ABSTRACT
ObjectiveTo explore the mechanism of Shenqi Gualou Xiebai Banxia Decoction （参芪瓜蒌薤白半夏汤， SGXBD） in the treatment of atherosclerosis. MethodsThirty Apolipoprotein E gene knockout （ApoE-/-） mice were randomly divided into five groups： model group， rosuvastatin group， low-， moderate-， and high-dose SGXBD， with six mice in each group. They were fed a high-fat diet to prepare for atherosclerosis model. Another six C57BL/6J wild-type mice were set as the blank group. After modeling， the low-， moderate-， and high-dose SGXBD groups were gavaged with 6.46， 12.92， and 25.84 g/（kg·d） of SGXBD， respectively. The rosuvastatin group was given 1.55 mg/（kg·d） of rosuvastatin tablets by gavage. The blank group and model group were given 0.5 ml saline by gavage. After four weeks， the total cholesterol （TC）， triglyceride （TG）， low-density lipoprotein cholesterol （LDL-C）， and high-density lipoprotein cholesterol （HDL-C） in the serum of each group were detected， as well as TC and TG in the liver. The serum bile acid level was detected by enzyme cycling colorimetry. The mRNA and protein expression of peroxisome proliferator-activated receptor γ （PPARγ）， sterol regulatory element-binding protein 2 （SREBP2）， 3-hydroxy-3-methylglutaryl-CoA reductase （HMGCR）， and cholesterol 7α-hydroxylase （CYP7A1） in the liver were detected by real-time RT-PCR and Western blot. ResultsCompared with the blank group， the model group showed significant increases in serum TG， TC， and LDL-C levels， and significant decreases in HDL-C and bile acid levels； the levels of TG and TC in the liver， as well as the expression of SREBP2 and HMGCR proteins and mRNA in the liver significantly increased， while the expression of PPARγ and CYP7A1 proteins and mRNA significantly decreased （all P＜0.01）. Compared with the model group， the rosuvastatin group and high-dose SGXBD group showed significant decreases in serum TG， TC， and LDL-C levels and liver TG and TC levels， and significant increases in bile acid levels； the expression of PPARγ and CYP7A1 proteins and mRNA increased， while the expression of SREBP2 and HMGCR proteins and mRNA decreased； the low-dose SGXBD group showed significant decreases in serum TC and LDL-C levels and liver TC level （P＜0.05 or P＜0.01）. Compared with the rosuvastatin group， the low-dose SGXBD group had a significantly higher liver TC level， while the high-dose SGXBD group had a significantly lower liver TC level， CYP7A1 mRNA level， and PPARγ protein expression level， and a significantly higher SREBP2 protein expression level （P＜0.05 or P＜0.01）. Compared with the low- and moderate-dose groups， the high-dose SGXBD group had significantly lower serum TG and liver TC levels （P＜0.05）. ConclusionSGXBD may improve blood lipid levels and exhibit anti-atherosclerotic effects by regulating the protein level of PPARγ and simultaneously affecting the synthesis of liver cholesterol and the conversion of cholesterol to bile acids.

Loss of appetite is called anorexia in traditional Chinese medicine (TCM), and it is one of the most common adverse reactions in the course of chemotherapy for treatment of patients with malignant tumor. Anorexia results in severe malnutrition and delay of the recovery of physical strength, that lead to the patients' intolerance to chemotherapy, incompliance with the treatment, no guarantee in quality of life, loss of confidence in treatment effect and finally, the therapeutic efficacy is seriously affected. According to an old classical TCM book: The " Kidney is the Gate of Stomach" is discussed in the book, so the author of this article has used tonifying spleen and kidney method to improve anorexia after tumor chemotherapy,and the efficacy is satisfactory. Most doctors believe that the formation of tumor is"due to deficiency". Mainly, the internal visceral organs are responsible for the weakness of healthy qi, particularly the Spleen and kidney. The basis of tumor formation is the weakness of the viscera, especially the deficiency of spleen and kidney. Based on clinical experiences, the spleen and kidney tonification method is discussed and used as the guidance to treat the anorexia after chemotherapy for malignant tumor patients, showing that the method is important for treatment of anorexia in such patients.

Objective:Based on the hospital information system (hospital information system,HIS) were studied in rheumatology clinical medicine data hidden behind the prescription pattern,aims to explore the treatment of rheumatoid arthritis with patterns or rules from the spleen.Methods:This study from May 2012 to December 2016 in the treatment of active rheumatoid arthritis patients as the research object in the division of Rheumatology at the First Affiliated Hospital of Anhui TCM university.The use of Office 2010,the complex network technology and IBM Clementine 14.2 SPSS Apriori software module for Chinese medicine core prescription analysis,frequency statistics,association rule analysis.Results:a total of 4 000 copies of medical records and 3 643 prescriptions of traditional Chinese medicine were included in the study,the main results are as follows:(1) the clinical medication can be divided into jianpihuashi drugs,Huoxue Huayu drugs,dispelling dampness and heat drugs in four categories,more than 60％ times the frequency of tuckahoe,liquorice,tangerine peel,coix seed,Salvia miltiorrhiza the kernel,safflower,Chinese yam,clematis root,peach kernel,core prescription:tuckahoe,liquorice,tangerine peel,Salvia miltiorrhiza,coix seed,safflower,Chinese yam,clematis root,peach kernel,dandelion;(2) drug association analysis:Poria and Oldenlandia,dandelion,Herba Siegesbeckiae,peach kernel,Chinese medicine jianpihuashi the compatibility of blood stasis,dispelling dampness and heat drugs for 20 drug combinations;(3) compare the index difference between the two groups before and after treatment,the experimental group Hs-CRP,RF,ESR,IgA,IgG and C3 compared with the control group decreased significantly after treatment,the difference was statistically significant (P＜0.05 or P＜0.01);(4) the analysis of drug and two indicators of Relevance:Jiang Banxia,Herba Siegesbeckiae and Hs-CRP,ESR decreased the incidence is higher,white atractylodes rhizome,Salvia miltiorrhiza and RF reduce the high degree of relevance,yam,dandelion and C3 to reduce the high degree of relevance,Du Zhonghe,IGA,IGG and Honghua Weilingxian to reduce the high correlation.Analysis of the three Association:Jiang Banxia,Herba Siegesbeckiae respectively with Hs-CRP,ESR,dandelion,walnuts and reduce the high degree of relevance,astragalus,salvia,licorice and RF reduce the high degree of relevance,yam,dandelion,Herba Siegesbeckiae and C3 reduce the high degree of relevance,malt,dandelion,Radix Clematidis and IGA to reduce the high degree of relevance,eucommia,safflower,licorice and IGG reduce high correlation.Conclusion:This study on the treatment of rheumatoid arthritis from the real world to the spleen,spleen wet medicine as the core,and the compatibility between blood stasis,antipyretic,rheumatism medicine prescription is the key,which can effectively improve the patients' immune inflammation and other laboratory indexes.
