ABSTRACT

BACKGROUND AND OBJECTIVE: Children with special health care needs (SHCNs) have significant medical and educational expenses affecting household finances. Housing instability can be detrimental to family well-being. Our objective was to evaluate housing instability in households of children with and without SHCNs. METHODS: Cross-sectional surveys (2013-2017) in English and Spanish of caregivers with children <4 years old were conducted at 5 hospitals. The children with SHCN screener and caregiver report of child Supplemental Security Income (SSI) receipt were used to categorize children into the following groups: (1) no SHCNs, (2) SHCNs and no SSI, or (3) SHCNs and receiving SSI. Housing instability was determined by positive endorsement of ≥1 adverse circumstance: behind on rent or mortgage, or moving twice or more in the past year, or homelessness in the child's lifetime. Analyses used multivariable logistic regression models, adjusting for demographics and housing subsidies. RESULTS: Of 14 188 children, 80% had no SHCNs, 16% had SHCNs and no SSI, and 4% had SHCNs and received SSI. Compared with the no-SHCNs group, the SHCNs-no-SSI group but not the SHCN-receiving-SSI group experienced significantly greater adjusted odds of being behind on rent or mortgage (adjusted odds ratio [aOR] 1.28 [95% confidence interval (CI) 1.14-1.44]; P < .001), multiple moves (aOR 1.29 [95% CI 1.05-1.59]; P = .01), and homelessness (aOR 1.44 [95% CI 1.20-1.72]; P < .001). CONCLUSIONS: Families of children with SHCNs are at risk for housing instability. Child SSI receipt decreased the risk of housing instability among families of children with SHCNs. Protecting families of young children with SHCNs from housing instability is an important investment.

El proceso de envejecimiento, que en la actualidad está afectando a un número significativo de personas con discapacidad intelectual, constituye una realidad social emergente, que se ha producido a causa del aumento de la esperanza de vida entre esta población que no se daba en épocas anteriores. El envejecimiento de las personas con DI plantea nuevos desafíos, nuevas necesidades, dado que muchas veces los recursos no están adaptados a esta nueva situación. Este fenómeno social, sin duda puede redundar en una doble vulnerabilidad (persona con Discapacidad Intelectual y envejecimiento), que repercutirá sobre las propias personas con discapacidad Intelectual, sus familias, al personal cuidador, las entidades y a las instituciones responsables del bienestar de las personas


The aging process, which nowadays is affecting a number of people with intellectual disability, is turning into an emerging social reality. The reason that lies beneath it, is the life expectancy growth in comparison with past times. The aging process that affects people with intellectual disability arises new challenges and needs, since current resources are usually not adapted to this new situation. This social phenomenon will represent, without a doubt, a double vunerability (person with intellectual disability and aging), which will have an effect on people with intellectual disability themselves, their families, caregiving staff, entities and those institutions in charge of people wellness

ABSTRACT

Attracting and retaining a stable and motivated home care workforce has become a top policy priority. We surveyed 402 former home care workers in Washington State. We compared these "leavers" to current home care workers recently surveyed. Those who left the profession were more highly educated, had higher household income, and were more likely to be White. Those newly employed have better benefits, wages, hours, and career mobility than in their home care jobs. The low status and poor pay of home care workers may result in the inability of the profession to retain those who face better prospects.

ABSTRACT

A partir del estudio de los accidentes del Grupo AMICA en los años 2006-2009 se determinó que ésta no era homogénea existiendo 2 puntos críticos: - AMICA: la movilización de personas con discapacidad en los centros de alojamiento, suponía más del 50% de los accidentes ocurridos. - SAEMA EMPLEO SL (Centro Especial de Empleo): aumento de la siniestralidad en un 50% entre los años 2006 a 2009. Frente a la reducción producida en el Grupo AMICA. Gerencia propuso desarrollar un plan de actuación específico para cada punto crítico, basado en la colaboración entre trabajadores y empresa, contando con el asesoramiento del ICSST. La aplicación práctica se llevó a cabo creando un grupo de trabajo en cada entidad compuesto por: - 2 delegados de prevención. - 2 técnicos del servicio de prevención. - 2 mandos intermedios. - Dirección técnica de AMICA / SAEMA EMPLEO SL. La dinámica de trabajo se estructuró realizando reuniones periódicas, en las que se analizaban los resultados de las medidas de mejora implantadas y se presentaban nuevas propuestas de actuación. Grupo de trabajo AMICA: a partir de los resultados de la evaluación ergonómica realizada, se decidió actuar sobre la formación de los trabajadores y mejorar las ayudas técnicas disponibles en los centros de trabajo. Grupo de trabajo SAEMA EMPLEO SL: se propuso mejorar la formación e información que reciben los trabajadores y revisar las condiciones de trabajo, actuando sobre las causas más frecuentes y las susceptibles de provocar los accidentes más graves (AU)


From the study of accidents of AMICA's Group in the years 2006 - 2009, it was determined that the accident rate was not homogeneous, there were two critical points: AMICA: moving and handling of disabled people in accommodation centers, was more than 50% of accidents. SAEMA EMPLEO S.L.: the accident rate increased by 50% between the years 2006-2009, against the reduction that took place in AMICA´s Group. Management proposed to develop a specific action plan for each critical point, based on the collaboration between workers and the company, with the advice of the ICASST. The practical application took place by creating a working party on each entity. They were composed of: 2 safety workers. 2 safety technicians of the Preventive Service. 2 senior managers. Technical manager of AMICA / SAEMA EMPLEO SL. Working dynamics was structured by holding regular meetings, where the results of the implemented improvement measures were analysed and new proposals for action were presented. AMICA working party: from the results of the ergonomic assessment, it was decided to act on the training of workers and to improve the technical aids available at the workplace. SAEMA EMPLEO SL working party: the proposal was to improve the training and information that workers receive and to review working conditions, acting on the most frequent accident causes and on the most likely to cause the most serious accidents (AU)
