ABSTRACT

UNLABELLED: The problem of the development of the new efficient methods for the treatment of the patients presenting with chronic bacterial vesiculitis (CBV) is currently considered among the important priorities. AIM: The objective of the present study was to provide a scientifically sound substantiation for the application of sinusoidal modulated currents (SMC), magnetic fields, and laser radiation in the combined treatment of the patients with CBV. PATIENTS AND METHODS: A total of 121 patients presenting with chronic bacterial vesiculitis were examined and treated during the latent phase of the inflammatory process. They were randomly divided into three groups. Group 1 (main) was comprised of 40 patients treated, in addition to basal pharmacotherapy, by supravascular contact laser irradiation of the cubital vein area followed after 2-3 hours by the application of sinusoidal modulated currents to the pubosacral region. Group 2 included 41patents given, besides basal pharmacotherapy, laser therapy in the same regimen as in group 1 supplemented after 2-3 hours by abdominal magnetic therapy. Group 3 (control) received traditional pharmacotherapy in the combination with antibacterial and anti-inflammatory medicines. RESULTS: It was demonstrated that the patients of group 2 exhibited the most pronounced positive dynamics of the clinical signs and symptoms estimated from the total National Institute of Health Chronic Prostatitis Symptom Index (NIH-CPSI) and of the characteristics of the quality of life evaluated based on the QLS scale. The patients of the two former groups experienced a more conspicuous decrease in the activity of the inflammatory process in seminal vesicles, restoration of their structural and anatomical features (as shown by the transrectal ultrasound study), improvement of testosterone metabolism, and normalization of the spermogram characteristics in comparison with controls. The analysis of the spermograms revealed the tendency toward the increase in the number of actively motile spermatozoa only in the patients of group 2. The combined treatment of the patients of the two former groups resulted in the decrease of the level of sex hormone-binding globulin and the increase of the free androgen index. CONCLUSION: The results of the study indicate that the application of the preformed physical factors for the treatment of the patients presenting with chronic bacterial vesiculitis enhances the effectiveness of pharmacotherapy and decreases both the frequency and the duration of relapses of the disease.

ABSTRACT

The article summarizes the results of the clinical trials on application of likoprofit in patients with a chronic prostatitis and prostate adenoma, which were conducted by the Russian urologists for the last 8 years. Application of likoprofit in patients after TURP contributes to significantly earlier and effective restoration of microcirculation, which decreases the risk of development of postoperative complications and accelerates rehabilitation of patients. Studies in which likoprofit was applied in patients with a chronic prostatitis and prostate adenoma, proved that likoprofit also has antiedematous effect, improves the urination act, improves ejaculate parameters, positively impacts on sexual function, and has a high safety profile.

ABSTRACT

BACKGROUND AND PURPOSE: Chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS) negatively affects quality of life and sexual function in men of all ages. Typical treatment with antibiotic and antimicrobial drugs often is not successful. The purpose of this case report is to describe a multimodal physical therapy intervention that included manual therapy techniques applied to the pelvic floor in 2 patients who were unsuccessfully treated with the biomedical model of prescription drug therapies. CASE DESCRIPTION: Two men, aged 45 years and 53 years and diagnosed with chronic prostatitis, were referred for physical therapy following unsuccessful pharmacological treatment. The patients were treated with manual therapy techniques applied to the pelvic floor and instructed in progressive muscle relaxation, flexibility exercises, and aerobic exercises. OUTCOMES: Changes in the patients' National Institutes of Health Chronic Prostatitis Symptom Index revealed differences between preintervention and postintervention scores reflecting decreased pain and improved quality of life. One patient improved from a score of 25 (total possible score = 43) before treatment to a score of 0 after treatment, and the other patient improved from a score of 29 to a score of 21. DISCUSSION: Manual therapy techniques applied to the pelvic floor and performed by a physical therapist specially trained in these techniques, along with progressive muscle relaxation, flexibility exercises, and aerobic exercises, appeared to be beneficial to both patients in reducing pain and improving sexual function.

ABSTRACT

Patients with chronic bacterial prostatitis complicated by erectile dysfunction received combined rehabilitative treatment based on the use of various combinations of physiobalneotherapeutic factors, such as ultrasound, local negative pressure, white and yellow turpentine or sodium chloride baths, supplemented by basal medicamentous therapy in the form of rectal suppositories. Efficiency of therapy involving sodium chloride, white and yellow turpentine baths was estimated at 85, 60, and 75% respectively.

ABSTRACT

Rhythmic patterns in the functioning of different systems of the body should be taken into consideration when developing new therapeutic modalities intended for the improvement of efficiency of health care measures. The study of chronobiological features of the work of organ systems in patients with different diseases residing in mid-latitude regions provided a rationale for the differential approach to the prescription of medicinal preparations of natural origin in selected periods of the annual cycle.

ABSTRACT

A total of 75 males from married couples with circulatory prostatitis aged 19-47 were examined and treated in Barnaulsky sanatorium. The patients were trained by walking with the load power 80-110 W for 2.5 to 3 hours. They also received phytocorrection with biologically active food additive Fitopan M. The treatment led to attenuation of pelvic symptoms, stabilization of morning erection, normalization of friction period and laboratory indices. The effect persisted for 3 years.

ABSTRACT

A total of 135 patients with chronic prostatitis have undergone rehabilitation in Belokurikha health resort. Of them, 35 patients of group 1 received basic therapy (nitrosiliceous baths, therapeutic exercise, massage of the prostate, psychotherapy); 40 patients of group 2 received basic therapy plus rectal tampons with sulphide mud from the lake Gorkoye of the Altai Administrative Area; 60 patients of group 3 were given basic therapy plus rectal lavage of the prostate location with nitrosiliceous thermal water of Belokurikha health resort. Treatment efficacy was assessed by scales of the symptoms and sexual disorders, ejaculate tests, testosterone levels, immune status, results of Doppler vascular mapping of the prostate. The highest effect was achieved in group 3 (87.3% response). This therapy relieves inflammation, stimulates a sexual function, improves hemodynamics in the prostate, raises the number and motility of spermatozoa, corrects immune disorders, normalizes testosterone concentration in the blood. In group 2 the response was obtained in 81.6% patients. This treatment improves hemodynamics, raises the number of spermatozoa. In group 1 the effect was observed in 60.4% patients, it manifested with an insignificant improvement in prostatic hemodynamics, spermatogenesis, hormonal and immune systems.

ABSTRACT

A combined treatment is proposed for patients with chronic prostatitis attended by reflex syndromes of lumbar osteochondrosis. The treatment consists in postisometric relaxation, massage of the prostate and transrectal mobilization of the pelvic ligaments. The results achieved in 106 patients proved that the proposed combined treatment is effective in compound pathology of the pelvic region.

ABSTRACT

Combined aftercare with mud applications, sinusoidal modulated currents (SMC) and UV radiation of autoblood was compared to conventional drug therapy in 56 and 30 patients, respectively, having chronic prostatitis with reproductive dysfunction. It was found that the combined aftercare raises the treatment efficiency due to a favourable effect on the immune system, inflammation in the prostate and spermatogenesis. It also promotes long-term remission.

ABSTRACT

103 patients with chronic prostatitis complicated by erectile impotence were given combined treatment including shock-wave massage, mud applications, local vacuum magnetotherapy. This combination was found to stimulate copulative function, urodynamics of the lower urinary tracts, to produce an antiinflammatory effect. These benefits allow to recommend the above physical factors for management of chronic prostatitis patients with copulative dysfunction.

ABSTRACT

Antibacterial antibiotics only, antibiotics in combination with immunostimulators, antibiotics plus immunostimulators plus physiotherapy were administered in 132 patients with urethroprostatitis caused by mixed infection (18, 45 and 69 patients, respectively). A complete recovery was achieved only in patients who had been treated with the antibiotics, immunostimulators and physiotherapy. After antibiotics, residual infection was registered in 11.1% of the patients, clinical symptoms persisted in 6 patients; after antibiotics combined with immunostimulators in 6.7% and 4 patients, respectively.

ABSTRACT

105 patients with chronic nonspecific prostatitis were examined and treated with papaverin electrophoresis using sinusoidal modulated currents (SMC) and local vacuum magnetotherapy (LVMT). Papaverin SMC electrophoresis and LVMT stimulated cavernous circulation. The highest stimulation was achieved at successive use of LVMT and the electrophoresis. LVMT followed by the electrophoresis maintained good cavernous circulation for 5-6 hours after the procedure in the course of which several spontaneous erections were observed.

ABSTRACT

Prostatic rheography findings evidence for marked disturbances of venous outflow in patients with chronic prostatitis. To correct the hemodynamics, rectal magnitotherapy was used in two regimens: with constant frequency of the magnetic field (5000 Hz) and fluctuating magnetic field (1-20000 Hz). The comparison of the results gave priority to the latter regimen because it provided reduction of the treatment duration and normalization of rheographic parameters in 37 (90.2%) patients. The authors recommend rectal magnitotherapy for combined treatment of chronic prostatitis.

ABSTRACT

In this paper, a therapeutic clinical series-based plan is submitted of a combined pathogenetically substantiated treatment of patients with chronic prostatitis. Sine-modulated flows were utilized as were centimetric waves, phonophoresis with methyluracil ointment to prostate according to the authors' own methods, as well as laser therapy. Positive results were obtained. Significant improvement of the consistency of the gland as well as indices of spermatogramme and microscopy of the prostate juice were recordable in 75%) of the patients. In summary, the proposed therapeutic alternative for patients with chronic prostatitis can be recommended for use along with other treatment options.

ABSTRACT

Health resort treatment with the purpose of rehabilitation was carried out in 129 patients with chronic prostatitis at health resorts of Lvov Province. Results of the treatment: recovery was noted in 105, improvement--in 19 patients, failures--in 5 patients. The author suggests that it is more rational to treat patients with chronic prostatitis in three stages; stage I--out patient or hospital treatment; stage II--health resort treatment; stage III--treatment at home. Such three-stage treatment of patients with chronic prostatitis increases the recovery rate, prolongs the remission time, reduces the number of readmissions, decrease the time of working capacity loss.
