ABSTRACT

A sin drome de jadassohn lewandowsky ou paquioniquia conjenita (PC) hereditaria autossomica dominante de ocorrencia rara com expressividade fenotipica altamente variavel. Caracteriza-se pela formação de quantidades excessivas de queratina nas unhas pele e mucosas, manifestada por hiperceratose ungueal simetrica hiperceratose palmoplantar e leucoceratose folicular. Apresenta-se o caso de um jovem portador dessa anomaliacom onicodistrofa generalizada de mâos e pes, hiperceratose palmoplantar dentes natis maculas no dorso comedões e cistos multiplos pelo corpo paquitriquia e placas alopecias ocipitais. Neste artigo citam-se aspectos geneticos compilam-se manifestações clinicas revisam-se propostas de classificação descreve-se o quadro histologico e aventam-se as possibilidades terapeiticas da PC.

ABSTRACT

The lunula is the visible portion of the distal nail matrix that extends beyond the proximal nailfold. It is white, half-moon-shaped, appears by week 14 of gestation, has unique histologic features. The lunula has a primary structural role in defining the free edge of the distal nail plate. Lunular anomalies include changes in form and structure and in color. Lunular dysmorphologic features can be characterized by macrolunula, microlunula or anolunula, and nonconvex lunula. Lunular dyschromias can be confluent or spotted or can be characterized by longitudinal colored bands that traverse the lunula. Alterations in the morphologic features or color (or both) of the lunula can be an indication of either a cutaneous or a systemic disorder.

ABSTRACT

Nail damage in 55 persons due to contamination by diluted paraquat in a group of 296 spray operators employed on a sugar estate in Trinidad is described. The commonest lesion seen was transverse white bands of discoloration, but loss of nail surface, transverse ridging, gross deformity of the nail plate, and loss of nails occurred. The index, middle, and ring fingers of the right hand were predominantly affected and this could be ascribed to leakage from the knapsack sprayer. It is emphasized that, although the degree of contamination was unusually gross, it is nonetheless important to recognize that diluted material can cause nail damage. Simple hygienic precautions and proper maintenance of spraying equipment can prevent its occurence. Periodic medical examinations are recommended even for workers exposed only to diluted paraquat. The distribution of the nail lesions confirms that they are the result of a local action. Following cessation of further exposure subsequent nail growth is normal. (AU)

ABSTRACT

An apparently healthy family with an incomplete type of leukonychia in IImembers is described. Both fingernails and toenail were affected, except in three members where only the thumbnails showed a marked leukonychia. The lunula covered the greater part of the affected nail was chalky white, and in between tlere was a narrow zone of normal red color. Microscopic examination of the whiter distal part of the nails revealed marked parakeratosis with an abundance of both basophilic nuclear elements and eosinophilic structures.The mechanism for the observed changesin nail color are discussed. the disorder seems to be inherited in a simple dominantfashion.
