Las unidades geriátricas de convalecencia tienen como objetivo la recuperaciónde funciones perdidas por una enfermedad . La eficacia deestas unidades y la necesidad de plazas hospitalarias son factores quehan contribuído a que los pacientes ingresados sean cada vez mascomplejos. La atención subaguda podrá realizarse con una mayordotación de personal y una revisión del sistema de financiación
The convalescent care geriatric units have as an objective the recoveryof the lost functions caused by a disease. The efficiency of these unitsand the need of hospital beds are factors which have contributed to thefact that patients will be every time more complex. The subacute carewill be able to be accomplished with a greater endowment of personneland a review of the financing system

El concepto de cuidados postagudos incluye toda la franja asistencialsituada entre el hospital de agudos y el domicilio (cuidados subagudos,intermedios, media estancia, convalecencia). Las unidades desubagudos son aquellas que atienden a pacientes en el periodopostagudo inmediato muy próximo a la atención hospitalaria.Los pacientes que requieren atención postaguda suelen ser de edadavanzada, que han superado la fase aguda de su enfermedad, que yaestán diagnósticados y que presentan discapacidad reciente. Dentro deestos, existen unos en los que la situación postaguda es inmediata ypresentan además inestabilidad clínica (sin riesgo vital) y todavíaprecisan tratamientos que requieren monitorización frecuente. Estosson los candidatos para las unidades de subagudos.Los cuidados postagudos no son una simple prolongación de la hospitalizaciónde agudos, sino que deben contribuir a mejorar los resultadosde salud, siendo necesario añadir a la atención subaguda, laposibilidad real de efectuar una valoración geriátrica. El espaciopostagudo cumple las condiciones ideales para ofrecer un programaeficaz de valoración geriátrica (intervención intensa 24 h al día, mastiempo para intervenir y pacientes con discapacidad reciente). Se proponeque en este entorno, la valoración geriátrica ha de ser completacon instrumentos que permitan evaluar todas las dimensiones y conprofesionales que lleven a cabo una intervención interdisciplinar real yeficaz
The concept of postacute care includes an extensive area between acutecare hospitals and home care (subacute care, intermediate care,transitional care convalescence). Subacute care units are those, inwhich patients are admitted, in the immediate postacute periode, closelyconnected with the acute care in the hospital.Postacute care is needed in patients with advanced age, in which acutefase of the illness has been overcome, the diagnostic process has beenfinished and functional decline in present. A subgroup of these patients,who are still in the immediate postacute periode, who are minimallymedically unstable (fluctuating conditions but not life-threatening) andwho still requires basic uncomplicated intermitent treatments, arecandidates for a subacute care unit.Postacute care is not a simply hospital acute care extension, but it hasto contribute to improve health outcomes in these patients. Acomprehensive geriatric assessment may be added to the subacutecare requirements.The postacute care environment has ideal conditions to offer an effectivegeriatric assessment program (stong intervention 24 h a day, moretime for intervention than in acute care hospital, patients with recentphysical decline). Geriatric assessment in the postacute care settingmust be complete, all dimensions must be evaluated with validatedinstruments and a real and effective interdisciplinary intervention mustbe performed
