ABSTRACT

OBJECTIVE: To synthesize the available evidence on the prevalence and odds for anxiety and depression in adults with pyoderma gangrenosum (PG). DATA SOURCES: Observational studies examining anxiety and depression in adults with PG were systematically searched using the MEDLINE, EMBASE, PsycINFO, and CINAHL databases from the inception of each database to March 11, 2020. STUDY SELECTION: Two authors independently screened references based on predetermined eligibility criteria. DATA EXTRACTION: Of the 244 articles identified, three met the eligibility criteria. Relevant data were extracted from included studies, and methodological quality was evaluated independently by two authors using the modified Newcastle-Ottawa Scale. DATA SYNTHESIS: Three observational studies comprising 183 participants with PG met the inclusion criteria. Estimated rates of depression in adults with PG ranged from 10% to 23%. None of the studies measured rates of anxiety. CONCLUSIONS: The current systematic review suggests that depression is a common psychological comorbidity in adults with PG. Additional research is required to further assess the psychological comorbidities in this population.

ABSTRACT

BACKGROUND: Pyoderma gangrenosum (PG) and hidradenitis suppurativa (HS) are painful and unsightly dermatologic disorders that have the potential to significantly decrease patients' quality of life (QOL). Due to the difficulties associated with curing these conditions, QOL improvement is imperative. OBJECTIVE: To determine if there are PG and HS disease-specific QOL measures and to assess whether QOL is being systematically studied in clinic populations or clinical trials. METHODS: A PubMed search was performed to identify QOL studies for patients with PG and HS. Sources from other papers were also utilized. RESULTS: QOL is rarely qualitatively or quantitatively assessed in HS and PG studies. We identified no validated disease-specific QOL measures for either PG or HS. CONCLUSION: Development of disease-specific QOL indices, clinical use of QOL measurement tool, and formal QOL evaluation in clinical trials would help to define the impact of PG and HS treatment in patients' lives.

ABSTRACT

Successful treatment of pyoderma gangrenosum depends on prompt diagnosis. Here, a patient describes how an inaccurate diagnosis resulted in severe pain and delayed treatment, with a markedly negative effect on quality of life.

El síndrome de Munchausen conjuga manifestaciones multisistémicas, fingidas y autoprovocadas, en cuyo contexto la piel tiene participación preponderante como órgano de expresión de conflictos y emociones. Severas perturbaciones psicológicas, entre ellas la compulsión por mentir, labilidad de lazos afectivos, soledad y marginación son la base conductual de estos pacientes. Se hace una revisión de la evolución histórica del síndrome, sus características y el rol de las dermatitis artefactas en su presentación clínica, con punto de partida en la vida aventurera de su inspirador, el Barón Karl von Müschhausen. Asimismo se sugiere un tratamiento interdisciplinario, basado en el trabajo conjunto de dermatólogos y psiquiatras, para mejorar el pronóstico generalmente pobre y en ocasiones incierto de los afectados por esta condición
