ABSTRACT

BACKGROUND: The aim of this study was to determine the incidence of different Kennedy's classes of partial edentulism during 18 months period. MATERIALS AND METHODS: Patients were clinically examined for various Kennedy's classes of partial edentulism in the outpatient department (OPD), prosthodontics, GPRDCH, Kurnool (Andhra Pradesh). RESULTS: Of the total 1,420 OPD patients, Kennedy's class III was the most frequent classification encountered (62%) and followed by Kennedy's class I (18%), class II (11%), and class IV (9%) in decreasing order. CONCLUSION: The patients with various Kennedy's classes of partial edentulism can be offered various treatment modalities like removable cast partial dentures, fixed partial dentures, over dentures and implant supported dentures. This study can be crucial for screening the population for incidence of tooth loss as a factor of gender and age. Clinical significance: Tooth loss appears to have an important role in the loss of esthetics and mastication. Study of incidence of various classes of partial edentulism provides clinically useful information for dental training and continuing education.

ABSTRACT

Introducción. El objetivo del presente estudio era valorar los resultados clínicos de la carga inmediata en el tratamiento con implantes oseointegrados de la mandíbula edéntula. Pacientes y métodos. Los pacientes edéntulos completos fueron tratados con implantes Microdent® con superficie con chorreado de arena y grabada con ácidos para la rehabilitación de la mandíbula edéntula mediante la colocación de 4 implantes en el sector anterior con carga funcional inmediata. Los implantes fueron evaluados durante 1 año. Resultados. 10 pacientes (6 varones y 4 mujeres) fueron tratados con 40 implantes (20 rectos y 20 inclinados) de 14-16 mm de longitud y fueron cargados de forma inmediata con una prótesis completa de resina atornillada. Los hallazgos clínicos indican una supervivencia y éxito de los implantes del 100% ya que ningún implante se ha perdido durante el año de seguimiento clínico. Conclusiones. Este estudio indica que la carga inmediata de los implantes en el tratamiento de la mandíbula edéntula constituye un tratamiento odontológico con una elevada tasa de éxito (AU)


Introduction. The aim of this study was to assess the clinical outcome of immediate loading in the treatment with osseointegrated implants of edentulous mandible. Methods. Edentulous patients were treated with Microdent® sandblasted-etched surface implants for rehabilitation of mandible by insertion of 4 implants in anterior area with immediate loading. A follow-up were of 12 months. Results. 10 patients (6 men and 4 women) were treated with 40 implants (20 straight and 20 inclined) with a length of 14-16 mm and were loaded immediately with a acrylic screw full denture. Clinical findings showed a survival and success rate of implants of 100%. No implants were lost during the follow-up period. Conclusions. This study indicate that immediate loading of implants in the treatment of edentulous mandible constitute a dental treatment treatment with a high success rate success (AU)

ABSTRACT

Patients afflicted with congenital/developmental anomalies of the oral/head and neck regions present the prosthodontist with unique esthetic and functional restorative challenges. This article reviews the various pediatric patient categories that would benefit from placement of "vital" overdentures. A long-term follow-up of patients seen over 25 years of practice is presented with a focus on complications such as temporomandibular joint stability, caries, soft tissue and bony support changes, and denture adaptation. Longitudinal follow-up of these patients indicates that, although problems occur with overdenture therapy, the benefits of this treatment far outweigh the complications.

ABSTRACT

PURPOSE: This article proposes a classification protocol for reporting the outcome of implant overdentures. MATERIALS AND METHODS: Review of the literature pertaining to postinsertion prosthodontic procedures for maxillary and mandibular implant overdentures revealed a wide range in the terminology used and methods of reporting outcome, usually as a result of the use of varying designs among different implant systems. A need for comparative evaluation of the prosthodontic success of implant overdentures was identified. RESULTS: An existing classification protocol for implant-fixed prostheses containing six objectively defined fields was modified for implant overdentures. This new protocol, which also uses the descriptive fields of success, survival, unknown, dead, and retreatment (repair or failure), provides a method whereby postinsertion prosthodontic procedures and subsequent time to retreatment can be quantified regardless of design, method of attachment, or implant system. A ratio method for identifying prosthodontic treatment cost effectiveness of implant overdentures is included. CONCLUSION: This protocol could assist clinicians in their choices of implant overdenture therapy by providing accurate comparisons of varying implant overdenture systems and designs, and by identifying prosthodontic treatment cost effectiveness.

ABSTRACT

La P.R.P. retenida con ganchos constituye un problema estético que puede ser minimizado a través de un adecuado análisis y diseño del caso. La utilización del aditamento ofrece la ventaja de obtener excelentes restultados estéticos al eliminar el brazo retentivo vestibular de los ganchos; sin embargo, la utilización de éstos representa trabajo adicional para el odontólogo y mayor costo para el paciente; además, el uso de estos aditamentos es un tema controversial y confuso en la especialidad debido a la multitud de alternativas, como son aditamentos intracoronales, extracoronales, intrarradiculares, extrarradiculares, de precisión, de smiprecisión, rígido y elásticos. En esta revisión se presentan algunas consideraciones estéticas en el diseño de retenedores directos, así como la nomenclatura, clasificación e indicaciones de los diferentes tipos de aditamentos utilizados como retenedores de P.P.R. y los criterios para su selección

ABSTRACT

A classification system for implant-supported overdentures is proposed in this article. Various types of implant-supported overdentures have been described in the literature, but there is considerable difference in the function of these designs. For example, an overdenture supported by freestanding implants does not have the retentive elements provided by a bar and, therefore, is less retentive than an overdenture supported by a straight bar. If more than two implants are configured in an arch, and if indirect retention is incorporated into the bar and overdenture design, function is improved over that of a straight-bar design. Another classification of overdenture is one that receives all of its support from an implant-supported bar with tissue contact to provide a mucosal seal. This tissue contact does not provide any actual support. By accepting and using this classification system, the clinician can design an overdenture that is appropriate for the functional requirements of the patient being treated.

ABSTRACT

Una sobredentadura parcial removible es aquella prótesis que usa dientes naturales y raíces retenidas para soporte y retención, siendo similar a una dentadura parcial removible convencional con la única excepción de que las bases protéticas cubren una o más raíces retenidas. El mantener a los dientes como parte del reborde residual, contribuye a disminuir la velocidad de reabsorción de éste, a la vez que se mantienen los impulsos propioceptivos, lo que permite conservar la discriminación direccional, la sensibilidad táctil a la carga y la diferenciación del grosor y consistencia de los alimentos, casi de la misma forma que en aquellos pacientes con dientes naturales, por lo que éstos se habitúan más fácilmente a estas prótesis. Al conservar el hueso alveolar también se produce menos trauma a los tejidos remanentes, aumentándose el soporte, la retención y la estabilidad protética. Las S.P.R. también constituyen soluciones rápidas y conservadoras para los múltiples problemas que presentan los pacientes con defectos congénitos y adquiridos y son además, usadas en muchos casos como prótesis transitorias. Sin embargo, la caries y la enfermedad periodontal son los principales problemas que se presentan con el uso de este tipo de prótesis. La conservación de los soportes, así como de los dientes naturales remanentes, depende en gran parte de la motivación y destreza del paciente para mantener un alto grado de higiene bucal

ABSTRACT

Although advances in methods and improved materials have contributed to better treatment results, caries and periodontal problems still remain significant threats to overdenture service life. The prevention of caries and recurrent periodontal disease thus becomes a most important aspect of postinsertion care. Undoubtedly, there are a number of successful overdenture methods. This article outlines two treatment sequences previously described by Brewer and Morrow that continue to be effective (Fig. 23). Common to most overdenture techniques is that an overdenture may be the only treatment that permits retention using the patient's last few retainable teeth. A conservative approach to root preservation still is a valid and practical measure in preventative dentistry.
