ABSTRACT

BACKGROUND: Pulsatile drug delivery of actives based on the body's biological rhythms came into sight as a novel and emerging concept in the field of drug delivery. The concept of late has given birth to another field of research worth exploring: multiple-pulse drug delivery. OBJECTIVE: Delivering a drug in multiple pulses has been applied to antibiotics for effective and patient compliant drug delivery. Delivering antibiotics in divided pulses results in better annihilation of microbes, as it prevents them going into a resistant/dormant stage and developing biological tolerance. The concept appears to have potential, and on 16 March 2009 MiddleBrook Pharmaceuticals, Inc. will launch the first of such once-daily product based on their proprietary pulsatile drug delivery technology, PULSYS. METHODS: This review focuses on the rationale, possible strategies and technologies employed for multiple-pulse delivery, as well as current status and future trends. CONCLUSION: The concept is in its infancy and promises great potential in the fight against microbial resistance; many approved formulations based on similar approaches with new and improved therapeutic paradigms are anticipated in the near future.

ABSTRACT

La radiodermitis crónica tras el tratamiento con radioterapia es una secuela frecuente que puede resultar preocupante para el paciente. En la literatura especializada se han publicado pocos casos en los que se han utilizado el láser de colorante pulsado en el tratamiento de telangiectasias aparecidas tras radioterapia por cáncer de mama. Presentamos una paciente con radiodermitis en el cuello tras radioterapia por carcinoma de cavum. La paciente recibió 5 sesiones de láser de colorante pulsado (pulso de 3 ms, diámetro de impacto de 7 mm y energía entre 7 y 12 J/cm 2). Se realizaron estudios histológicos secuenciales. La respuesta al tratamiento fue muy buena con una desaparición casi completa de las lesiones. La paciente quedó muy satisfecha del resultado. El láser de colorante pulsado es un tratamiento seguro y eficaz de la radiodermitis crónica del cuello tras radioterapia


Chronic radiodermatitis after radiotherapy is a frequent sequela that may be worrying for the patient. Few cases have been published in the literature in which pulsed-dye laser has been used in the treatment of telangiectasias that appeared after radiotherapy for breast cancer. We present a female patient with radiodermatitis on the neck after radiotherapy for nasopharyngeal carcinoma. The patient received five sessions of pulsed-dye laser treatment (3 ms pulse duration, 7 mm spot size, fluence between 7 and 12 J/cm 2). Sequential histological studies were performed. The response to treatment was very good, with the lesions almost completely disappearing. The patient was very satisfied with the result. Pulsed-dye laser is a safe and effective treatment for chronic radiodermatitis of the neck after radiotherapy

ABSTRACT

A pilot study has been undertaken to compare the efficacy of small dose pulsed betamethasone therapy with need based oral steroids in chronic recurrent erythema nodosum leprosum (ENL) patients. Though this mode of therapy was well tolerated, no advantage with intermettnt steroid administration was observed. This coul have been on account of small dose of steroid given monthly. Treatment of chronic recurrent erythema nodosum leprosum (ENL) patients continues to be unsatisfactory, particularly, because of non-availability of thalidomide. Though corticosteroids are effective in suppressing all the manifestations and even restoring partially or fully the functional impairment, their side effects and dependence are equally troublesome. Based on (a) the reported efficacy and safety of intermittent use of corticosteroids in several immune complex mediated disorders (Cathcart et al 1976, Kimberly et al 1979), Lieblin et al 1981 and Pasricah & Gupta 1984) and (b) ENL (type II) reactions having similar pathology, a pilot study has been undertaken to see the efficacy and the tolerance of pulsed steroids in chronic ENL patients

ABSTRACT

This review provides both a biological and clinical perspective on Time-Dependent Sensitization (TDS), an ancient amplified memory response to threat manifest in the ability of both drugs and nondrug stressors to induce neuronal and behavioral effects which strengthen entirely as a function of the passage of time following even a single or acute exposure. Evidence is presented to show that TDS may be involved in the development of a spectrum of diseases and how drug regimens based on the principles of TDS could provide a novel and revolutionary means of treating psychiatric and other illnesses.
