ABSTRACT

BACKGROUND: Breast cancer is among the most common malignancies worldwide. With progress in treatment methods and levels, the overall survival period has been prolonged, and the demand for quality care has increased. AIM: To investigate the effect of individualized and continuous care intervention in patients with breast cancer. METHODS: Two hundred patients with breast cancer who received systemic therapy at The First Affiliated Hospital of Hebei North University (January 2021 to July 2023) were retrospectively selected as research participants. Among them, 134 received routine care intervention (routing group) and 66 received personalized and continuous care (intervention group). Self-rating anxiety scale (SAS), self-rating depression scale (SDS), and Functional Assessment of Cancer Therapy-Breast (FACT-B) scores, including limb shoulder joint activity, complication rate, and care satisfaction, were compared between both groups after care. RESULTS: SAS and SDS scores were lower in the intervention group than in the routing group at one and three months after care. The total FACT-B scores and five dimensions in the intervention group were higher than those in the routing group at three months of care. The range of motion of shoulder anteflexion, posterior extension, abduction, internal rotation, and external rotation in the intervention group was higher than that in the routing group one month after care. The incidence of postoperative complications was 18.18% lower in the intervention group than in the routing group (34.33%; P <0.05). Satisfaction with care was 90.91% higher in the intervention group than in the routing group (78.36%; P <0.05). CONCLUSION: Personalized and continuous care can alleviate negative emotions in patients with breast cancer, quicken rehabilitation of limb function, decrease the incidence of complications, and improve living quality and care satisfaction.

ABSTRACT

BACKGROUND: Several groups proved kidney-sparing surgery (KSS) had equivalent oncological outcomes compared with radical nephroureterectomy (RNU) for the low-risk upper urinary tract urothelial carcinoma (UTUC) patients. Whereas, the clinical efficacy of KSS for high-risk UTUC, especially for distal high-risk ureteral carcinoma, remains unclear. OBJECTIVE: To evaluate the feasibility of KSS for patients with distal high-risk ureter cancer. MATERIALS AND METHODS: Our study included 22 patients who diagnose the distal high-risk ureter cancer and underwent KSS between May 2012 and July 2021 in the First Affiliated Hospital of Chongqing Medical University. Overall survival (OS), confirmed as the primary endpoint of present study, was assessed by a blinded independent review committee (BIRC). The secondary endpoints included the postoperative SF-36 (the short form 36 health survey questionnaire) score, progression-free survival (PFS), postoperative complications, and so on. RESULTS: Overall, 17 (77.3%) and 5 (22.7%) patients underwent segmental ureterectomy (SU) and endoscopic ablation (EA), respectively. By the cut-off date, the mean OS was 76.3 months (95% Cl: 51.3-101.1 months) and the mean PFS was 47.0 months (95% Cl: 31.1-62.8 months), respectively. And the SF-36 score in a majority of patients was >300 (90.9%). CONCLUSION: This is a daring endeavor to explore the clinical efficacy of KSS in distal high-risk ureter cancer based on the high-risk UTUC criteria, which shows satisfactory results in the long-term prognosis and operation-associated outcomes. However, future randomized or prospective multicenter studies are necessary to validate our conclusions.

The purpose of this humanbecoming hermeneutic sciencing was to discover the emergent meanings in the living quality of humanuniverse, Albert Einstein. The author's dialogue with various media centered on the question: What are the emergent meanings in the living quality of the legendary figure, Albert Einstein? For this humanbecoming hermeneutic sciencing, the works investigated by the author were limited to only three media sources: Einstein Revealed, a public television production; the book that inspired the work, Einstein: His Life and Universe, by Walter Isaacson; and the BBC-DVD video, Einstein and Eddington. There are hundreds of books, essays, films, and other works about Einstein that were not included in this hermeneutic investigation.

The article builds on the previous review conducted by Doucet and Bournes in 2007, which addressed studies conducted from 1985-2006. ABSTRACT

OBJECTIVES: To investigate the effect of continuous nursing on rehabilitation of patients with acute ischemic stroke (AIS). METHODS: In this prospective study, 150 patients were treated for AIS at our hospital from January 2017 to December 2019. The patients were separated into two groups: the conventional group (n = 75) that received conventional nursing and the continuous nursing group (n = 75) were treated with continuous nursing intervention. Patients were followed up for compliance, negative mood, living quality, nursing satisfaction, and daily living abilities. RESULTS: The compliance rate of the continuous nursing group (93.33%) was obviously higher than that of the conventional nursing group (85.33%, P<0.05). There was no obvious difference in the negative mood scores between the two groups before intervention (P>0.05); however, the negative mood scores of the two groups decreased after intervention. In particular, the scores in continuous nursing group were obviously lower than that in the conventional nursing group (P<0.05). After intervention, the living quality scores in the continuous nursing group were obviously higher than that in the conventional nursing group (P<0.05). The satisfaction of nurses in the continuous nursing (96.00%) was obviously higher than that in the conventional nursing group (70.00%, P<0.05). The Barthel index (BI) scores in the continuous nursing group were obviously higher than those in the conventional nursing at 1 d, 7 d, and 30 d after intervention (P<0.05). CONCLUSIONS: The results demonstrated that continuous nursing can enhance the rehabilitation compliance of patients with AIS, alleviate the negative mood of patients, enhance the living quality and daily living ability of patients.

ABSTRACT

OBJECTIVE: The effects of continuing nursing care under cognitive behavioral intervention on the psychological state and living quality of patients with double J catheter after ureterolithiasis surgery were analyzed in this research. METHODS: The research subjects in this study were 142 patients who were discharged from our hospital with a double J tube after ureterolithiasis surgeries from June 2019 to June 2020. According to the patient care plan, the subjects were divided into the control group (n=71) and the observation group (n=71) and respectively treated with routine basic nursing measures and continuing nursing care under cognitive-behavior intervention. Subsequently, the psychological state, living quality and the complications that occurred were compared between the two groups. RESULTS: The depression and anxiety scores of the two groups after 4 weeks of intervention, comparing to thoes before intervention, were seen with remarkably decreased significance (P<0.05). The scores of living quality and self-care quality of patients in the observation group were obviously higher than those in the control group (P<0.05). In addition, the incidence of complications in the observation group was critically lower than that in control group (P<0.05). CONCLUSION: Continuous nursing care under cognitive behavioral intervention can substantially improve the psychological state, living quality and self-care ability of patients with double J tube after ureterolithiasis surgery, and reduce the incidence of complications, which is worthy of clinic promotion.

ABSTRACT

OBJECTIVE: In this study, we investigated the effects of 5A nursing mode combined with fine nursing management on perioperative self-efficacy and living quality of hysteromyoma. METHODS: 116 hysteromyoma patients admitted to our hospital during August 2018 to August 2020 were enrolled as the research objects, and divided into control group and observation group by random-number-table method. Each group contained 58 cases. The control-group patients were treated with conventional basic nursing, while the observation-group patients received combined treatment of 5A nursing mode and fine nursing management. Subsequently, the changes of self-efficacy, living quality of the two groups of patients before and after interventions, and the occurrence of postoperative complications in both groups were recorded and compared. RESULTS: The self-efficacy scores of both groups after intervention were markedly higher than those before intervention (P < 0.05), and the score of the observation-group after intervention was obviously higher than that of control-group (P < 0.05). The scores of the quality of life of the two groups after intervention were remarkably higher than those before intervention (P < 0.05), and the score of observation group after intervention was substantially higher than that of control group (P < 0.05). In addition, the incidence of complications in observation group was notably lower than that in control group, and the difference was statistically significant (10.34%, 29.31%, χ2 = 6.5619, P = 0.0104). CONCLUSION: The combined treatment of 5A nursing mode and fine nursing management has showed good nursing concept and standard nursing methods. It can effectively improve the self-efficacy and quality of life in hysteromyoma patients during perioperative period.

ABSTRACT

OBJECTIVES: This study analyzed the effect of multi-platform extended care on postoperative self-efficacy and quality of life in patients with osteoporotic vertebral compressive fracture (OVCF). METHODS: 162 OVCF patients who underwent percutaneous vertebroplasty (PVP) or percutanous kyphoplasty (PKP) surgery in our hospital from January 2018 to June 2019 were classified into a control group (n=78) and an observation group (n=84) based on the admission time. The control group was given conventional health guidance and follow-up by telephone, and the observation group got multi-platform extended care. The postoperative incidence of re-fracture, Oswestry dysfunction index (ODI) before and after intervention, self-efficacy and quality of life were compared between the two groups. RESULTS: Incidence of re-fracture in the observation group was higher than that of the control group (P<0.05). The ODI scores of the two groups 3, 6, and 12 months after operation were lower than those on discharge (P<0.05), and the observation group had lower OD scores than the control group 6 and 12 months after operation (P<0.05). The self-efficacy scores of the two groups 6 months after discharge were higher than that on discharge (P<0.05), and the index in the observation group was higher than that of the control group (P<0.05). In addition, the scores of all dimensions of quality of life in two groups 6 months of discharge were higher than those on discharge (P<0.05), and the scores in the observation group were higher than those of the control group (P<0.05). CONCLUSION: Multi-platform extended care can effectively reduce the risk of postoperative re-fracture in OVCF patients, facilitate the improvement of patients' lumbar function, self-efficacy, and quality of life, and improve the prognosis of patients, which is worthy of clinical promotion.

ABSTRACT

OBJECTIVE: This study explored and analyzed the effect of Internet-based self-management on pulmonary function rehabilitation and living quality in patients with chronic obstructive pulmonary disease (COPD). METHODS: Altogether, 106 stable-staged COPD patients admitted to our hospital from June 2018 to September 2019 were enrolled as the research subjects, and divided into the control group (n=51) and the observation group (n=55) according to the order of visit. The control group received conventional health education, and the observation group was treated with an educational model by Internet-based self-management. Afterwards, the changes of self-management ability, pulmonary function and living quality before and after intervention were compared between the two groups. RESULTS: The scores of each dimension of self-management behavior in both groups of patients after intervention were critically higher than those prior to intervention (P<0.05), and the scores in observation-group patients were remarkably higher than thoes in control-group patients (P<0.05). Besides, the two groups of subjects were observed with elevated pulmonary function indexes FVC, FEV1 and 6MWD after intervention compared with that in prior-intervention (P<0.05), and the indexes in the observation-group after intervention were obviously higher than those in the control-group (P<0.05). In addition, the scores of each dimension of living quality in both groups after the intervention had a conspicuously rise compared with the prior-intervention period (P<0.05), and the scores in the observation group were remarkably lower than those in the control group (P<0.05). The satisfaction in observation group was obviously higher than that in control group (P<0.05). CONCLUSION: Internet-based self-management can effectually improve the self-management ability of COPD patients. It is conducive to improve the patients' pulmonary function, and their living quality, thus it is worthy of clinical popularization and application.

ABSTRACT

OBJECTIVE: To determine the impact of tinnitus-related handicap on daily living of tinnitus sufferers and factors associated with tinnitus severity. METHODS: The cross-sectional study was conducted from November 2016 to April 2017at Al-Nafees Medical College Hospital, Islamabad, Capital Development Authority Hospital, Islamabad, Lahore General Hospital, Lahore, Bahawal Victoria Hospital, Bahawalpur; and Wassay Ear Nose Throat Clinic, Muzaffarabad, Pakistan. Tinnitus patients of either gender aged 15-75 years were included. Detailed history was taken and examination was conducted. Data was collected using the Tinnitus Handicap Inventory, and was analysed using SPSS 21. RESULTS: Of the 152 patients, 87(57.20%) were males. The overall mean age was 44.10±16.06 years. Tinnitus was severe in 53(34.87%), while catastrophic severity was noted in 26(17.10%). Cases with hearing loss and male gender suffered significantly more in daily life activities (p<0.05). Severity of tinnitus handicap was associated with the type of tinnitus sound (p<0.05). CONCLUSIONS: Gender, hearing loss and type of tinnitus sound were found to be associated with the severity of the condition.

ABSTRACT

PURPOSE: As the most poorly tolerated and debilitating form of spinal malalignment, sagittal imbalance is becoming an increasingly recognized cause of pain and disability in adults. However, there is evidence showing that sagittal imbalance has a weak or no correlation with health-related quality-of-life (HRQoL) outcomes. The objective of this study was to describe the direct factor associated with HRQoL in terms of Oswestry Disability Index (ODI) assessment. PATIENTS AND METHODS: This study retrospectively evaluated the clinical and radiographic information of 179 elderly patients with degenerative lumbar disorders and suboptimal sagittal standing posture (sagittal vertical axis>50 mm). Patient-reported outcomes were assessed using ODI. Patients with ODI≥40% were assigned to Group D (disability), while those with ODI<40% were assigned to Group ND (non-disability). RESULTS: Compared with Group ND (n=104), patients in Group D (n=75) had greater thoracolumbar kyphosis, pelvic incidence-lumbar lordosis (PI-LL), sagittal vertical axis (SVA), T1 pelvic angle, and fat infiltration, and smaller LL and muscle mass ratio. Pearson analysis revealed a high correlation between the percentage of fat infiltrated and ODI (r=768, P<0.01) and moderate correlation between SVA and ODI (r=0.408, P<0.001). Linear regression results indicated that fat infiltration was an independent factor associated with ODI. ODI significantly correlated with SVA in patients with major fat infiltration (r=0.328, P=0.001), while having no correlation with SVA in those with moderate or minor fat infiltration (r=0.083, P=0.464). CONCLUSION: Lumbar muscle fat infiltration is an independent factor associated with the living quality in terms of ODI assessment in the elderly population with degenerative lumbar disorders, which has more correlations with ODI scores than the sagittal imbalance. The relationship between HRQoL outcomes and sagittal imbalance depends on the quality of lumbar muscle.

Suddenly hearing she had breast cancer, M.A., one of the authors, a nurse with discerning witness, ponders her experience of rolling stones. At the time she worked in a center dedicated to living humanbecoming with person-family-community concerned with a cancer illness. Humanbecoming paradigm was the specific discipline guiding the care in the center. Moment-to-moment, the author (M.A.) was moving with emerging meaningful experiences of having breast cancer, shapeshifting her living quality with surprising uncertainties. The humanbecoming principles structuring meaning, configuring rhythmical patterns, and cotranscending with the possibles illustrate her story with the metaphorical expression of mountains as meaningful in a Swiss novel.
