ABSTRACT

BACKGROUND: Uterine fibroids, are prevalent benign tumors affecting women of reproductive age. However, surgical treatment is often necessary for symptomatic hysteromyoma cases. This study examines the impact of humanized nursing care on reducing negative emotions and postoperative complications in patients receiving hysteromyoma surgery. AIM: To investigate the impact of humanized nursing care on patients undergoing hysteromyoma surgery. METHODS: Among patients who underwent hysteromyoma surgery at the Fudan University Affiliated Obstetrics and Gynecology Hospital, 200 were randomly assigned to either the control group (n = 100) or the humanized nursing care group (n = 100). The control group received traditional nursing care, while the humanized nursing care group received a comprehensive care plan encompassing psychological support, pain management, and tailored rehabilitation programs. In addition, anxiety and depression levels were assessed using the hospital anxiety and depression scale preoperatively and postoperatively. Postoperative complications were evaluated during follow-up assessments and compared between both groups. RESULTS: The humanized nursing care group demonstrated a significant decrease in anxiety and depression levels compared to the control group (P < 0.05). The rate of postoperative complications, including infection, bleeding, and deep venous thrombosis, was also markedly lower in the humanized nursing care group (P < 0.05). CONCLUSION: Humanized nursing care can effectively alleviate negative emotions and reduce the incidence of postoperative complications in patients undergoing hysteromyoma surgery. This approach should be considered a crucial component of perioperative care for these patients. Further research may be needed to explore additional benefits and long-term outcomes of implementing humanized nursing care in this population.

ABSTRACT

BACKGROUND: Uterine intravenous leiomyomatosis is defined as leiomyoma tissue invading the vein outside the leiomyoma. Reports of extension to the right pulmonary artery are relatively rare. CASE SUMMARY: We describe a 31-year-old woman with a benign leiomyoma that extended into the right ventricular lumen, causing mechanical obstruction and corresponding clinical symptoms. Tumors located in the pulmonary artery can cause pulmonary artery obstruction. After diagnosis, surgical treatment should be performed as soon as possible. CONCLUSION: In this case, the uterine leiomyoma extended to the right pulmonary system, which is clinically rare.

BACKGROUND: Hysteromyoma is not a rare tumor among pregnant women. During pregnancy, the symptoms caused by hysteromyoma can be improved through conservative treatment in most cases. However, in order to ensure the safety of mothers and children, surgeries are necessary in some special cases. CASE SUMMARY: We report a case of pregnancy complicated with hysteromyoma red degeneration. The patient had peritonitis after sudden abdominal pain during the 20th week of pregnancy. Laparoscopic exploration suggested rupture and bleeding of hysteromyoma, which were improved after drainage and an anti-inflammatory treatment. A cesarean section was performed after full term. This case shows the complications of rupture after red degeneration of hysteromyoma during pregnancy. CONCLUSION: We should be alert to rupture of hysteromyoma during pregnancy, and active laparoscopic exploration is essential to improve the prognosis of such patients.

ABSTRACT

In this study, a minimally invasive test method for cervical cancer in vitro was proposed by comparing Raman spectroscopy with support vector machine (SVM) model and deep belief network (DBN) model. The serum Raman spectra of cervical cancer, hysteromyoma, and healthy people were collected. After data processing, SVM classification model and DBN classification model were built respectively. The experimental results show that when the DBN network algorithm is used, the sample test set can be divided accurately and the result of cross-validation is ideal. Compared with the traditional SVM algorithm, this method firstly screened the effective feature matrix from the data, and then classified the data. With high efficiency and accuracy, based on 445 samples collected, this method improved the accuracy by 13.93%±2.47% compared with the SVM method, and provided a new direction and idea for the in vitro diagnosis of cervical diseases.

ABSTRACT

OBJECTIVES: To determine the value of dual-sequence magnetic resonance imaging (MRI)-based radiomics in predicting the efficacy of high-intensity focused ultrasound (HIFU) ablation for hysteromyoma. METHODS: A total of 142 patients with 172 hysteromyomas (95 hysteromyomas from the sufficient ablation group, and 77 hysteromyomas from the insufficient ablation group) were enrolled in the study. The clinical-radiological model was constructed with independent clinical-radiological risk factors, the radiomics model was constructed based on the optimal radiomics features of hysteromyoma from dual sequences, and the two groups of features were incorporated to construct the combined model. A fivefold cross validation procedure was adopted to validate these models. A nomogram was constructed, applying the combined model in the training cohort. The models were assessed with receiver operating characteristic (ROC) curves and integrated discrimination improvement (IDI). An independent test cohort comprising 40 patients was used to evaluate the performance of the optimal model. RESULTS: Among the three models, the average areas under the ROC curves (AUC) of the radiomics model and combined model were 0.803 (95% confidence interval (CI): 0.726-0.881) and 0.841 (95% CI: 0.772-0.909), which were better than the clinical-radiological model in the training cohort. The IDI showed that the combined model had the best prediction accuracy. The combined model also showed good discrimination in both the validation cohort (AUC = 0.834) and the independent test cohort (AUC = 0.801). CONCLUSION: The combined model based on the dual-sequence MRI radiomics is the most promising tool from our study to assist clinicians in predicting HIFU ablation efficacy.

ABSTRACT

Purpose: This paper aims to explore the safety and feasibility of a single-hole laparoscopic myomectomy through an abdominal scar approach. Method: The clinical data of seven patients who underwent the single-hole laparoscopic myomectomy via the abdominal scar approach from January to November 2021 in the Department of Gynecology, the Affiliated Changzhou No. 2 People's Hospital of Nanjing Medical University, were studied retrospectively. The duration of operation, the intraoperative blood loss, the decrease of postoperative hemoglobin, and the postoperative visual analogue score (0 points: no pain, 10 points: maximum pain) were recorded. Results: All seven patients received the operation successfully, without changing to the conventional laparoscopic operation or open appendectomy. The average blood loss was 101.42 ± 7.89 ml, the average length of hospital stay was 5 ± 0.53 days, the average operation duration was 130 ± 26.86 min, and the 24-h pain score was 1.57 ± 0.53. The seven patients had no intraoperative or postoperative complications and no damage to the ureter or bladder. All patients could urinate spontaneously without urinary retention or urinary tract infection after catheter removal. No analgesic drugs were used after the operation. Conclusion: The single-hole laparoscopic myomectomy via the abdominal scar approach is a more aesthetic and feasible option for eligible patients, but more cases and studies are needed for further confirmation.

ABSTRACT

OBJECTIVE: In this study, we investigated the effects of 5A nursing mode combined with fine nursing management on perioperative self-efficacy and living quality of hysteromyoma. METHODS: 116 hysteromyoma patients admitted to our hospital during August 2018 to August 2020 were enrolled as the research objects, and divided into control group and observation group by random-number-table method. Each group contained 58 cases. The control-group patients were treated with conventional basic nursing, while the observation-group patients received combined treatment of 5A nursing mode and fine nursing management. Subsequently, the changes of self-efficacy, living quality of the two groups of patients before and after interventions, and the occurrence of postoperative complications in both groups were recorded and compared. RESULTS: The self-efficacy scores of both groups after intervention were markedly higher than those before intervention (P < 0.05), and the score of the observation-group after intervention was obviously higher than that of control-group (P < 0.05). The scores of the quality of life of the two groups after intervention were remarkably higher than those before intervention (P < 0.05), and the score of observation group after intervention was substantially higher than that of control group (P < 0.05). In addition, the incidence of complications in observation group was notably lower than that in control group, and the difference was statistically significant (10.34%, 29.31%, χ2 = 6.5619, P = 0.0104). CONCLUSION: The combined treatment of 5A nursing mode and fine nursing management has showed good nursing concept and standard nursing methods. It can effectively improve the self-efficacy and quality of life in hysteromyoma patients during perioperative period.

ABSTRACT

Objective: The present study aimed to explore the application value of magnetic resonance imaging (MRI) histograms with multiple sequences in the preoperative differential diagnosis of endometrial stromal sarcoma (ESS) and degenerative hysteromyoma (DH). Methods: The clinical and preoperative MRI data of 20 patients with pathologically confirmed ESS and 24 patients with pathologically confirmed DH were retrospectively analyzed, forming the two study groups. Mazda software was used to select the MRI layer with the largest tumor diameter in T2WI, the apparent diffusion coefficient (ADC), and enhanced T1WI (T1CE) images. The region of interest (ROI) was outlined for gray-scale histogram analysis. Nine parameters-the mean, variance, kurtosis, skewness, 1st percentile, 10th percentile, 50th percentile, 90th percentile, and 99th percentile-were obtained for intergroup analysis, and the receiver operating curves (ROCs) were plotted to analyze the differential diagnostic efficacy for each parameter. Results: In the T2WI histogram, the differences between the two groups in seven of the parameters (mean, skewness, 1st percentile, 10th percentile, 50th percentile, 90th percentile, and 99th percentile) were statistically significant (P < 0.05). In the ADC histogram, the differences between the two groups in three of the parameters (skewness, 10th percentile, and 50th percentile) were statistically significant (P < 0.05). In the T1CE histogram, no significant differences were found between the two groups in any of the parameters (all P > 0.05). Of the nine parameters, the 50th percentile was found to have the best diagnostic efficacy. In the T2WI histogram, ROC curve analysis of the 50th percentile yielded the best area under the ROC curve (AUC; 0.742), sensitivity of 70%, and specificity of 83.3%. In the ADC histogram, ROC curve analysis of the 50th percentile yielded the best area under the ROC curve (AUC; 0.783), sensitivity of 81%, and specificity of 76.9%. Conclusion: The parameters of the mean, 10th percentile and 50th percentile in the T2WI histogram have good diagnostic efficacy, providing new methods and ideas for clinical diagnosis.

ABSTRACT

OBJECTIVE: To improve the accuracy of clinical diagnosis by analyzing different contrast-enhanced ultrasound (CEUS) imagines of specific subtypes of uterine leiomyomas. METHODS: A total of 147 female patients received preoperative CEUS examination. The scanning plane of the biggest tumors for CEUS was found by common B-mode ultrasonographic scanning on pelvic cavity, then 1.5 mL SonoVue were injected into the median cubital vein. According to the CEUS images, the lesion enhanced time, enhanced level and enhanced morphology were recorded. The time-intensity curve was acquired and analyzed, meanwhile, the relevant parameters were calculated, including rise time (RT), peak intensity (PI), time to peak (TTP) and mean transit time (MTT). RESULTS: In cellular uterine leiomyoma group, the percentage of high enhancement, early enhancement was higher, equal enhancement and synchronic enhancement were lower than those in the common uterine leiomyomas group. In hysteromyoma with hyaline degeneration group, the percentage of high enhancement, early enhancement was lower, while low enhancement and delayed enhancement were higher than those in the common uterine leiomyomas group. The ratio of PI in cellular uterine leiomyoma group was the highest, but the ratios of RT, TTP and MTT were the lowest of the three benign groups. The ratio of PI in hysteromyoma with hyaline degeneration group was the lowest, while the ratios of RT and TTP was the highest among the three benign groups. CONCLUSION: Different pathological types of uterine leiomyomas have their own signal performance on CEUS. CEUS can be used to infer their pathological types and help differential diagnosis.

ABSTRACT

OBJECTIVE: To characterize endometrial structural and functional state and receptivity in women with hysteromyoma (HM) and chronic endometritis (CE) in infertile couples. MATERIAL AND METHODS: A total of 130 patients who had HM and/or CE in an infertile couple were examined. Group 1 included 64 women with HM and CE; Group 2 consisted of 24 patients with HM; group 3 comprised 42 patients with EC. A control group included endometrial biopsy specimens from 20 healthy women planning a pregnancy. RESULTS: Patients with HM and CE were found to develop endometrial dysfunction. Its structural signs were damage to the endometrial surface epithelium; intense stromal fibrosis with the high expression of type III and IV collagens; dysregulated processes of high angiogenesis; impaired maturation of pinopodia by the implantation window; high estrogen receptor (ER) expression in the nuclei of endometrial glandular and stromal cells; low progesterone receptor (PR) expression; and impaired secretory endometrial transformation. CONCLUSION: HM concurrent with CE is responsible for endometrial dysfunction. Its structural manifestations are mainly associated with CE.

ABSTRACT

ETHNOPHARMACOLOGICAL RELEVANCE: Rhizoma Curcumae and Rhizoma Sparganii (RCRS), a celebrated traditional Chinese medicine drug pair, has been used to treat hysteromyoma (HY). AIM OF THE STUDY: We aimed to identify the endogenous biomarkers of RCRS against HY. MATERIALS AND METHODS: HY rat model was established by injecting intramuscularly estradiol benzoate and progesterone injection from inner thigh in sequence. Body weight, uterus morphological indexes, immunohistochemistry (IHC) and hematoxylin and eosin (HE) staining experiments were used to evaluate the efficacy of RCRS (The rats were treated with RCRS extract, which was made by soxhlet reflux method. The rats were administrated intragastrically with 2 mL of RCRS extract). UPLC-Q-TOF-MS based metabonomics was adopted to analyze the serum and urine biomarkers from HY rats before and after RCRS treatment. Principle component analysis (PCA) and orthogonal partial least squares-discriminant analysis (OPLS-DA) were utilized to identify differences of metabolic profiles in rats among the four groups. RESULTS: 16 potential biomarkers from serum and 18 potential biomarkers from urine in both positive and negative mass spectrometry detection modes were identified, primarily related to Linoleic acid metabolism and Glyoxylate and dicarboxylate metabolism. RCRS drug pair has therapeutic effects on rats with HY via the regulation of multiple metabolic pathways. CONCLUSIONS: This study provides a useful method to get insight into the integrated metabonomic mechanism of RCRS drug pair on HY rats.

ABSTRACT

OBJECTIVE: To investigate difference in the extraction of specimens between via the middle part incision of posterior vagina and via morcellation.METHODS: A retrospective analysis was performed in 70 cases of laparoscopic hystero myomectomy admitted to the gynecological ward from June 2017 to June 2018 in Shenyang Women and Children's Hospital;tissue extraction was accomplished in a specimen retrieval bag via middle part incision of posterior vagina in 36 cases,while 34 cases through morcellation.RESULTS: All the 70 patients with uterine fibroids underwent laparoscopic surgery successfully.The specimen was extracted via middle part incision of posterior vagina or through morcelation.The retrieval bags were not damaged and no complications occurred.The operation time and blood loss were compared between the two groups,and there was no significant difference[(106.58±10.24)min vs.(104.32±10.58)min,(88.89±41.60)mL vs.(88.82±46.76)mL,P>0.05].The vaginal incision healed well in follow-up.CONCLUSION: The middle part incision of posterior vagina took great advantage of the nature path.The way of the extraction is an efficacious and minimally morbid technique for removing the intact entrapped specimen after laparoscopic myomectomy.It is feasible and easy to grasp.

ABSTRACT

Objective: To explore the pathological diagnosis features and differential diagnosis of one case of patient with solid pseudopapUlary neoplasm (SPN) of pancreas occurred in the majus omentum, and to improve the understanding of extra-pancreatic SPN. Methods : One female patient was admitted to hospital because of abdominal mass found 20 d ago. The patient was diagnosed as multiple hysteromyoma by gynecological color ultrasound, and the results of abdominal ultrasound had no obvious abnormity. The patient underwent hysterotomy, and a well-defined mass in the majus omentum was found during operation, so the patient received hysterectomy and omental tumor resection. Results : The patient was confirmed as multiple hysteromyoma by the postoperative pathological examination. The patient was clearly diagnosed as SPN arising from the majus omentum on the basis of pathological characteristics and immunohistochemical staining results. There were no ectopic pancreas tissue arround the tumor and no SNP metastasis in examination of ultrasound and CT. No recurrence and metastasis were found in the follow-up of 3 years. Conclusion: SPN arising from the majus omentum is extremely rare. The tumor has clear boundary, typical morphology and immunohistochemical appearances.

Objective To study and observe the effects of cold knife excision by hysteroscopy for the local microcirculation and inflammatory reaction of patients with hysteromyoma.Methods 86 patients with hysteromyoma admitted in our hospital from March 2016 to July 2017 were selected as the research objects,and the patients were divided into control group with 43 cases and observation group with 43 cases according to the method of random number table.The control group were treated with monopolar electric knife excision by hysteroscopy,while the observation group treated with cold knife excision by hysteroscopy.Then the local microcirculation and inflammatory reaction indexes of two groups at different time before and after the operation were detected.Results The local microcirculation and inflammatory reaction indexes of two groups before the operation were compare,no significant differences found (P>0.05);the local microcirculation indexes of observation group after the operation were significantly better than those of the control group,the inflammatory reaction indexes were significantly lower than those of the control group,both with statistical significance (P<0.05) .Conclusion The cold knife excision by hysteroscopy has better clinical effects in the control of local microcirculation and inflammatory reaction in patients with hysteromyoma,so it has higher application value in the patients with hysteromyoma.

Objective To observe the effect of the long-term low-dose mifepristone on the treatment of hysteromyoma.Methods From March 2016 to December 2016 in Haiyan maternal and child health-care center,120 cases with hysteromyoma were selected as the object study and were given long term oral low-dose mifepristone.The uterus size, follicle stimulating hormone, myoma size, luteinizing hormone, estrogen and endometrium were observed Resultsafter treatment, the patients with uterine fibroids situation improved markedly, and appeared amenorrhea.At the same time, in patients with clinical symptoms were improved to a certain extent, six months after a drug follicle-stimulating hormone, luteinizing hormone and estrogen levels significantly decreased, and there is no other significant side effects.In addition, on the analysis of the effectiveness of the treatment,86 cases were markedly improved,28 cases effective,6 cases ineffective, the total effective rate was 95%.Conclusion Long-term low-dose mifepristone has worked well in the treatment of uterine fibroids, can effectively reduce uterine fibroids, high security, in the process of follow-up of patients, no recurrence no satisfaction related data, the result can't reach this Conclusion , the authors modify please.Is the ideal way of conservative treatment of uterine fibroids, and is worth popularizing in the process of clinical application.

ABSTRACT

Objective: To investigate the molecular maechanism and potential active constituents population of Guizhi Fuling Capsule (GFC) for the treatment of dysmenorrhea, pelvic inflammatory disease (PID), and hysteromyoma. Methods: One handred and thirty target proteins related with dysmenorrhea, PID, and hysteromyoma were selected through mining literature, retrieving in DrugBank and TTD database, the main active constituents and potential target proteins from GFC were computed and analyzed by DOVIS2.0 and Cytoscape 3.0. The potential target proteins were then projected into the KEGG databases to illustrate the molecular mechanism of GFC. Results: The results of data analysis showed that the 115 active molecules with stronger interaction with protein were distributed in Cinnamomi Ramulus and Poria. The High network degree and betweenness of molecules were found to be pentacyclic triterpenes and steroids by further analysis of network characteristics. The most of the potential target proteins (78.57%) interacted wth the compounds in GFC were from 15 biological pathways closely related with dysmenorrhea, PID, and hysteromyoma in KEGG database, which was involved in cell proliferation, angiogenesis, coagulation, dysregulation of inflammatory process, uterine contractions, and release of estrogen or progesterone in uterus. As well as the synthesis or release of inflammatory factors such as prostaglandin and the regulation of calcium channels and so on. Conclusion: GFC has the function by the interaction of pentacyclic triterpenes and steroids with multi target proteins, such as arachidonic acid metabolism, calcium signaling pathway, GnRH signaling pathway, complement and coagulation cascades, and progesterone-mediated oocyte maturation, to alleviate the pain of dysmenorrhea and PID, or improve the quality of life for the patients with hysteromyoma through inhibiting uterine contractions, improving microcirculation, and reducing the release of estrogen or promegestone and inflammatory response (such as PGE2, PGF2α, and leukotriene B4).

Mediator Complex Subunit 12 (MED12) is a subunit of the mediator complex, which is believed to regulate global, as well as gene-specific, transcription. It has been reported that MED12 is mutated at high frequency in hysteromyoma. Recent studies have also shown that MED12 presents with different mutation frequencies in hysteromyoma patients of different populations. However, there are few studies with regard to the MED12 gene mutation in hysteromyoma patients in the Chinese Han population. In the present study, the MED12 mutations of 171 patients with hysteromyoma were analyzed; the results showed that 93 patients exhibited different MED12 mutations, including 131G→T, 131G→A, 130G→A, 146C→T, 130G→A, 130G→C, 128A→C, 130G→T, 127Ins27, 118_132Del15, 117_134Del18, 131_148Del18 and 141_165Del15. The mutation frequency was similar to that found in individuals of African descent or individuals of other non-Caucasian ethnicities, and lower than that in the Finnish or North American populations. Further analysis of 141 patients whose hysteromyoma was measured showed that the mutation frequency of MED12 in patients with large hysteromyomas was significantly lower than that in those with small hysteromyomas. These results suggested that MED12 mutation was important in the development of hysteromyomas in the Chinese Han population and that the size of the hysteromyoma may negatively correlate with the mutation frequency of MED12. This study supplemented current information on MED12 mutations in different races and may aid in developing personalized diagnoses for patients with hysteromyoma in the future.

Objective To investigate the effect of Yiqihuayu herb on serum NF-κB p65 and myocardin in patients with hysteromyoma. Methods 56 cases patients with hysteromyoma were selected and divided into two groups.The control group of 28 cases were treated with mifepristone, and the experiment group of 28 cases were treated with Yiqihuayu herb.The clinical curative effect, uterine myoma volume, expression of NF-κB p65 and myocardin level were compared after treatment.Results The clinical curative effect of the experiment group was 89.29%, which was higher than 67.86% of control group(χ2 =3.82,P<0.05).Compared with control group, NF-κB p65 expression level was lower(P<0.05), myocardin expression level was higher ( P<0.05 ) , the volume of uterus and uterine myoma were smaller ( P<0.05 ) .Conclusion Yiqihuayu recipe could reduce the expression of NF-κB p65 in serum, increase myocardin level, and reduce the volume of uterus and myoma, which has good clinical curative effect in the treatment of patients with hysteromyoma.

ABSTRACT

Objective To explore the targeted distribution of flurbiprofen axetil (FA) in hysteromyoma and its influence on prostaglandin E2 (PGE2).Methods A total of 40 female patients undergoing myomectomy (ASA-Ⅱ) were enrolled in the present study. The patients were randomly divided into blank control group and FA groups of three different doses. The four groups were injected with normal saline, 0.5, 1, or 1.5 mg/kg of FA 15 min before the operation. Tumor tissues and normal tissues were collected by surgical operation and the sampling time was recorded. The concentration of flurbiprofen (FP), which was the active metabolite of FA, was detected in the tumor tissues and normal tissues by HPLC. The concentrations of PGE2 were detected by ELISA in the two samples. Results FP was not detected in the tumor tissues or normal tissues of the blank group. With the increase of FA dose in the three groups, the concentration of FP had an increasing tendency in the tumor tissues, with significant differences found among the three groups (P 0.05). The concentrations of FP in the tumor tissues were significantly higher than those in the normal tissue in the three FA groups (P2 had a tendency to decrease in the tumor tissues; and there were significant differences between blank group and 1, 1.5 mg/kg FA groups (P 2 in the tumor tissues were significantly higher than those in the normal tissues in the four groups (P 0.05). Conclusion FA has a certain targeted distribution in hysteromyoma and can reduce the level of PGE2.
