ABSTRACT

The aim of our article is to highlight the history of pain management. The multidisciplinary team (MDT) concept in confronting pain was first conceptualized by the Hippocratics, and has evolved through time and become a trend in medicine over recent decades. Documentary research was conducted to unveil the story of the evolution of MDTs. From the early 1950's the idea of an MDT approach to deal with various types of pain was sporadically introduced in medicine. Studies encouraged health institutions to support this concept by providing health professionals with training, alongside the necessary facilities and resources. Specialized care programs started with Dame Cicely Mary Strode Saunders as one of the pioneers. CONCLUSIONS: Team work and continuous interdisciplinary treatment of pain have rendered MDTs essential for health systems. Barriers in flexibility, information flow and personal issues give rise to the need for better organization and training. Pain and terminal disease palliation call for MDTs, and educated leaders to run them. Present and future health MDTs are considered necessary in all medical fields.

ABSTRACT

Los cuidados paliativos (CP) evolucionaron a partir de un movimiento contracultural en consonancia con otros movimientossociales de los años 60. Cicley Saunders puso en marcha una revolución silenciosa: un modelo científico, compasivo y eficientede “Cuidado total”1. En el siglo xxi el acceso a los CP es un derecho aún no tan accesible, ni antes ni durante la pandemiapor COVID-19. Se espera que aumente la necesidad de CP aproximadamente un 87 % en las próximas 4 décadas, afectandode forma desproporcionada a las personas que viven en la pobreza y a los mayores de 70 años, entre otros grupos2. En elmundo, cerca del 85 % de las personas no tienen garantizado ese derecho3. Cada uno/a de nosotro/as es parte interesada en este problema. (AU)

ABSTRACT

The discipline of palliative medicine in Canada started in 1975 with the coining of the term "palliative care." Shortly thereafter, the provision of clinical palliative medicine services started, although the education of the discipline lagged behind. In 1993, the Canadian Society of Palliative Care Physicians (CSPCP) started to explore the option of creating an accredited training program in palliative medicine. This article outlines the process by which, over the course of 20 years, palliative medicine training in Canada went from a mission statement of the CSPCP, to a 1 year of added competence jointly accredited by both the Royal College of Physicians and Surgeons of Canada (Royal College) and the College of Family Physicians of Canada, to a 2-year subspecialty of the Royal College with access from multiple entry routes and a formalized accrediting examination.
