ABSTRACT

BACKGROUND: Alcohol withdrawal can lead to severe complications including seizures, delirium tremens, and death if not treated appropriately. Nurses are critical to the safety and outcomes of these patients. OBJECTIVE: The objective of this retrospective study was to determine if nursing education on a community hospital's alcohol withdrawal protocol led to improved nursing compliance. METHODS: This is a quality improvement project involving a two-part retrospective review-an initial needs assessment followed by nursing education and a subsequent posteducation retrospective review. The initial needs assessment included 65 patients. The subsequent posteducation group included 50 patients. RESULTS: Nursing compliance of 1-hour assessments increased after the educational intervention; however, there was no statistically significant difference in 6-hour assessment or medication administration protocol compliance between preeducation and posteducation groups. CONCLUSION: Nursing education is a good place to start in improving compliance with an alcohol withdrawal protocol, but physicians need to be included to increase standardization within the institution. Future study should look at the effectiveness of different assessment frequency intervals and its impact on patient-centered outcomes.

ABSTRACT

To develop minimum standards for nursing care for people intoxicated by alcohol and treated in the emergency units. Method: documental research, built upon the experience of the authors as members of the nursing care team of a center of toxicological assistance, based on literature review. Results: we presented the results in two units. Firstly, through a brief literature review on patient safety and alcohol users attended in emergency department, and secondly through a description of minimum standards for initial nursing care necessary for the safety of users of alcohol in three aspects: clinical/biological, psycho-emotional and social. ABSTRACT

Alcohol health workers (AHWs) have been identified as an effective means of tackling alcohol-related hospital admissions. However, there is no understanding of the national coverage, or the extent and diversity of the services provided by hospital-based AHWs. Using a cross-sectional questionnaire, this is the first study to explore the current provision and remit of AHWs in acute hospitals across England.The data was analysed using SPSS. Significant differences were found with regards to the extent and diversity of AHW provision across England. This research provides a point of comparison for current and future hospital-based AHW provision.Further research is necessary to examine different 'service types', establish effective ways of working, and determine whether sources of funding could and should more accurately reflect the remit of hospital-based AHW roles.

ABSTRACT

Estudo qualitativo, com base na Análise de Conteúdo, a partir da interrogação sobre como o usuário de drogas em situação de tratamento para a dependência química percebe-se como trabalhador, e qual sua relação com o mundo do trabalho. O referencial teórico apoiou-se nas definições de drogas psicotrópicas, dependência química e trabalho, das seguintes fontes, respectivamente, Organização das Nações Unidas (ONU), Classificação Internacional das Doenças (CID-10) e Ministério do Trabalho (MT) e a teoria da Psicodinâmica do trabalho. A metodologia baseou-se na Análise de Conteúdo Temática. A coleta de dados desenvolveu-se por meio de entrevistas em profundidade, e pela obtenção de dados sociodemográficos e de trabalho a partir dos registros. Foram realizadas trinta e oitos entrevistas semi estruturadas com dez mulheres, quatorze homens usuários de múltiplas drogas e quatorze homens usuários somente de bebidas alcoólicas. Todos os sujeitos eram trabalhadores em tratamento no Centro de Atenção Psicossocial-álcool e drogas Centro Estadual de Tratamento e Reabilitação de Adictos (Caps-ad CENTRA-RIO). Foram organizadas e analisadas seis categorias: conciliação ente o trabalho e o consumo de drogas; trabalho e angústia; o mundo do trabalho favorecendo/estimulando o consumo; beneficio e UD; trabalho pleno; perspectivas de vida do UD. Discutiu-se a intensa relação de sofrimento que permeia o tempo todo o trabalhador usuário de drogas, as diversas alternativas experimentadas para conseguir conciliar o binômio trabalho e drogas, a submissão aos valores construídos no ambiente de trabalho, a função de integradora intragrupo dos trabalhadores e terapêutica das drogas para conseguir cumprir o trabalho real, sua função relaxante e domadora da angústia e do medo. Nas conclusões, discutem-se os limites do mundo do trabalho em acompanhar a evolução do campo da saúde mental e a Reforma Psiquiátrica, e a importância para a enfermagem do trabalho...


Qualitative study, based on Content Analysis, interrogating how the dependent drug user is perceived as a worker, and his/her relationship with workplace. Theoretical referential used definitions of drugs psychotropic drug, chemical dependence and work of the following sources, respectively: Word Health Organization – WHO; United Nations Organization (UN), International Classification of Diseases (CID-10); Brazilian National Department of Labor (MT) and Work Psychodynamics’ Theory. Methodology was based on the Thematic Content Analysis. Data collection was developed through in-depth interviews, also using sociodemographic data from medical records.. Thirty eight semi-structured interviews were accomplished with ten women, fourteen men multiple drugs users, and fourteen men alcohol-only users. All the subjects were workers in treatment in the State Psychosocial Attention Center for Alcohol and Drugs - Treatment and Rehabilitation of Addicted Center (Caps-AD - CENTRA-RIO). Results were organized and analyzed in six categories: conciliation between work and drug consumption; the work world making consumption easier; Legal benefits and DU; delightful work; DU’s life perspectives. The intense suffering relationship that permeates the whole time that theses workers used drugs was discussed, and also the several alternatives experienced in order to reconcile the work-and-drugs binomial, the submission to the values built in the work atmosphere, the integrative and therapeutic function of drug that aloud workers to accomplish the real work, and it’s relaxing and taming function of the anguish and fear. ABSTRACT

O objetivo deste estudo foi compreender os conceitos e estratégias que norteiam a assistência dos profissionais que atuam em um Centro de Atenção Psicossocial para atendimento de pacientes com transtornos, decorrentes do uso e dependência de substâncias psicoativas, na tentativa de subsidiar as ações dos demais serviços de saúde que atendem tais pacientes. Optou-se pela pesquisa qualitativa, seguindo a metodologia de estudo de caso; assim, nove profissionais foram entrevistados e o eixo norteador das questões foi referente à concepção de serviço e as ações assistenciais ali desenvolvidas. Os dados foram analisados à luz do conceito da representação social. Valendo-se dos temas emergentes foram elaboradas duas representações centrais da pesquisa: Concepção sobre o serviço de atenção psicossocial. Ações assistenciais aos pacientes com transtornos decorrentes de uso prejudicial e ou dependência de álcool e outras drogas. A análise final orienta-se para a compreensão de que o uso prejudicial do álcool e outras drogas decorrem de fatores multifacetados, propondo nessa linha de raciocínio a assistência psicossocial.

ABSTRACT

This study utilized a life history approach to describe clients' psychological experiences of the alcohol withdrawal process while voluntarily residing in a specialist alcohol withdrawal facility. Reflection on the past and anticipation of the future frequently occupied the thoughts of participants as they sought insight in to their lives. These insights were associated with a range of emotions that included embarrassment, shame, optimism, feelings of support and a sense of loss of control. The findings provide additional information on the human experience of alcohol withdrawal and, thus, increase empathy, understanding and knowledge. This increased understanding can be utilized to improve the quality of nursing care provided to this complex client group.

ABSTRACT

Substance abuse is a widely identified problem in our society. Alcohol abuse is the most problematic, yet most undertreated. Primary care practitioners often do not screen the geriatric population appropriately for at-risk drinking, so the condition goes undiagnosed. This oversight is complicated by the fact that many manifestations of alcohol abuse do not present themselves until well into the disease process. This article explores the practitioner's role in geriatric alcohol abuse assessment and treatment and the impact of geriatric alcohol abuse on society.
