ABSTRACT

The Olympic and Paralympic Games are a sporting event that welcomes more than ten thousand top-level athletes. These athletes require daily care before and after training and competition, but are also prone to serious injuries. The polyclinic, set up in the athletes' village, will provide a wide range of high-quality care for the various delegations. Urgent care will be provided by the hospitals of the Assistance publique-Hôpitaux de Paris, which are responsible for the administration of the polyclinic.

ABSTRACT

OBJECTIVE: To optimize the diagnosis of abdominal pain syndrome occurring under the «mask¼ of acute pancreatitis via algorithms for clinical and laboratory examination. MATERIAL AND METHODS: We have retrospectively and prospectively analyzed patients with abdominal pain syndrome. We selected all patients with acute pancreatitis who repeatedly applied with abdominal pain syndrome to polyclinics and hospitals between 2017 and 2021. A personalized algorithm for patients with abdominal pain syndrome occurring under the «mask¼ of acute pancreatitis has been developed. This algorithm underlies an information system for decision-making support. RESULTS: An optimal diagnostic algorithm is needed in reception departments of hospitals and polyclinics. This one depends on equipment of hospitals and needs to be constantly improved. When refusing hospitalization and discharging patients with recurrent pain syndromes, physicians should orient the doctors of general network to use more informative methods. Indeed, the last ones may be unavailable in reception departments and various districts due to technical, qualification and organizational shortcomings. CONCLUSION: Advanced clinical and laboratory diagnostic methods based on the above-described algorithm are necessary for recurrent abdominal pain syndrome occurring under the «mask¼ of acute pancreatitis.

ABSTRACT

Based on clinical models of problem situations, for the first time in the form of structured algorithms, the organization of a doctor's work is described - the process of operational activity of a specialist when considering medical, legal, departmental and interdepartmental issues in child psychiatry. The activity of a children's specialist, requiring intensification during outpatient admission, in its content represents a complex of sequential operational actions, built in stages, logical algorithms for making productive professional decisions. The algorithms of activity take into account the legal, diagnostic, clinical, therapeutic, rehabilitation and preventive stages in the organization of providing assistance. The productivity and efficiency of the work of a child psychiatrist, his specialization, experience, competencies, orientation in legislation and modern scientific and practical industry issues of diagnostics and assistance are indispensable components of the optimal process of organizing a doctor's activities. The availability and quality of assistance in modern society are achieved by observing the continuity of the activities of specialists, saving human resources, and developing service units. Of particular importance is the adoption of the right decisions by the health authorities and the achievement of an effective balance between community care, dispensary, hospital replacement technologies and inpatient specialized care.

ABSTRACT

The analysis summarises the experience of a sample of patient at the University Polyclinic in Messina (Italy) and provides a detailed assessment of the satisfaction of patients experiencing healthcare at different Departments. Information collected through a specific survey allowed to build a dataset with more than 350 observations. Regressors were carefully selected and compared through a radar chart. The estimation of a logistic model was then carried out. The results outlined the relevant factors for patient satisfaction: they depend both on the ambulatory where the care is provided and the judgement about quality of care. Other crucial factors in determining a higher satisfaction were the availability of parking lots, the cleaning of structures and the judgment on physicians, the latter endorsing the probability of being highly satisfied when expectations on physicians' competences and professionalism are confirmed. The "Contact details", i.e., the indications of the people to contact in case of need, strengthen the overall patients' positive experience. The study enriches the existing literature on patient satisfaction and is aimed at rethinking the organization of the health assistance offered at University Polyclinics, outlining the aspects to improve, with the objective to guarantee the highest patient satisfaction.

ABSTRACT

Objectives: The study aimed to determine the prevalence of self-medication with antibiotics among attendants of the Out-patient Department (OPD) at Madina Polyclinic before seeking medical consultation and associated factors. Design: Cross-sectional study. Setting: The study was conducted at Madina Polyclinic. Participants: The study involved 319 general OPD attendants aged 18 years and above accessing healthcare services at the Madina Polyclinic between May and June 2019. Main outcome measures: The prevalence of antibiotic self-medication and the factors associated with this practice. Results: From the study, 46.4% (95% CI 40.8%- 52.0%) had self-medicated with antibiotics before presenting for medical consultation at the hospital. Less than half of the respondents (44.5%) had adequate knowledge about the use of antibiotics. Having a tertiary level of education was significantly associated with self-medication (aOR= 8.09, 95% CI 2.31-28.4, p = 0.001), whilst adequate knowledge on the use of antibiotics reduced the odds of self-medication by 53% (aOR= 0.47, 95% CI 0.23- 0.66, p<0.001). The level of education modified the relationship between knowledge and self-medication with antibiotics. Conclusion: The practice of antibiotic self-medication is rife among OPD attendants. Therefore, adequate public education on the use of antibiotics and the effects of using them inappropriately must be done. The Antimicrobial Resistance (AMR) policy ought to be reinforced and made known to all, especially among the pharmacies that dispense antibiotics indiscriminately. Funding: None declared.

Introduction: Patient satisfaction is one of the most important components of measuring healthcare quality. Objectives: The study aimed to evaluate the validity and reliability of the patient satisfaction scale with the quality of health services and its associated factors. Methods: A cross-sectional study was conducted to collect data on patient satisfaction with 301 outpatients at one polyclinic in Hanoi, Vietnam. Results: The overall outpatient satisfaction was 53.5%. There were five factors (facilities, services provision results, information transparency and administrative procedures, accessibility, and interaction and communication of staff) including one major factor with high Eigenvalues coefficient, 22.5 for satisfaction with facility, and four others with lower Eigenvalues coefficient, 3.2, 2.0, 1.5, and 1.2 for satisfaction with service provision results, information transparency and administrative procedures, accessibility, and interaction and communication of staff respectively. All satisfaction-factors show internal consistency reliability, with a Cronbach's Alpha of over 0.9. The insured are 3.5 times (95% CI: 1.9-6.2) more likely to be satisfied with health services than the uninsured. Conclusion: The patient satisfaction measurement tool should be used for intervention to improve the quality of health services at the clinic.

ABSTRACT

BACKGROUND: There is unfinished reform in primary care in Russia and other former Soviet Union (FSU) countries. The traditional 'Semashko' multi-specialty polyclinic model has been retained, while its major characteristics are increasingly questioned. The search for a new model is on a health policy agenda. It is relevant for many other countries. OBJECTIVES: In this paper, we explore the strengths and weaknesses of the multi-specialty polyclinic model currently found in Russia and other FSU countries, as well as the features of the emerging multi-disciplinary and large-scale primary care models internationally. The comparison of the two is a major research question. Health policy implications are discussed. METHODS: We use data from two physicians' surveys and recent literature to identify the characteristics of multi-specialty polyclinics, indicators of their performance and the evaluation in the specific country context. The review of the literature is used to describe new primary care models internationally. RESULTS: The Semashko polyclinic model has lost some of its original strengths due to the excessive specialization of service delivery. We demonstrate the strengths of extended practices in Western countries and conclude that FSU countries should "leapfrog" the phase of developing solo practices and build a multi-disciplinary model similar to the extended practices model in Europe. The latter may act as a 'golden mean' between the administrative dominance of the polyclinic model and the limited capacity of solo practices. The new model requires a separation of primary care and outpatient specialty care, with the transformation of polyclinics into centers of outpatient diagnostic and specialty services that become part of hospital services while working closely with primary care. CONCLUSION: The comprehensiveness of care in a big setting and potential economies of scale, which are major strengths of the polyclinic model, should be retained in the provision of specialty care rather than primary care. Internationally, there are lessons about the risks associated with models based on narrow specialization in caring for patients who increasingly have multiple conditions.

ABSTRACT

To improve quality and efficiency of primary health care and to strengthen its preventive directivity is possible through health care modernization including implementation of management technologies, development of database for evaluation and control. The purpose of the study is to develop method of evaluating activity of stomatologic clinic. The methodology was based on concepts of "functional systems" and "decision making". The listing of informative criteria was based on data analysis of functioning of stomatologic service of the Chechen Republic in 2002-2016 and results of sociological survey of stomatologists (n=181), health care administrators in stomatology (n=45), paramedics (n=220) and patients (n=359). The experts (n=13) developed required number of criteria and their gradation. The algorithm of evaluating activity of stomatologic clinic was developed using systematic approach. The listing of 52 criteria and their 10 blocks were established. The experts established coefficients of significance of criteria (0.0084-0.0781; ≥0.0224 - more significant, <0.0224 - less significant). Three levels were determined for each criterion (according principle of minimization) that became a basis for calculation of integrated indicator of efficiency of functioning of stomatologic polyclinic. The expanded methodological approaches ensure comparartivity of estimates of stomatologic polyclinic in various time periods. The health care authorities have opportunity to objectify analysis of functioning of a number of polyclinics in different periods.

ABSTRACT

The analysis of opinions of district pediatricians about state and perspectives of development of primary medical sanitary care for children in the Kabardino-Balkarian Republic permitted to obtain competent assessment of quality of polyclinic medical care. The positive tendencies in activities of children polyclinics were established. However, the problem of achieving higher quality of medical care of children continues to be actual. According to survey results, character and direction of organizational managerial decision making concerning improvement of material and technical equipment of children polyclinics in accordance with modern requirements (37.6%), elimination of defects in organization of free medication support (63.6%), improvement of organization of free nutrition supply of children (50%), etc. Among the priority tasks of pediatric service is the solution of the medical personnel issue and full staffing of pediatric districts with physicians (66.7%), paramedical personnel (29.6%) and medical specialists (61.7%), as well as the need to include in staff of the polyclinic positions of child psychologist (54.0%) and social worker (31.5%). One of the main problems of primary medical sanitary care of modern pediatric service remains high work overload of district pediatricians that which undoubtedly impacts quality of medical and preventive activities. The low readiness of district pediatricians to active acceptance of innovative transformations in functioning of children polyclinic, requiring psychological readjustment of medical personnel is established. This was most strikingly manifested by process of implementing information technologies and the National project "Lean Polyclinic". All issues identified by sociological survey significantly strained during coronavirus pandemic in 2020. The pandemic inputted its own adjustments in medical care support of children in the region. Despite the fact that disease did not affect children to large extent, due to prolonged absence of physicians suffering from coronavirus, quality of dynamic monitoring and treatment of children suffered. Factually, no dispensarization activities were implemented and prevention measures as well. The results of survey contributes to the formation of monitoring of quality of out-patient care of children at the regional level and makes the research data highly relevant.

ABSTRACT

The comprehensive diagnosis of abnormalities of development of children during the first years of life is one of the most important targets and the first stage of system of early medical care based on the intersectoral interaction of the experts. The health care facilities play paramount role in identifying children at risk, including ones born with signs of perinatal damage of central nervous system. The article presents results of the experimental study of efficiency of common diagnostic methods of development of children during the first years of life in conditions of polyclinic services. The article also proves the necessity of complementing actual diagnostic instruments by new methods of diagnostic based on complex evaluation of main parameters of psychomotor development of children that will result in timely diagnostic and inclusion of children of risk group to the system of early medical care.

ABSTRACT

The article considers main issues of implementing the lean polyclinic project in practice of Russian polyclinic institutions. The main problems that can be solved by implementing the principles of economical production, optimizing operations and eliminating losses, including financial ones, are identified. It is concluded that saved funds can be distributed to motivate employees in the form of higher salaries and bonuses, as well as to provide training opportunities, to attend conferences by specialists. This approach results in reasonable use of the Federal funds, the mandatory medical insurance Fund, and regional budgets.

ABSTRACT

Background: A hospitals' superior service is expected to be of higher value than other available provisions, which consequently differentiates the facility from others, as the branding easily attracts the community attention. The purpose of this study, therefore, is to identify the most needed and profitable health services from existing hospitals. Design and methods: This was a descriptive research performed with a cross sectional study approach. The variables studied include the number of visits, and revenue based on National Health Insurance (JKN). Results: Findings show that the polyclinics were the highest number of visits between 2016 and 2017 include Cardiac, Internal Medicine, Medical Rehabilitation, Nerve, General Surgery, and also Dental & Mouth. Conversely, those with the most significant income include Heart, Polyclinics, Dental & Mouth, as well as General Surgery Polyclinics. Moreover, the Medical Rehabilitation and Internal Medicine outpatient installations demonstrated negative INA income, while the already running featured Services in high demand were Heart, Nerve, Dental & Oral, and also General Surgery polyclinics. Conclusions: In can be concluded that not all polyclinics with high traffic generate positive income, hence it is necessary to monitor and analyze National Health Insurance (JKN) monthly income.

ABSTRACT

The organization of out-patient care of elderly people has its own clinical and organizational characteristics. The study was carried out to establish quality factors of ambulatory polyclinic medical care of elderly people from the position of attending physician. The sampling consisted of 80 physicians interviewed. Out of them 70 were females (87.5%), 50 local therapists and general practitioners and 30 physicians of various specialties. The genuinely elaborated questionnaire was applied. Results and discussion. It is established that most physicians are aware of the need of improving quality of medical care of elderly patients and developing geriatric care. The percentage of elderly patients in ambulatory polyclinic network makes up to 50%, and in particular cases up to 60-70%. The majority of elderly patients visit their physician 1-2 times per month. More than 50% of physicians consider elderly patients have to be observed by physicians of geriatric profile that confirms actual need in this medical specialty. The social and organizational factors of quality of medical care are identified: insufficient duration of physician admission, low level of preferential medications support, lacking of cash resources in patients to purchase prescribed medications, difficulties in making an appointment to medical specialist and district physician, difficult access of medical organization. The orientation of doctors in provision of care to elderly patients only to medication therapy is established. Conclusion. The specific characteristics of identified factors of quality of medical care of elderly patients are to be considered in developing directions of optimizing medical care for this contingent.

ABSTRACT

To identify the most effective areas of out-patient care of children, an in-depth sociological study was carried out in the Kabardino-Balkaria Republic. The questionnaire allowed to get answers on satisfaction of parents with the quality, volume and availability of out-patient care, as well as to study the social status of respondents. The survey included 495 mothers aged of 18-45 years. Out of them, 70.0% had 1-2 children, 30.0% had 3 or more children. The orientation of the majority of families to the birth of 1-2 children formed one of the lowest in the republics of the North Caucasus total fertility rate of 1.61. The results of the study of parental satisfaction with the level of medical care of children in out-patient settings established that the main problem is sufficiently correlated with the data of similar studies in different regions of Russia. According to the data obtained, 61.0% of the respondents were satisfied with the overall health care in the clinic, and only 12.0% of the respondents were fully satisfied. At the same time, a quarter of the respondents expressed their dissatisfaction with various aspects of medical care. The main reasons for dissatisfaction are the low level of examination, organizational issues that need to be addressed, a significant workload of doctors, often the lack of doctors of narrow specialties. According to parents opinion, the clinic is lacking continuity in the work and dynamic monitoring of a sick child, as well as awareness of patients about the ongoing therapeutic and preventive measures. The low efficiency of preventive work is established. The dissatisfaction with the availability and quality of medical care of children is the basis for the necessary measures to improve functioning of the children's clinic, including introduction of new and effective organizational and medical technologies, improvement of material and technical equipment, improvement of the professionalism of medical personnel. The prerequisite for the success of the clinic is also compliance with ethical principles in Pediatrics. The results obtained on the basis of the analysis of the parents' opinion are valuable for development of the perspective programs of development and improvement of primary health care of children in the Republic of Kabardino-Balkaria.

ABSTRACT

The article deals with the study of problem areas and prospects for the implementation of the «Moscow polyclinic standard¼ (SME). The aim of the work was to analyze the factors on the part of medical workers that affect the effectiveness of the implementation of innovative projects in health care. The assessment of the doctor's and nurse's attitude to the changes caused by the introduction and willingness to work in a new format was studied. During the pilot study, which covered 730 medical workers, including 358 doctors and 374 nurses working in outpatient medical organizations of the capital, a sociological survey was conducted on specially designed questionnaires. It was found that the introduction of SMEs contributed to the revision of a number of responsibilities and activation of medical personnel, focusing on meeting the needs and requirements of the population in health services. Thus, the SME fits into the WHO strategy of orienting primary health care institutions to integrate into the integrated model of care delivery. At the same time, the SME has significantly freed up the time of doctors to work directly with patients. The activity of nurses was changed, they received much more independence in the work. Therefore, the scope of the received results are the health authorities of Moscow and also the management of outpatient medical organizations. In General, doctors and nurses positively assesses the changes. New working conditions are to some extent stressful factors and need to be corrected, which should be taken into account by the heads of medical organizations.

OBJECTIVE: This paper aims to describe the medical service of two polyclinics of the PyeongChang Winter Olympic Games and to analyse the injury and illness of athletes who visited the polyclinics during the oilympic period in order to provide some insight with respect to the future construction and operation of polyclinics in mass gathering events such as the Olympic Games. METHODS: The PyeongChang Olympic Village was located near the Olympic Stadium for snow sports athletes and the Gangneung Olympic Village was located near the ice venues for ice sports athletes. During the Olympic Games, polyclinics were consisted of emergency service and outpatient clinics. We retrospectively analysed the electronic medical record data of athletes who visiting polyclinics between 9 February 2018 and 25 February 2018. RESULTS: During the Olympics, there were 1639 athlete encounters in both polyclinics. Among those, injuries of athletes were 237 (14% of all athlete encounters) in total, and the most common injured site was knee joint. Upper respiratory infection was the most frequent case in diseases of athlete encounters. Total 223 cases of image study were done, MRI was 44 cases. CONCLUSION: The PyeongChang Winter Olympic Games had the highest number of participants in the history of Winter Olympic Games. Overall 48% of athletes encountered polyclinics due to disease during the Games period. Upper respiratory infection and other seasonal diseases were more frequent this Olympic Games than before. Polyclinics were managed healthcare of athletes as well as injury and illness of athletes. In winter sports, a polyclinic and similar medical facilities should be prepare for diseases considering geography, weather as well as injuries and endemic diseases when planning future mass gathering events.

INTRODUCTION: Precancerous cervical lesion is significantly a health problem globally. Thus, screening targeting women between the ages of 17-60 is being undertaken in developing countries, including Cameroon. Over 50% (7.8 per 100,000) women die of cervical cancer every year. This study was to determine the prevalence of precancerous cervical lesion, the age demography and access the risk factor. METHODS: A hospital-based cross-sectional study was conducted from August 09th to October 17th 2017. A total of 60 women participated, and were screened for precancerous cervical lesion. Data were collected by using a questionnaire. Visual inspection with acetic acid and visual inspection with Lugol's iodine was applied for the screening. SPSS version 16.0 was used for data entry and analysis. Logistic regression analysis was fitted and odds ratios with 95% confidence intervals and p-values were computed to identify factors associated with precancerous cervical cancer lesion. RESULTS: Out of 60 study participants, 2(3.33%) were found to be positive for precancerous cervical cancer lesion. CONCLUSION: The prevalence of precancerous cervical lesion in women that consulted at the Mezam polyclinic is high.

ABSTRACT

PURPOSE: The 2018 PyeongChang Winter Olympic Games involved 2925 elite athletes, and providing proper health care services for these elite athletes was a critical priority. We established an emergency department (ED) in the Pyeongchang Mountain Polyclinic during the Olympics, which served staff and athletes from many countries. This experience, as well as a description of illnesses and injuries encountered during the games, may provide useful information for planning medical care at similar events in the future. MATERIALS AND METHODS: The polyclinic ED operated from January 25 to February 27, 2018. All cases were enrolled in this study, and their data were analyzed by date and category. In addition, the number of injuries by body part, number of illnesses by organ system, and illness symptoms and causes were analyzed. RESULTS: In total, 288 patients were encountered in the ED. These included 113 injuries and 175 illnesses. We consulted with 153 staff members and 75 athletes, and reported that the fingers were the most commonly injured body part, followed by the knee. The respiratory system was the most commonly involved organ system, and the most common cause of illness was infection. Thirty-eight influenza tests were performed, among which the results of seven were positive. We performed 17 norovirus tests, among which the results of four were positive. CONCLUSION: Our analysis of our ED experience will aid arrangements for medical services in future Winter Games. RESUMEN Introducción: En Cuba, la enfermera de la familia se distingue por tener múltiples áreas de trabajo y para cada una son necesarios diferentes recursos. Se desconoce si es mucho o poco el recurso consumido de cara a las actividades realizadas. Objetivo: Caracterizar la eficiencia de la actividad de enfermería en consultorios del médico y enfermera de la familia. Métodos: Estudio descriptivo transversal con la totalidad de consultorios de tres policlínicos del municipio 10 de Octubre. Se resolvió un modelo de análisis envolvente de datos de maximización de resultados y rendimientos constantes a escala con seis indicadores de resultados y cuatro de recursos. Resultados: La media de la eficiencia estuvo entre 86,66 por ciento y 95,63 por ciento. Para mejorar la eficiencia se deben alcanzar los siguientes valores promedios de cumplimiento del seguimiento: atención materno infantil, entre 85,32 por ciento y 88,58 por ciento; para cada paciente con enfermedades crónicas, entre 8,99 por ciento y 41,67 por ciento; para pacientes con enfermedades transmisibles, entre 34,87 por ciento y 54,55 por ciento; para el adulto mayor, entre 65,70 por ciento y 90,23 por ciento; y para las citologías vaginales, entre 91,97 por ciento y 162,72 por ciento. Además, reducir en promedio el gasto en material gastable, entre 11,25 por ciento y 47,28 por ciento; en medicamentos, entre 15,31 por ciento y 107,22 por ciento; y en recursos humanos entre 15,01 por ciento y 32,72 por ciento. Conclusiones: Se determinaron niveles altos de eficiencia técnica pura para la actividad de enfermería en los consultorios, aunque se verificó la presencia de unidades ineficientes. Existen unidades con ineficiencia estructuralmente determinada, y para solucionarla es necesario un análisis del consumo de recursos(AU)


ABSTRACT Introduction: In Cuba, the family nurse is distinguished by having multiple work areas, and for each one different resources are necessary. It is not known if the resource consumed is too much or too little for the activities carried out. Objective: To characterize the efficiency of the nursing activity in family doctor and nurse's offices. Methods: Cross-sectional, descriptive study carried out with all the family medical offices of three outpatient polyclinics in Diez de Octubre Municipality. An analysis model was solved that involved maximization of results and constant returns to scale data with six results indicators and four resource indicators. Results: The average efficiency was between 86.66 percent and 95.63 percent. In order to improve efficiency, the following average values of monitoring compliance must be achieved: mother and child care, between 85.32 percent and 88.58 percent; for each patient with chronic diseases, between 8.99 percent and 41.67 percent; for patients with communicable diseases, between 34.87 percent and 54.55 percent; for the elderlies, between 65.70 percent and 90.23 percent; and for vaginal Pap smear tests, between 91.97 percent and 162.72 percent. In addition, there was an average reduction on consumption of materials that can be used, between 11.25 percent and 47.28 percent; in medicines, between 15.31 percent and 107.22 percent; and in human resources, between 15.01 percent and 32.72 percent. Conclusions: High levels of pure technical efficiency were determined for the nursing activity in the outpatient polyclinics, although the presence of inefficient units was verified. There are units with structurally determined inefficiency, and to solve it, an analysis of the consumption of resources is necessary(AU)

ABSTRACT

PURPOSE: The 2018 PyeongChang Winter Olympic Games involved 2925 elite athletes, and providing proper health care services for these elite athletes was a critical priority. We established an emergency department (ED) in the Pyeongchang Mountain Polyclinic during the Olympics, which served staff and athletes from many countries. This experience, as well as a description of illnesses and injuries encountered during the games, may provide useful information for planning medical care at similar events in the future. MATERIALS AND METHODS: The polyclinic ED operated from January 25 to February 27, 2018. All cases were enrolled in this study, and their data were analyzed by date and category. In addition, the number of injuries by body part, number of illnesses by organ system, and illness symptoms and causes were analyzed. RESULTS: In total, 288 patients were encountered in the ED. These included 113 injuries and 175 illnesses. We consulted with 153 staff members and 75 athletes, and reported that the fingers were the most commonly injured body part, followed by the knee. The respiratory system was the most commonly involved organ system, and the most common cause of illness was infection. Thirty-eight influenza tests were performed, among which the results of seven were positive. We performed 17 norovirus tests, among which the results of four were positive. CONCLUSION: Our analysis of our ED experience will aid arrangements for medical services in future Winter Games. Additionally, given our new experience, we will now be able to provide better medical services for future winter sports events.
