ABSTRACT

La radiografía de tórax sigue siendo fundamental para la evaluación de patologías torácicas en lactantes. Antes de iniciar su interpretación, muchos autores sugieren revisar la técnica radiológica, ya que a esta edad se presentan varias particularidades técnicas que deben tenerse en cuenta para evitar errores interpretativos y no confundir hallazgos técnicos con patologías. Entre estas particularidades técnicas se deben evaluar: el centraje transversal o rotación, el centraje longitudinal o posición lordótica, el grado de inspiración, la posición de la vía aérea superior, la penetración o exposición de la radiografía, tipos de proyecciones y el movimiento. El objetivo de esta revisión es comentar y ejemplificar las peculiaridades técnicas que presenta la radiografía de tórax en lactantes y que pueden llevar a interpretaciones erróneas.


The chest X-ray remains essential for evaluating thoracic pathology in infants. Before beginning its interpretation, many authors recommend assessing the radiographic technique, as several technical peculiarities must be considered at this age to avoid interpretive errors and prevent mistaking technical artifacts for pathology. The technical aspects to be evaluated include transverse centering or rotation, longitudinal centering or lordotic position, degree of inspiration, upper airway positioning, radiograph penetration or exposure, projection types, and movement. The objective of this review is to discuss and illustrate the technical peculiarities of infant chest X-rays that can lead to erroneous interpretations.

ABSTRACT

Objetivo: Describir los hallazgos imagenológicos en radiografías de tórax y ecografías pulmonares de pacientes con síndrome post-COVID-19. Métodos: estudio descriptivo, prospectivo y transversal que incluyó pacientes con síndrome post-COVID-19, sometidos a radiografías de tórax y ecografías pulmonares en el Servicio de Neumonología Clínica del Hospital Dr. José Ignacio Baldo, entre enero y octubre de 2022, con la finalidad de establecer su evolución imagenológica pulmonar. Se utilizó estadística descriptiva, chi-cuadrado de Pearson y prueba kappa de concordancia, considerando significativo un valor de p < 0,05. Resultados: La muestra consistió en 58 pacientes con una edad media de 55 ± 13 años, predominando el sexo femenino (58,6%). El 60,3% mostró alteraciones en la radiografía de tórax; un 74,3% con patrón intersticial bilateral y un 25,7% con patrón intersticial unilateral. La ecografía reveló patrón intersticial en el 43,1% de los casos y se observaron dos microconsolidaciones subpleurales. Conclusiones: Las radiografías de tórax y las ecografías pulmonares son herramientas imagenológicas eficaces, accesibles y económicas para detectar alteraciones en pacientes con síndrome post-COVID-19. Objective: To describe imaging findings in chest radiographs and lung ultrasounds of patients with post-COVID-19 syndrome. Methods: A descriptive, prospective, and cross-sectional study was carried out that included patients with post-COVID-19 syndrome, who underwent chest radiographs and lung ultrasounds at the Clinical Pneumonology Service of Dr. José Ignacio Baldo Hospital, between January and October 2022. Descriptive statistics, Pearson's chi-square, and kappa concordance test were used, considering a p-value < 0.05 significant. Results: The sample consisted of 58 patients with an average age of 55 ± 13 years, with a predominance of females (58.6%). 60.3% showed alterations in the chest radiograph; 74.3% with a bilateral interstitial pattern and 25.7% with a unilateral interstitial pattern. The ultrasound revealed an interstitial pattern in 43.1% of the cases and two subpleural microconsolidations were observed. Conclusions: Chest radiographs and lung ultrasounds are effective, accessible, and economical imaging tools to detect alterations in patients with post-COVID-19 syndrome. (AU)

ABSTRACT

RESUMO O pectus excavatum é considerado uma patologia da parede torácica, onde há convexidade no aspecto ventral do esterno, o que gera complicações secundárias, como alongamento ventrodorsal do tórax e aumento da pressão intratorácica, entre outras. No presente caso, o corpo de um cachorro Buldogue Francês nasceu com sinais prévios de decaimento e baixo consumo de leito materno, além de uma avaliação médica forense. De acordo com a avaliação patológica, foi encontrada uma fenda no peito esternal relacionada às esternas caudais próximas à cartilagem xifóide, conteúdo espumoso na cartilagem epiglótica da laringe e sinais graves de enfisema nos lobos caudal, craniano e médio pulmonar. Na avaliação cardiológica, foi observada assimetria morfológica invaginante no septo interventricular na mesma área em que a fenda esternal ocorre, por sua vez, na avaliação radiográfica, evidencia uma depressão dorsal do terço caudal do esterno com alterações consideráveis na silhueta cardíaca. Na Colômbia não há relatos de ninhadas completas que apresentem esta alteração, portanto, pretende-se que este seja o primeiro relatório anatomopatológico e de imagem que descreva a patologia em caninos.


ABSTRACT Pectus excavatum is considered a pathology of the chest wall where there is convexity in the ventral aspect of the sternum, which generates secondary complications, such as ventrodorsal stretching of the chest and increased intrathoracic pressure, among others. In the present case, the body of a French bulldog dog was born with previous signs of decay and low consumption of maternal waste, in addition to a forensic medical evaluation. According to the pathological evaluation, a crack was found in the sternal chest related to the caudal sternum close to the xiphoid cartilage, frothy content in the laryngeal epiglottis cartilage and severe signs of emphysema in the caudal, cranial and middle lung lobes. In the cardiological evaluation, an invaginating morphological asymmetry was observed in the interventricular septum in the same area in which the sternal cleft occurs, in turn, in the radiographic evaluation, it shows a dorsal depression of the caudal third of the sternum with considerable changes in the cardiac silhouette. In Colombia there are no reports of complete litters that present this alteration, so it is intended that this is the first anatomopathological and imaging report that describes the pathology in canines.

ABSTRACT

La actual pandemia de COVID-19 ha afectado múltiples sectores económicos y sociales a escala mundial, con especial afectación sobre el sector de la salud, ya que constituye un reto adicional para la atención primaria, dada la marcada escasez de recursos. La infección por SARS-CoV-2 ocasiona morbilidad y mortalidad crecientes, por lo que el diagnóstico mediante imágenes es una herramienta imprescindible en la práctica clínica; sin embargo, el acceso limitado a algunos medios como la tomografía axial computarizada en diferentes niveles de atención, justifica el uso de la radiografía de tórax como una opción costo-efectiva y accesible en muchas regiones. En el presente artículo se exponen los hallazgos asociados a procesos infecciosos virales sugestivos de infección por el nuevo coronavirus y una serie de escalas de clasificación que buscan estandarizar la lectura e interpretación radiográfica por parte del personal médico.


The pandemic of COVID-19 has affected multiple economic and social sectors worldwide, with special effects on the health sector, since it constitutes an additional challenge for the primary health care, given the marked lack of resources. The infection due to SARS-CoV-2 causes increasing morbidity and mortality, reason why the diagnosis by means of images is an indispensable tool in the clinical practice; however, the limited access to some means as the computerized axial tomography in different levels of care, justifies the use of the thorax x-ray as a cost-effective and accessible option in many regions. Findings associated with suggestive viral infectious processes of infection due to the new coronavirus and a series of classification scales that seek to standardize reading and radiographic interpretation by the medical staff are exposed in this work.

RESUMEN Introducción : La radiografía posteroanterior (PA) de tórax es utilizada rutinariamente como examen complementario en la evaluación de trabajadores. Sin embargo, existe la controversia sobre su indicación y utilidad en todos los trabajadores. El presente estudio tuvo como objetivo describir los factores asociados a hallazgos anormales en radiografías digitales de tórax en trabajadores sin síntomas respiratorios. Materiales y métodos : Se realizó un estudio observacional transversal analítico, utilizando registros de placas radiografías digitales de tórax PA de 24 069 trabajadores asintomáticos respiratorios, evaluados en un establecimiento de salud acreditado para la realización de exámenes médicos ocupacionales en el Perú, entre los años 2016 - 2019. Resultados : De los 24 069 trabajadores asintomáticos, 1166 (4,84 % IC 95 % [4,58 - 5,12]) presentaron radiografía anormal. Los principales hallazgos se encontraron en el parénquima pulmonar y de estas, las imágenes secuelares de tuberculosis fueron las más frecuentes. Se observó que los trabajadores mayores de 50 años (RP(a) = 6,34 IC 95 % [1,68 - 2,01]), los que presentaron bajo peso (RP(a) = 2,71 IC 95 % [1,88 - 3,91]) y los que tuvieron antecedentes patológicos pulmonares (RP(a) = 6,19 IC 95 % [5,54 - 6,91]) presentaron mayor probabilidad de presentar anormalidades en la radiografía pulmonar. Conclusiones : Debido a la baja frecuencia de radiografías digitales anormales de tórax, creemos que no resulta de utilidad como detección precoz de patología respiratoria en la población estudiada y que debería considerarse en trabajadores mayores de 50 años, con antecedente de enfermedad respiratoria y/o con bajo peso. Es importante realizar mayor cantidad de estudios que confirmen nuestros resultados.


ABSTRACT Introduction : Posteroanterior Chest X-Ray (CXR) is usually performed as a routine exam for workers' health surveillance, but their benefits are discussed and controversial. Our investigation described the associated factors with abnormal digital CXR in workers without respiratory symptoms. Materials and methods : An analytical cross-sectional observational study was performed, including 24 069 digital CXR obtained from workers without respiratory symptoms who performed their workers' health surveillance at an accredited occupational health center in Lima, Perú, from 2016 to 2019. Results : Of the 24 069 asymptomatic workers, 1166 (4.84 %, 95 % CI [4.58-5.12]) had abnormal radiography. The main findings were found in the lung parenchyma and of these, sequential images of tuberculosis were the most frequent. It was observed that workers older than 50 years (RP(a) = 6.34, 95 % CI [1.68 - 2.01]), those with low weight (RP(a) = 2.71 95 % CI [1.88 - 3.91]) and those with a history of pulmonary pathology (RP(a) = 6.19 95 % CI [5.54 - 6.91]) were more likely to have abnormalities on pulmonary radiography. Conclusions : Due to the low frequency of abnormal digital Chest X-Ray (CXR), it is not useful as an early detection of respiratory pathology in the population studied, however it should be considered in those over 50 years with a history of respiratory disease and/or low weight. It is important to carry out more studies that confirm our results.

RESUMEN: Los pulmones son el sitio predominante en la infección por COVID-19. Esta puede conducir al síndrome distrés respiratorio agudo (SDRA). Frente a su sintomatología severa, la ventilación mecánica (VM), y sus valores de mecánica ventilatoria aparecen como una herramienta fundamental. Un complemento, para analizar el estado de avance de esta patología es la radiografía de tórax (RT), aunque en ocasiones esta depende de la experiencia del equipo de salud. Así el objetivo de esta investigación fue explorar la relación de las medidas de mecánica ventilatoria y radiográficas con el tiempo de conexión a VM en pacientes COVID-19. Estudio retrospectivo, que incluyó a 23 pacientes en VM. Se recolectó información de variables de mecánica ventilatoria; PEEP, presión plateau, presión de distensión y compliance estática. Desde la RT se midió, altura y ancho pulmonar, ángulo costodiafragmático y espacio intercostal. Los resultados indicaron que las variables de mecánica ventilatoria tales como el PEEP y el plateau se relacionaron significativamente con el tiempo de conexión a VM (r=0,449; p=0,035 y r=0,472; p=0,026), mientras que las variables radiográficas construidas en base al ángulo costodiafragmático y el espacio intercostal presentaron similares comportamientos (r= 0,462; p=0,046 y r=-0,543; p=0,009). En conclusión, la presión resultante de la programación del ventilador mecánico junto a cambios estructurales observados en la RT, se relacionan con el tiempo de conexión a VM.


SUMMARY: The lungs are the predominant site of COVID-19 infection. This can lead to severe acute respiratory síndrome (ARDS). In view of its severe symptoms, mechanical ventilation (MV) and its ventilatory mechanics values appear as a fundamental tool. Chest radiography (CR) is a complement to analyze the state of progress of this pathology, although this sometimes depends on the experience of the health team. Thus, the aim of this research was to explore the relationship of ventilatory mechanics and radiographic measures with connection time to MV in COVID-19 patients. Retrospective study, which included 23 patients on MV. Information on ventilatory mechanics variables was collected; PEEP, plateau pressure, distension pressure and static compliance. And from CR, lung height and width, costodiaphragmatic angle and intercostal space were measured. The results indicated that ventilatory mechanics variables such as PEEP and plateau were significantly related to connection time to MV (r = 0.449; p = 0.035 and r = 0.472; p = 0.026), while the radiographic variables Constructed on the basis of the costodiaphragmatic angle and the intercostal space, they showed similar behaviors (r = 0.462; p = 0.046 and r = -0.543; p = 0.009). In conclusion, the pressure resulting from mechanical ventilator programming, together with the structural changes observed in CR, are related to the connection time to MV.

En la actual pandemia por SARS-COV-2 la elección de una modalidad de imagen que ayude al basa en las condiciones clínicas del paciente, las pruebas de laboratorio y la disponibilidad de equipos de imágenes en los establecimientos de salud. La Tomografía (TC) y la radiografía de tórax (RxT) son las modalidades de imágenes más usadas; la RxT, con menor sensibilidad que la TC, es un método accesible, menos costoso y de menor exposición al personal de salud, se recomienda su uso en las emergencias y en los servicios de hospitalización. El objetivo del artículo es orientar en la toma de decisiones para elegir una modalidad de imagen de acuerdo a escenarios, teniendo en cuenta sus potenciales beneficios y profundizando en la descripción de las características radiográficas de sospecha de infección por SARS-COV-2 que pueden servir en las emergencias y que permiten evaluar la progresión de la enfermedad usando un sistema de puntuación.


In the current SARS-COV-2 pandemic, the choice of an imaging modality to aid diagnosis is based on the patient's clinical conditions, laboratory tests, and the availability of imaging equipment in health facilities. Tomography (CT) and chest radiography (CXR) are the most widely used imaging modalities; CXR, with less sensitivity than CT, is an accessible method, less expensive and less exposure to health personnel, its use is recommended in emergencies and in hospital services. The objective of the article is to guide decision-making to choose an imaging modality according to scenarios, taking into account its potential benefits and deepening in the description of the radiographic characteristics of suspected SARS-COV-2 infection that may serve in emergencies and we assess disease progression using a scoring system.

ABSTRACT

The objective of this study was to correlate cardiovascular parameters with obesity in adult cats. Forty mixed-breed cats (20 obese and 20 controls) underwent echocardiographic measurements and thoracic radiography to establish a relationship between the obtained data and the body condition score (BCS), body mass index (BMI), and vertebral heart size (VHS). Systolic arterial pressure (SAP) was also studied in 15 obese cats. Compared to the control cats, obese cats presented higher VHS values across several measurements, but among the echocardiographic parameters, only the difference in the left ventricular free wall at diastole was statistically significant. A positive relationship was established between body weight, left ventricular internal diameter at diastole, and left ventricular free wall at diastole. Positive relationships were also observed between BMI and diameter of the aorta, BCS and the left ventricular free wall at diastole, VHS and the left atrium, and the left atrial to aortic root diameter ratio and the left ventricular internal diameter at diastole. SAP was higher in obese cats and presented a positive correlation with body weight. Obesity correlated with an increase in SAP and with some echocardiographic and radiographic cardiac measurements in adult cats. Further studies are needed to better assess the effects of obesity on the cardiovascular system of cats and to reassess the parameters related to body weight and condition.(AU)


O objetivo deste estudo foi correlacionar parâmetros cardiovasculares com obesidade em gatos adultos. Foram utilizados 40 gatos mestiços (20 obesos e 20 controles) avaliados por meio de medidas ecocardiográficas e radiografia de tórax, estabelecendo relação entre os dados obtidos e escore de condição corporal (ECC), índice de massa corporal (IMC) e tamanho do coração vertebral (VHS). A pressão arterial sistólica (PAS) também foi estudada em 15 gatos obesos. Os gatos obesos apresentaram valores mais altos de VHS nas várias medições realizadas em comparação com gatos normais, mas apenas a diferença na medida da parede livre do ventrículo esquerdo na diástole foi estatisticamente significativa nos parâmetros ecocardiográficos. Uma relação positiva foi estabelecida entre o peso corporal, o diâmetro interno do ventrículo esquerdo na diástole e a parede livre do ventrículo esquerdo na diástole. Outras relações positivas estabelecidas, incluem o IMC com o diâmetro da aorta (AO), ECC com parede livre do ventrículo esquerdo na diástole, VHS com o átrio esquerdo (AE), e da relação do átrio esquerdo com o diâmetro da raiz da aorta (AE: Ao) com diâmetro interno do ventrículo esquerdo na diástole. A PAS foi maior em gatos obesos e apresentou correlação positiva com o peso corporal. A obesidade se correlacionou com aumento da PAS, e com aumento em medidas ecocardiográficas e radiográficas em gatos. Mais estudos são necessários para melhor avaliar os efeitos da obesidade no sistema cardiovascular da espécie, e reavaliar os parâmetros relacionados ao peso e condição corporal.(AU)

La pandemia de COVID­19 ha resultado en una emergencia de salud global. Los estu­dios de imagen utilizados en esta enfermedad son la radiografía de tórax (RX) y la to­mografía computarizada (TC). Ambas modalidades tienen sus hallazgos descritos, pero no son específicos dado que muchas enfermedades pueden producir patrones simila­res, particularmente las neumonías virales. Los RX de tórax muestra hallazgos consis­tentes en opacidades alveolares las cuales son múltiples, periféricas, bilaterales y basales, mientras que la tomografía de tórax sus hallazgos más frecuentes son presen­cia de patrón en vidrio deslustrado, consolidaciones, engrosamiento septal, patrón en empedrado, dilatación bronquial y engrosamiento peri bronquial, broncograma, patrón de halo invertido y patrón de neumonía organizada. Los hallazgos por imagen depen­den del tiempo de evolución de la enfermedad ya que en etapas tempranas puede ser normal tanto en la RX como la TC. El riesgo de trombo embolismo pulmonar es alto y más frecuente que en pacientes con COVID­19 negativo


The COVID­19 pandemic has resulted in a global health emergency. The imaging stu­dies used in this disease are chest radiography (CXR) and computed tomography (CT). Both imaging modalities findings have had their findings. These findings described are not specific since many diseases can produce similar patterns. CXR shows somewhat consistent findings consisting of alveolar opacities which are multiple, peripheral, bilate­ral and basal, while CT the most frequent findings are the presence of grounded glass pattern, consolidations, septal thickening, crazy paving pattern, bronchial dilation and peribronchial thickening, air bronchograms, inverted halo sign and organized pneumo­nia. Imaging findings depends on the evolution time of the disease since in the early sta­ges both chest radiography and tomography may be normal. The risk for pulmonary embolism is high and more frequent than in patients with negative COVID­19

ABSTRACT

The objectives of this study were to evaluate the accuracy of thoracic radiology as a screening test for congenital heart diseases in dogs, to identify the main contributions and limitations of this modality, and to verify the reproducibility of the evaluations by three observers with different levels of training. An interobserver, observational, retrospective and prospective study was carried out, including ninety dogs: thirty healthy animals, thirty with acquired heart diseases and thirty with congenital heart diseases, which all had thoracic radiographs and a confirmed echocardiographic diagnosis. The cases were separated and randomized by a mediator who did not participate in the reading of the radiographic examinations, and no evaluator had access to the patients' data. Sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV) and accuracy of each observer were calculated in relation to the correct classification of dogs to groups of normal or acquired and congenital heart diseases, as well as identification of enlargement of the cardiac silhouette and large vessels of dogs with congenital heart diseases. Finally, the Kappa coefficient was obtained between the observers to verify the reproducibility of the radiological evaluations performed. In general, sensitivity, PPV and accuracy were unsatisfactory ( 70%), and the agreement ranged from poor to reasonable (between 0 and 0.39). Although greater accuracy was achieved in the differentiation of healthy dogs from those with acquired and congenital heart diseases by thoracic radiography, when compared to the other studies, the modality was able only to identify healthy patients, and could not differentiate the individuals with different forms of heart disease or define the cardiac malformations. In addition, there was low reproducibility between observers, therefore, this technique should not be used as a sole screening method in dogs with suspected congenital heart diseases.(AU)


Os objetivos do estudo foram avaliar a acurácia da técnica radiográfica de tórax como exame de triagem nas cardiopatias congênitas em cães, identificar as principais contribuições e limitações dessa modalidade, e verificar a reprodutibilidade das avaliações realizadas por três observadores com diferentes graus de treinamento. Realizou-se um estudo interobservador, observacional, retrospectivo e prospectivo, o qual foram selecionados 90 cães, sendo 30 saudáveis, 30 com cardiopatias adquiridas e 30 com cardiopatias congênitas, que possuíam radiografia de tórax e diagnóstico ecocardiográfico confirmado. Os casos foram separados e randomizados por um mediador que não participou da leitura dos exames radiográficos, sendo que nenhum avaliador obteve acesso aos dados dos pacientes. Calculou-se os índices de sensibilidade, especificidade, valor preditivo positivo (VPP), valor preditivo negativo (VPN) e acurácia de cada observador em relação a identificação dos pacientes saudáveis, com cardiopatias adquiridas e congênitas, bem como para identificação de aumento da silhueta cardíaca e região de grandes vasos dos cães com cardiopatias congênitas. Por fim, foi obtido o coeficiente de Kappa entre os observadores a fim de verificar a reprodutibilidade das avaliações radiográficas realizadas. De modo geral, a sensibilidade, VPP e acurácia foram insatisfatórios (< 70%), enquanto a especificidade e VPN foram satisfatórios (> 70%), sendo que a concordância variou de ruim a razoável (entre 0 e 0,39). Apesar de ter sido alcançado acurácia maior na identificação de cães saudáveis, com cardiopatias adquiridas e congênitas pela radiografia de tórax quando comparado aos demais estudos, confirma-se que essa modalidade se mostrou capaz apenas de identificar o paciente saudável, não de diferenciar os indivíduos cardiopatas entre si ou de definir com maior detalhe as malformações cardíacas especificamente. Além disso, o exame radiográfico de tórax apresentou baixa reprodutibilidade entre os observadores, portanto, essa técnica não deve ser considerada como método único de triagem na suspeita de cães com cardiopatias congênitas.(AU)

ABSTRACT

The objective of this study was to correlate cardiovascular parameters with obesity in adult cats. Forty mixed-breed cats (20 obese and 20 controls) underwent echocardiographic measurements and thoracic radiography to establish a relationship between the obtained data and the body condition score (BCS), body mass index (BMI), and vertebral heart size (VHS). Systolic arterial pressure (SAP) was also studied in 15 obese cats. Compared to the control cats, obese cats presented higher VHS values across several measurements, but among the echocardiographic parameters, only the difference in the left ventricular free wall at diastole was statistically significant. A positive relationship was established between body weight, left ventricular internal diameter at diastole, and left ventricular free wall at diastole. Positive relationships were also observed between BMI and diameter of the aorta, BCS and the left ventricular free wall at diastole, VHS and the left atrium, and the left atrial to aortic root diameter ratio and the left ventricular internal diameter at diastole. SAP was higher in obese cats and presented a positive correlation with body weight. Obesity correlated with an increase in SAP and with some echocardiographic and radiographic cardiac measurements in adult cats. Further studies are needed to better assess the effects of obesity on the cardiovascular system of cats and to reassess the parameters related to body weight and condition.


O objetivo deste estudo foi correlacionar parâmetros cardiovasculares com obesidade em gatos adultos. Foram utilizados 40 gatos mestiços (20 obesos e 20 controles) avaliados por meio de medidas ecocardiográficas e radiografia de tórax, estabelecendo relação entre os dados obtidos e escore de condição corporal (ECC), índice de massa corporal (IMC) e tamanho do coração vertebral (VHS). A pressão arterial sistólica (PAS) também foi estudada em 15 gatos obesos. Os gatos obesos apresentaram valores mais altos de VHS nas várias medições realizadas em comparação com gatos normais, mas apenas a diferença na medida da parede livre do ventrículo esquerdo na diástole foi estatisticamente significativa nos parâmetros ecocardiográficos. Uma relação positiva foi estabelecida entre o peso corporal, o diâmetro interno do ventrículo esquerdo na diástole e a parede livre do ventrículo esquerdo na diástole. Outras relações positivas estabelecidas, incluem o IMC com o diâmetro da aorta (AO), ECC com parede livre do ventrículo esquerdo na diástole, VHS com o átrio esquerdo (AE), e da relação do átrio esquerdo com o diâmetro da raiz da aorta (AE: Ao) com diâmetro interno do ventrículo esquerdo na diástole. A PAS foi maior em gatos obesos e apresentou correlação positiva com o peso corporal. A obesidade se correlacionou com aumento da PAS, e com aumento em medidas ecocardiográficas e radiográficas em gatos. Mais estudos são necessários para melhor avaliar os efeitos da obesidade no sistema cardiovascular da espécie, e reavaliar os parâmetros relacionados ao peso e condição corporal.

Resumen: Introducción: El objetivo de este estudio fue desarrollar un modelo predictivo sobre la presencia de tuberculosis pulmonar activa utilizando datos clínico-epidemiológicos y hallazgos de radiografía simple (Rx) y tomografía computadorizada (TC) de tórax. Material y métodos: Se realizó un estudio observacional, retrospectivo, descriptivo y analítico, que recopiló 22 variables clínico-epidemiológicas, 11 hallazgos radiológicos en Rx de tórax y 23 en la TC, que se realizaron en pacientes con sospecha clínica de tuberculosis pulmonar durante un período de 10 años. Se aplicó un modelo de regresión logística multivariado a los predictores potenciales de cultivo positivo, obteniendo un modelo predictivo. Resultados: Se recogieron 1.540 pacientes con sospecha clínica de tuberculosis a los que se les realizó Rx y TC torácico. El cultivo fue positivo en 101 casos. Se utilizó un proceso de eliminación hacia atrás para obtener el mejor conjunto de variables predictivas. Se obtuvieron 24 variables que fueron significativas (6 clínicas, 5 de Rx y 13 de TC) y se les asignó una puntuación. A la suma de estas puntuaciones se restó la edad en años multiplicada por 0,03. El modelo sugiere el diagnóstico de tuberculosis pulmonar activa en pacientes con una puntuación superior a 1,845. Obtuvo una sensibilidad de 85,1%, especificidad de 83,6%, valor predictivo positivo de 26,6%, y valor predictivo negativo de 98,7%. El área bajo la curva ROC fue de 0,9163. Conclusión: Este sistema de puntuación basado en criterios clínico-epidemiológicos y hallazgos radiológicos puede ayudar a diagnosticar tuberculosis pulmonar activa en casos de sospecha diagnóstica.


Abstract:Introduction: The objective of this study was to develop a predictive model on the presence of active pulmonary tuberculosis using clinical-epidemiological data and findings of chest radiography and thoracic computed tomography (CT). Material and methods: An observational, retrospective, descriptive and analytical study was conducted, which collected 22 clinical and epidemiological variables, 11 radiological findings on chest x-ray and 23 on CT that were performed in patients with clinical suspicion of pulmonary tuberculosis during a period of 10 years. A multivariate logistic regression model was applied to the potential predictors of positive culture, obtaining a predictive model. Results: We collected 1,540 patients with clinical suspicion of tuberculosis who underwent radiography and thoracic CT. The culture was positive in 101 cases. A backward elimination process was used to obtain the best set of predictive variables. We obtained 24 variables that were significant (6 clinical, 5 of chest plain films and 13 of CT) and were assigned a score. The sum of these scores was subtracted from the age in years and multiplied by 0.03. The model suggests the diagnosis of active pulmonary tuberculosis in patients with a score higher than 1.845. The model obtained a sensitivity of 85.1%, specificity of 83.6%, positive predictive value of 26.6, and negative predictive value of 98.7%. The area under the ROC curve was 0.9163. Conclusion: This scoring system based on clinical-epidemiological criteria and radiological findings can help rule out active pulmonary tuberculosis in cases of diagnostic suspicion.

ABSTRACT

Eighty-year old woman with a medical history of arterial hypertension, rheumatic valvular heart disease with mitral and aortic valve replacement, chronic atrial fibrillation and congestive heart failure dependent on domiciliary chronic oxygen for 17 daily hours. The reason for arrival to the hospital was a stroke of cardioembolic etiology.


Mujer de 80 años con antecedentes de hipertensión arterial, valvulopatía reumática con recambio valvular mitral y aórtica, fibrilación auricular crónica e insuficiencia cardiaca congestiva dependiente de oxígeno crónico domiciliario durante 17 horas diarias. El motivo de llegada al hospital fue ictus cerebral de etiología cardioembólica.

Resumen La neumonía se define clínicamente como la presencia aguda de fiebre acompañada de síntomas y signos respiratorios que pueden estar o no acompañados de hallazgos radiográficos. Sin embargo, los signos y síntomas que acompañan la enfermedad en la edad pediátrica son inespecíficos. La radiografía de tórax continúa siendo la prueba de referencia para el diagnóstico de neumonía. Contrario a esto, la radiografía de tórax no se recomienda de forma rutinaria ante la sospecha de neumonía. La tomografía y la resonancia magnética ofrecen mejores herramientas diagnósticas, aunque todas conllevan a una exposición a radiación ionizante y a un mayor costo en los servicios de salud. Es conocido que la radiación ionizante es capaz de causar potencialmente daños a los tejidos y más aún en los niños. En los últimos años, diferentes estudios proponen la ecografía pulmonar como ayuda diagnóstica para la neumonía al ofrecer ventajas como no requerir uso de sedantes, no expone a radiación ionizante, fácil transporte y ofrece mejor resolución en lo referente al diagnóstico de septos. Muchos estudios han demostrado la efectividad de la ecografia pulmonar para el diagnóstico de la neumonía. Un metaanálisis reciente reportó una sensibilidad de 96 % (IC95 % 94-97 %) y una especificidad del 93 % (IC95 % 90-96 %) de la ecografía pulmonar para el diagnóstico para la neumonía. Deduciendo que la ecografía pulmonar se proyecta como una herramienta diagnóstica para la neumonía, llegando a ser igual o superior a la radiografía de tórax.


Abstract Pneumonia is defined clinically as the acute presence of fever accompanied by respiratory symptoms and signs that may or may not be accompanied by radiographic findings. However, the signs and symptoms that accompany the disease in the pediatric age are nonspecific. Chest radiography continues to be the gold standard for the diagnosis of pneumonia. Contrary to this, chest radiography is not routinely recommended in case of suspected pneumonia. The tomography and the magnetic resonance offer better diagnostic tools, nevertheless these, along with the x-ray, lead to an exposure to ionizing radiation and to a greater cost in the health services. It is known that ionizing radiation is capable of potentially causing damage to tissues and even more so in children. In recent years, different studies have proposed pulmonary ultrasound as a diagnostic aid for pneumonia, offering advantages such as not requiring the use of sedatives, exposure to ionizing radiation, easy transport and offering better resolution regarding the diagnosis of septa. Many studies have demonstrated the effectiveness of pulmonary ultrasound for the diagnosis of pneumonia. A recent meta-analysis reported a sensitivity of 96% (95% CI 94-97%) and a specificity of 93% (95% CI 90-96%) for pulmonary ultrasound for the diagnosis of pneumonia. Deducing that the pulmonary ultrasound is projected as a diagnostic tool for pneumonia, being equal or superior to chest radiography.

Resumen Comunicamos el caso clínico de un paciente de 54 años de edad que ingresó al servicio por dificultad respiratoria. Inició su padecimiento actual un mes previo a su ingreso con tos productiva con expectoración purulenta abundante, fiebre no cuantificada y pérdida de peso, dos días previos a su ingreso se agregó dificultad respiratoria que se exacerbó aproximadamente tres horas previas a su ingreso. A la exploración física se observó paciente consciente con signos vitales: frecuencia cardiaca 130 lpm, frecuencia respiratoria 27 rpm, presión arterial 115/73 mmHg, temperatura 36.4ºC, con disnea, uso de músculos accesorios, taquicardia, taquipnea, con saturación a aire ambiente de 89%, como antecedentes de importancia destacaron toxicomanías positivas; se integraron datos clínicos de neumonía adquirida en la comunidad; la radiografía de tórax evidenció borramiento del ángulo costofrénico y costodiafragmático por lo que se sospechó derrame pleural; sin embargo, se decidió realizar ultrasonido pulmonar para confirmar derrame pleural vs consolidación pulmonar, en el que se observaron datos compatibles con escaso derrame pleural y zona de consolidación pulmonar basal derecha; se dio tratamiento con doble esquema antibiótico. Con este artículo se demuestra que el ultrasonido pulmonar resulta ser una herramienta efectiva y confiable en el diagnóstico temprano de neumonía en el servicio de Urgencias, sin necesidad de realizar radiografía de tórax, incluso tiene sensibilidad mucho mayor para el diagnóstico de derrame pleural en comparación con la radiografía convencional.


Abstract This paper reports the clinical case of a 54-year-old male patient, which entered to the service due to respiratory difficulty. Patient initiated his current suffering a month before the hospital admission with productive cough with purulent, abundant expectoration, not quantified fever, and loss of weight, two days before to his hospital admission respiratory difficulty was added that was exacerbated approximately 3 hours prior to income. To the physical exploration conscious patient was observed with vital signs: HR 130 bpm, RR 27 bpm, blood pressure 115/73 mmHg, temperature 36.4ºC, with shortness of breath, use of accessory muscles, tachycardia, tachypnea, with saturation to air ambience of 89%; precedents of importance: positive drug dependency; clinic data were integrated of community-acquired pneumonia, X-ray chest evidenced effacement of the costophrenic and costodiaphragmatic angle; thus, it was suspected pleural effusion; however, it was decided to perform pulmonary USG to confirm effusion vs pleural pulmonary consolidation, in which there were data compatible with low pleural effusion and right basal pulmonary consolidation area; handling was started with double antibiotic scheme. This article demonstrates that pulmonary USG turns out to be an effective and reliable tool in the early diagnosis of pneumonia in the Emergency Department, without necessity of chest X-ray, it has even much larger sensitivity for diagnosis of pleural effusion in comparison with conventional radiography.

ABSTRACT

BACKGROUND: Bronchogenic cyst is a malformation of the ventral portion of the intestine, which is limited by bronchial epithelium and produces alterations in the development of the tracheobronchial tree. They may be single or multiple, and are usually confined to one lung or to the mediastinum, rarely to the neck, which is a subcutaneous tissue. OBJECTIVE: The case of a 9 year old girl is reported, who presented with a clinical picture characterized by a slow-growing, asymptomatic tumour on the left side of the neck of 4 years onset. Chest X-ray, neck ultrasound and computed tomography of the neck and chest ruled out any other injury. A complete resection was performed, and the histopathological study confirmed the diagnosis of bronchogenic cyst. CONCLUSION: The symptomatology of a bronchogenic cyst is due to the compression of the vascular, digestive or air structures, as well as its size, infection and location. The treatment of choice is a surgical resection, even when asymptomatic.

El derrame pleural tiene una prevalencia mundial de aproximadamente 400 de cada 100.000 habitantes y Venezuela tiene cifras similares. Se relaciona con múltiples patologías, por lo que determinar sus características podría ayudar a obtener un mejor diagnóstico y tratamiento. Métodos: Se realizó un estudio de casos, retrospectivo y descriptivo, obteniendo información de las historias de pacientes hospitalizados con derrame pleural en el servicio de Medicina Interna del Hospital Dr. Domingo Luciani en el período Enero 2010- Abril 2015. Resultados: La edad promedio fue de 49±19 años, género masculino (53%). Motivo de consulta: disnea (81%), dolor torácico (44%) y tos (37%). Los síntomas: disnea (92%), dolor pleurítico (58%) y fiebre (54%). Antecedentes personales: HTA (32%), DM (22%) e IC (20%). Radiografía de tórax: (60%) derrame pleural derecho y (26%) izquierdo. Tomografía de tórax realizada en (77%). Citoquímicos: (85%) exudado (53% mononuclear y 32% polimorfonucleares). La prueba de ADA positiva en 25%, cultivo para bacterias realizado en 89 casos, positivos 18%. Bloque celular con resultado inflamatorio (80%). Biopsia pleural realizada (22%): inflamatorio (36,4%), seguido por ADC metástasico (31,8%). Estancia hospitalaria > 15 días (76%) y el diagnóstico final fue infeccioso (51%). Conclusión: Contando con estos datos clínicos- epidemiológicos se puede caracterizar el comportamiento del derrame pleural en nuestro centro para el rápido y acertado diagnóstico, igualmente proponer una investigación prospectiva donde se analice el comportamiento de dicha enfermedad, y crear protocolos de actuación(AU)


Pleural effusion has a worldwide prevalence of approximately 400 per 100,000 inhabitants and Venezuela has similar statistics. It is related to multiple pathologies, which determine their characteristics which could help for better diagnosis and treatment. Methods: A retrospective descriptive case study was conducted, obtaining information from the charts of hospitalized patients with pleural effusion in Internal Medicine Dr. Domingo Luciani Hospital Venezuela in the period January 2010-April 2015. Results: Mean age 49 ± 19 years, male genre (53%). Complaints: dyspnea (81%), chest pain (44%) and cough (37%). Symptoms: dyspnea (92%), pleuritic pain (58%) and fever (54%). Personal history: hypertension (32%), DM (22%) and HF (20%). Chest x-ray: right pleural effusion (60%), left (26%). Chest tomography performed on (77%). Cytochemical: exudate: 85% (53% mononuclear and polymorphonuclear 32%). ADA testing positive in 25%. For bacteria culture: performed in 89 cases, 18% positive. Cell block inflammatory (80%). Pleural followed by metastatic ADC (31.8%). Hospital stay> 15 days (76%) and final diagnosis was infection (51%). Cause of discharge from hospital: improvement (80%). Conclusion: Having these clinical and epidemiological data can characterize the behavior of pleural effusion for quick and accurate diagnosis(AU)

ABSTRACT

Kerley lines represent a radiology sign present on chest radiographs in patients with pulmonary septal thickening. They were first described in 1933 by Sir Peter James Kerley, an eminent English radiologist; as a result of rigorous scientific observation in an era with limited technology resources. With three display patterns (A, B and C); it was determined by histopathological correlation that they corresponded to thickening of the septal connective tissue in different dispositions, in the lung tissue. Today they are a sign which is absolutely current and clinically useful in patients studied for suspected cardiogenic pulmonary edema using simple chest radiography.


Las líneas de Kerley representan un signo radiológico presente en radiografías de tórax en pacientes con engrosamiento septal pulmonar. Fueron descritas por primera vez en 1933, por Sir Peter James Kerley, un eminente radiólogo inglés; fruto de observación científica rigurosa en una época con limitados recursos tecnológicos. Con tres patrones de presentación (A, B y C); mediante correlación histopatológica se determinó que correspondían a engrosamiento del tejido conectivo septal en su distinta disposición en el tejido pulmonar. Hoy en día son un signo plenamente vigente y de gran utilidad clínica en pacientes estudiados con radiografía simple de tórax con sospecha de edema pulmonar cardiogénico.

Introdução: A hipertrofia ventricular esquerda (HVE) é um fator preditor independente de risco cardiovascular em pacientes com doença renal crônica (DRC) em hemodiálise (HD). Objetivo: Mostrar a utilidade da radiografia de tórax no diagnóstico de HVE em pacientes com DRC em HD. Métodos: Estudo transversal que incluiu 100 pacientes (58 homens e 42 mulheres), idade média de 46,2 ± 14,0 anos, com DRC de todas as etiologias, há pelo menos seis meses em HD. Foram obtidos ecocardiograma e radiografia de tórax dos pacientes, sempre até uma hora após o término das sessões de HD. Resultados: A HVE foi detectada em 83 pacientes (83%), dos quais 56 (67,4%) apresentavam o padrão concêntrico e 27 (32,6%) a padrão excêntrico de HVE. Cardiomegalia - definida por índice cardiotorácico (ICT) > 0,5 - esteve presente em 61 pacientes (61%). Foram os seguintes os valores de sensibilidade, especificidade e acurácia, respectivamente, para a variável ICT: 66,2%, 70,5% e 68,0%. A correlação de Pearson entre ICT e índice de massa do ventrículo esquerdo (IMVE) foi de 0,552 (p < 0,05) e razão de verossimilhança positivo de 2,2. Conclusão: A radiografia de tórax é um exame seguro e útil como ferramenta diagnóstica de HVE em pacientes com DRC em HD. .


Introduction: Left ventricular hypertrophy (LVH) is an independent predictor of cardiovascular risk in patients with chronic renal disease (CRD) on hemodialysis (HD). Objective: To show the usefulness of chest radiography in the diagnosis of LVH in CRD patients on HD. Methods: Cross-sectional study including 100 patients (58 men and 42 women), mean age 46.2 ± 14.0 years, with CRD of all causes, for at least six months on HD. Were obtained echocardiogram and chest x-rays of patients, always up to one hour after the end of HD sessions. Results: LVH was detected in 83 patients (83%), of whom 56 (67.4%) had the concentric pattern and 27 (32.6%) with eccentric pattern of LVH. Cardiomegaly - defined by cardiothoracic index (CTI) > 0.5 - was present in 61 patients (61%). The following were the sensitivity, specificity and accuracy, respectively, for the variable ICT: 66.2%, 70.5% and 68.0%. The Pearson correlation between ICT and index of left ventricular mass (LVMI) was 0.552 (p < 0.05) and positive likelihood ratio of 2.2. Conclusion: Chest radiography is a safe and useful as a diagnostic tool of LVH in CKD patients on HD. .
