ABSTRACT

INTRODUCTION: Human beings are constantly exposed to complex acoustic environments every day, which even pose challenges for individuals with normal hearing. Speech perception relies not only on fixed elements within the acoustic wave but is also influenced by various factors. These factors include speech intensity, environmental noise, the presence of other speakers, individual specific characteristics, spatial separatios of sound sources, ambient reverberation, and audiovisual cues. The objective of this study is twofold: to determine the auditory capacity of normal hearing individuals to discriminate spoken words in real-life acoustic conditions and perform a phonetic analysis of misunderstood spoken words. MATERIALS AND METHODS: This is a descriptive observational cross-sectional study involving 20 normal hearing individuals. Verbal audiometry was conducted in an open-field environment, with sounds masked by simulated real-word acoustic environment at various sound intensity levels. To enhance sound emission, 2D visual images related to the sounds were displayed on a television. We analyzed the percentage of correct answers and performed a phonetic analysis of misunderstood Spanish bisyllabic words in each environment. RESULTS: 14 women (70%) and 6 men (30%), with an average age of 26 ±â€¯5,4 years and a mean airway hearing threshold in the right ear of 10,56 ±â€¯3,52 dB SPL and in the left ear of 10,12 ±â€¯2,49 dB SPL. The percentage of verbal discrimination in the "Ocean" sound environment was 97,2 ±â€¯5,04%, "Restaurant" was 94 ±â€¯4,58%, and "Traffic" was 86,2 ±â€¯9,94% (p = 0,000). Regarding the phonetic analysis, the allophones that exhibited statistically significant differences were as follows: [o] (p = 0,002) within the group of vocalic phonemes, [n] (p = 0,000) of voiced nasal consonants, [r] (p = 0,0016) of voiced fricatives, [b] (p = 0,000) and [g] (p = 0,045) of voiced stops. CONCLUSION: The dynamic properties of the acoustic environment can impact the ability of a normal hearing individual to extract information from a voice signal. Our study demonstrates that this ability decreases when the voice signal is masked by one or more simultaneous interfering voices, as observed in a "Restaurant" environment, and when it is masked by a continuous and intense noise environment such as "Traffic". Regarding the phonetic analysis, when the sound environment was composed of continuous-low frequency noise, we found that nasal consonants were particularly challenging to identify. Furthermore in situations with distracting verbal signals, vowels and vibrating consonants exhibited the worst intelligibility.

ABSTRACT

Purpose: To describe the acoustic characteristics of a classroom, voice quality, fatigue, and vocal load of university professors. Methods: Exploratory, observational, longitudinal, and descriptive study with a single group of participants, including vocal monitoring data over two weeks. Acoustic characterization of the classroom, perceptual-auditory evaluation, and acoustic analysis of voice samples were conducted before and after classes. Vocal dosimetry was performed during classes, and the Vocal Fatigue Index (VFI) was assessed at the beginning of each week. Descriptive analysis of the findings was conducted, and randomization test was performed to verify the internal reliability of the judge. Results: All participants reported speaking loudly in the classroom, with the majority reporting vocal changes in the past six months, and only one participant reported a current vocal change. The classroom had acoustical measures and estimations that deviated from established standards. The professors used high vocal intensities during classes. After the classes, an increase in the absolute values of the aggregated data for CAPE-V, jitter, and fundamental frequency was found, varying within the range of normality. Furthermore, there was an observed increase in both post-lesson intensity and VFI when comparing the two-week period. Conclusions: Vocal intensities and VFI were possibly impacted by the acoustics of the classroom. The increase in average VFI between the weeks may be attributed to a cumulative fatigue sensation. Further research with a larger number of participants and in acoustically conditioned classrooms is suggested in order to evaluate collective intervention proposals aimed at reducing the vocal load on teachers.


Objetivo: Describir las características acústicas, calidad vocal, fatiga y carga vocal de profesores universitarios. Métodos: Estudio exploratorio, observacional, longitudinal, descriptivo con un solo grupo de participantes y datos de monitoreo vocal durante dos semanas. Se realizó caracterización acústica de la sala, evaluación auditiva-perceptiva y acústica de muestras de voz antes y después de las clases. Se realizó dosimetría vocal durante las clases y se verificó el Índice de Fatiga Vocal (IFV) en dos semanas. Se realizó un análisis descriptivo de los hallazgos y una prueba de aleatorización para verificar la confiabilidad interna del juez. Resultados: Todos los participantes informaron hablar en voz alta en clase, la mayoría informó cambios vocales en los últimos seis meses y solo uno informó cambios vocales actuales. La sala presentó mediciones y estimaciones acústicas fuera de las normas establecidas. Los profesores utilizaron intensidades vocales altas durante las clases. Hubo un aumento en los valores absolutos de los datos agrupados para CAPE-V, jitter y frecuencia fundamental, variando dentro de los límites normales, después de las clases. La intensidad después de las clases y el IFV, en la comparación entre las dos semanas, mostraron un aumento. Conclusiones: La dosis vocal y el IFV posiblemente se vieron afectados por la acústica del aula. El aumento del IFV medio entre semanas pudo deberse a la sensación de cansancio acumulada. Se sugieren nuevas investigaciones con un mayor número de participantes y que se realicen en la sala acondicionada acústicamente para evaluar propuestas de intervención colectiva, con el objetivo de reducir la carga vocal de los docentes.

ABSTRACT

Anthropogenic noise is one of the fastest growing, globally widespread pollutants, affecting countless species worldwide. Despite accumulating evidence of the negative impacts of wind turbines on wildlife, little is known about how the noise they generate affects ecological systems. Songbirds may be susceptible to noise pollution due to their reliance on vocal communication and thus, in this field study, we examined how songbirds are affected by wind turbine noise. We broadcasted noise produced by one wind turbine in a migratory stopover site during the nonbreeding season. Throughout the study, we repeatedly monitored the acoustic environment and songbird community before, during, and after the noise treatments with passive acoustic monitoring and mist netting. We employed generalized linear mixed effects models to assess the impact of experimental noise treatment on birds behavior and likelihood ratio tests to compare models with variables of interest with null models. The daily number of birds in the presence of wind turbine noise decreased by approximately 30% compared with the before and after phases. This reduction had a significant spatial pattern; the largest decrease was closer to the speaker and on its downwind side, fitting measured sound propagation. Although we found no impact on species diversity, two out of three most common species showed clear avoidance behavior: 45% and 36% decrease in abundance for the lesser whitethroat (Sylvia curruca) and Sardinian warbler (Sylvia melanocephala momus), respectively. In the after phase, there were lingering effects on the lesser whitethroat. The age structure of the lesser whitethroat population was affected because only juvenile birds showed avoidance behavior. No difference in avoidance extent was found between migratory and nonmigratory species, but the impacts of displacement on migrants during stopover are especially troubling from a conservation perspective. Our results stress the need to address the impacts of noise pollution on wildlife when planning noise-generating infrastructures, such as wind turbines, to allow for sustainable development without threatening already declining songbird populations.


El ruido antropogénico es uno de los contaminantes con mayor crecimiento y distribución a nivel mundial, por lo que afecta a incontables especies en todo el mundo. A pesar de acumular evidencia sobre el impacto negativo que tienen las turbinas eólicas sobre la fauna, se sabe muy poco sobre cómo el ruido que generan afecta a los sistemas ecológicos. Las aves canoras pueden ser susceptibles a la contaminación sonora ya que dependen de la comunicación vocal y, por lo tanto, en este estudio de campo, analizamos cómo les afecta el sonido producido por las turbinas eólicas. Transmitimos ruido producido por una turbina en un punto de parada migratorio durante la temporada no reproductiva. Durante el estudio, monitoreamos repetidas veces el entorno acústico y la comunidad de aves canoras antes, durante y después de los tratamientos de ruido con monitoreo acústico pasivo y redes de niebla. Empleamos modelos de efectos lineales mixtos generalizados para evaluar el impacto del ruido experimental sobre el comportamiento de las aves y pruebas de probabilidad de proporción para comparar los modelos con variables de interés con los modelos nulos. El número diario de aves en la presencia del ruido de turbinas eólicas disminuyó aproximadamente un 30% en comparación con las fases de antes y después. Esta reducción tuvo un patrón espacial significativo: la mayor disminución ocurrió más cerca a la bocina y en el lado de sotavento, lo que se ajusta a la medida de la propagación del sonido. Aunque no encontramos impacto alguno sobre la diversidad de especies, dos de tres de las especies más comunes mostraron un comportamiento de evasión evidente: 45% y 36% de disminución en la abundancia de Sylvia curruca y Sylvia melanocephala momus, respectivamente. Durante la fase posterior al ruido, observamos efectos prolongados en S. curruca. La composición de edades de la población de S. curruca se vio afectada porque sólo los individuos juveniles mostraron un comportamiento de evasión. No encontramos una diferencia en el grado de evasión entre las especies migratorias y no migratorias, pero el impacto del traslado sobre las migrantes durante el punto de parada es de preocupación especial desde una perspectiva de conservación. Nuestros resultados acentúan la necesidad de abordar el impacto de la contaminación sonora sobre la fauna cuando se planean estructuras que producen ruido, como las turbinas eólicas, para permitir el desarrollo sustentable sin amenazar a las poblaciones de aves canoras que ya están en declive. Efectos del ruido de turbinas eólicas sobre el comportamiento de las aves canoras durante la temporada no reproductiva.

Resumo Introdução: a subnotificação da Perda Auditiva Induzida por Ruído (Pair) relacionada ao trabalho é um problema que contribui para a invisibilidade e falta de prioridade deste agravo nos programas de Saúde do Trabalhador (ST). Logo, ações de educação permanente voltadas ao planejamento estratégico-situacional podem contribuir para o enfrentamento desta realidade. Objetivo: apresentar e discutir a experiência de uma intervenção educativa com técnicos da ST de uma macrorregião de saúde do estado da Bahia, Brasil, sobre o uso do Planejamento e Programação Local em Saúde (PPLS) como ferramenta para ampliação da notificação da Pair relacionada ao trabalho Métodos: a intervenção consistiu em uma oficina estruturada em cinco etapas, as quais proporcionaram discussão sobre a Pair, conhecimento do PPLS, prática do PPLS, socialização dos resultados e avaliação do evento. Resultados: a prática do PPLS, a partir de um problema real, viabilizou o delineamento de ações concretas de enfrentamento da subnotificação da Pair a serem implementadas nos Centros de Referência em Saúde do Trabalhador (Cerest) e no estado como um todo. Conclusão: a experiência mostrou-se bem-sucedida ao fomentar a tomada de consciência e mobilização dos técnicos para enfrentamento da subnotificação da Pair, ao tempo que propiciou qualificação em PPLS dos participantes.


Abstract Introduction: underreported occupational noise-induced hearing loss (ONIHL) is an issue that increases the invisibility and lack of priority of this condition in occupational health (OH) programs. Hence, permanent education actions focused on strategic-situational planning may help address the situation. Objective: to present and discuss the experience of an educational intervention in OH technicians of a larger health region in the state of Bahia, Brazil, approaching the use of the Local Health Planning and Program (PPLS, in Portuguese) as a toll to increase reports of ONIHL. Methods: the intervention had a workshop structured into five stages, which approached ONIHL discussions, PPLS knowledge, PPLS practice, result socialization, and event evaluation. Results: real-problem PPLS practice made it possible to outline concrete actions to address underreported ONIHL, which can be implemented in Occupational Health Reference Centers (Cerest) and other settings statewide. Conclusion: the experience proved to be positive, raising technicians' awareness of underreported ONIHL and mobilizing them to address the underreporting of ONIHL, while also effectively training participants to use PPLS.

RESUMO Objetivo Avaliar a contribuição da tecnologia de escuta assistida em usuários de implante coclear (IC) em situações de reverberação e ruído. Métodos Estudo transversal prospectivo aprovado pelo Comitê de Ética Institucional (CAAE 8 3031418.4.0000.0068). Foram selecionados adolescentes e adultos usuários de IC com surdez pré ou pós-lingual. Para usuários bilaterais, cada orelha foi avaliada separadamente. O reconhecimento de fala foi avaliado por meio de listas gravadas de palavras dissílabas apresentadas a 65 dBA a 0° azimute com e sem o Mini Microfone2 (Cochlear™) conectado ao processador de fala Nucleus®6. A reverberação da sala foi medida como 550 ms. Para avaliar a contribuição do dispositivo de escuta assistida (DEA) em ambiente reverberante, o reconhecimento de fala foi avaliado no silêncio. Para avaliar a contribuição do DEA em reverberação e ruído, o reconhecimento de fala foi apresentado a 0° azimute com o ruído proveniente de 8 alto-falantes dispostos simetricamente a 2 metros de distância do centro com ruído de múltiplos falantes usando relação sinal-ruído de +10dB. Para evitar viés de aprendizado ou fadiga, a ordem dos testes foi randomizada. A comparação das médias foi analisada pelo teste t para amostras pareadas, adotando-se nível de significância de p<0,005. Resultados Dezessete pacientes com idade média de 40 anos foram convidados e concordaram em participar, sendo 2 participantes bilaterais, totalizando 19 orelhas. Houve contribuição positiva significante do Mini Mic2 na reverberação e ruído+reverberação (p<0,001). Conclusão DEA foi capaz de melhorar o reconhecimento de fala de usuários de IC tanto em situações de reverberação quanto ruidosas.


ABSTRACT Purpose This study aimed to evaluate the contribution of assistive listening technology with wireless connectivity in cochlear implant (CI) users in reverberating and noise situations. Methods Prospective cross-sectional study approved by the Institutional Ethics Committee (CAAE 8 3031418.4.0000.0068). Adolescents and adults CI users with pre- or post-lingual deafness were selected. For bilateral users, each ear was assessed separately. Speech recognition was assessed using recorded lists of disyllabic words presented at 65 dBA at 0° azimuth with and without the Wireless Mini Microphone 2 (Cochlear™) connected to the Nucleus®6 speech processor. Room reverberation was measured as 550 ms. To assess the contribution of the assistive listening device (ALD) in a reverberating environment, speech recognition was assessed in quiet. To assess the contribution of the ALD in reverberation and noise, speech recognition was presented at 0° azimuth along with the noise coming from 8 loudspeakers symmetrically arranged 2 meters away from the center with multi-talker babble noise using signal to noise ratio of +10dB. To avoid learning bias or fatigue, the order of the tests was randomized. Comparison of means was analyzed by t test for paired samples, adopting significance level of p <0.005. Results Seventeen patients with a mean age of 40 years were invited and agreed to participate, with 2 bilateral participants, totaling 19 ears assessed. There was a significant positive contribution from the Mini Mic2 in reverberation, and noise+reverberation (p <0.001). Conclusion ALD was able to improve speech recognition of CI users in both reverberation and noisy situations.

ABSTRACT

RESUMO Objetivo Investigar a confiabilidade do Teste de Palavras no Ruído em um grupo de adultos normo-ouvintes. Método Participaram da pesquisa 45 sujeitos adultos normo-ouvintes. O intervalo entre a primeira e a segunda avaliação foi de 14 a 28 dias, realizadas no mesmo turno do dia e pelo mesmo avaliador. A análise de comparação entre teste e reteste foi realizada considerando o resultado geral das orelhas, totalizando 90 orelhas avaliadas. A análise inferencial incluiu a comparação das situações na primeira e segunda avaliação por meio do Teste de Wilcoxon, cálculo e interpretação do Índice de Correlação Intraclasse. Resultados Houve diferença estatisticamente significante entre os desempenhos no teste e reteste. Os coeficientes de correlação intraclasse obtidos foram indicativos de boa confiabilidade (r=0,759; p<0,001) para o estímulo monossilábico e de moderada confiabilidade (r=0,631; p<0,001) para o dissilábico. Conclusão O Teste de Palavras no Ruído demonstrou satisfatória confiabilidade tanto para o estímulo monossilábico, quanto para o dissilábico.


ABSTRACT Purpose To investigate the reliability of the Word-with-Noise Test in a group of normal-hearing adults. Methods Forty-five normal-hearing adult subjects participated in the research. The interval between the first and second assessment was 14 to 28 days, performed during the same time of the day and by the same evaluator. The comparison analysis between the test and the retest was performed considering the general result of the ears, totaling 90 ears evaluated. The inferential analysis included the comparison of the situations in the first and second assessment using the Wilcoxon Test, calculation, and interpretation of the Intraclass Correlation Index. Results There was a statistically significant difference between the test and retest performances. The intraclass correlation coefficients obtained were indicative of good reliability (r=0.759; p<0.001) for the monosyllabic stimulus and moderate reliability (r=0.631; p<0.001) for the disyllabic stimulus. Conclusion The Word-with-Noise Test demonstrated satisfactory reliability for both the monosyllabic and disyllabic stimuli.

ABSTRACT

RESUMO Objetivo Propor um instrumento para a avaliação do reconhecimento de fala na presença de ruído competitivo. Definir sua estratégia de aplicação, para ser aplicado na rotina clínica. Obter evidências de validade de critério e apresentar seus valores de referência. Método Estudo realizado em três etapas: Organização do material que compôs o Teste de Palavras no Ruído (Etapa 1); Definição da estratégia de aplicação do instrumento (Etapa 2); Investigação da validade de critério e definição dos valores de referência para o teste (Etapa 3), por meio da avaliação de 50 sujeitos adultos normo-ouvintes e 12 sujeitos com perda auditiva. Resultados O Teste de Palavras no Ruído é composto por listas de vocábulos mono e dissilábicos e um ruído com espectro de fala (Etapa 1). Foi definida como estratégia de aplicação do teste, a realização do Limiar de Reconhecimento de Fala com ruído fixo em 55 dBNA (Etapa 2). Quanto à validade de critério, o instrumento apresentou adequada capacidade de distinção entre os sujeitos normo-ouvintes e os sujeitos com perda auditiva (Etapa 3). Foram definidos como valores de referência para o teste, os pontos de corte expressos em relação sinal/ruído de 1,47 dB para o estímulo monossilábico e de -2,02 dB para o dissilábico. Conclusão O Teste de Palavras no Ruído demonstrou ser rápido e de fácil aplicação e interpretação dos resultados, podendo ser uma ferramenta útil a ser utilizada na rotina clínica audiológica. Além disso, apresentou evidências satisfatórias de validade de critério, com valores de referência estabelecidos.


ABSTRACT Purpose To propose an instrument for assessing speech recognition in the presence of competing noise. To define its application strategy for use in clinical practice. To obtain evidence of criterion validity and present reference values. Methods The study was conducted in three stages: Organization of the material comprising the Word-with-Noise Test (Stage 1); Definition of the instrument's application strategy (Stage 2); Investigation of criterion validity and definition of reference values for the test (Stage 3) through the evaluation of 50 normal-hearing adult subjects and 12 subjects with hearing loss. Results The Word-with-Noise Test consists of lists of monosyllabic and disyllabic words and speech spectrum noise (Stage 1). The application strategy for the test was defined as the determination of the Speech Recognition Threshold with a fixed noise level at 55 dBHL (Stage 2). Regarding criterion validity, the instrument demonstrated adequate ability to distinguish between normal-hearing subjects and subjects with hearing loss (Stage 3). Reference values for the test were established as cut-off points expressed in terms of signal-to-noise ratio: 1.47 dB for the monosyllabic stimulus and -2.02 dB for the disyllabic stimulus. Conclusion: The Word-with-Noise Test proved to be quick to administer and interpret, making it a useful tool in audiological clinical practice. Furthermore, it showed satisfactory evidence of criterion validity, with established reference values.

ABSTRACT

Objectives: This was a single-subject study, aimed to demonstrate different vocal demand situations that are typical for primary school and teacher's vocal demand response under two acoustical conditions, with and without voice amplification, during five working days. Methods: The long-term voice dosimetry with Vocal Holter Med (PR.O. Voice Srl) was carried out on a 49-year-old female teacher with voice disorders during daily teaching activities. A sound field amplification system (SFAS) PentaClass Runa was installed in the classroom. Voice dosimetry was provided under two different acoustical conditions: without SFAS (2 days) and with SFAS (3 days). Results: Phonation time percentage, sound pressure level (SPL), SPL SD, fundamental frequency (F0), F0 SD, cycle, and distance doses were investigated in seven communication scenarios (lessons, group/individual classes, sports lessons in the gym and schoolyard, breaks, lunch breaks, and other activities). The median scores of all voice parameters differed significantly between different vocal demand contexts. The significant statistical difference in the vocal demand response was in the communication situations with and without SFAS. In addition, the number of children, reverberation time, and ambient air relative humidity impacted voice SPL and the cycle dose. Conclusions: Lessons, sports lessons held in the gym or schoolyard, breaks, and lunch breaks were considered as high vocal demand communication situations requiring higher voice intensity and fundamental frequency, higher phonation time percentage, cycle, and distance doses. Group/individual work and other teacher activities during the day, unrelated to direct work with students, were categorized as low vocal demand communication scenarios.


Objetivos: Este fue un estudio de sujeto único, cuyo objetivo fue demostrar diferentes situaciones de demanda vocal típicas de la escuela primaria y la respuesta vocal de los docentes bajo dos condiciones acústicas, con y sin amplificación de voz, durante cinco días laborables. Métodos: Se llevó a cabo dosimetría vocal a largo plazo con Vocal Holter Med (PR.O. Voice Srl) durante las actividades diarias de enseñanza en una docente de 49 años con trastornos de la voz. Se instaló un sistema de amplificación de campo sonoro (SFAS) PentaClass Runa en el aula. La dosimetría vocal se realizó bajo dos condiciones acústicas diferentes: sin SFAS (2 días) y con SFAS (3 días). Resultados: Se investigaron el porcentaje de tiempo de fonación, el nivel de presión sonora (SPL), SPL SD, la frecuencia fundamental (F0), F0 SD, ciclos y dosis de distancia en siete escenarios de comunicación diferentes (clases, clases grupales/individuales, clases de educación física en el gimnasio y el patio de la escuela, recreos, almuerzos y otras actividades). Las puntuaciones medias de todos los parámetros vocales diferían significativamente entre los diferentes contextos de demanda vocal. La diferencia estadísticamente significativa en la respuesta a la demanda vocal se observó en las situaciones de comunicación con y sin SFAS. Además, el número de niños, el tiempo de reverberación y la humedad relativa del aire ambiente afectaron al SPL de la voz y la dosis de ciclo. Conclusiones: Las lecciones, las clases de educación física en el gimnasio o el patio de la escuela, los recreos y los almuerzos se consideraron situaciones de comunicación de alta demanda vocal, que requerían una mayor intensidad y frecuencia fundamental de la voz, un mayor porcentaje de tiempo de fonación y dosis de ciclo y distancia más altas. El trabajo grupal/individual y otras actividades del profesor durante el día no relacionadas con el trabajo directo con los estudiantes se categorizaron como escenarios de comunicación de baja demanda vocal.

ABSTRACT

Objective: The aim of this study was to identify if cochlear implant (CI) users are perceiving a decrease in life quality due to voice problems. This study evaluated 43 CI user's perception of their voice and how it affects their quality of life through a survey. Approach: Forty-three CI users responded to a survey regarding their demographics, details about their CI, the Hearing Health Quick Test (HHQT), the Voice Related Quality of Life (V-RQOL), and the Voice Handicap Index-10 (VHI-10). The survey responses were analyzed using univariate linear regression analysis. Results: Few of the CI users scored below the cut off for normal voice related quality of life. CI users averaged 93.4 out of 100 on the V-RQOL and only four scored abnormally for the VHI-10. Lower scores on the V-RQOL were correlated with the participants having an associate degree and with participants visiting friends, family, and neighbors less often due to hearing loss. The VHI-10 scores were correlated with gender, education levels, difficulty in social situations due to hearing loss, noise exposure, and tinnitus. Limitations of the study: The small n was the primary limitation of this study. Originality: This study was one of the first to examine the voice-related quality of life in CI users. Conclusions: Overall, respondents did not perceive much voice-related difficulty. However, they were more likely to perceive voice-related difficulty if they experienced difficulty hearing in noise and avoided social situations due to hearing loss.


Objetivo: Este estudio identificó si los usuarios de implantes cocleares (IC) están percibiendo una disminución en la calidad de su vida debido a problemas de voz. Además, evaluó la percepción de la voz de 43 usuarios de IC y cómo afecta su calidad de vida a través de una encuesta. Enfoque: Cuarenta y tres usuarios de IC respondieron a una encuesta sobre su demografía, detalles sobre su IC, la Hearing Health Quick Test (HHQT), la Voice Related Quality of Life (V-RQOL) y el Voice Handicap Index-10 (VHI-10). Las respuestas de la encuesta se analizaron mediante un análisis de regresión lineal univariado. Resultados: Pocos usuarios de IC puntuaron por debajo del límite para calidad de vida relacionada con la voz. El promedio V-RQOL fue de 93,4/100; solo 4 participantes tuvieron puntuación anormal en VHI-10. Las bajas puntuaciones en V-RQOL se correlacionaron con título de asociado y menos visitas por pérdida auditiva; las puntuaciones VHI-10, con sexo, educación, dificultad en situaciones sociales, exposición al ruido y tinnitus. Limitaciones del estudio: La pequeña n fue la principal limitación de este estudio. Originalidad: Este estudio fue uno de los primeros en examinar la calidad de vida relacionada con la voz en usuarios de CI. Conclusiones: En general, los encuestados no percibieron mucha dificultad relacionada con la voz. Sin embargo, era más probable que percibieran dificultades relacionadas con la voz si tenían dificultades para oír en ruido y evitaban situaciones sociales debido a la pérdida auditiva.

Introdução: a exposição ao ruído no ambiente escolar pode gerar desconforto, sendo apontada como um dos fatores relacionados às dificuldades de aprendizagem e concentração. Objetivo: definir a relação entre a exposição ao ruído e os efeitos auditivos e não auditivos na percepção de professores do ensino infantil e fundamental I e II em uma escola privada no Vale do Itajaí. Metodologia: coleta de dados com aplicação de questionário online respondido por 29 professores, contendo perguntas de múltipla escolha, com análise estatística descritiva. Resultados: 89,66% dos professores têm jornada de trabalho semanal superior a 20 horas/aula; 20% percebem a presença de ruído na escola, sendo esta de intensidade moderada; 68,97% apontaram como principal fonte de ruído os próprios alunos e 62,07% o intervalo e a quadra de esportes; 41,4% percebem prejuízos na saúde, porém desconhecem a existência de normas regulamentadoras sobre o ruído na escola; 100% relatam sintomas de estresse, ansiedade, irritabilidade e/ou insônia e 68,96% relacionam estes com a exposição ao ruído; 34,48% percebem agravos dos sintomas após as aulas; 55,17% nunca receberam informações ou participaram de atividades sobre o ruído, embora 86,21% "fazem combinados" [estabelecem regras] com os alunos para minimizar os efeitos do ruído no ambiente escolar. Conclusão: os efeitos do ruído na escola são percebidos pelos professores, principalmente no intervalo das aulas, sendo os sintomas de estresse, ansiedade, irritabilidade e/ou insônia os mais citados. Introduction: exposure to noise in the school environment can cause discomfort, being identified as one of the factors related to learning and concentration difficulties. Objective: to define the relationship between exposure to noise and auditory and non-auditory effects on the perception of teachers of kindergarten and elementary education I and II in a private school in Vale do Itajaí. Methodology: data collection using an online questionnaire answered by 29 teachers, containing multiple choice questions, with descriptive statistical analysis. Results: 89.66% of teachers have a weekly working day of more than 20 hours/class; 20% perceive the presence of noise at school, which is of moderate intensity; 68.97% pointed to the students themselves as the main source of noise and 62.07% to recess and the sports court; 41.4% perceive health losses, but are unaware of the existence of regulatory standards regarding noise at school; 100% report symptoms of stress, anxiety, irritability and/or insomnia and 68.96% relate these to noise exposure; 34.48% notice worsening of symptoms after classes; 55.17% never received information or participated in activities about noise, although 86.21% "made arrangements" [established rules] with students to minimize the effects of noise in the school environment. Conclusion: the effects of noise at school are noticed by teachers, especially during class breaks, with symptoms of stress, anxiety, irritability and/or insomnia being the most cited. (AU)

Objetivo:definir la relación entre la exposición al ruido y los efectos auditivos y no auditivos en la percepción de profesores de jardín de infantes y educación primaria I y II en una escuela privada del Vale do Itajaí. Metodología: recolección de datos mediante cuestionario en línea respondido por 29 docentes, que contiene preguntas de opción múltiple, con análisis estadístico descriptivo. Resultados: el 89,66% de los docentes tienen una jornada laboral semanal superior a 20 horas/clase; El 20% percibe la presencia de ruido en el colegio, el cual es de intensidad moderada; El 68,97% señaló a los propios alumnos como principal fuente de ruido y el 62,07% al recreo y la cancha deportiva; El 41,4% percibe pérdidas en la salud, pero desconoce la existencia de normas regulatorias en materia de ruido en la escuela; El 100% reporta síntomas de estrés, ansiedad, irritabilidad y/o insomnio y el 68,96% los relaciona con la exposición al ruido; el 34,48% notan empeoramiento de los síntomas después de clases; El 55,17% nunca recibió información ni participó en actividades sobre ruido, aunque el 86,21% "hizo arreglos" [estableció reglas] con los estudiantes para minimizar los efectos del ruido en el ambiente escolar. Conclusión: los efectos del ruido en la escuela son notados por los profesores, especialmente durante los recreos de clase, siendo los síntomas de estrés, ansiedad, irritabilidad y/o insomnio los más citados. Objetivo: verificar os níveis de pressão sonora, a influência do turno e das ocorrências nesses níveis em uma unidade neonatal do Rio de Janeiro. Método: estudo longitudinal, exploratório, descritivo, com dados coletados entre novembro de 2021 e janeiro de 2022. As variáveis categóricas foram descritas por meio de frequências absoluta e relativa. A aderência à distribuição normal foi verificada pelo teste Shapiro-Wilk. Para a análise da associação entre variáveis de exposição e desfecho, utilizou-se teste Mann-Whitney, Kruskal Wallis e Qui-quadrado. Considerou-se estatisticamente significativos resultados de p<0,05. Resultados: foram coletados 10.122 registros de decibéis e 56 eventos relacionados as atividades, que ocorreram predominantemente no período diurno. Houve variação de 38,6 a 93,1 decibéis (dB). Em 96,3% do período observado, os níveis de pressão sonora estiveram acima do recomendado. Na comparação entre os períodos, o noturno apresentou menores níveis (p=0,000). Conclusão: os níveis de pressão sonora registrados estão acima do recomendado para um ambiente seguro(AU)


Objective: to verify the sound pressure levels (SPL) and the influence of the shift and the occurrences in these levels in a neonatal unit in Rio de Janeiro. Method: longitudinal, exploratory, descriptive study. Collected from November 2021 to January 2022. Categorical variables were described using absolute and relative frequencies. To verify adherence to the normal distribution, the Shapiro-Wilk test was used. For the association between exposure and outcome variables, the Mann-Whitney, Kruskal Wallis and Qui-square tests were used. Results of p < 0.05 were considered statistically significant. Results: 10,122 decibel records and 56 events related to activities were collected, these occurred predominantly during the day. There was a variation from 38.6 to 93.1 decibels (dB). In 96.3% of the observed period, the NPS were above the recommended. Comparing the periods, nighttime had lower SPL (p=0.000). Conclusion: the registered NPS are above the recommended for a safe environment(AU)

Introdução: a poluição sonora está presente na escola, sendo assim, é importante conhecer as fontes de ruído, o impacto na saúde do professor e no ensino. Objetivo: avaliar a percepção de professores do ensino fundamental I quanto ao impacto do ruído no ambiente escolar, bem como seus sintomas vocais. Método: estudo observacional, descritivo, analítico, transversal. Aplicou-se inventário, do qual participaram 26 professoras da rede municipal da cidade de Santo Amaro da Imperatriz, SC. Aplicou-se um questionário com questões referentes à idade, sexo, tempo de trabalho, carga horária diária, impacto e percepção do ruído no ambiente escolar. Utilizou-se o Índice de Triagem para Distúrbios da Voz, uma triagem para identificação de risco para distúrbio de voz em professores. Para a análise de associação dos resultados foi utilizado o teste de Qui Quadrado, com nível de significância de 5%. Resultados: 53,57% das professoras responderam que há excesso de ruído na escola e 57,14% consideram que o ruído dentro da sala prejudica as atividades. As fontes externas e internas de ruído que mais atrapalham são: pátio, corredor, conversas paralelas e ventilador. Quanto aos sintomas vocais referidos, destacaram-se: garganta seca, cansaço ao falar, pigarro e rouquidão. Professoras que lecionam em período integral apresentaram mais queixas para o pigarro. Conclusão: os sujeitos têm a percepção de que o ambiente escolar é excessivamente ruidoso e causa prejuízos no desenvolvimento das atividades. As participantes têm poucas queixas relacionadas à voz, sendo a mais frequente, garganta seca. Além disso, professoras que lecionam em período integral apresentaram, significativamente, mais queixas de pigarro. (AU)


Introducción: la contaminación acústica está presente en la escuela, entonces es importante conocer las fuentes del ruido, el impacto en la salud del docente y en la enseñanza. Propósito: evaluar la percepción de docentes sobre el impacto del ruido en el ambiente escolar, así como sus síntomas vocales. Método: estudio observacional, descriptivo, analítico, transversal. Se aplicó un inventario, en el que participaron 26 docentes de la red municipal de la ciudad de Santo Amaro da Imperatriz, (SC). con preguntas relacionadas con edad, género, tiempo de trabajo, carga diaria de trabajo, impacto y percepción del ruido en el ambiente escolar. Se utilizó el Índice de Detección de Trastornos de la Voz para identificar el riesgo de trastornos de la voz. Para el análisis de asociación se utilizó la prueba de chi-cuadrado, con un nivel de significación del 5%. Resultados: 53,57% respondió que en la escuela hay exceso de ruido y 57,14% consideram que el ruido dentro del salón perjudica las actividades. Las fuentes de ruido más molestas son: patio, pasillo, conversaciones paralelas y ventilador. En cuanto a los síntomas vocales reportados destacan sequedad de garganta, cansancio al hablar, carraspeo y ronquera. Los maestros de tiempo completo se quejaron más de carraspear. Conclusión: los sujetos tienen la percepción de que el ambiente escolar es excesivamente ruidoso y provoca perjuicios para el desarrollo de las actividades. Los participantes tienen pocas quejas relacionadas con la voz, siendo la garganta seca la más frecuente. Además, los maestros que enseñan a tiempo completo tenían muchas más quejas de carraspeo. Introduction: noise pollution is present at school; therefore, it is important to know the sources of noise, the impact on the teacher's health and on teaching. Purpose: to evaluate the perception of elementary school teachers regarding the impact of noise in the school environment, as well as their vocal symptoms. Method: observational, descriptive, analytical, cross-sectional study. An inventory was applied, in which 26 teachers from the municipal network of the city of Santo Amaro da Imperatriz, (SC). participated. A questionnaire was applied with questions related to age, gender, working time, daily workload, impact and perception of noise in the school environment. The Voice Disorders Screening Index was used, a screening to identify risk for voice disorders in teachers. For the analysis of the association of results, the chi-square test was used, with a significance level of 5%. Results: 53.57% of the teachers answered that there is excessive noise in the school and 57.14% consider that the noise inside the room harms the activities. The most disturbing external and internal sources of noise are courtyard, corridor, parallel conversations and fan. As for the reported vocal symptoms, the following stand out: dry throat, tiredness when speaking, throat clearing and hoarseness. Full-time teachers complained more about throat clearing. Conclusion: the subjects have the perception that the school environment is excessively noisy and causes damage to the development of activities. The participants have few voice-related complaints, the most frequent being dry throat. In addition, teachers who teach full-time had significantly more complaints of throat clearing. (AU)

This article has the objective of comment the usefulness of early identification of the hidden hearing loss after CSNE based on audiological markers and in changes in the clinical methodology in clinical groups for its searching.Method: Review of related literature in scientific databases and narrative description of results.Results: CSNE results in a hidden hearing loss in patients with normal pitch audiogram, mainly workers or indi-viduals exposed to high noise levels. The main studies of identification have been performed mainly in groups of students from college or musicians.Conclusions: Is necessary adjustments in hearing health policy for an wide early identification of CSNE in at risk populations for the identification of the hidden hearing loss and fight for its damage regulation. Antecedentes y objetivos: Las investigaciones que involucran el análisis acústico no siempre consideran el nivel de ruido de fondo del recinto donde van a realizar el proceso de grabación o si lo hacen el valor máximo de ruido de fondo que utilizan varía de autor en autor. El objetivo de esta investigación es determinar si los parámetros: f0, Jitter, Shimmer y Alpha ratio varían en relación con los distintos niveles de ruido de fondo del recinto donde se realiza la grabación, y determinar un valor máximo de este. Materiales y métodos Se utilizó el programa Praat® para obtener el valor promedio de los parámetros: f0, Jitter, Shimmer y Alpha ratio a partir de una muestra de voz en 33 sujetos al interior de una cabina Audiométrica cuyo nivel de ruido de fondo fue de 28,1dB(A). Se realizó una comparación estadística entre el valor promedio de cada parámetro al ir incrementando el nivel de ruido de fondo en relación con los valores promedio obtenidos con el nivel de ruido de fondo basal de la cabina para cada parámetro. Resultados Los parámetros f0 y Alpha ratio, muestran valores confiables hasta un nivel de ruido de fondo de 47,7dB(A), mientras que los parámetros Jitter y Shimmer lo hacen hasta un nivel de ruido de fondo de 43,8dB(A). Conclusiones Considerando que los parámetros acústicos se obtienen todos de la misma grabación, podemos señalar que el nivel de ruido de fondo máximo recomendable en el recinto donde se realicen las grabaciones debiese ser no superior a los 43,8dB(A). Background and objectives: Research involving acoustic analysis does not always consider the level of background noise in the room where the recording process is going to be carried out, or if they do, the maximum value of background noise used varies from author to author. The objective of this research is to establish if the parameters: f0, Jitter, Shimmer and Alpha ratio vary in relation to different levels of background noise in the room where the recording process is carried out and to establish a maximum value of this. Materials and methods The Praat program was used to get the average value of the parameters: f0, Jitter, Shimmer and Alpha ratio from a voice sample of 33 subjects inside an audiometric booth whose background noise level was 28.1dB(A). A statistical comparison was made between the average value of each parameter as the background noise increased in relation to the average values obtained with the baseline background noise of the cabin for each parameter. Results The f0 and Alpha ratio parameters show reliable values up to a background noise level of 47.7dB(A), while the Jitter and Shimmer parameters do so up to a background noise level of 43.8dB(A). Conclusions Considering that the acoustic parameters are all obtained from the same recording, we can point out that the recommended maximum background noise level in the room where the recordings are made should not exceed 43.8dB(A). Professional musicians have significant occupational exposure to high levels of noise. Use of hearing protection among musicians could substantially prevent hearing damage, though the rate of use is quite low. METHODS AND MATERIAL: A questionnaire on the use of protective hearing devices, hearing care, and subjective judgments of hearing difficulties was completed by a group of classical musicians from Spain. We analysed the frequency of device use by instrument based on contingency tables analysed by χ2 tests. RESULTS: One hundred and ninety-four Spanish classical orchestral musicians voluntarily completed the questionnaire. The percentage of musicians who reported using hearing protection in our survey was very low and varied with the type of instrument played. However, we found a high prevalence of subjective auditory disorders within this group. CONCLUSION: Few Spanish musicians use hearing protection. Training on hearing-loss prevention in this field and better protective devices could increase device use and improve the auditory health of this group.

Introducción: La hipoacusia neurosensorial motivada por la exposición al ruido es una de las causas más frecuentes de sordera. Los músicos profesionales tienen una exposición ocupacional significativa a los altos niveles de ruido. El uso de protección auditiva entre los músicos podría prevenir sustancialmente el daño auditivo, aunque su tasa de utilización es bastante baja.Métodos y materialUn grupo de intérpretes de orquesta de música clásica españoles completó un cuestionario sobre el uso de dispositivos de protección auditiva, cuidados auditivos, y opiniones subjetivas sobre las dificultades auditivas. Analizamos la frecuencia del uso de dispositivos por instrumento basada en tablas de contingencia analizadas mediante pruebas χ2.ResultadosCiento noventa y cuatro intérpretes de orquesta de música clásica españoles completaron el cuestionario voluntariamente. El porcentaje de músicos que reportaron el uso de protección auditiva en nuestra encuesta fue muy bajo, y variable dependiendo del tipo de instrumento interpretado. Sin embargo, encontramos una alta prevalencia de trastornos auditivos subjetivos dentro de este grupo.ConclusiónPocos músicos españoles utilizan protección auditiva. La formación sobre prevención de hipoacusia en este campo y la mejora de los dispositivos protectores podría incrementar el uso y mejorar la salud auditiva de este colectivo. (AU)

ABSTRACT

El ruido en el medio hospitalario conlleva consecuencias serias para la salud. La evidencia demuestra que los pacientes neurológicos hospitalizados, se ven más afectados por la contaminación acústica, ya que ciertos estímulos están alterados, siendo más sensibles a luz y ruido. El descanso nocturno de los pacientes, es un aspecto relevante dentro del proceso de recuperación. Estudios previos, en relación al ruido hospitalario concluyeron que, el índice de ruido intrahospitalario supera las recomendaciones de la Organización Mundial de la Salud, que cifra por debajo de 35 decibelios la noche, y 45 decibelios el día. En este contexto, la Comisión de Calidad Percibida y Humanización estableció un programa específico de intervención sobre el ruido, para la mejora de la calidad percibida de los pacientes ingresados, calidad de vida y sueño. Se planteó un estudio de investigación cuantitativo observacional de tipo transversal para describir los factores relacionados con la percepción de ruido hospitalario en la unidad de neurociencias, y en relación a la medición real de decibelios en diferentes áreas de la misma unidad. Los resultados estiman que es posible reducir la contaminación acústica hospitalaria, haciendo cambios de mínima intervención y bajo coste, pero de gran impacto en la percepción de los pacientes y en la disminución del ruido, como es el mobiliario, ajuste de horarios de visita y concienciación de los profesionales de la salud sobre el ruido generado en la realización de la propia actividad asistencial. Como estudio de extensión, se propone el estudio de la efectividad de medidas propuestas. (AU)


Noise in the hospital environment has serious consequences for health. The evidence shows that hospitalized neurological patients are more affected by light and noise pollution, since certain stimuli are altered, being more sensitive to light and noise. The night rest of the patients is a relevant aspect within the recovery process. Previous studies, in relation to hospital noise, concluded that the intrahospital noise index exceeds the recommendations of the World Health Organization, which is below 35 decibels at night and 45 decibels during the day. In this context, the Perceived Quality and Humanization Commission established a specific noise intervention program to improve the perceived quality of hospitalized patients, quality of life and sleep. A cross-sectional observational quantitative research study was proposed to describe the factors related to the perception of hospital noise in the neuroscience unit, and in relation to the actual measurement of decibels in different areas of the same unit. The results estimate that it is possible to reduce hospital noise pollution, making changes with minimal intervention and low cost, but with a great impact on the perception of patients and on the reduction of noise, such as furniture, adjustment of visiting hours and awareness of health professionals about the noise generated in the performance of the care activity itself. As an extension study, the study of the effectiveness of the proposed measures is proposed. (AU)

BACKGROUND AND OBJECTIVES: Research involving acoustic analysis does not always consider the level of background noise in the room where the recording process is going to be carried out, or if they do, the maximum value of background noise used varies from author to author. The objective of this research is to establish if the parameters: f0, Jitter, Shimmer and Alpha ratio vary in relation to different levels of background noise in the room where the recording process is carried out and to establish a maximum value of this. MATERIALS AND METHODS: The Praat program was used to get the average value of the parameters: f0, Jitter, Shimmer and Alpha ratio from a voice sample of 33 subjects inside an Audiometric booth whose background noise level was 28,1 dB(A). A statistical comparison was made between the average value of each parameter as the background noise increased in relation to the average values ​​obtained with the baseline background noise of the cabin for each parameter. RESULTS: The f0 and Alpha ratio parameters show reliable values ​​up to a background noise level of 47,7 dB(A), while the Jitter and Shimmer parameters do so up to a background noise level of 43,8 dB(A). CONCLUSIONS: Considering that the acoustic parameters are all obtained from the same recording, we can point out that the recommended maximum background noise level in the room where the recordings are made should not exceed 43,8 dB(A).

RESUMO Objetivo Investigar a queixa de dificuldade de inteligibilidade na presença de ruído, em sujeitos sem perda auditiva e compará-la com o desempenho em um teste de fala no ruído. Método A casuística foi constituída por 39 sujeitos de 18 a 59 anos e 11 meses foram divididos em quatro grupos em função da década de vida. Estes foram submetidos à audiometria, timpanometria, testes de processamento auditivo, mini exame do estado mental, autorrelato da percepção auditiva com o Pt-AIADH e a um teste de sentenças no silêncio e no ruído. Resultados Todos os grupos pontuaram para os domínios do Pt-AIADH, com maior pontuação média para o domínio de inteligibilidade no ruído. Houve diferenças entre o G18 vs G40, G18 vs G50, e G30 vs G50 para a autopercepção auditiva na inteligibilidade no ruído; e diferenças entre o G18 e os demais grupos no teste de fala com ruído, com menor relação sinal-ruído para os adultos mais velhos. Houve correlação moderada e significativa para a inteligibilidade no ruído e o teste de fala no ruído. Conclusão Normo-ouvintes de todas as faixas etárias estudadas apresentaram queixa de inteligibilidade no ruído. Quanto maior a dificuldade auditiva neste domínio pior o desempenho no teste de fala com ruído, sendo mais significativo em adultos de meia idade.


ABSTRACT Purpose To investigate complaints of difficulty understanding speech in the presence of noise in subjects without hearing loss and their performance on a speech-in-noise test. Methods Thirty-nine subjects aged 18 to 59 years and 11 months were divided into four groups according to their decade of life. They underwent audiometry, tympanometry, auditory processing tests, the Mini-Mental State Examination, a self-report on auditory perception combined with the Amsterdam Inventory for Auditory Disability and Handicap (Pt-AIADH), and a sentence test in silence and in noise. Results All groups scored high on the Pt-AIADH domains, with the highest average score obtained for the noise intelligibility domain. There were differences between G18 vs. G40, G18 vs. G50, and G30 vs. G50 for auditory self-perception in noise intelligibility, and differences between the youngest and all other groups on the speech-in-noise test in particular, with a lower signal-to-noise ratio for older adults. We also identified a moderate and significant correlation between intelligibility in noise and the speech-in-noise test. Conclusion Normal hearers of all age groups complained of intelligibility in noise. We found that the higher an individual's auditory difficulty in this domain, the worse their performance on the speech-in-noise test; this is especially true for middle-aged adults.
