ABSTRACT

Seborrheic alopecia is a chronic dermatological disease caused by multiple factors. It occurs frequently in young and middle-aged men aged 20-30 years. The main clinical manifestations are greasy hair, itching, excessive dandruff, receding hairline, sparse hair on the top of the head, and progressive hair loss in the frontotemporal area. Seborrheic alopecia is not fatal, but it affects the appearance of patients, seriously harming their self-esteem and bringing great psychological distress to them. The Wnt/β-catenin signaling pathway widely exists in multicellular eukaryotes and is a basic growth regulatory pathway which regulates cell proliferation and differentiation, maintains stem cells activity and organ homeostasis, and affects cell migration. At present, it has been reported in China and abroad that Wnt/β-catenin signaling pathway is closely related to the occurrence and development of seborrheic alopecia and the action mechanism of drugs. Traditional Chinese medicine (TCM) has multi-component, multi-target, and multi-pathway advantages, and it can promote the formation of hair follicle laminae, the proliferation and differentiation of hair follicle stem cells, and the periodic changes in hair follicles by regulating the Wnt/β-catenin signaling pathway, thereby alleviating seborrheic alopecia. This article reviewed the relationship of Wnt/β-catenin signaling pathway and its key target protein factors with seborrheic alopecia to clarify the important role of Wnt/β-catenin signaling pathway in seborrheic alopecia. At the same time, the TCM that targeted the Wnt/β-catenin signaling pathway to relieve seborrheic alopecia were summarized, so as to provide reference for the treatment of seborrheic alopecia and further development of new drugs.

ABSTRACT

BACKGROUND: Seborrheic alopecia (SA) is a common dermatological disease with a long disease course, and treatment for this disease usually exhibits slow effects. Currently, Western medicine treatments have shown some effects; however, they also have certain limitations. In recent years, Chinese medicine has made breakthroughs in treating SA. The efficacy of plum blossom needle acupuncture with qi-invigorating superficies-consolidating therapy for SA was observed, and its clinical effects were investigated in this study. METHODS: A total of 87 patients with SA treated at the First Affiliated Hospital of Guangzhou University of Chinese Medicine from September 2018 to September 2019 were enrolled as the research subjects. They were divided into a Western medicine group and a Chinese medicine group by the random number table method. The 43 patients in the Western medicine group were treated with conventional Western medicine, and the 44 patients in the Chinese medicine group were treated with a comprehensive traditional Chinese medicine regimen of plum blossom needling with qi-invigorating superficies-consolidating therapy. The treatment effects, changes in estradiol (E2) and testosterone (T) levels, and scores for various body signs (hair growth and hair loss) of the two groups before and after treatment were compared. RESULTS: The difference in total effective rate between the Chinese medicine group and the Western medicine group (95.45% vs. 81.40%) was statistically significant (P<0.05). After treatment, the T levels of both groups were lower than before treatment, and the E2 levels of both groups were higher than before treatment; the difference between the two groups was statistically significant (P<0.05). After treatment, the hair growth and hair loss scores of the two groups were lower than those before treatment, and those of the Chinese medicine group were lower than those of the Western medicine group; the differences were statistically significant (P<0.05). CONCLUSIONS: Plum blossom needle acupuncture combined with qi-invigorating superficies-consolidating therapy is significantly effective for treating patients with SA. It can effectively adjust the patient's hormone levels, improve hair loss, and promote hair growth. It has the advantages of simplicity, tolerability, and low cost. It cures the cause of the disease and is worth promoting.

ABSTRACT

Objective To investigate the therapeutic effect of dense-packing auto hair grafting technique on the restoration of seborrheic alopecia. Methods With local anesthesia, a scalp strip was harvested from the back of the head. Under operating microscope (Various graft was created from the scalp strip, including micro-grafts with 1-2 hairs, mini-grafts with 3-4 hairs and sliver graft with 5-6 hairs. In the alopecia recipient area, micro slots were made with a small triangle-edged needle for the micro-grafts,mini slits were made with mini blade for the mini-grafts and foramen ovale were made with a slot punch. The grafts were then implanted into these holes. Results 32 cases of seborrheice alopecia were treated with the above mentioned technique from March 2007 to July 2009. Postoperative following up for 12-24 month showed that the grafted hairs were growing well with average 90 % survival of the hair. 81 % of the patients obtained satisfactory results with only one stage operation. Six patients needed the second operation to improve the appearance. All of the patients were satisfied with the appearance. Conclusions The dense-packing hair grafting technique with various grafts not only saves time of operation, but also obtains dense grafted hair and well appearance. The results are satisfactory to most patients with only one stage operation.
