ABSTRACT

ABSTRACT Objectives: To investigate the efficacy of traditional Chinese medicine (TCM) in the treatment of female stress urinary incontinence (SUI). Method: PubMed, Cochrane, Web of Science, Embase, CNKI, Wanfang, and VIP databases were searched for articles published up to September 2022. Variables were analyzed using weighted mean difference (WMD), standardized mean difference (SMD), odds ratios (OR), and 95% confidence interval (CI). Results: Eight studies containing 744 patients were included in this study. The results demonstrate that TCM therapy had more advantages in improving the clinical outcome of SUI patients (OR = 2.90, 95%CI:1.92-4.37, P = 0.000), reducing the International Consultation on Incontinence Questionnaire Short-Form (ICIQ-SF) score (WMD = -2.41, 95%CI:-2.83- -1.98, P = 0.000), reducing 1-h urinary pad leakage urine volume (WMD = -1.86, 95%CI:-2.23- -1.49, P = 0.000) and increasing Maximum urethral closure pressure (MUCP) (SMD = 0.86, 95%CI: 0.61-1.11, P = 0.000). Conclusion: TCM therapy is effective in improving urinary incontinence symptoms, urodynamics, and quality of life in patients with SUI. This article provides a reference for the application of TCM therapy in women with urinary incontinence.


RESUMEN Objetivo: Investigar la eficacia de la medicina tradicional china (MTC) en el tratamiento de la incontinencia urinaria de esfuerzo (IUE) femenina. Método: Se llevaron a cabo búsquedas de artículos publicados hasta septiembre de 2022 en las bases de datos PubMed, Cochrane, Web of Science, Embase, CNKI, Wanfang y VIP. Las variables se analizaron mediante la diferencia de medias ponderada (DMP), la diferencia de medias estandarizada (DME), la razón de momios (RM) y el intervalo de confianza (IC) del 95%. Resultados: Este estudio incluyó ocho investigaciones con la participación de 744 pacientes. Los resultados indican que la terapia de medicina tradicional china (MTC) presentó mejoras significativas en los resultados clínicos de pacientes con incontinencia urinaria de esfuerzo (IUE) (RM = 2.90, IC del 95%: 1.92-4.37, P = 0.000), al reducir la puntuación del Cuestionario Internacional sobre la Incontinencia y su impacto en la calidad de vida (ICIQ-SF) (DMP = -2.41, IC del 95%: -2.83- -1.98, P = 0.000), así como la disminución del volumen de orina a la 1 hora de fuga de la almohadilla urinaria (DMP = -1.86, IC del 95%: -2.23- -1.49, P = 0.000) y el aumento de la presión de cierre uretral máxima (PCUM) (DME = 0.86, IC del 95%: 0.61-1.11, P = 0.000). Conclusión: La terapia de medicina tradicional china (MTC) resulta efectiva en la mejora de los síntomas de la incontinencia urinaria, la urodinámica y la calidad de vida en pacientes con incontinencia urinaria de esfuerzo (IUE). Este artículo ofrece una referencia para la aplicación de la terapia de MTC en mujeres con incontinencia urinaria.

As variáveis foram analisadas por meio da diferença de média ponderada (DMP), da diferença média padronizada (DMP), da razão de chances (RC) e do intervalo de confiança (IC) de 95%. Resultados: Esta pesquisa envolveu oito estudos, contando com a participação de 744 pacientes. Os resultados indicam que a terapia da medicina tradicional chinesa (MTC) apresentou melhorias significativas nos resultados clínicos de pacientes com incontinência urinária de esforço (IUE) (RM = 2.90, IC de 95%: 1.92-4.37, P = 0.000), ao reduzir a pontuação do Questionário Internacional sobre a Incontinência Urinária - Versão Breve (ICIQ-SF) (Diferença de Média Ponderada = -2.41, IC de 95%: -2.83- -1.98, P = 0.000), assim como na diminuição do volume de urina na 1 hora de vazamento do absorvente urinário (Diferença de Média Ponderada = -1.86, IC de 95%: -2.23- -1.49, P = 0.000) e no aumento da pressão máxima de encerramento uretral (PMCU) (Diferença Média Padronizada = 0.86, IC de 95%: 0.61-1.11, P = 0.000). Conclusão: A terapia da medicina tradicional chinesa (MTC) é eficaz na melhoria dos sintomas da incontinência urinária, na urodinâmica e na qualidade de vida em pacientes com incontinência urinária de esforço (IUE). Este artigo fornece uma referência para a aplicação da terapia da MTC em mulheres com incontinência urinária.

ABSTRACT

A terapia com moxabustão envolve combustão de uma mistura de ervas, composta predominantemente por Artemisia vulgaris. Pode ser aplicada em diversas patologias sendo de grande utilidade para cicatrização de tecidos. O objetivo deste artigo foi abordar o uso da moxabustão em dois cães com feridas contaminadas, em que os tratamentos foram destinados a promover a cicatrização por segunda intenção. É descrito o caso de uma cadela da raça pastor-alemão com deiscência de pontos (caso 1) e o de uma cadela sem raça definida com ferida traumática por mordedura (caso 2). Nos dois casos, o tratamento com a moxabustão, modalidade da Medicina Tradicional Chinesa (MTC), foi efetivo no tratamento das feridas por segunda intenção, melhorando os aspectos de oxigenação, circulação e revitalização de tecidos. Após dois anos no caso 1, o tecido apresenta-se pigmentado e alopécico; no caso 2, a epiderme encontra-se queratinizada, uniforme e com presença de pelos, em menor dimensão há tecido pigmentado.(AU)


Moxibustion therapy involves burning a mixture of herbs, consisting predominantly of Artemisia vulgaris. It can be applied in several pathologies and is very useful for tissue healing. The aim of this article was to present the use of Moxibustion in two dogs with contaminated wounds, whose treatment aimed to achieve healing by second intention. It is presented the case of a German Shepherd dog with dehiscence of points (case 1) and one of a mixed breed dog with traumatic wound by bite (case 2). In both cases, the treatment with Moxibustion, a modality of Traditional Chinese Veterinary Medicine, demonstrated effectiveness in the treatment of wounds by secondary intention, improving aspects of oxygenation, circulation and tissue revitalization. After two years in case 1 the tissue is pigmented and alopecic; in case 2 the epidermis is keratinized, uniform and with the presence of hairs, in a smaller dimension there is pigmented tissue.(AU)

ABSTRACT

Concern with antimicrobial resistance in animal production systems increases the interest toward integrative therapies. The objective of the present report was to report an integrative approach to a goat undergoing rumenotomy. A goat with ruminal acidosis underwent rumenotomy, corrective rumen suture, moxibustion, and treatment of the surgical wound (TFO) with sugar. In the first twenty days, acupuncture was performed on ST36, indirect moxibustion on Sp6 and CV8, and TFO with antibiotic ointment and sugar. The wound contracted 2cm, there was gradual return of ruminal dynamics, and moderate pain. Acupuncture continued from D21 to D50 on Yin tang, BL13, BL20, BL23, and BL22. Although there was drainage of ruminal fluid, the wound contracted 4cm during this period, pain was absent, and ruminal dynamics normal. During the last thirty days, the acupuncture technique surround the dragon was used and BL13 point needled, with complete closure of the wound. The TFO from D21 was performed solely with sugar. During treatment, there was modulation of the inflammatory response, with formation of granulation tissue and neovascularization. On D84, fibrinogen was 100mg/dL. The authors conclude that the use of sugar, acupuncture, and moxibustion contributed to return of normal ruminal motility, wound contraction, and complete tissue reepithelization.(AU)


A preocupação com a resistência antimicrobiana nos sistemas de produção animal aumenta o interesse pelas terapias integrativas. Objetiva-se relatar a abordagem integrativa ao paciente caprino submetido à ruminotomia. Uma cabra com acidose ruminal foi submetida à ruminotomia, ruminorrafia, moxabustão, acupuntura e tratamento da ferida operatória (TFO) com açúcar. Nos primeiros vinte dias realizou-se acupuntura em ST36, moxa indireta em Sp6 e CV8, TFO com pomada antibiótica e açúcar. A ferida contraiu 2cm, houve retorno gradual da dinâmica ruminal e dor moderada. A acupuntura continuou de D21 a D50 em Yin tang, BL13, BL20, BL23 e BL22. Apesar da ferida drenar fluido ruminal teve contração de 4cm nesse período, ausência de dor e retorno fisiológico da dinâmica ruminal. Nos últimos trinta dias foi realizada a técnica de acupuntura cercar o dragão e punturou-se o ponto BL13, com fechamento completo da ferida. O TFO a partir do D21 foi realizado somente com açúcar. Durante tratamento houve modulação da resposta inflamatória com formação de tecido de granulação e neoangiogênesse, o D84 revelou fibrinogênio 100mg/dL. Conclui-se que a utilização do açúcar, acupuntura e moxabustão contribuíram para o retorno fisiológico da motilidade ruminal, contração da ferida e completa reepitelização tecidual.(AU)

ABSTRACT

OBJECTIVE: To investigate the effects of acupuncture and moxibustion on the repair of excisional skin injuries on the back of adult female Wistar rats. METHODS: 90 animals were divided into three groups: C, control; A, acupuncture treatment (needled at traditional acupuncture points BL13, BL17 and ST36); M, moxibustion treatment (overlying same traditional acupuncture points). They were euthanased on days 7, 14 and 21 after injury for removal and preparation of tissue for analysis. RESULTS: The treated groups (A and M) showed no changes regarding the structural analysis relative to the control (C) group. The total number of fibroblast cells in the A and M groups were significantly higher than those in the C group on days 14 and 21. The number of granulocytes was significantly less in the A and M groups compared with the C group on days 14 and 21. The total number of newly formed vessels increased on day 21 and was significantly higher in the A and M groups. The amount of birefringent collagen fibre detected on day 21 was significantly higher in the C group. The amount of glycosaminoglycan and hydroxyproline was similar between the groups. The amount of collagen I did not differ between the groups in any period, despite the increased amount detected over time. The amount of type III collagen did not differ between the groups but the detected amount decreased over the course of the experiment. The amount of transforming growth factor ß1 (TGF-ß1) and vascular endothelial growth factor (VEGF) in the A and M rats was similar but inferior to C rats across all experimental periods. CONCLUSIONS: Acupuncture and moxibustion stimulated fibroblast proliferation and neoangiogenesis, and extended the period of collagen fibre reorganisation in the repair of excisional injuries in adult female rats.

Large snakes of the Boidae family, such as Eunectes murinus, require special techniques to facilitate their care when they are sick. Thus, an acupuncture technique called moxibustion was applied that utilizes burning of the weed Artemisia vulgaris for heating at specific points on the skin without handling the animals. The objective of this case report was to describe the use of moxibustion acupuncture as an auxiliary treatment for snakes. A female, 4.0 meters long and weighing about 30 kg, belonging to a Biological Museum of the Butantan Institute, showed symptoms of weight loss, anorexia and hyperemia in the ventral region. The snake was examined and treated with ciprofloxacin antibiotics and a month showed an area of necrosis that exposed the muscles in the dorsal cranial region. The female was treated with moxibustion and application of silver sulfadiazine cream at the site of the lesion. After seven months of treatment, the animal showed signs of good healing with complete wound closure. This technique of moxibustion could be easily applied without restraint, an important detail given the size of the snake, and with successful closure of the lesion.(AU)

ABSTRACT

La recomendación de la Organización Mundial de la Salud, en relación al porcentaje de partos que deberían tener vía abdominal es 15%, esto dado por condiciones no prevenibles o patologías materno-fetales que contraindican el nacimiento vía vaginal. En Colombia, la Encuesta Demográfica Nacional de 2010, reporta la realización de cesárea en un 34% de los nacimientos. Una de las indicaciones del parto por vía abdominal, es la mala posición fetal, sin embargo, si está circunstancia es diagnosticada antes del término de la gestación, puede ser manejada con técnicas de la Medicina Tradicional China. Se revisaron los textos clásicos y los estudios publicados recientemente, con relación al uso de Medicina Tradicional China en el manejo de la mala posición fetal, encontrándose que existe suficiente información con relación a la seguridad y efectividad del manejo con moxibustión asociada o no a acupuntura en el punto Vejiga 67 (Zhi yin) para mujeres con gestaciones entre 33 y 35 semanas en ausencia de otro factor de riesgo materno o fetal.

ABSTRACT

Fundamento: el uso de los medios naturales, como métodos terapéuticos en el tratamiento de las enfermedades dolorosas constituye en la actualidad un reto para las ciencias médicas. Objetivo: evaluar la efectividad de la aplicación de la Medicina Natural y Tradicional como método terapéutico en el servicio de urgencias del Hospital General Docente Mártires de Mayarí. Métodos: se realizó un estudio observacional, descriptivo, prospectivo, en pacientes con enfermedades dolorosas en la consulta de Medicina Natural y Tradicional, ubicada en el servicio de urgencias del Hospital General Docente Mártires de Mayarí del referido municipio, de octubre 2013 a marzo del 2014. El universo estuvo constituido por 180 pacientes con afecciones dolorosas que acudieron al servicio y que les fue indicado la aplicación de las diferentes modalidades de la terapéutica alternativa. La muestra no probabilística quedó constituida por 150 pacientes, que cumplieron con los criterios de inclusión establecidos para esta investigación, a los que se les aplicó tratamiento natural. Resultados: el grupo etáreo entre 50 y 60 años constituyó el de mayor incidencia, predominó el sexo masculino, el mayor porciento de pacientes pertenecen al nivel cultural obrero, solo el 20 % de la muestra tuvieron accesibilidad anterior a la terapéutica. La bursitis del hombro fue la enfermedad de mayor incidencia en tanto la moxibustión constituyó la técnica más utilizada. La mayoría de los pacientes tratados tuvieron evolución satisfactoria, su situación inicial se modificó con una única sesión de tratamiento y se recomendó la continuidad terapéutica en su área de salud. Conclusiones: la Medicina Natural y Tradicional es un método cómodo y barato que no requiere grandes recursos para su aplicación, no se reportan complicaciones durante la aplicación del tratamiento y se reduce el consumo de medicamentos al mínimo por parte de los mismos.


Background: the use of natural means, like therapeutic methods in the treatment of painful diseases, is a challenge for medical sciences nowadays. Objective: to asses the effectiveness of the application of Natural and Traditional Medicine as a therapeutic method in the emergency department of Mártires de Mayarí General Teaching Hospital, Mayarí, Holguín. Methods: an observational, descriptive, prospective study was conducted in patients with painful diseases who were seen at the Natural and Traditional Medicine consultation of the emergency department of Mártires de Mayarí General Teaching Hospital, Mayarí, Holguín from October 2013 to March 2014. The universe was composed of 180 patients with painful conditions who underwent the application of different treatments of alternative therapeutics. The non-probabilistic sample was composed of 150 patients who matched the inclusion criteria established for the research. These patients underwent natural treatment. Results: the age group between 50 and 60 years predominated as well as male sex. The greatest percent of patients belonged to the working class. Only the 20 % of the sample had had previous accessibility to the therapeutics. Shoulder bursitis was the disease with the greatest incidence, while moxibustion was the most used technique. Most patients had a satisfactory progress and got better with only one treatment session. It was recommended to continue the treatment in the health area of each patient. Conclusions: Natural and Traditional Medicine is a convenient and cheap method that does not require great resources for its application. Besides, no complications are reported during the treatments and the consumption of medicaments is minimized.

ABSTRACT

A moxabustão é uma técnica da medicina tradicional chinesa que utiliza o calor para tratamento de doenças crônicas, do frio e de deficiência. O tratamento com moxabustão é realizado utilizando-se ervas nomeadas Artemisia vulgaris, as quais são queimadas geralmente logo acima da pele nos acupontos. O calor produzido diminui a viscosidade e melhora a circulação sanguínea, aumentando a perfusão de sangue aos órgãos e tecidos.Além disso, a moxabustão estimula o sistema imune, por produção de mediadores imunológicos e aumento do número de linfócitos e neurotransmissores. Baseando-se na utilização de moxabustão indireta no tratamento de úlceras de pressão em humanos este trabalho visa descrever a utilização de moxabustão indireta como uma tentativa de tratamento para feridas de difícil cicatrização, como as causadas por decúbito prolongado em equinos. A moxabustão apresentou-se como uma alternativa de baixo custo e eficiente, promovendo a cicatrização deferidas abertas, profundas e infeccionadas.


The moxibustion is a technic from the Chinese Traditional Medicine which uses heat as a treatment forchronic, "cold" and deficiency diseases. EI tratamiento con moxibustíon se realiza utilizando Ia hierba Artemisia vulgaris, qué son quemados por lo general cerca de la piel en 105 acupuntos (puntos de acupuntura). EI calor producido disminuye Ia viscosidad y mejora Ia circulación sanguínea, aumentando Ia perfusión de Ia sangreen 105 órganos y tejidos. ABSTRACT

BACKGROUND: Obesity is a major health problem worldwide for which conventional therapy efficacy is limited. Traditional Chinese medicine, particularly body acupoint stimulation, provides an alternative, effective, and safe therapy for this medical challenge. The present study was designed to compare the effects of distinct methods to stimulate the same set of acupoints, on anthropometric and biochemical parameters in obese women. MATERIALS AND METHODS: Ninety-nine obese women were randomly assigned to six groups of treatment: Acupuncture with moxibustion, long needle acupuncture with moxibustion, electroacupuncture (EA), EA with moxibustion, embedded catgut with moxibustion (CGM) and sham acupuncture as control. Obesity-related parameters, including body weight, body mass index (BMI), waist and hip circumferences, waist/hip ratio, biochemical parameters (triglycerides, cholesterol, glucose, insulin) and homeostasis model of assessment - insulin resistance (HOMA-IR) index, were determined before and after each treatment. RESULTS: Body weight and BMI were significantly reduced in response to all treatments. Interestingly, acupoint catgut embedding therapy combined with moxibustion was the only treatment that produced a significant reduction in body weight (3.1 ± 0.2 kg, P < 0.001), BMI (1.3 ± 0.1 kg/m(2), P < 0.001), insulin (3.5 ± 0.8 mcU/ml, P < 0.1) and HOMA-IR (1.4 ± 0.2 units, P < 0.01) in comparison with sham group. Furthermore, this treatment was able to bring back obese women to a state of insulin sensitivity, indicating that acupoint catgut embedding therapy combined with moxibustion could be useful as a complementary therapy to reduce the risk of diabetes associated to obesity in women. CONCLUSION: Overall, our results confirmed the effectiveness of acupoints stimulation to assist in the control of body weight in women. They also highlighted the more favorable effects of embedded catgut-moxibustion combination that may be due to the extended and consistent stimulation of acupoints.

Antecedentes: Las revisiones sistemáticas (RS) sobre la eficacia de las intervenciones en salud son consultadas frecuentemente para apoyar la toma de decisiones en salud, al ser de la más alta evidencia, para la Medicina Basada en la Evidencia (MBE). La moxibustión es una intervención terapéutica de la Medicina Tradicional China (MTCh) cuya eficacia, como intervención única, ha sido poco estudiada por la MBE. Es importante evaluar la calidad de las RS publicadas y disponibles sobre esta intervención y hacer una crítica externa de estas revisiones, a la luz del uso de los principios de la MTCh. Métodos: Búsqueda exhaustiva en 9 bases de datos de libre acceso. Preselección y selección de RS. Aplicación de criterios de inclusión, exclusión y de las herramientas de evaluación de calidad de publicación y metodológica (PRISMA y AMSTAR). Extracción de datos. Lo anterior realizado por dos revisores independientes. Resultados: 3 RS, sobre moxibustión para la colitis ulcerativa, la hipertensión y el estreñimiento, se incluyeron. Al evaluar su calidad de publicación, se observó un cumplimiento promedio de 84% y al evaluar su calidad metodológica de un 67%.Todas ellas concluyen que la evidencia no es suficiente para afirmar la eficacia terapéutica de la moxibustión y que se requieren más estudios clínicos rigurosos. Con respecto al uso de los principios de la MTCh, es bajo de un 16.6% y la experticia de quien aplica la técnica no se menciona. Conclusiones y recomendaciones: La calidad de las RS incluidas es baja, en términos generales, principalmente porque los ensayos clínicos controlados aleatorizados que incluyen son de baja calidad. Los principios de la MTCh no son tenidos en cuenta, en la mayoría de estudios. Se requieren de más estudios clínicos que tengan en cuenta las particularidades de esta intervención terapéutica de la MTCh.

El asma es una de las enfermedades crónicas no transmisibles de mayor impacto por su morbilidad y mortalidad. Objetivo: evaluar el tratamiento con moxibustión en los pacientes con crisis ligera del asma bronquial en el servicio de urgencias del policlínico Carlos Juan Finlay desde enero a mayo de 2008. Método: se realizó un estudio de intervención analítico prospectivo en los pacientes que acudieron al servicio de urgencias con crisis ligera de asma bronquial. Constituyeron el universo de estudio 84 pacientes. La muestra se conformó por 42 pacientes con criterios de estertores roncos y/o sibilantes, se tomó en consideración el examen físico realizado por el médico de guardia para la detección de las manifestaciones clínicas. El estudio constó de tres fases para su realización, una de detección, otra de intervención con la técnica de tratamiento de moxibustión indirecta y una última de evaluación por el médico de guardia. Resultados: las manifestaciones predominantes fueron: la tos, la disnea y los estertores secos en el 100 %; seguido por la expectoración, la ansiedad con 88,1 % y 57,1 % respectivamente, 36 de los pacientes necesitaron dos sesiones de moxa para eliminar su manifestación clínica inicial. Conclusiones: la totalidad de los pacientes expresaron sentirse satisfechos con la terapia (AU)

Objective: to evaluate the treatment with moxibustion in patients with slight crisis of bronchial asthma in the emergency service at the polyclinic Carlos Juan Finlay during the year 2008. Method: a prospective analytic intervention study was carried out in patients that came to the emergency service with slight crisis of bronchial asthma. The universe of study was made up of 84 patients. The sample was constituted by 44 patients with sibilant and/or rhonchus rales criteria; it was taken in consideration the physical examination carried out by the doctor on call for the detection of clinical manifestations. The study consisted of three phases for its realization, one for detection, another for intervention with the indirect moxibustion treatment technique and a last one of evaluation for the doctor on call. Results: the predominant manifestations were: cough, the dyspnea and the dry rale in 100 %; followed by expectoration, the anxiety with 88,1 % and 57,1 % respectively, 36 of patients needed two sessions of moxa to eliminate their initial clinical manifestation. Conclusions: all the patients expressed to feel satisfied with the therapy (AU)

Se realizó un estudio observacional, descriptivo y longitudinal sobre la evolución del tratamiento con moxibustión en 150 pacientes con sacrolumbalgia aguda que fueron diagnosticados en la consulta de Medicina natural y tradicional (MNT) del Policlínico Mantilla, durante el primer semestre del año 2008, con el objetivo de determinar su estado evolutivo según criterio clínico al cabo de la décima sesión, caracterizarlos en cuanto a edad, sexo y ocupación, y describir las manifestaciones clínicas de interés antes de someterse a la primera sesión y después de culminada la última. Se midieron las variables: edad, sexo, ocupación, realización de esfuerzo físico, dolor, intensidad, inflamación e irradiación. Se realizaron entrevistas para corroborar los aspectos cualitativos en la aplicación del tratamiento empleado y con los datos obtenidos se confeccionaron tablas para su mejor comprensión. Como resultado más relevante se obtuvo que 90 por ciento de los pacientes que llegaron a la consulta con dolor, refirieran no sentir dolor alguno al finalizar el tratamiento. Los resultados que se derivan de esta investigación se están utilizando en la preparación y actualización del personal médico y de enfermería en la Atención Primaria de Salud(AU)
