ABSTRACT

Abstract: 14. The aim of this study is to determine the ethical experiences and the level of moral sensitivity and related factors of pediatric nurses working in hospital settings. Designed as descriptive and cross-sectional, the study was undertaken at three public hospitals in Konya and Ankara, Turkey and included the participation of a total of 200 pediatric nurses. The Moral Sensitivity Questionnaire (MSQ) was used for evaluation of the ethical sensitivities of the nurses. Of the 200 nurses, 46.5% had working experience of between 1-5 years, 58% were married, 18.6% had not undergone any ethics education, and 59.5% held a bachelor's degree. A large majority (95%) of the pediatric nurses reported that they had encountered ethical problems. The mean moral sensitivity score of the nurses was 95.89±24.34, with higher scores in this area being observed in the nurses who were in the older age group and had worked longer than others (p<0.05). The mean moral sensitivity score of the nurses was determined to be at a medium level and was found to be influenced by the age group they were in and the length of time they had worked.


Resumen: 18. El objetivo de este estudio consiste en determinar las experiencias éticas y el nivel de sensibilidad moral y factores relacionados de enfermeras pediatras que trabajan en hospitales. El diseño del estudio es descriptivo y transversal y se realizó en tres hospitales públicos en Konya y Ankara en Turquía, incluyendo la participación de un total de 200 enfermeras pediatras. Para evaluar la sensibilidad ética de las enfermeras se usó el Cuestionario sobre Sensibilidad Moral (CSM). De las 200 enfermeras, 46,5% tenía experiencia de trabajo entre uno y cinco años, 58% era casada, 18,6% no había recibido educación en ética y 59,5% era licenciada. Una gran mayoría (95%) de las enfermeras pediatras informó que había experimentado problemas éticos. El puntaje medio de sensibilidad moral fue de 95.89±24.34, con puntajes más altos para las enfermeras de mayor edad y que habían trabajado más tiempo (p<0.05). Se determinó que el puntaje medio de sensibilidad moral era de nivel medio y que estaba influenciado por el grupo etario y el tiempo de experiencia de trabajo.


Resumo: 22. O objetivo deste estudo é determinar as experiências éticas e o nível de sensibilidade moral e fatores relacionados da atuação da enfermagem pediátrica no ambiente hospitalar. Concebido como descritivo e transversal, o estudo foi realizado em três hospitais públicos em Konya e Ankara (Turquia) e contou com a participação de um total de 200 enfermeiros pediátricos. O questionário de sensibilidade Moral (MSQ) foi usado para avaliar a sensibilidade ética dos enfermeiros. Dos 200 enfermeiros, 46,5% tinham experiência profissional entre 1 a 5 anos, 58% eram casados, 18,6% não tinham qualquer instrução de estudos de ética e 59,5% tinham graduação. Uma grande maioria (95%) dos enfermeiros pediatras relataram que já encararam problemas éticos. A pontuação média acerca da sensibilidade moral dos enfermeiros foi de 95.89±24.34, com notas mais altas no grupo em que os enfermeiros estavam na faixa etária mais velha e tinham trabalhado mais do que outros (p < 0,05). A pontuação média de sensibilidade moral dos enfermeiros foi determinada em um nível médio e foi identificada pela influência da faixa etária e de tempo que tinham trabalhado.

ABSTRACT

OBJECTIVES: To measure the level of moral distress in PICU and neonatal ICU health practitioners, and to describe the relationship of moral distress with demographic factors, burnout, and uncertainty. DESIGN: Cross-sectional survey. SETTING: A large pediatric tertiary care center. SUBJECTS: Neonatal ICU and PICU health practitioners with at least 3 months of ICU experience. INTERVENTIONS: A 41-item questionnaire examining moral distress, burnout, and uncertainty. MEASUREMENTS AND MAIN RESULTS: The main outcome was moral distress measured with the Revised Moral Distress Scale. Secondary outcomes were frequency and intensity Revised Moral Distress Scale subscores, burnout measured with the Maslach Burnout Inventory depersonalization subscale, and uncertainty measured with questions adapted from Mishel's Parent Perception of Uncertainty Scale. Linear regression models were used to examine associations between participant characteristics and the measures of moral distress, burnout, and uncertainty. Two-hundred six analyzable surveys were returned. The median Revised Moral Distress Scale score was 96.5 (interquartile range, 69-133), and 58% of respondents reported significant work-related moral distress. Revised Moral Distress Scale items involving end-of-life care and communication scored highest. Moral distress was positively associated with burnout (r = 0.27; p < 0.001) and uncertainty (r = 0.04; p = 0.008) and inversely associated with perceived hospital supportiveness (r = 0.18; p < 0.001). Nurses reported higher moral distress intensity than physicians (Revised Moral Distress Scale intensity subscores: 57.3 vs 44.7; p = 0.002). In nurses only, moral distress was positively associated with increasing years of ICU experience (p = 0.02) and uncertainty about whether their care was of benefit (r = 0.11; p < 0.001) and inversely associated with uncertainty about a child's prognosis (r = 0.03; p = 0.03). CONCLUSIONS: In this single-center, cross-sectional study, we found that moral distress is present in PICU and neonatal ICU health practitioners and is correlated with burnout, uncertainty, and feeling unsupported.

ABSTRACT

PURPOSE: The purpose of this study was to explore the perceptions of child maltreatment among inpatient pediatric nurses. DESIGN AND METHODS: A cross-sectional survey was used to obtain responses to an online survey designed to examine perceptions of child maltreatment from inpatient pediatric nurses. RESULTS: Many nurses surveyed (41.25%) indicated that they had not received adequate training or had never received training on child maltreatment identification and many (40%) also indicated they were not familiar with the applicable reporting laws. CONCLUSIONS: Due to the serious immediate and long term effects of child maltreatment, it is imperative that pediatric inpatient nurses have adequate training on how to identify potential abuse and neglect cases, as well as legal reporting requirements, since they are in a unique position to identify potential cases of maltreatment. PRACTICE IMPLICATIONS: There is a continuing need for training on child maltreatment identification and reporting laws for inpatient pediatric nurses.

La globalización es un fenómeno reciente, que avalado por adelantos científicos técnicos interrelaciona todos los países del mundo, esta interacción en esferas: económicas, políticas, socioculturales ha creado una cultura global que en muchos casos ha resquebrajado el sistema de valores de cada país. Nuestro sistema de valores respaldado y formado por años de historia y lucha, ha creado un pueblo identificado por sus ansias de solidaridad e internacionalismo manifiesto, a pesar de todo, la crisis económica mundial ha incidido directamente en la formación de valores en los profesionales de la salud de nuestro país sobre todo el personal de enfermería y específicamente los enfermeros de cuidados progresivos. Se diseñó un estudio longitudinal y prospectivo en el Hospital Pediátrico General Docente Eliseo N. Caamaño, con el objetivo de cualificar y reforzar el conjunto de valores específicos que se consideran ideales para un enfermero de nuestros tiempos. El universo de estudio se constituyó por la totalidad de los enfermeros físicos(112), se definió una muestra de estudio de forma aleatoria conformada por 60 enfermeros, y una pequeña representación comparativa de familiares acompañantes.(30) Creamos un panel de apoyo compuesto por varios especialistas que tipificaron de forma práctica los valores morales específicos ideales para nuestros enfermeros, de esta manera conformamos un cuestionario con las variables objetos de estudio que se aplicó en dos tiempos, antes y después de impartir un taller didáctico sobre el tema que nos ocupa, los datos obtenidos en ambos tiempos se tabularon y compararon demostrando la eficacia de nuestro proyecto para reforzar valores morales específicos y necesarios para nuestro personal de enfermería (AU).


Globalization is a recent phenomenon interrelating all the countries around the world supported by scientific and technical advances. This interrelation in the economic, political and socio-cultural spheres has created a global culture that has crashed the values´ system of every country. Cuba is not out of the reach of these globalization´s negative effects. Our values´ system, formed in years of history and struggles, has created a sui generis people identified by its freedom eagerness, solidarity and internationalism. The world economic crisis has directly struck on the health professionals´ values formation in our country, especially in the case of the Nursing staff and mostly in progressive care nurses. This motivated the design of a prospective, longitudinal study in the Teaching Pediatric Hospital ¨Eliseo N. Caamaño¨ to qualify and strengthen the whole of specific values considered ideal for a nurse. The study universe were the totality of the hospital´s nurses, and the sample was randomly formed by 60 nurses and a little comparative representation of the family companions. It was created a panel of specialists who typified the ideal moral values specific for nurses. That way a questionnaire was elaborated with the variables that were object of study, to be applied before and after carrying out a didactic workshop on the theme. The obtained data were tabulated and compared, showing the efficacy of the project to reinforce moral values that are necessary and specific in the nursing staff (AU).

In reality, it falls rather short of that and seems, at times, to be lengthy, repetitive, rather obscure, bland and agree to a low benchmark of performance.
