ABSTRACT

Background: Asthma and allergic diseases have increased in recent decades and are more common in industrialized countries. Industrial areas with a considerably high number of inhabitants and vehicles can favor the presence of serious air pollution and therefore the appearance and exacerbation of respiratory allergy symptoms. The objective of this study was to determine the relationship between exposure to environmental pollutants with exacerbation of respiratory allergy. Methods: A total of 240 subjects above 6 years old who lived in the metropolitan area of Monterrey, Nuevo León, Mexico, with diagnosis of allergic rhinitis and/or asthma, were included. The subject's address was registered in the database and the rhinitis control assessment test (RCAT) and the asthma control test (ACT) were applied. Environmental data were obtained from the Environmental Monitoring System (SIMA) of Nuevo León. Geolocation of industries and avenues in proximity of subject's addresses and SIMA stations were obtained through geographic information systems using ArcGis software. Results: The relation between pollutants and subjects' RCAT, ACT, and spirometry results in the 14 stations was established. PM10 and forced vital capacity (FVC) had an r = 0.074 with p = 0.005, PM10 and absolute FEV1/FVC ratio presented an r = -0.102 with a p = 0.000; The distance found to be associated with a worsening of respiratory symptoms was living 165 m from a main road or 241 m from an industrial establishment. Conclusions: Exposure to pollutants present in the environment are factors associated with increased symptoms in subjects with respiratory allergies.

ABSTRACT

Chemical Respiratory Allergy (CRA) is triggered after exposure to Low Molecular Weight (LMW) sensitizers and manifests clinically as asthma and rhinitis. From a risk/toxicity assessment point of view, there are few methods, none of them validated, for evaluating the respiratory sensitization potential of chemicals once the in vivo-based models usually employed for inhalation toxicity addressment do not comprise allergenicity endpoints specifically. Based on that, we developed, characterized, and evaluated the applicability of a 3D-tetraculture airway model reconstructed with bronchial epithelial, fibroblasts, endothelial and monocytic cell lines. Moreover, we exposed the tissue to maleic anhydride (MA) aerosols to challenge the model and subsequently assessed inflammatory and functional aspects of the tissue. The reconstructed tissue presented phenotypic biomarkers compatible with human bronchial epithelium, and MA aerosol exposure triggered an increased IL-8 and IL-6 production, reactive oxygen species (ROS) formation, and apoptosis of epithelial cells. Besides, augmented IL-8 production by monocytic cells was also found, correlating with dendritic cell activation within the co-culture model after MA exposure. Our results demonstrated that the 3D-tetraculture bronchial model presents hallmarks related to human airways' structure and function. Additionally, exposure to a respiratory sensitizer induced inflammatory and functional alterations in the reconstructed tissue, rendering it a valuable tool for exploring the mechanistic framework of chemically induced respiratory sensitization.

ABSTRACT

Anti-Ascaris lumbricoides (Asc) IgE and IgG can immunomodulate the allergy; however, the influence of these isotypes has not been investigated in the giardiasis and allergy. Therefore, the frequency of respiratory allergy (RA) symptoms in Giardia lamblia-infected children, with or without anti-Asc IgE, IgG1, or IgG4 and Th1, Th2/Treg, and Th17 cytokine production, was evaluated. We performed a case-control study with children aged 2-10 years old selected by questionnaire and stool exams to form the groups: infected or uninfected with RA (G-RA, n = 55; nG-RA, n = 43); infected and uninfected without RA (G-nRA, n = 59; nG-nRA, n = 54). We performed blood leukocyte counts and in vitro culture. Cytokine levels in the supernatants (CBA), serum total IgE and anti-Asc IgE (ImmunoCAP), IgG1, IgG4, and total IgA (ELISA) were measured. Infection was not associated with allergy. Infected children showed increased levels of anti-Asc IgG1, IL-2, IFN-γ, IL-4, and IL-10. There was a lower frequency of allergy-related symptoms in anti-Asc IgG1-positive children than IgG1-negative (OR = 0.38; CI = 0.17-0.90, p = 0.027) and few eosinophils in G-RA than in G-nRA and more in G-nRA than in nG-nRA, whereas TNF-α levels were higher in the G-RA than in the nG-nRA group. For infected and positive anti-Asc IgG1, there was higher TNF-α and IL-10 production than G/-IgG1. IL-10 levels were lower in nG/ + IgG1 than in infected or non-infected, and both were negative for anti-Asc IgG1. Th1/Th2/IL-10 profiles were stimulated in the infected patients, and in those with circulating anti-Asc IgG1, the TNF-α production was strengthened with a lower risk for respiratory allergy symptoms.

ABSTRACT

Respiratory sensitization encompasses a group of diseases that manifest through airway hyperresponsiveness and airflow limitation. Although the concerns regarding human health, to date there are still no validated methods for preclinical assessment of this class of toxicants once the chemical respiratory allergy mechanistic framework is not fully understood. As Dendritic Cells (DCs) are the bridging elements between innate and adaptative immune responses, we preliminarily investigated the biological alterations triggered by seven different LMW respiratory allergens in the DC model THP-1. The results have shown that exposure to respiratory allergens promoted alterations in DCs maturation/activation status and triggered pro-inflammatory changes in these cells through increased expression for the CD86/HLA-DR/CD11c surface biomarkers and enhancement in IL-8 and IL-6 production by exposed THP-1 cells. Therefore, evidence was found to support the startpoint for chemical respiratory allergy pathogenesis elucidation, subsidizing the contribution of dendritic cells in such pathomechanisms.

ABSTRACT

OBJECTIVE: Environmental control includes measures to prevent exposure to common aeroallergens in an individual's home. Questionnaires are part of the clinical practice of health assessment, and are also widely used in research. Our aim was to develop and validate a questionnaire to identify possible sources of aeroallergens present in the indoor environment. METHODS: This study describes the development, validation and application of a questionnaire. For content validation the Content Validation Index and Ordinal Cronbach's Alpha Index have been used; Polychoric Correlations for the agreement between judges; and an Exploratory Factor Analysis for the structure of the questionnaire, while for reliability assessment, Intraclass Correlation Coefficient has been applied. RESULTS: Twenty-one doctors participated as judges to validate the questionnaire, which 204 patients answered. The Content Validity Index for all the questions on the "Clarity" aspect was 0.846 ± 0.152 and on the "Relevance" aspect, 0.954 ± 0.080. Cronbach's alpha coefficient for the "Clarity" aspect was 0.88 with a 95% confidence intervals (CI) and the "Relevance" aspect, 0.94 with a 95% CI. The average Intraclass Correlation Coefficient was 0.94 and all the F tests were highly significant. CONCLUSIONS: The questionnaire developed by our group was considered valid and reliable, and is capable of portraying the home environment without the need for a personal visit to the patient's home. This questionnaire would be a good tool to use in research or during patient consultations to assess the patient's home environment, as this latter assessment is essential for the management of patients with respiratory allergies.

ABSTRACT

OBJECTIVES: We measured the production of cytokines, chemokines and antibodies involved in allergic responses and sCD23 levels during Schistosoma mansoni infection. METHODS: Individuals (n = 164) were selected using the ISAAC questionnaire and parasitological exams. The subjects were divided as follows: those infected individuals with allergy-related symptoms (A-I), those with allergy-related symptoms only (A-NI); those only infected (NA-I); and those non-infected individuals without allergy-related symptoms (NA-NI). We used supernatants from cell culture (mitogenic stimulation) to measure cytokine and chemokine levels using cytometric bead arrays. Serum levels of anti-Ascaris lumbricoides (Asc) and anti-Blomia tropicalis IgE were measured using ImmunoCAP, and sCD23 was measured using ELISA. RESULTS: Schistosoma mansoni infection was associated with a lower risk of allergy-related symptoms. In A-I, there were higher levels of TNF-α, IL-10, IL-6, IFN-γ and CXCL8 than in NA-NI group, with TNF-α and IL-6 also at higher levels compared to A-NI group. Levels of IL-6, CXCL8, total and anti-Asc IgE, as well as the numbers of eosinophils, were higher in NA-I than in NA-NI, and the antibodies were also lower in A-NI than in NA-I group. In AI and NA-I, there was less production of CCL2 than in NA-NI. There were no differences in the levels of IL-2, IL-4, IL-17, CCL5, sCD23 and anti-Blomia IgE. CONCLUSIONS: Patients with allergy-related symptoms and infected (simultaneously) had higher levels of IL-10; due to the infection, there was increased production of IL-6 and CXCL8 and less CCL2. These data may characterize deviation to Th1 or attenuation of the Th2 response in allergy sufferers in areas endemic for schistosomiasis.

ABSTRACT

BACKGROUND: The skin prick test is recognized as the gold standard for the specific diagnosis of allergy. OBJECTIVE: To determine the sensitivity and specificity of the epicutaneous patch test that has been modified for the diagnosis of respiratory allergy to Dermatophagoides pteronyssinus. METHODS: An observational, interpretative, prolective, and transversal study. The included patients were between 6 and 49 years old, with symptoms of a respiratory allergy, and a skin test was performed on them by using the European standardized prick test and a modified epicutaneous patch test with the variable of the scarification that occurred before the antigen was applied, and its homogenization with a wooden applicator, which occurred after the antigen was applied. The levels of sensitivity, specificity, and prevalence influence and positive and negative predictive values were calculated. RESULTS: 104 patients were included, of which 72 (69.2%) were women. The age category of the patients was 24 ± 11.3 years of age. 78.9% of the patients were diagnosed with allergic rhinitis, 1.9% were diagnosed with asthma, and 19.2% were diagnosed with both rhinitis and asthma. The prevalence of the respiratory allergy to Dermatophagoides pteronyssinus was of 62.5%; 94.23% with a correct diagnosis. The sensitivity and specificity of the test was of 93.85% and 94.87%; the positive and negative predictive values were of 96.83% and 90.24%, and the positive and negative likelihood ratios were of 18.30 and 0.06. CONCLUSIONS: The modified epicutaneous patch test was highly sensitive and specific; it decreased the risk of allergen carryover and it lowered the cost of the material that was used.

ABSTRACT

BACKGROUND: As part of the etiology of respiratory allergy we have genetics, prenatal factors and sensitivity to various airborne allergens, between these fungi are found. Relationship has been found between sensitization to fungal in skin tests and allergy pathogenesis and aggravation. There is a few literature in Mexico and in the north of the country it is lacking regarding this problem. OBJECTIVE: Assess the prevalence of sensitization to fungi in patients with respiratory allergy in skin tests to airborne allergens; determine the most prevalent fungus and prevalence of sensitization to each species of fungus per year, to assess the prevalence of sensitization to fungi by years. MATERIAL AND METHOD: Cross-sectional, observational and descriptive study conducted from 1 January 2010 to 31 December 2014 in patients treated at the Regional Center of Allergy and Clinical Immunology (Monterrey, Mexico) where we reviewed a database with patients whom performed skin tests, sensitization to 6 species of fungi were evaluated. We performed tables of data capture and statistical analysis. RESULTS: 4880 patients had respiratory allergy, a 17.1% prevalence of sensitization to fungal skin tests was determined. The fungus specie most prevalent was Alternaria alternata with 5.5%. The year range with the highest prevalence of sensitization was 0-10 years with a 6.7. CONCLUSIONS: The prevalence of fungi sensitization was higher than the global prevalence found, but lower than the prevalence found in other researches in Mexico.

ABSTRACT

This study aims to compare the concentration of viable fungi, especially those of the genus Aspergillus in the respiratory tract of stabled horses with and without Recurrent Airway Obstruction (RAO). Thirty two housed horses from four equestrian centers in Brazil were included in the study. These animals were submitted to clinical examination and to a respiratory sample collection. They were categorized into two groups: healthy and RAO-affected horses. Samples obtained by tracheobronchial washes were evaluated for fungal microscopy, quantitative culture and Aspergillus spp. quantification. Eighteen healthy and 14 RAO-affected horses were studied. Fungi were more frequently recovered in the RAO group, in comparison to controls, for both fungal microscopy (P<0.0001), fungal culture (P<0.0001) and Aspergillus spp. quantitative culture (p=0.001). In conclusion, horses with RAO have significantly higher fungal load in the respiratory tract in comparison to healthy horses. The implications of these findings in terms of the pathogenesis of RAO deserve additional investigation...

ABSTRACT

BACKGROUND: Sensitization to mites and cockroach are a common cause of respiratory allergy, especially in children. There is little information about its cross-reactivity with shrimp, which is common in adult patients. OBJECTIVES: To determine the frequency of sensitization to house dust mites (Dermatophagoides pteronyssinus, Dermatophagoides farinae) and cockroach species Periplaneta americana, in adult patients attending to a third level hospital and to determine the frequency of sensitization to shrimp in patients with positive skin test to house dust mite and/or cockroach, in patients with clinical manifestations suggestive of allergy to shrimp. MATERIAL AND METHOD: A retrospective study was done in which results were reviewed of 672 patients clinical records, attending to the Allergy and Clinical Immunology department in a third level hospital, those patients with positive skin test results to Dermatophagoides and/or Periplaneta americana were investigated for sensitization to shrimp through Prick-to-Prick test. RESULTS: Data from 672 patients were obtained, 69.8% (469) males, with a mean age of 33.5 years. Dermatophagoides positivity was 72.2% (486/672), and its relationship with other invertebrates that also have tropomyosin was 5.6% (4.9% with cockroach and 0.74% with shrimp). CONCLUSIONS: In our population, house dust mite sensitization was similar to that reported in the international literature. There was low positivity to cockroach, probably due to geographical conditions. There is very little information about shrimp's cross-reactivity with Dermatophagoides, despite its clinical significance, which requires more specific studies such as measurement of specific IgE to tropomyosin.

ABSTRACT

Objective: To determine the frequency of IgE antibodies to food allergens in patients with respiratory allergic diseases using molecular analysis. Method: This cross-sectional study included101 participants aged 6-18 years, diagnosed with allergic rhinitis (89.1% with associated asthma),and with no history of food allergy. Analysis of serum specific IgE was carried out using the ImmunoCAP ISAC method, which applies molecular biology tools to the detection of different allergens, including 42 derived from foods (pineapple, celery, peanut, hazelnut, codfish, shrimp, carp, cashew nut, Brazil nut, carrot, sesame, kiwi, cow’s milk, apple, egg, peach, soy, and wheat).Values ≥ 0.3 ISAC standardized units (ISU) were considered to be positive. Descriptive statistical analysis was used. Results: Twenty seven (26.7%) patients presented specific IgE to at leastone of the food allergens analyzed. Among the 42 allergic components tested, 20 (47.6%) were associated with IgE responses in at least one patient. The allergens with the highest frequencies of IgE reactivity were shrimp (Pen a 1 15.8%, Pen i 1 16.8%, Pen m 1 16.8%) and peach (Pru p 3 5.9%).Conclusions: The present study showed that molecular-based evaluation of food allergies should take several elements into consideration, particularly the correlation with clinical symptoms andthe knowledge available on IgE cross-reactivity among allergens from different sources. Presence of specific IgE to one allergen means sensitization, but not necessarily allergy. Misdiagnosis of food allergies may lead to inappropriate treatment, with unnecessarily restrictive diets which could affect the nutritional status of patients.

ABSTRACT

Respiratory allergies, especially rhinitis and asthma, are becoming increasingly common. Given that in northwestern Paraná there is little data on the subject, our objective was to verify the causative agents of respiratory allergies and their complications in patients treated at a private pulmonology service in the municipality of Maringá, Paraná. The study was observational, retrospective, lasting from January 2004 to December 2005. Patients were evaluated clinically, and the Prick TestR was performed. The age, positive test, major allergen reagents, presence or absence of atopy, rhinitis or asthma were noted for each individual. Of 396 individuals, 236 (59.3%) were positive for one or more allergens, whereas 80 (20.2%) were positive for three antigens and 85 (21.5%) to four or more. The reactivity of individuals was more frequent for house dust (207), Dermatophagoides pteronyssimus (184), Dermatophagoides farinae (158) and Blomia tropicalis (95). House dust, D. pteronyssimus and D. farinae occurred mainly in subjects with moderate to severe atopy. Over 70% of individuals from six to 20 years of age had atopy. Considering the high level of atopy in patients seen by the pulmonology service in Maringá, it is essential to undertake environment programs, along with monitoring and medical treatment of atopic individuals.
