ABSTRACT

AIMS: To describe the experience of using high concentration nitrous oxide (N(2)O) relative analgesia administered by nursing staff in children undergoing minor procedures in the emergency department (ED) and to demonstrate its safety. METHOD: Data were collected over a 12 month period for all procedures in the ED performed under nurse administered N(2)O sedation. All children greater than 12 months of age requiring a minor procedure who had no contraindication to the use of N(2)O were considered for sedation by this method. The primary outcome measure was the incidence of a major complication namely respiratory distress or hypoxia during the procedure. Secondary outcome measures were minor complications and the maximum concentration of N(2)O used. RESULTS: Data were collected for a total of 224 episodes of nurse administered N(2)O sedation over a 12 month period. In 73.2% of children no complications were recorded. One major complication was recorded (respiratory distress) and the most common minor complication was mask intolerance in 17%. The mean maximum concentration of N(2)O used was 60.2%. CONCLUSIONS: N(2)O is a safe analgesic in children over the age of 1 year undergoing painful or stressful procedures in the ED. It may safely be administered in concentrations of up to 70% by nursing staff after appropriate training.

ABSTRACT

Objetivo. Demostrar que la administración de líquidos en el paciente postoperatorio de cirugía menor no digestiva puede efectuarse antes de las dos horas de haber finalizado la anestesia sin un incremento de las complicaciones. Material y métodos. Ochenta pacientes sometidos a cirugía menor fueron asignados aleatoriamente para recibir ingesta precoz (IP; antes de las dos horas de finalizada la anestesia) o ingesta tardía (IT; después de las dos horas de finalizada la anestesia).Resultados. Sólo tres pacientes del grupo IP y dos del grupo IT presentaron vómitos (p = NS). La intolerancia a la ingesta no se relacionó con la duración del ayuno ni con ningún factor estudiado (edad, sexo, duración de la anestesia u otros).Conclusiones. La ingesta precoz (antes de las 2 horas) postoperatoria es segura y recomendable como método rutinario de cuidados de enfermería en el postoperatorio de cirugía menor (AU)

ABSTRACT

This course is immensely popular with students. Many express a sense of accomplishment in knowing that they have performed a specific procedure at least once before facing the requirement in the clinical setting. Students who complete the course have a firm foundation in minor procedures and are knowledgeable about indications, contraindications, and methods to perform the procedure before facing that first patient in the clinical setting. They have also accumulated a set of procedures, patient discharge instructions, and a bibliography that can be reviewed before performing a procedure for the first time in the clinical setting. Combining clinical hands-on skills with the experience of writing a step-by-step procedure reinforces learning and is a valuable skill that can be used in clinical practice after graduation. Although many of the students will only apply a portion of the skills they learn in this course, they express a significant boost in self-confidence, a decrease in anxiety level, a sense of accomplishment in their skills, and a definite edge in securing a nurse practitioner position in a competitive healthcare marketplace.
