ABSTRACT

Objetivo: Mapear a produção do conhecimento sobre os principais cuidados de enfermagem realizados aos recém-nascidos submetidos a fototerapia em unidades neonatais. Método: Trata-se de um protocolo de revisão de escopo, realizado de acordo com a metodologia do Joanna Briggs Institute e checklist do Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviews (PRISMA-ScR), seguindo as seguintes etapas: seleção da questão de pesquisa; busca por estudos relevantes; seleção dos estudos; extração e análise dos dados; e, agrupamento, resumo e apresentação dos resultados. Para identificar os documentos as seguintes bases de dados bibliográficas serão pesquisadas: Medical Literature Analysis and Retrievel System Online, Cumulative Index to Nursing & Allied Health Literature, Web of Science, SciVerse Scopus, Base de Dados de Enfermagem, Literatura Latino-Americano e do Caribe em Ciências da Saúde, Scientific Electronic Library Online, Cochrane Library, Catálogo de Teses e Dissertações da Capes e Google Acadêmico. O resultado do fluxo de seleção, desta etapa metodológica, será apresentado em forma de figura, conforme o Preferred Reporting Items for Systematic Reviews and Meta-Analyses. Resultados: O mapeamento dos dados permitirá o agrupamento dos cuidados além de evidenciar a necessidade destes para os recém-nascidos submetidos à fototerapia. Conclusão: Espera-se salientar a indispensabilidade dos cuidados de enfermagem a este público e reforçar a necessidade de educação continuada aos profissionais.


Objetivo: Mapear la producción de conocimiento sobre los principales cuidados de enfermería brindados a las criaturas recién nacidas sometidas a fototerapia en unidades neonatales. Revisión: Se trata de un protocolo de revisión del alcance realizado, según la metodología del Instituto Joanna Briggs y la lista de verificación Elementos de informe preferidos para revisiones sistemáticas y extensión de metaanálisis para revisiones de alcance (PRISMA-ScR). Lo anterior, siguiendo los siguientes pasos: selección de la pregunta de investigación, buscar estudios relevantes, selección de estudios, extracción y análisis de datos y, agrupar, resumir y presentar los resultados. Para identificar los documentos, se buscará literatura en las siguientes bases de datos: Sistema de recuperación y análisis de literatura médica en línea, Índice acumulativo de enfermería y Literatura relacionada con la salud, Web of Science, SciVerse Scopus, base de datos de Enfermería, Literatura Latinoamericana y del Caribe en Ciencias de la Salud, Scientific Electronic Library Online, Cochrane Library, Capes Theses and Dissertations Catalog y Google Scholar. El resultado del flujo de selección de este paso metodológico se presentará en forma de figura, de acuerdo con PRISMA-ScR. Resultados: El mapeo de datos permitirá la agrupación de cuidados, además, resaltar la necesidad de estos para las criaturas recién nacidas sometidas a fototerapia. Conclusión: Se espera resaltar la indispensabilidad del cuidado de enfermería para este público y reforzar la necesidad de educación continua para las personas profesionales en esta área.


Objective: To map the production of knowledge on the main nursing care provided to newborns undergoing phototherapy in neonatal units. Method: This is a scoping review protocol carried out following the Joanna Briggs Institute methodology and the Preferred checklist Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviews (PRISMA-ScR). The following the following steps were followd: selection of the research question, search for relevant studies, selection of studies, data extraction and analysis, and, grouping, summarizing and presenting the results. The following databases will be consulted to identify the documents: Medical Literature Analysis and Retrievel System Online, Cumulative Index to Nursing & Allied Health Literature, Web of Science, SciVerse Scopus, Database of Nursing, Latin American and Caribbean Literature in Health Sciences, Scientific Electronic Library Online, Cochrane Library, Capes Theses and Dissertations Catalog, and Google Scholar. The result of this methodological step's selection flow will be presented in figure form as per the Preferred Reporting Items for Systematic Reviews and Meta-Analyses. Results: The data mapping will allow the grouping of care and it will also highlight the need for care in newborns undergoing phototherapy. Conclusion: It is expected to highlight the indispensableness of nursing care for this public and reinforce the need for continuing education in professionals.

ABSTRACT

Objetivo: Verificar o conhecimento da equipe de enfermagem relacionado ao recém-nascido em fototerapia. Métodos: Trata-se de uma pesquisa transversal, do tipo descritiva e exploratória e com abordagem quantitativa. Os dados foram coletados no período de agosto a outubro de 2018. Foi aplicado um questionário sobre o conhecimento dos cuidados com o recém-nascido em fototerapia para os profissionais de enfermagem. Os dados obtidos foram organizados e analisados estatisticamente. Resultados: Responderam ao questionário 22 profissionais de enfermagem. Quando perguntado sobre quais os cuidados são necessários ao RN em fototerapia foram significativamente recordados pelos grupos: utilizar proteção ocular; monitorização da temperatura; manter aleitamento materno; retirar proteção ocular durante amamentação; realizar mudança de decúbito e aferição de peso diária. Questionados sobre as possíveis complicações e efeitos colaterais ao RN em fototerapia foram significativamente mais recordados pelos grupos: desidratação; aumento do número de evacuações; queimaduras e possível lesão de retina e efeitos negativos no relacionamento mãe-recém-nascido. Conclusão: Ficou evidenciado que mesmo com o bom conhecimento dos profissionais de enfermagem, treinamentos contínuos devem ser realizados para tornar ainda mais eficaz a assistência prestada ao recém-nascido. ABSTRACT

Objetivos: analisar a percepção materna sobre fototerapia e descrever a avaliação das mães sobre uma ferramenta educativa na mesma temática. Método: estudo descritivo, com abordagem qualitativa. Participaram 30 mães, depois de firmado o Termo de Consentimento Livre e Esclarecido. Realizaram-se as entrevistas, em seguida, submetidas à Técnica de Análise de Conteúdo na modalidade Análise Temática. Após coletados os dados, identificou-se a necessidade de um material educativo, o qual foi elaborado e avaliado. Resultados: percebeu-se conhecimento limitado em relação ao tratamento. Quanto ao material educativo, houve a satisfação e sua recomendação por 100% das mães. Conclusão: as mães não se sentem seguras com as informações que recebem dos profissionais de saúde, o que compromete a sua confiança e sua contribuição diante da terapêutica. Verificou-se elevada relevância do uso de uma ferramenta educativa que atenda às necessidades reais das mães.

ABSTRACT

Objetivo: conhecer a prática dos profissionais de enfermagem quanto à realização dos cuidados oculares ao recém-nascido exposto à fototerapia. Método: estudo descritivo, tipo censo, com aborgagem quantitativa, no de agosto a outubro de 2014, por meio da aplicação de formulário, com 97 profissionais de enfermagem que assiste RN nos setores de UCIN, UTIN e ALCON de dois hospitais-escola públicos. Os dados foram inseridos em planilhas de Excel. Em seguida, foram formatados em uma figura e em tabelas, calculadas as frequências relativas e absolutas das variáveis para as análises descritiva e analítica. Resultados: a maioria dos profissionais demonstrou conhecer parcialmente os cuidados oculares investigados. Ainda, eles relatam a improvisação da proteção ocular, a não verificação do posicionamento como rotina, a não retirada do protetor ocular nos momentos pertinentes e a não realização da higiene ocular. Conclusão: necessita-se de maior atenção da equipe de enfermagem quanto aos cuidados oculares, pois o estudo sugere a possibilidade de que os RN em fototerapia estão sendo expostos a riscos oculares passíveis de prevenção.(AU)


Objective: to know the practice of nursing professionals pertaining the eye care provided to newborns exposed to phototherapy. Method: descriptive study, census-based, with quantitative approach and carried out from August to October 2014 through application of a form to 97 nursing professionals that make up the staff who assist NB in the sectors of NICU, NITCU and ROOM of two public teaching hospitals. Data were entered into Excel spreadsheets, sorted by professional category and sector and then formatted into a figure and tables that served to calculate the absolute and relative frequencies of variables for descriptive and analytical analysis. Results: most professionals showed to have partial know on the investigated eye care. It was also evident that they make reference to the improvisation of eye protector, not checking its position as routine, non-removal of the eye protector in relevant moments and non-realisation of eye hygiene. Conclusion: more attention from the nursing staff regarding eye care is needed, as the study suggests the possibility that the NB undergoing phototherapy is exposed to risks to the eye that could be prevented.(AU)

Resultados: la mayoría de los profesionales demostraron conocer parcialmente los cuidados oculares investigados. Aún, se evidenciaron que ellos relatan improvisación de la protección ocular, la no verificación del posicionamiento como rutina, la no retirada del protector ocular en los momentos pertinentes y la no realización de la higiene ocular. Conclusión: se necesita una mayor atención del equipo de enfermería cuanto a los cuidados oculares, pues el estudo sugiere la posibilidad de que los RN en fototerapia están siendo expuestos a riesgos oculares pasibles de prevención.(AU)

ABSTRACT

Phototherapy is the use of visible light for the treatment of hyperbilirubinemia in the newborn. This relatively common therapy lowers the serum bilirubin level by transforming bilirubin into water-soluble isomers that can be eliminated without conjugation in the liver. The dose of phototherapy is a key factor in how quickly it works; dose in turn is determined by the wavelength of the light, the intensity of the light (irradiance), the distance between the light and the baby, and the body surface area exposed to the light. Commercially available phototherapy systems include those that deliver light via fluorescent bulbs, halogen quartz lamps, light-emitting diodes, and fiberoptic mattresses. Proper nursing care enhances the effectiveness of phototherapy and minimizes complications. Caregiver responsibilities include ensuring effective irradiance delivery, maximizing skin exposure, providing eye protection and eye care, careful attention to thermoregulation, maintaining adequate hydration, promoting elimination, and supporting parent-infant interaction.

ABSTRACT

Hyperbilirubinemia is the most common condition requiring evaluation and treatment in newborns. The clinical manifestation of hyperbilirubinemia-jaundice-occurs in 60% of normal newborns and nearly all preterm infants. Compared with conditions that require advanced pharmacologic and technologic treatment strategies, hyperbilirubinemia seems to be overshadowed and may lose the attention it deserves as a condition that has potentially devastating effects. Nurses must be vigilant when caring for babies with "just jaundice" by monitoring bilirubin levels, identifying infants at risk for developing severe hyperbilirubinemia, and implementing prescribed treatment effectively when indicated.

ABSTRACT

The syndrome of seasonal affective disorder (SAD) is defined as a history of major depressive episodes that recur regularly at a particular time of year, typically fall or winter, and completely remit in the spring. It has been hypothesized that photoperiod-related changes in the duration of melatonin secretion may be involved in the seasonal mood cycles of SAD in human beings, suggesting that artificial bright lights (mimicking daylight) might be used to treat SAD. This article reviews studies on the use of bright light therapy and antidepressant medication for the treatment of SAD. Studies have found that bright light therapy and antidepressant medication are both effective for the treatment of SAD. Bright light therapy may also be effective for treating nonseasonal depression.

La vía clínica consiste en un documento escrito que refleja la secuencia de un proceso, basado en protocolos y recomendaciones. Permite mejorar la atención, disminuyendo la variabilidad en la práctica clínica; debe adaptarse al medio en que se aplica y a las nuevas evidencias científicas en que se apoya, permitiendo emprender acciones de mejora. ABSTRACT

Bright light therapy is a method of maintaining or restoring the natural circadian rhythm by assisting daytime awakening using bright lights. Postoperative delirium is one of the potential complications encountered by patients receiving postoperative care in the intensive care unit (ICU), but there have been no studies on the use of light for the prevention of postoperative delirium. The objective of this study was to examine whether the circadian rhythms of patients after surgery for oesophageal cancer can be adjusted and whether the postoperative delirium crisis rate can be reduced by bright light therapy. The subjects were 11 patients operated on for oesophageal cancer in Osaka University Hospital. After informed consent was obtained, they were divided into a study group and a control group by a random sampling method. After removal of the endotracheal tube, the study group was exposed to light. The light intensity was about 5000lx immediately before the eyes, and the distance from the light source was about 100 cm. The control group was placed in a natural lighting environment after extubation. In both groups, the rhythms of physical activities and autonomic activities were monitored after surgery, and delirium was evaluated. A significant difference was observed in the delirium score between the study group and control group on the morning of day 3 of bright light therapy by the Mann-Whitney U-test (P=0.014). The study group could begin ambulation about 2 days earlier than the control group. Bright light therapy may reduce the rate of postoperative delirium and make early ambulation possible. However, our study involved a very small sample size. We want to increase the sample in the future after having reviewed clinical application methods.

ABSTRACT

Neuropsychiatric behaviors are common in people with Alzheimer's disease (AD) and make both professional and lay caregiving difficult. Light therapy has been somewhat successful in ameliorating disruptive behaviors. This randomized trial tested the effects of morning or afternoon bright light exposure compared with usual indoor light on the presence, frequency, severity, and occupational disruptiveness of neuropsychiatric behaviors in nursing home residents with AD. Light was administered for 1 hr daily (Monday-Friday) for 10 weeks. The Neuropsychiatric Inventory-Nursing Home was used to assess behavior at baseline and end of the intervention. Analyses revealed statistically significant differences between groups on agitation/aggression, depression/dysphoria, aberrant motor behavior, and appetite/eating disorders. The magnitude of change was small and may not represent clinically significant findings. Agitation/aggression and nighttime behaviors commonly occurred and were highly correlated with occupational disruptiveness. Interventions that decrease the presence and/or severity of neuropsychiatric behaviors have the potential to significantly decrease caregiver burden.

Phototherapy may be used as a single therapy or as combination therapy with other treatments. Health care professionals providing phototherapy treatment are responsible for educating patients regarding the benefits and risks of phototherapy.

ABSTRACT

A fototerapia é o tratamento inicial da icterícia neonatal. Objetivamos conhecer a percepção da mãe acerca da fototerapia e identificar as suas dificuldades, relacionadas ao tratamento fototerápico. Estudo descritivo com abordagem qualitativa, realizado no Alojamento Conjunto (AC) de um hospital público em Fortaleza-CE, com 8 puérperas na faixa etária entre 13 e 25 anos. Os dados foram coletados em maio e junho/2006, mediante entrevista com duas questões de pesquisa: O que representa para a senhora ver o seu filho sob fototerapia; quais são as dificuldades enfrentadas em relação aos cuidados com o seu bebê na fototerapia? Na análise das falas, identificamos as categorias: percepção da mãe com relação ao cuidado com o bebê sob a fototerapia, dificuldades enfrentadas pela mãe com seu filho em fototerapia, e a mãe após as orientações recebidas. Concluímos que existe a necessidade de repensar as orientações e assumir autenticamente os cuidados de enfermagem ao binômio mãe-filho.
