ABSTRACT

Resumo O objetivo deste estudo foi descrever as mudanças na frequência do consumo de refeições de adolescentes residentes na região metropolitana do Rio de Janeiro, em período de 5 anos. Utilizaram-se dados de dois estudos transversais, de base populacional, que foram realizados por meio de visitas domiciliares. Em 2005, a amostra final foi de 1089 domicílios com 511 adolescentes (de 12 a 18 anos), e em 2010 de 1121 domicílios com 314 adolescentes. A frequência do consumo de refeições foi obtida por meio de questionário autopreenchido e a avaliação da adequação de peso dos adolescentes foi realizada com base nos pontos de corte do IMC por sexo e faixa etária. Houve aumento da substituição diária do almoço tradicional por lanche (3,7% para 13,7%) e no consumo do jantar tradicional (62,9% para 72,0%). Os adolescentes com sobrepeso consumiram o desjejum com menor frequência do que aqueles sem sobrepeso (2005: 68,3% vs 79,3% p=0,02, 2010: 59,5% vs 77,4% p=0,03). Conclui-se que houve aumento da substituição diária do almoço tradicional por lanche e no consumo do jantar tradicional no período de 5 anos. Ademais, a prática do desjejum diário se associou com a classificação de IMC nos dois momentos do estudo, de modo que aqueles com sobrepeso consomem o desjejum com menor frequência.


Abstract This study described changes in meal frequency over a 5-year period among adolescents living in the Rio de Janeiro metropolitan region. The data used were from two cross-sectional, population-based studies conducted by home visits. In 2005 the final sample was 1089 households with 511 adolescents (aged 12 to 18 years) and in 2010, 1121 households with 314 adolescents. Meal frequency was obtained through self-administered questionnaire and the adolescents were assessed for appropriate weight by BMI cut-off points, by sex and age group. Increasingly, traditional daily lunch was replaced by snacks (from 3.7% to 13.7%) and traditional dinner was eaten (62.9% to 72.0%). Overweight adolescents ate breakfast less often than those not overweight (in 2005, 68.3% and 79.3%, p=0.02 and, in 2010, 59.5% and 77.4%, p=0.03). Traditional daily lunch was increasingly replaced by snacks and consumption of traditional dinner increased over the 5-year period. Also, eating breakfast every day was associated with BMI classification at both study times: those who were overweight consumed breakfast less frequently.

BACKGROUND: Food timing affects circadian rhythms involved in weight control. Regular consumption of breakfast may affect body weight. OBJECTIVE: We examined the relation between breakfast frequency with weight change in middle-age women over a 3-y period. METHODS: We used data from 65,099 nonpregnant women aged >20 y participating in the Mexican Teachers' Cohort (MTC) who at baseline (2006-2008) were cancer free and for whom self-reported breakfast frequency at baseline was available. We analyzed body weight change between baseline and the first follow-up (2011) according to breakfast frequency. Participants were classified according to baseline breakfast frequency 0, 1-3, 4-6, or 7 d/wk and meal frequency 1-2, 3-4, or ≥5 meals/d. We used linear and modified Poisson regression to analyze body weight change as a continuous variable and for weight gain ≥5 kg (yes/no), respectively. Models were adjusted for sociodemographic and lifestyle confounders. RESULTS: At baseline, 25% of participants were daily breakfast consumers and 18.4% of women increased ≥5 kg between 2008 and 2011. The prevalence of weight gain ≥5 kg among daily breakfast consumers was 7% lower than among those who skipped breakfast (prevalence ratio: 0.93; 95% CI: 0.89, 0.97; P-trend = 0.02). The association was stronger among normal-weight women at baseline with a corresponding estimate of 0.87 (95% CI: 0.79, 0.97; P-trend = 0.02). CONCLUSION: Daily breakfast consumption was inversely associated with weight gain ≥5 kg over 3 y in middle-aged Mexican women. Regular breakfast may be an important dietary factor for body weight change.

ABSTRACT

Calcium is an essential micronutrient that plays a role in growing and pregnancy, and its necessity is increased during pregnancy in adolescence. Thus, the aim of the study is to describe the daily calcium intake and its associations with dietetic habits, sociodemographic data and perinatal outcomes among pregnant adolescents. A prospective cohort study was conducted among primiparous adolescents who started prenatal care before 20 weeks of gestation. Sociodemographic data, weight and height, 24-h dietary recall (24hRec) and perinatal outcomes were collected over four meetings (three during pregnancy and one in puerperium). All 24hRecs were analysed by the Nutrition Data System for Research (NDSR)® programme, and descriptive analysis and univariate and multivariate logistic regression were done. A total of 150 pregnant adolescents were included, with a mean of daily calcium intake of 659.9 mg (50% of recommended intake). Adolescents who ate more than three meals per day (89.3%), and ate breakfast every day (69.3%), were shown to have higher daily calcium intake, odds ratio (OR CI 95%) of 3.4 (1.0, 11.0) and 16.8 (1.0, 302.1), respectively. No correlation was observed between calcium daily intake and sociodemographic data or perinatal outcomes. Dairy products were the foods that mostly contributed to achieving recommended daily calcium intake. In our cohort, pregnant adolescents had a low daily calcium intake. They should be advised to eat more than three meals per day, eat breakfast in particular, increase the consumption of calcium rich-foods, such as dairy products and green leafy vegetables, and consider calcium supplementation.

O objetivo desse trabalho foi comparar o intervalo de tempo de registro de cinco minutos com quatro intervalos de tempo (10, 15, 20 e 30 minutos), para avaliação dos comportamentos comendo, ruminando, em ócio, em pé, apresentados por bovinos confinados individualmente. Foram utilizados 62 machos Nelore desmamados, confinados durante 112 dias. O comportamento foi registrado a cada 28 dias, durante 24h, por observações diretas com rota de amostragem scan e intervalos de registro de 5 minutos, para o cálculo das seguintes características: tempo de alimentação, tempo de ruminação, tempo em ócio, tempo em pé e frequência de alimentação. Foram comparados os intervalos de registro por meio de variâncias, correlações e médias das características. Observou-se para as características, tendência de aumento da variância residual à medida que o intervalo de tempo entre dois registros aumentou, com exceção da frequência alimentar, em que foi observada tendência de diminuição da variância residual com o aumento do intervalo de registro. As médias ajustadas para as medidas de comportamento ingestivo obtidas nos intervalos de registro de 5, 10, 15, 20 e 30 minutos não diferiram significativamente (P>0,05) entre si, com exceção de tempo de alimentação, que apresentou menor valor (P<0,05) no intervalo de registro de 30 minutos em comparação ao intervalo de 5 minutos. Observou-se que até o intervalo de registro de 15 minutos, as correlações com o intervalo de registro de 5 minutos podem ser consideradas altas. Entretanto, acima de 15 minutos de intervalo de registro, a correlação com o intervalo de registro de 5 minutos cai abruptamente, deixando claro que intervalos de registros superiores a 15 minutos são inadequados para estimar as características de comportamento ingestivo de bovinos avaliadas.

Sixty-two weaned Nellore males housed in individual pens for 112 days were used. Ingestive behavior was evaluated every 28 days for 24 h. Direct observations were made using scan sampling and instantaneous sampling at intervals of 5 minutes. The recording intervals were evaluated using error variance, correlations, and adjusted means. For all traits, the residual variance tended to increase with increasing recording interval, except for meal frequency whose residual variance decreased with increasing recording interva. The adjusted means for the ingestive behavior measurements obtained at intervals of 5, 10, 15, 20 and 30 minutes did not differ significantly (P>0.05), except for meal frequency which was lower (P<0.05) in the 30-minute recording interval. The correlations with the 5-minute recording interval were considered high up to the 15-minute interval. However, for longer recording intervals, the correlation with the 5-minute recording interval decreased abruptly, showing that recording intervals longer than 15 minutes are inadequate to estimate ingestive behavior traits of cattle.

Methods: This was a cross-sectional study with a representative sample of 708 schoolchildren aged 7-14 years. Data on personal information, socioeconomic status, physical activity and number of meals were obtained through semi-structured questionnaire and consumption by 24-h recall and food record. Weight and height measurements were also performed to calculate the body mass index. Finally, blood samples were collected for analysis of total cholesterol, high- and low density lipoprotein, triglyceride, and glucose levels. Descriptive statistics, the Mann-Whitney test, and Poisson regression were used in statistical analysis. Results: Meal frequency <4 was associated in children, family income <3 Brazilian minimum wages (PR = 5.42; 95% CI: 1.29-22.77; p = 0.021) and adolescents, the number of sons in the family >2 (PR = 1.53; 95% CI: 1.11-2.11; p = 0.010). Even in the age group of 10-14 years, <4 meals was related to higher prevalence of body mass index (PR = 1.33; 95% CI: 1.02-1.74; p = 0.032) and low-density lipoprotein (PR = 1.39; 95% CI: 1.03-1.87; p = 0.030) higher after adjustments. Conclusion: Lower frequency of meals was related to lower income in children and adolescents, larger number of sons in the family, and increased values of body mass index and low-density lipoprotein.


Resumo: Objetivo: Investigar a distribuição da frequência de refeições diárias e sua relação com fatores demográficos, socioeconômicos, comportamentais, antropométricos e bioquímicos em crianças e adolescentes. Métodos: Estudo transversal feito com uma amostra representativa de 708 escolares 7 e 14 anos. Os dados sobre informações pessoais, nível socioeconômico, atividade física e número de refeições foram obtidos por meio de questionário semiestruturado e o consumo por meio de recordatório de 24 horas e registro alimentar. Medidas de peso e altura também foram feitas para cálculo do índice de massa corporal. Por fim, coletaram-se amostras de sangue para análises de colesterol total, lipoproteína de baixa e de alta densidade, triglicerídeos e glicemia. Análises descritivas, teste de Mann-Whitney e regressão de Poisson foram usados nas análises estatísticas. Resultados: Frequência de refeições < 4 se associou, em crianças, a renda familiar < 3 salários (RP = 5,42; IC 95%: 1,29-22,77; p = 0,021) e, em adolescentes, ao número de filhos na família > 2 (RP = 1,53; IC 95%: 1,11-2,11; p = 0,010). Ainda na faixa de 10 a 14 anos, < 4 refeições se relacionou a maior prevalência de índice de massa corporal (RP = 1,33; IC 95%: 1,02-1,74; p = 0,032) e lipoproteína de baixa densidade (RP = 1,39; IC 95%: 1,03-1,87; p = 0,030) elevados após ajustes. Conclusão: Menor frequência de refeições se associou a menor renda em crianças e em adolescentes a maior número de filhos na família e valores aumentados de índice de massa corporal e lipoproteína de baixa densidade.

ABSTRACT

Appropriate feeding behaviours are important for child growth and development. In societies undergoing nutrition transition, new food items are introduced that may be unfavourable for child health. Set in rural Nicaragua, the aim of this study was to describe the infant and young child feeding (IYCF) practices as well as the consumption of highly processed snack foods (HP snacks) and sugar-sweetened beverages (SSBs). All households with at least one child 0- to 35-month-old (n = 1371) were visited to collect information on current IYCF practices in the youngest child as well as consumption of SSBs and HP snacks. Breastfeeding was dominant (98%) among 0- to 1-month-olds and continued to be prevalent (60%) in the second year, while only 34% of the 0- to 5-month-olds were exclusively breastfed. Complementary feeding practices were deemed acceptable for only 59% of the 6- to 11-month-old infants, with low dietary diversity reported for 50% and inadequate meal frequency reported for 30%. Consumption of HP snacks and SSBs was frequent and started early; among 6- to 8-month-olds, 42% and 32% had consumed HP snacks and SSBs, respectively. The difference between the observed IYCF behaviours and World Health Organization recommendations raises concern of increased risk of infections and insufficient intake of micronutrients that may impair linear growth. The concurrent high consumption of SSBs and HP snacks may increase the risk of displacing the recommended feeding behaviours. To promote immediate and long-term health, growth and development, there is a need to both promote recommended IYCF practices as well as discourage unfavourable feeding behaviours.
