ABSTRACT

Background: Alveolar osteitis (AO) or "dry socket" affects the quality of life of patients, and there is a high clinical demand for its effective treatment. Objective: To evaluate the effect of Er:YAG laser therapy (ErLT) on AO after mandibular third molar surgery. Methods: Eighty-three patients were randomly divided into Er (n = 43) and control groups (n = 40). In the Er group, the Er:YAG laser (2940 nm; AT Fidelis Fotona, Ljubljana, Slovenia) was used to irradiate the AO site directly in micro short-pulsed mode (pulse duration 0.1 ms, pulse energy 100 mJ, frequency 40 Hz, water 4, and air 2) until all debris and necrotic material had been removed, exposing fresh bone and soft tissue surfaces with blood exudation. The control group received mechanical therapy until the treated lesions resembled those in the Er group. Pain assessment was performed at baseline and on days 1-7 post-intervention using the visual analog scale (VAS). Wound healing was assessed using the wound healing index (WHI). The operating times of the two therapies were also recorded. Results: Group Er had lower VAS scores than the control group on days 1-3 (p = 0.00). There was no significant difference between the two groups on days 4-7 (p = 0.15). The WHI scores were better in the Er group than those in the control group (t = 2.65, p = 0.01), especially in terms of redness (t = 2.70, p = 0.01). There was no significant difference in the operating time between the two groups (t = 0.76, p = 0.45). Conclusions: Compared with mechanical therapy, ErLT for AO provides rapid pain relief and improved wound healing.

ABSTRACT

Alveolar osteitis (AO) is a common complication following the extraction of the teeth, particularly the lower third molars. It starts within a few days after the extraction and manifests mainly as pain in the extraction site. Several strategies of treatment are available in order to relieve pain and heal the extraction wound. Recently, a novel medical device combining hyaluronic acid (HA) and octenidine (OCT) was introduced for the treatment of AO. This series of case reports aims to summarize the initial clinical experiences with this new device and to highlight factors possibly interfering with this treatment. The medical documentation of five patients with similar initial situations treated for AO with HA + OCT device was analyzed in detail. Smoking and previous treatment with Alveogyl (Septodont, Saint-Maur-des-Fossés, France) were identified as factors interfering with the AO treatment with the HA + OCT device. In three patients without these risk factors, the treatment led to recovery within two or three days. The patient pretreated with Alveogyl and the smoker required six and seven applications of the HA + OCT device, respectively. According to these initial observations, it seems smoking and previous treatment with Alveogyl prolong the treatment of AO using the HA + OCT device that, in turn, shows a rapid effect if these risk factors are not present.

ABSTRACT

Introducción: la osteítis alveolar es una complicación postoperatoria que acontece tras la extracción dentaria, se realizó la investigación por la frecuencia de esta enfermedad en el servicio de urgencias y la manipulación errónea de los casos. Objetivo: describir las características clínico-epidemiológicas de la osteítis alveolar en pacientes que acudieron al servicio de urgencia de la Clínica Estomatológica Docente de Güines. Métodos: se realizó un estudio descriptivo transversal de marzo del 2014 a marzo del 2015; la población objeto de estudio estuvo integrada por 94 pacientes, de 19 y más años, de diferentes sexos, con osteítis alveolar. Se realizó interrogatorio y examen clínico bucal. Se confeccionó una planilla de recolección de datos. Se empleó como medida resumen el porciento, se presentaron tablas de doble entrada, la prueba estadística se realizó usando la tabla de contingencia a través de la dócima X2, en los casos en que se alcanzó diferencias significativas se aplicó la dócima de comparación Múltiple de Duncan. Resultados: predominaron las féminas (64.89 por ciento) y el grupo de 19-34 años (46.81 por ciento). Resultó más común la osteítis seca (69.15 por ciento) con mayor frecuencia en arcada inferior (63.83 por ciento) y en molares (50.0 por ciento). Conclusiones: la osteítis alveolar predominó en féminas y en el grupo de 19-34 años. El tipo más común fue la osteítis seca con mayor frecuencia en el grupo de molares de la arcada inferior. Los factores de riesgo más frecuentes fueron la higiene bucal deficiente y el hábito de fumar y como causa de la extracción dentaria, la enfermedad pulpar(AU)


Introduction: alveolar osteitis is a postoperative complication that occurs after tooth extraction, the investigation was performed due to the frequency of this disease in the emergency department and the wrong handling of the cases. Objective: to describe the clinical and epidemiological characteristics of alveolar osteitis in patients who attended the emergency department of the Güines Clinical Stomatology Clinic. Methods: a cross-sectional descriptive study was carried out from March 2014 to March 2015; The study population consisted of 94 patients, aged 19 and over, of different sexes, with alveolar osteitis. We performed a questionnaire and oral clinical examination. A data collection sheet was made. A summary measure was used, double entry tables were presented, the statistical test was performed using the contingency table through the 12th decimal point, in The cases where significant differences were reached were applied the Duncan Multiple comparison decimal. Results: women predominated (64.89 percent) and the group aged 19-34 (46.81 percent). Dry osteitis (69.15 percent) was more common, with lower frequency in the lower arch (63.83 percent) and molars (50.0 percent). Conclusions: alveolar osteitis predominated in females and in the group of 19-34 years. The most common type was dry osteitis most frequently in the group of molars of the lower arch. The most frequent risk factors were poor oral hygiene and smoking and as a cause of tooth extraction, pulp disease(AU)

ABSTRACT

AIM: The development of oral implant techniques and the demand for the treatment of increasingly complex cases have drawn the attention of researchers and clinicians towards those systems and materials able to promote bone regeneration. The aim of this paper is to evaluate the clinical behavior and in particular the resorption times of the material used as a filler in postextractive alveolus with the intent of preserving the ridge volume, a prerequisite for successive insertion of osteointegrated fixtures. METHODS: A group of 12 patients, aged between 30-40 years, have been selected at the Oral Surgery Unit of the San Giovanni Calibita Hospital in Rome. They all required an endosseous implant following the loss of a dental element due to radicular fracture of periodontal pathology. The grafting material used is OsteoBiol Putty, an antigen-free bone paste composed of 80% granulated mix and 20% pure collagen. RESULTS: The results emerging from the histological analysis show that 3 months after insertion of this osteoconductive material in the receiving site it can no longer be detected in the samples collected and it has been completely substituted by trabecular bone tissue. CONCLUSIONS: The conclusion is drawn that the heterologous material used in this study shows excellent manageability allowing the operator to easily and accurately remove and shape it in the receiving site.

ABSTRACT

Following extraction of a maxillary left first molar tooth in an eight year-old retriever, the dog re-presented five days later because of oral pain, which did not respond to analgesic therapy. The extraction site contained a foul-smelling fluid, but did not contain a clot or granulation tissue. Alveolar osteitis (dry socket) was diagnosed. The alveolus was curetted and flushed, and the dog was given cefalexine and prednisolone. The alveolus was filling with healthy granulation tissue one week later and the dog was no longer in pain.

ABSTRACT

O objetivo deste trabalho foi verificar se a anti-sepsia intrabucal com soluçäo de clorexedina a 0.2 por cento exerce influência sobre a reparaçäo de feridas de extraçäo dental. Foram empregados 45 ratos, divididos em 3 grupos (controle, soro fisiológico e clorexedina). A anti-sepsia (clorexedina) ou a simulaçäo da anti-sepsia (soro fisiológico) foi realizada antes da exodontia. Os animais foram sacrificados aos 3, 10 e 21 dias após a exodontia. Os autores concluíram que a anti-sepsia com soluçäo de digluconato de clorexedina 0.2 por cento näo interfere na cronologia da reparaçäo de feridas de extraçäo dental

ABSTRACT

The records of 61 horses undergoing tooth repulsion for treatment of alveolar periostitis were reviewed. Seventeen of 36 horses (47%) in which maxillary teeth were removed had serious postoperative complications, such as infection of a second tooth, bone sequestration, chronic sinusitis, draining tracts, retained dental packing, feed impaction of the alveolus or sinus, suture-line dehiscence, or skin-flap sloughs. Eight horses required at least one additional surgical procedure. Eight of 25 horses (32%) in which mandibular teeth were removed had serious postoperative complications, and four horses required an additional surgical procedure. Hospitalization lasted 2 to 61 days (median, 22 days) for maxillary teeth and 3 to 35 days (median, 8 days) for mandibular teeth. Long-term follow-up (at least 5 months) was possible in 47 horses. Twenty-four of 30 horses (80%) with maxillary tooth repulsion healed without further problems; six horses had persistent nasal discharge. Fourteen of 17 horses (82%) with mandibular tooth repulsion healed with no further problems or with only minor complications; three horses had a chronic draining tract.

ABSTRACT

A buccal plate destroyed by the inflammatory process as a result of a longitudinal root fracture was completely restored by the combined use of an IMZ implant placed in the alveolus of the fractured maxillary central incisor and the principles of guided tissue regeneration. A Gengiflex membrane was used to cover the implant, and porous hydroxyapatite was used as grafting material. At the 6-month reentry, the defect around the implant was completely filled by mineralized tissue.

ABSTRACT

Os autores relatam um caso clínico de transplante autógeno de dente para a cavidade óssea em fase de cicatrizaçäo, após terem enucleado um odontoma e extraído as raízes do 51 e 52 localizados na regiäo anterior da maxila. Concluem que o reparo periodontal processa-se rapidamente, com pouca ocorrência de reabsorçäo radicular e anquilose dental, mesmo quando o dente contém rizogênese completa. Discutem os bons resultados da técnica e a consideram viável para os transplantes autógenos de dentes. Lembram que esses resultados observados näo podem ser extrapolados para os transplantes homógenos e heterógenos pois, estes últimos, exibem quadro de rejeiçäo imunológica. Para prevenir os acidentes e transtornos das inclusöes dentais, os autores enfatizam a importância dos exames radiográficos, como rotina dos pacientes com faixa etária de 4-6, 10-12 e 16-18 anos, respectivamente para as regiöes dos incisivos superiores, caninos e pré-molares e terceiros molares

ABSTRACT

Bone tissue regeneration and abatement of inflammatory processes in the well after tooth extraction were examined in experimental dogs treated with iodoform and crushed brefobone preserved in 0.5 percent formalin. Histologic study has confirmed the efficacy of crushed brefobone employment for the inflamed well plasty, for such treatment was conducive to a sooner abatement of the inflammation and regeneration of bone tissue in the well.

ABSTRACT

Os autores compararam a limpeza cirúrgica alveolar, a aplicaçäo de Alveosan sem limpeza cirúrgica e a limpeza cirúrgica com aplicaçäo de Alveosan em alvéolos acometidos por alveolite provocada experimentalmente em ratos. Os animais foram sacrificados aos 6, 15, 21 e 28 dias pós-operatórios, e as peças foram tratadas, coradas em hematoxilina e eosina e analisadas na microscopia óptica. Foi possível concluir que todos os tratamentos aceleraram o reparo alveolar em comparaçäo o grupo com alveolite e com näo tratado e, entre os grupos experimentais, o que apresentou melhor resultado foi o da limpeza cirúrgica com aplicaçäo de Alveosan

An overview of the various possibilities for oral surgery urgent care to the oral region has been presented. Modern treatment procedures have been described. The recent advances in dental science have become superior to what they were just a few short years ago; however, we must never forget the variabilities of human responses to any of our treatment techniques, and we must not be dogmatic in our approach. We should all be able to diagnose, interpret, and alter any of our treatment techniques as necessary.
