ABSTRACT

BACKGROUND: Mesenteric lymphatic malformations (MLs) are a heterogeneous group of benign diseases of the lymphatic system that present with cystic dilated lymphatics of the mesentery. MLs are rare and represent less than 5% of all lymphatic malformations. The aims of this study were to analyze the characteristics of MLs in children and to suggest a modified classification. PATIENTS AND METHODS: We investigated 25 patients who underwent ML surgery. The clinical data and pathological findings were reviewed retrospectively. We divided the patients into 4 groups according to the operative findings. Group 1 included patients with MLs involving the intestinal walls. Group 2 included patients with pedicle-type MLs with no relationship to the mesenteric vessels. Group 3 patients presented with MLs located in the mesenteric boundaries near the mesenteric vessels. Group 4 patients had multicentric and diffusely infiltrated MLs. RESULT: The male-to-female ratio was 11:14, and the median age at diagnosis was 5years of age. The most common symptom was abdominal pain. The jejunal mesentery was the most frequently involved site in this study. Five patients showed the macrocystic type and 20 patients showed the mixed cystic type. With the exception of one patient with a large mixed cystic-type ML who underwent incomplete mass excision, 24 patients underwent complete mass excision. The group 1 patients (n=14) underwent mass excision performed with segmental resection of the bowel. The group 2 patients (n=3) only underwent mass excision surgery. The patients in group 3 (n=7) underwent mass excision with segmental resection of the intestine because ML excision altered the blood supply of the adjacent intestines. The group 4 patients (n=1) presented with MLs involving the entire mesentery and underwent incomplete excision. CONCLUSION: The relationships between MLs and the neighboring organs determine the surgical strategy, and the size and location of MLs affect the operative methods. The modified classification based on these findings can facilitate effective treatment planning.

ABSTRACT

Intra-abdominal lesions, such as mesenteric cysts, are uncommon disorders. Most are discovered incidentally during routine abdominal examinations. They generally do not show typical clinical findings. Laparoscopic surgery has been used to remove a wide variety of mesenteric cysts. We report a patient with a mesenteric cyst, who was treated by laparoscopic enucleation.

ABSTRACT

Presentamos un nuevo caso de seudoquiste no pancreático de origen mesentérico en una mujer de 39 años de edad sin antecedentes personales de interés que acudió a urgencias al sufrir un cuadro abdominal agudo. En la ecografía abdominal se objetivó la existencia de una lesión quística a nivel de mesogástrio, de pared gruesa, con contenido ecogénico y nivel líquido-líquido interno; en la TC realizada posteriormente se demostró que la pared se realzaba con el contraste y la lesión procedía del mesenterio; también existía gran cantidad de líquido peritoneal libre. La paciente se intervino urgentemente, extrayéndosele un quiste mesentérico perforado. El estudio anatomopatológico de la pieza dio como resultado el diagnóstico de seudoquiste mesentérico (AU)

ABSTRACT

OBJECTIVE: Mesenteric cysts are rare intra-abdominal masses generally omited or briefly reported in textbooks. Their rarity had fostered a lack of information and difficulty in classification. The goal of the study is to present a simple, comprehensive, and reproducible classification of mesenteric cysts. METHODS: A large review of the literature with particular attention to recent series was performed in order to identify and characterize the different groups of mesenteric cysts. RESULTS: Although cysts of lymphatic and mesothelial origin are those most frequently encountered, they were confounded or omitted in previous classifications. However, their distinction is important because their incidence and mode of presentation differ. Lymphangiomas predominate in male children, may cause acute abdominal pain, and frequently require resection of adjacent structures. In contrast, cysts of mesothelial origin such as benign cystic mesothelioma are seen more frequently in young and middle-aged women, usually present with more indolent symptoms, and often recurs. CONCLUSIONS: A new classification based essentially on histopathological features should include the 6 following groups: (1) cysts of lymphatic origin (simple lymphatic cyst and lymphangioma); (2) cysts of mesothelial origin (simple mesothelial cyst, benign cystic mesothelioma, and malignant cystic mesothelioma); (3) cysts of enteric origin (enteric cyst and enteric duplication cyst); (4) cysts of urogenital origin; (5) mature cystic teratoma (dermoid cysts), and (6) pseudocysts (infectious and traumatic cysts).

ABSTRACT

E descrito um caso mesenterico, em paciente do sexo masculino, cuja forma de apresentacao clinica se deu de forma aguda. O diagnostico foi realizado atraves de exame fisico, ultrassonografico e radiologico. O tratamento realizado foi a excisao completa do tumor. O paciente apos 12 anos encontra-se bem, sem sinais de recidiva.
