ABSTRACT

Ameloblastoma is a rare benign epithelial odontogenic neoplasm, but with great clinical implications, as despite its benignity and slow growth, most cases are locally aggressive with a significant recurrence rate. Histological, cellular, or molecular analyses of its pathogenesis have confirmed the complexity of this neoplasm. We present the case of a 20-year-old patient with a suggestive clinical and radiographic diagnosis of ameloblastoma. An incisional biopsy was obtained confirming the diagnosis of conventional ameloblastoma. Left hemimandibulectomy and plate reconstruction were performed. Histopathological analysis of the surgical specimen confirmed the conventional ameloblastoma with a plexiform pattern and significant areas of cystic degeneration and amyloid-like-like deposits. Additionally, a microarray was carried out with bioinformatic analysis for the enrichment, protein interaction, and determination of eight hub genes (CRP, BCHE, APP, AKT1, AGT, ACTC1, ADAM10, and APOA2) related to their pathogenesis.

ABSTRACT

A osteomielite fibrosante é um processo inflamatório crônico, acompanhado pela destruição e substituição do tecido ósseo por fibroblastos, causado por um microorganismo infectante. Seu diagnóstico é baseado na história, achados radiográficos, histopatologia, cultura e antibiograma. O objetivo do presente estudo foi descrever um caso de osteomielite bacteriana crônica em felino doméstico (Felis catus) na região da mandíbula, acompanhado de osteólise. Após o exame, foi confirmado o diagnóstico de osteomielite fibrosante causada pela bactéria Pseudomonas aeruginosa. O tratamento de escolha foi hemimandibulectomia da porção afetada associada à antibioticoterapia. A pacientea presentou boa recuperação pós-operatória, concluindo que a técnica cirúrgica associada ao uso de antibióticos sensíveis às bactérias mostrou-se eficiente na resolução.(AU)


Firming osteomyelitis is a chronic inflammatory process, accompanied by destruction and replacement of bone tissue by fibroblasts, caused by an infecting microorganism. Its diagnosis is based on history, radiographic findings, histopathology, culture and antibiogram. The aim of the present study was to describe a case of chronic bacterial osteomyelitis in domestic feline (Felis catus) in the mandible region, accompanied byosteolysis. After examination, the diagnosis of fibrosing osteomyelitis caused by the bacterium Pseudomonas aeruginosa was confirmed. The treatment of choice was hemimandibulectomy of the affected portion associated with antibiotic therapy. The patient had a good postoperative recovery, thus concluding that the surgical technique associated with the use of bacteria-sensitive antibiotics proved to be efficient in resolving.(AU)

Por lo general las mejores opciones de rehabilitación para el paciente hemimandibulectomizado quedan fuera del alcance del poder adquisitivo de la mayoría de ellos, esto hace que el abordaje reconstructivo protésico- quirúrgico para éstos se tenga que restringir al uso de materiales más accesibles tales como el clavo de Kirschner y las prótesis inmediatas de metilmetacrilato, siendo estas últimas las más recomendables por su bajo costo, fácil manejo, por evitar la rotación mandibular y por prevenir la atrofia de los tejidos blandos. Se reporta el caso clínico de un paciente masculino de 25 años de edad el cual ingreso al Servicio de Oncología del Hospital General de México debido a la presencia de un aumento de volumen en el área correspondiente al ángulo mandibular izquierdo, diagnosticándose microscópicamente como un tumor mixto maligno (diagnóstico histopatológico final). Se decidió hacer la hemimandibulectomía de la zona involucrada, con reconstrucción del segmento óseo perdido mediante la colocación de una prótesis inmediata implantada confeccionada con metilmetacrilato de metilo termocurable, la cual fue fijada con alambre para osteosínteis aproximadamente a 3 y 6 milímetros por arriba de la cortical inferior mandibular en ambos bordes de sección.


In general terms, the best rehabilitation options for patients previously subjected to hemi-mandibulectomy are far beyond their financial possibilities. For this reason surgical-prosthetic reconstructive approach is mainly restricted to the use of more affordable materials such as Kirschner wire and methyl-methacrylate immediate prostheses. The latter are widely recommended due to their low cost, ease of handling, and because they prevent soft tissue atrophy. A clinical case is reported in this article: 25 year old male patient seeking treatment at the Oncology Service of the Hospital General de Mexico (Mexico's General Hospital) due to the presence of a volume increase in the area of the left mandibular angle. Microscopic analysis revealed presence of mixed malign tumor ( final histopathological diagnosis). It was decided to perform hemi-mandibulectomy of involved area, with reconstruction of lost bone segment by means of positioning an immediate methyl methacrylate prosthesis (thermosetting methyl). The prosthesis was fixated with osteosynthesis wire at both resection margins, at 3 mm above the cortex.

As neoplasias em cavidade oral ocupam o quarto local de maior incidência de tumores em cães e gatos,sendo o carcinoma de maior incidência na espécie felina. Entretanto, as glândulas salivares são raramenteacometidas por esta afecção, porém, quando ocorre, as mais acometidas são a mandibular e a parótida.O presente artigo tem como objetivo relatar um caso de um felino, sem raça definida, macho, com 11anos de idade, atendido do HCV-UFRGS, o qual apresentava um aumento de volume submandibular háquatro meses e que nos últimos 20 dias estava crescendo progressivamente. Os sinais clínicos observadosforam disfagia, sialorréia e perda de peso. Após o exame clínico, foi indicado a realização de examescomplementares, destacando-se a radiografia do crânio e exame citológico do local afetado, os quaisrevelaram radioluscência do ramo horizontal mandibular direito e presença de células compatível comcarcinoma de células escamosas (CCE) de glândula salivar, respectivamente. Realizou-se o tratamentocirúrgico com ressecção em bloco da glândula salivar em conjunto com o ramo mandibular afetado, alémdo linfonodo mandibular, que encontrava-se aumentado. Decorrido dois meses do procedimento, houverecidiva local do tumor e o proprietário optou pela realização da eutanásia.(AU)


Cancer in oral cavity occupies the fourth place of the highest incident tumors in dogs and cats, andcarcinoma is most incident in the feline species. However, the salivary glands are rarely affected by thisdisease, but when it does, the most affected are mandibular and parotid glands. We report a case of a cat,mixed breed, male, 11 years old, accepted in HCV-UFRGS, with submandibular swelling for four monthsand about the last 20 days it was growing progressively. Clinical signs were dysphagia, drooling andweight loss. After clinical examination, he was submitted to additional tests, especially X-ray of the skulland cytological examination of affected region. X-ray revealed radiolucency of the right mandibular horizontalbranch and cytological examination indicated the presence of cells consistent with carcinoma ofthe salivary gland. Surgery was performed through bloc resection of the salivary gland, in conjunctionwith the affected ramus and mandibular lymph node, which was enlarged. Two months after proceeding,tumor had recurred locally and the owner chose to proceed with euthanasia.(AU)

Ameloblastoma es un tumor de origen odontogénica que presenta crecimiento lento como una de las características principales, localmente invasivo y raramente causa metástasis. Pueden alcanzar grandes dimensiones si no tratado precozmente, convirtiéndose en un desafío su exéresis y reconstrucción en fases más avanzadas. El actual artículo tiene como objetivo relatar un caso clínico de un ameloblastoma multiquístico que afecta toda la región de la mandíbula derecha, en la cual fue realizado hemimandibulectomia, seguido de una reconstrucción con placa de 2.4mm y prótesis de cóndilo mandibular confeccionada en resina acrílica termopolimerizada, cubierta por el colgajo del músculo esternocleidomastoideo


The ameloblastoma is a tumor of odontogenic origin that present as main characteristics slow growth, locally invasive and they rarely cause metastasis. They can reach great dimensions if relegated the precocious treatment, turning his it complete removal and reconstruction in more advanced phases. The present article has as objective tells the clinical case of an ameloblastoma multicístico involving every area right mandibular, in which hemimandibulectomy was accomplished, following by reconstruction with plate 2.4mm and prosthesis of mandibular condylar made in resin acrylic thermopolimerizate, associated to shred of the muscle externocleidomastoideo
