ABSTRACT

Las metrorragias en el tercer trimestre de la gestación que parecen un cuadro de placenta previa pueden corresponder a una llamada de atención del paciente hacia el médico. Esta situación clínica nace de alteraciones facticias que permiten al paciente adoptar el papel de enfermo. Ello puede conducir a una intervención quirúrgica inútil: cesárea por metrorragias, por ejemplo. La principal dificultad de la situación es diagnóstica. El tratamiento está basado en la psicoterapia, dirigida a mejorar la relación ulterior madre-hijo y puede prevenir actos de automutilación (AU)


Metrorrhagia during the third trimester of pregnancy, simulating placenta previa, may be a sign of the patient calling for help to the doctor. This clinical situation is one where the woman mimics an illness, in order to play the role of sick patient. The risk is that this may lead to unnecessary surgical procedures, in this case caesarean section. The main difficulty in caring for these patients is to make the diagnosis. Treatment is based on psychotherapy, which can contribute to an improved mother-infant relationship and perhaps prevent acts of self-mutilation (AU)

ABSTRACT

Analiza que el síndrome de Munchausen by proxy (SMBP) es una forma de maltrato infantil y describe a niños con síntomas producidos por los padres, los mismos que proporcionan una historia médica falsa, determinando ingresos hospitalarios múltiples, estudios diagnósticos, procedimientos invasivos, uso de fármacos innecesarios y en muchos casos la muerte. Se hace una revisión de la psicopatología, criterios diagnósticos y manejo. Además se presentan dos casos, registrados en un período de 7 años en el Hospital Baca Ortíz de Quito; concluyéndose que existe un subregistro de éste síndrome, y se recalca en la importancia de un mayor conocimiento de la enfermedad y de la participación del sistema médico en la génesis y mantenimiento de dicha entidad.

ABSTRACT

The differential diagnosis of chronic diarrhea is broad. Most of the illnesses causing diarrhea can be diagnosed through a carefully taken medical history, stool examination, few complementary laboratory tests as well as an endoscopy and, if needed, a radiological examination. However, a few causes could still remain undiagnosed and special diagnostic techniques must then be implemented. The diagnosis of self-induced diarrhea by concealed laxative abuse is a special challenge and should be considered in every case of diarrhea where the cause remains unclear. This paper reports such a case where the diagnosis of the habitual laxative abuse with Bisacodyl was made with the help of the pathologist on the basis of stepwise taken colonic biopsies. The most important histological criteria are a champagne glass-like ectasia of the deep curved crypts, combined with increased goblet cell content and increased mucus production in the presence of an intact surface-epithelium.

A case of morbid self-violence (Münchhausen Syndrome) leading to death and a case of criminal self-violence (cutaneous emphysema) are reported. The issue of such cases is discussed with regard to the consequences concerning judicial procedure of punishment and insurance.

ABSTRACT

This case study reports on a suicide made to look like murder. The case highlights the issue of false claims of criminal victimization and suggests the need for a reexamination of the dynamics of factitious allegations of criminal victimization. Factitious complaints of mental or physical illness are typically self-reported to psychiatric or medical authorities. This paper illustrates a different kind of phenomenon: one in which an injury or disorder is brought to medical attention by way of the police, who report the death or injury as arising from criminal victimization.
