liver disease (NAFLD) is well established, but whether cardiovascular health (CVH) metrics is associated with NAFLD had not been fully studied. Thus, we examined the association between CVH metrics and NAFLD in the middle-aged Korean population. METHODS: We used data of 2,928 (851 men and 2,077 women) participants aged 30-64 years from the Cardiovascular and Metabolic Disease Etiology Research Center study. CVH metrics were measured using a modified version of Life's Simple 7 by the American Heart Association. NAFLD diagnosis was based on the fatty liver index or liver-to-spleen ratio on computed tomography. A multiple logistic regression model was used to investigate the cross-sectional and longitudinal associations between CVH metrics and NAFLD. RESULTS: In the cross-sectional analysis, the odds ratio for NAFLD was lower in participants with ideal CVH (odds ratio [OR], 0.13; 95% confidence interval [CI], 0.08-0.18), while it was higher in individuals with poor CVH (OR, 2.87; 95% CI, 2.13-3.86). Similarly, the risk of new-onset NAFLD was lower in participants with ideal CVH (OR, 0.28; 95% CI, 0.11-0.74), and higher in individuals with poor CVH (OR, 2.20; 95% CI, 0.50-9.72) in the longitudinal analysis of a subgroup. CONCLUSIONS: Ideal CVH was associated with a lower risk of NAFLD while poor CVH was associated with a higher risk of NAFLD. These findings suggest that making efforts to encourage people to manage their CVH to the ideal level may prevent and manage NAFLD.

Objetivo: Analizar la tendencia de las infecciones del torrente sanguíneo asociadas a la instalación de catéter venoso central en el sistema sanitario de Chile. Material y Método: Estudio ecológico, con análisis de tendencia de la prevalencia anual de las infecciones de torrente sanguíneo asociadas a instalación de catéter venoso central en los hospitales chilenos desde el año 2001 a 2019. Para el análisis univariado se utilizaron frecuencias, porcentajes, media y desviación estándar. En el análisis bivariado se utilizó la prueba de X 2 y T de Student en función de la naturaleza de las variables, con un nivel de significación de p=0,0034/0,0039), con un 5% de crecimiento promedio interanual de la tasa. El análisis por regiones muestra, en general, una tendencia estática. Conclusión: La infección del torrente sanguíneo por catéter venoso central es una complicación creciente en Chile, por lo que se hace necesario desarrollar programas e intervenciones que permitan controlar el avance de esta complicación.


Objective: To analyze the trend of bloodstream infections associated with central venous catheter insertion in the Chilean healthcare system. Materials and Methods: Ecological study with trend analysis of the annual prevalence of bloodstream infections associated with central venous catheter insertion in Chilean hospitals from 2001 to 2019. For univariate analysis, frequencies, percentages, mean and standard deviation were used. For bivariate analysis, the X 2 test and Student's t-test were used, according to the nature of the variables, with a significance level of p<0.01. For the trend analysis, a linear trend analysis of the annual prevalence was performed using the Prais-Winsten autoregression method, which allows the estimation of a global trend of a series of data of a quantitative indicator. The annual percent change (APC) and its 95% confidence intervals (CI-95%) were calculated. Results: A total of 27,197,232 cases were analyzed. An overall increasing and significant trend was observed throughout Chile (APC=0.00370; 95% CI=0.0034 / 0.0039), with an average interannual growth rate of 5%. In general, the analysis by region shows a static trend. Conclusion: Central venous catheter-related bloodstream infection is a growing complication in Chile. Therefore, it is necessary to develop programs and interventions to control the progression of this complication.

Foi calculada a variação percentual anual (APC sigla em inglês) e os respectivos intervalos de confiança a 95% (IC-95%). Resultados: Foram analisados 27.197.232 casos. Observou-se uma tendência global crescente e significativa em todo o Chile (APC=0,00370; IC 95%=0,0034/0,0039), com uma taxa média de crescimento anual de 5%. A análise por região mostra, em geral, uma tendência estática. Conclusão: A infecção da corrente sanguínea por cateter venoso central é uma complicação crescente no Chile. Portanto, é necessário desenvolver programas e intervenções para controlar a progressão desta complicação.

Purpose: This study aimed to develop quality indicators for the care of older adults with disabilities in long-term care facilities (LTCFs) based on Maslow's hierarchy of needs. Methods: The draft of the quality indicators was drawn up based on a literature review and research group discussion. The quality indicators were finalized by two rounds of expert consultation (involving 15 experts) using the Delphi method. The Analytic Hierarchy Process was applied to calculate the indicators' weight. Results: The response rates of the two rounds of consultation were 100% and 93%, and the expert authority coefficients were 0.86 and 0.87. After two rounds of consultation, the expert opinion coordination coefficients of the first-, second- and third-level indicators were 0.42, 0.25, and 0.96, respectively (P < 0.05), and the variation coefficient was ≤0.25. The final quality indicators for the care of older adults with disabilities in LTCFs included 7 first-level, 19 second-level, and 107 third-level indicators. Conclusion: The quality indicators for the care of older adults with disabilities in LTCFs are reliable, scientific, comprehensive, and practical and specify the content of person-centered care needs. This can provide a reference for evaluating and improving care quality in LTCFs.

ABSTRACT

Background: The COVID-19 pandemic has highlighted the importance of mental wellbeing. The identification and implementation of quality measures can improve health outcomes and patient experience. The objective was to identify and define a core set of valid and relevant pediatric mental health quality measures that will support health system evaluation and quality improvement in British Columbia, Canada. Methods: The study consisted of four phases. First, a comprehensive database search identified valid pediatric quality measures focused on mental health and substance use (MH/SU). Second, the identified quality measures were mapped to focus areas, which were then prioritized by two stakeholder groups consisting of 26 members. Third, up to two representative measures for each prioritized focus area were pre-selected by an expert panel (n = 9). And fourth, a three-step modified Delphi approach was employed to (1) assess each quality measure on a 7-point Likert scale against three relevance criteria (representative of a quality problem, value to intended audience and actionable), (2) discuss the results, and (3) select and rank the most relevant measures. Forty-eight stakeholders were invited to participate; of those 24 completed the round 1 survey, 21 participated in the round 2 discussion and 18 voted in the round 3 selection and ranking survey. For round 1, consensus was determined when at least 70% of the response rates were within the range of five to seven. For round 3, Kendall's coefficient of concordance W was used as an estimator of inter-rater reliability. Results: One-hundred pediatric mental health quality measures were identified in the database search. Of those, 37 were mapped to ten focus areas. Pre-selection resulted in 19 representative measures moving forward to the Delphi study. Eleven measures met the consensus thresholds and were brought forward to the round 2 discussion. Round 3 ranking showed moderate to strong raters' agreement (Kendall's W = 0.595; p < 0.01) and resulted in the following five highest-ranked measures: level of satisfaction after discharge from inpatient admission due to MH/SU, number of patients experiencing seclusion or restraint, length of time from eating disorder referral to assessment, number of ED visits due to MH/SU, and number of readmissions to ED. Conclusion: The selected core set of valid and relevant pediatric quality measures will support sustainable system change in British Columbia. The five top-ranked measures will be refined and tested for data collection feasibility before being implemented in the province.

Introducción: En Cuba el sistema nacional de salud con acceso universal y cuidado de los individuos, la familia y la comunidad es una fortaleza reconocida internacionalmente. La Clínica Central Cira García es un centro de atención médica especializada de referencia internacional en la atención a pacientes extranjeros. Su calidad ha sido certificada por distintas organizaciones, no obstante, trabaja intencionadamente por elevar los estándares alcanzados. Objetivo: Proponer un sistema de indicadores para evaluar la calidad del subproceso servicios generales de la Clínica Central Cira García. Métodos: Se aplicaron métodos teóricos y empíricos, a través del análisis y la síntesis, el método sistémico-estructural-funcional y el análisis de contenido, se analizó la documentación existente relativa a la gestión de la calidad de la institución y en particular de servicios generales, se sistematizaron los resultados y se propuso el sistema de indicadores de calidad a evaluar. Se utilizaron la observación y la entrevista en la caracterización del funcionamiento de las distintas esferas de trabajo que comprende el subproceso servicios generales con lo que se obtuvieron criterios del público interno. Se aplicó una encuesta de satisfacción a pacientes no hispanohablantes de países desarrollados. Se trianguló la información y se detectaron las brechas de mejora. Resultados: Se propuso un sistema de cuatro dimensiones, 14 variables y 54 criterios para evaluar la gestión de calidad del subproceso servicios generales el cual prácticamente no era tomado en cuenta en los indicadores de calidad utilizados hasta el momento. En las encuestas de satisfacción la mayoría de los criterios establecidos recibieron niveles satisfactorios por al menos el 90 por ciento de los encuestados. El tiempo de espera para recibir la habitación y el dominio de idiomas fueron los criterios que mostraron mayor insatisfacción. Conclusiones: Para garantizar un turismo de salud competitivo que cumpla con las expectativas de pacientes que seleccionen con este fin el destino Cuba, se necesita contar con un sistema de indicadores para evaluar la calidad de los procesos no clínicos que apoyan el desarrollo de los procesos clave que puedan generalizarse a las instituciones que desarrollan turismo de salud(AU)


Introduction: In Cuba, the national health system with universal access and care for individuals, the family and the community is an internationally recognized strength. Cira García Central Clinic is a specialized medical care center of international reference in the care of foreign patients. Its quality has been certified by different organizations, however, it intentionally works to raise the standards achieved. Objective: Propose a system of indicators to evaluate the quality of the sub-process called general services in Cira García Central Clinic. Methods: Theoretical and empirical methods were applied, through analysis and synthesis, the systemic-structural-functional method and content analysis, the existing documentation related to the quality management of the institution and in particular of general services was analyzed, the results were systematized and the system of quality indicators to be evaluated was proposed. Observation and interview were used in the characterization of the functioning of the different areas of work comprising sub-process called General Services, thus obtaining criteria from the internal public. A satisfaction survey was applied to non-Spanish-speaking patients from developed countries. Information was triangulated and improvement gaps were detected. Results: A system of four dimensions, 14 variables and 54 criteria was proposed to evaluate the quality management of the sub-process general services which was practically not taken into account in the quality indicators used so far. In the satisfaction surveys, most of the established criteria received satisfactory levels by at least 90percent of the respondents. The waiting time to receive the room and the mastery of languages were the criteria that showed the biggest dissatisfaction. Conclusions: To guarantee a competitive health tourism that meets the expectations of patients who select Cuba destination for this purpose, it is necessary to have a system of indicators to evaluate the quality of non-clinical processes that support the development of key processes that can be generalized to the institutions that develop health tourism(AU)

OBJECTIVES: To explore the challenges of secondary use of routinely collected data for analyzing nursing-sensitive outcomes in Austrian acute care hospitals. METHOD: A convergent parallel mixed methods design was performed. We conducted a quantitative representative survey with nursing managers from 32 Austrian general acute care hospitals and 11 qualitative semi-structured interviews with nursing quality management experts. Both results were first analyzed independently and afterward merged in the discussion. RESULTS: On average, 76% of nursing documentation is already electronically supported in the surveyed Austrian hospitals. However, existing nursing data is seldom used for secondary purposes such as nursing-sensitive outcome analyses. This is due to four major reasons: First, hospitals often do not have a data strategy for the secondary use of routine data. Second, hospitals partly lack the use of standardized and uniform nursing terminologies, especially for nursing evaluation. Third, routine nursing data is often not documented correctly and completely. Fourth, data on nursing-sensitive outcomes is usually collected in specific documentation forms not integrated into routine documentation. CONCLUSION: The awareness of the possibilities for secondary use of nursing data for nursing-sensitive outcome analyses in Austrian hospitals is still in its infancy. Therefore, nursing staff and nursing management must be trained to understand how to collect and process nursing data for nursing-sensitive outcome analyses. Further studies would be interesting in order to determine the factors that influence the decision-making processes for the secondary use of nursing data for outcome analyses.

Introdução: A segurança do paciente é considerada uma prioridade global pela Organização Mundial da Saúde, e o Brasil, como país-membro, lançou, em 2013, o Programa Nacional de Segurança do Paciente, recomendando práticas seguras para todos os serviços de saúde. Embora obrigatório, muitos estabelecimentos não possuem Núcleos de Segurança do Paciente estruturados para o gerenciamento de seus riscos e incidentes. Objetivo: Descrever a experiência na implantação de um programa de segurança para pacientes submetidos a procedimentos hemodinâmicos. Métodos: Estudo observacional, descritivo e retrospectivo da criação do Núcleo de Segurança do Paciente e do programa de gerenciamento de riscos. Resultados: O núcleo foi constituído em 2014, e o programa foi desenvolvido em quatro fases: criação do plano de segurança, mapeamento de riscos, criação de instruções de trabalho e gerenciamento de incidentes. Entre dezembro de 2014 e dezembro de 2019, foram monitorados 6.913 pacientes e identificado 146 incidentes, sendo 44,52% classificados como inconformidade a protocolos, 20,54% como circunstância notificável, 13,69% como quase-erro, 10,95% como incidente sem dano e 10,27% como evento adverso (incidente com dano). Entre 2017 e 2019, observaram-se redução no número de inconformidade de protocolo (0,66% versus 0,42%; p=0,01) e circunstância notificável (0,5% versus 0,32%; p=0,32), com incremento na comunicação de quase erro (0% versus 0,07%; p=0,018). Ao longo do estudo, observou-se aumento na adesão às metas dos indicadores relacionados à notificação de incidentes, ao tempo porta-balão e às horas- treinamento. Conclusão: A criação do Núcleos de Segurança do Paciente, somada à incorporação de um plano efetivo, é ferramenta efetiva, que contribui para a construção de uma cultura de excelência.


Background: Patient safety is considered a global priority by the World Health Organization, and Brazil, as a Member State, launched the National Patient Safety Program in 2013, recommending safe practices for all health services. Although mandatory, many facilities do not have structured Patient Safety Units to manage their risks and incidents. Objective: To describe the experience in implementing a safety program for patients undergoing invasive cardiology procedures. Methods: This was an observational, descriptive, and retrospective study on the creation of a patient safety unit and a risk management program. Results: The unit was established in 2014, and the program was developed in four phases: creation of the safety plan, risk mapping, creation of work instructions, and incident management. Between December 2014 and December 2019, a total of 6,913 patients were monitored and 146 incidents were identified, with 44.52% classified as non-compliance to protocol, 20.54% as reportable circumstances, 13.69% as near-misses, 10.95% as no harm incidents, and 10.27% as adverse events (harmful incidents). Between 2017 and 2019, there was a reduction in amount of non-compliance to protocol (0.66% versus 0.42%; p=0.01) and reportable circumstances (0.5% versus 0.32%; p=0.32), with an increase in near-miss reporting (0% versus 0.07%; p=0.018). Throughout the study, there was an increase in adherence to indicator targets related to incident reporting, door-to-balloon time, and training hours. Conclusion: The creation of Patient Safety Units, together with employing an effective plan, is an effective tool that contributes to the construction of a culture of excellence.

OBJECTIVES: This quality improvement study evaluates the impact of a caries risk assessment (CRA) registry on the following: percentage of children with a documented CRA, receipt of preventive and restorative services, and costs of care. METHODS: We used 2014-2019 data for patients aged 0-17 years from 22 locations in a group practice in Wisconsin. Paired t-tests and Wilcoxon signed-rank tests were used to evaluate changes over time in the following practice-level outcomes: CRA documentation, fluoride receipt, continuing care procedures, restorative procedures, total procedures, and inflation-adjusted costs of care. The same tests were used to compare average procedures and cost for patients a) enrolled and not enrolled in the registry, b) with and without CRA documentation, and c) at high and low caries risk. RESULTS: CRA documentation increased from 13 percent in 2014 to 87 percent in 2019 (P < 0.0001). There were statistically significant increases in the average number of continuing care procedures (from 1.47 to 1.54, P < 0.001), average total procedures (from 7.40 to 8.36, P < 0.001), and inflation-adjusted average cost (from $491.51 to $553.37, P < 0.001) after accounting for multiple comparisons. The average number of restorative procedures decreased, with borderline statistical significance. Average cost was stable for registry-enrolled patients and increased for those not enrolled. CONCLUSIONS: The registry achieved the primary goal of improving CRA documentation among children. This quality improvement initiative appears to have had value-enhancing effects by promoting increased receipt of preventive services and decreased restorative services, while maintaining stable average cost of care for registry-enrolled patients over time.

This study aims to evaluate, via a mixed methods study, the implementation of the screening process for pulmonary tuberculosis (PTB) within indigenous population of the Department of Cauca, Colombia, during the 2016-2018 period. Indicators assessing the PTB screening process were elaborated and estimated. Subsequently, an evaluation of the indicators were performed based on a sampling process from health care providers of the municipalities with the highest and lowest PTB incidence and from key agents' perspective. Screening indicators were estimated and thematic analysis was performed based on the interviews conducted with key agents. Finally, a triangulation of quantitative and qualitative findings was performed. From the total population expected to have respiratory symptomatics (n = 16,711), the health care providers were able to identify 42.3% of them. Out of the individuals identified as respiratory symptomatics (n = 7,064), they were able to examine 93.2% (n = 6,585) with at least one acid-fast bacilli smear test. The reported positivity index from acid-fast bacilli smear test was 1.87%. The explanations from key agents revolved around the possibility of an overestimated targeted amount of respiratory symptomatics; insufficient personnel for the search of symptomatic individuals; high costs for the search in areas of difficult access; the need to request permissions from indigenous authorities; culturally ingrained stigma; use of traditional medicine and self-medication; and patient's personal beliefs. This study revealed barriers in the implementation of the screening process for PTB within the indigenous population from the Department of Cauca, mainly in the identifying process of the respiratory symptomatics.


El objetivo fue evaluar la implementación del proceso y los indicadores en las pruebas para detectar tuberculosis pulmonar (TBP) en población indígena del Departamento de Cauca, Colombia, durante el periodo de 2016-2018, a través de un estudio de métodos mixtos. Fueron elaborados y estimados indicadores para el tamizaje de TBP. Posteriormente, a través de un proceso de muestreo de los proveedores de cuidados de salud de las municipalidades con las incidencias más altas y más bajas de TBP, y desde la perspectiva de actores clave, se intentó encontrar una explicación para los resultados de la primera fase. Se estimaron los indicadores de las pruebas y se realizó un análisis temático de las entrevistas dirigidas a los actores clave. Finalmente, se realizó la triangulación de los hallazgos cuantitativos y cualitativos. Del total de sintomáticos respiratorios esperados (n = 16.711), los proveedores de servicios de salud fueron capaces de identificar a un 42,3% de ellos, y de estos sintomáticos respiratorios identificados (n = 7.064) fueron capaces de examinar un 93,2% (n = 6.585) con al menos una prueba de frotis de bacilos ácidorresistentes. El índice de positividad informado en la prueba de la flema analizada en el microscopio, mediante la prueba de frotis de bacilos ácidorresistentes, fue 1.87%. Las explicaciones de los actores clave giraron alrededor de la percepción de un objetivo posiblemente sobreestimado de sintomáticos respiratorios; insuficiente personal para la consulta; altos costes para las consultas en áreas geográficas de difícil acceso; solicitud de permisos a las autoridades indígenas; estigma; medicina tradicional, automedicación y creencias. Este estudio reveló barreras en la implementación de las pruebas para la TBP en la población indígena del Departamento de Cauca, principalmente en el proceso de identificación de sintomáticos respiratorios.


O estudo teve como objetivo avaliar a implementação do processo e os indicadores de triagem para tuberculose pulmonar (TBP) numa população indígena do Departamento de Cauca, Colômbia, no período 2016-2018. Foi realizado um estudo com métodos mistos. Foram elaborados e estimados os indicadores para triagem da TBP. Em seguida, procurou-se explicar os achados da primeira fase, com base em um processo de amostragem de provedores de saúde dos municípios com os coeficientes mais altos e mais baixos de incidência de TBP, e da perspectiva dos atores-chave. Os indicadores de triagem foram estimados e a análise foi realizada das entrevistas com os atores-chave. Finalmente, foi feita a triangulação dos achados quantitativos e qualitativos. Do total de sintomáticos respiratórios esperados (n = 16.711), o provedor de saúde conseguiu identificar 42,3%, e destes sintomáticos respiratórios identificados (n = 7.064) conseguiram examinar 93,2% (n = 6.585) com pelo menos um exame de escarro (teste de BAAR). O índice de positividade do teste de BAAR foi de 1,87%. As explicações dos atores chave giraram em torno da percepção de uma proporção possivelmente superestimada de sintomáticos respiratórios, pessoal insuficiente para a busca, custos elevados da busca em áreas de difícil acesso geográfico, solicitação de autorização pelas autoridades indígenas, estigma, medicina tradicional, automedicação e crenças. O estudo revelou barreiras para a implementação da triagem para TBP na população indígena do Departamento de Cauca, principalmente no processo de identificação de sintomáticos respiratórios.

ABSTRACT

Objetivo: Analizar la tendencia del indicador "Infección del tracto urinario asociado a instalación de catéter urinario a permanencia" en Chile. Material y Método: Estudio ecológico, de series temporales en el sistema sanitario de Chile, correspondiente a un total de 15 regiones, desde el año 2001 al año 2017 (27.087.087 casos). Se incluyeron datos cuyo egreso hospitalario era infección de tracto urinario asociado a catéter urinario a permanencia, según la Clasificación Internacional de Enfermedades (CIE 10). El análisis descriptivo y correlacional (X2 y T de Student, respectivamente) se realizó en función del sexo, tipo de previsión, condición de egreso y operación quirúrgica. El análisis se realizó con el programa SPSS 24 y el nivel de significación estadística fue p< 0,05. La estimación, basada en análisis lineal, estimó cambios porcentuales anuales (APC: Annual Percentage Change) y sus intervalos de confianza al 95%. Se empleó el método de auto-regresión de Prais Winsten. El resguardo ético siguió lo dispuesto en la Ley N° 20.585. Resultados: Se observó un 2% de crecimiento interanual (APC= 0,00007; IC: 95%= 0,000069-0,000073). En los años 2005, 2008, 2009 y 2011 existió un aumento en el porcentaje de cambio de 2 a 5 puntos. Conclusión: El indicador analizado es un evento que ha presentado una tendencia creciente y significativa en Chile desde el año 2001, lo que resulta paradójico, puesto que el ordenamiento de los procesos hospitalarios, asociado a la Reforma de Salud, debería demostrar una mejora en el comportamiento de este tipo de indicadores.


Objective: To analyze the trend indicator "Urinary tract infection associated with indwelling urinary catheter insertion" in Chile. Material and Method: Ecological, time series study, carried out in the Chilean health system, corresponding to a total of 15 administrative regions, from 2001 to 2017 (27.087.087 cases). Data included information on patients whose hospital discharge was urinary tract infection associated with indwelling urinary catheter, according to the International Classification of Diseases (ICD-10). Descriptive and correlational analysis (X2 and Student's t-test, respectively) was performed according to sex, type of health insurance, hospital discharge condition and surgical operation. The analysis was performed using SPSS 24 software and the level of statistical significance was p<0.05. The estimation was based on linear analysis, estimating annual percentage changes (APC) and their 95% confidence intervals. The Prais-Winsten autoregression method was used. Ethical considerations followed the provision of Law No. 20.585. Results: A 2% interannual growth rate was observed (APC= 0.00007; CI: 95% = 0.000069-0.000073). In the years 2005, 2008, 2009 and 2011 there was an increase in the percentage change of 2 to 5 points. Conclusion: The analyzed indicator corresponds to an event that has shown a significant and increasing trend in Chile since 2001, which seems paradoxical, since hospital processes management, associated with the health care reform, should show an improvement regarding this type of trend indicator.


Objetivo: Analisar a tendência do indicador "Infecção do trato urinário associada à instalação de cateter urinário permanente" no Chile. Material e Método: Estudo ecológico, e de series temporais realizado no sistema de saúde chileno, correspondente a um total de 15 regiões administrativas, de 2001 a 2017 (27.087.087 casos). Os dados incluíram informações sobre pacientes cuja alta hospitalar foi infecção do trato urinário associada ao uso do cateter urinário permanente, de acordo com a Classificação Internacional de Doenças (CID-10). A análise descritiva e correlacional (X2 e teste t de Student, respectivamente) foi realizada segundo sexo, tipo de seguro de saúde, condição de alta hospitalar e operação cirúrgica. A estimativa foi baseada na análise linear, estimando as mudanças percentuais anuais (APC) e seus intervalos de confiança de 95%. Foi utilizado o método Prais-Winsten de auto-regressão. As considerações éticas seguiram o disposto na Lei No. 20.585. Resultados: Foi observada uma taxa de crescimento interanual de 2% (APC= 0,00007; IC:95%=0,000069-0,000073). Nos anos 2005, 2008, 2009 e 2011 houve um aumento na variação percentual de 2 a 5 pontos. Conclusão: O indicador analizado corresponde a um evento que tem mostrado uma tendência significativa e ascendente no Chile desde 2001, o que parece paradoxal, dado que a organização dos processos hospitalares associados à reforma do sistema de saúde deveria mostrar uma melhoria no comportamento deste tipo de indicador.
