ABSTRACT

BACKGROUND: Early identification of ST-elevated myocardial infarction (STEMI) on electrocardiograms (ECGs) is vital to patient outcomes. Therefore, nurses need to have the knowledge to quickly recognize this lethal dysrhythmia. METHOD: The purpose of this pretest-posttest study was to increase nursing's knowledge on accurate and timely identification of an STEMI using an online educational intervention delivered via social media. RESULTS: This study included a convenience sample of 31 RNs employed in an urban hospital in the Midwest. The mean score on the pretest was 7.53 of 10 (p = .028), which improved to 9.11 of 10 (t = 6.273; p = .000) on the posttest. CONCLUSION: The use of social media to reach adult learners may be an effective means of delivering continuing education. Additional work is needed to further explore the use of educating bedside nurses on the importance of using the 12-lead ECG as a screening tool. [J Contin Educ Nurs. 2019;50(10):475-480.].

ABSTRACT

A doença cardíaca isquémica é a causa mais comum de morte e a sua frequência está a aumentar (Ibanez et al., 2017). Na Europa, é responsável por cerca de 1,8 milhões de mortes ao ano (Townsend et al., 2016). No ano de 2014 registaramse 4619 mortes, isto é, morre uma pessoa a cada duas horas em Portugal devido a Enfarte Agudo Miocárdio (Instituto Nacional de Estatística, 2016). A existência de uma equipa multidisciplinar, tem um papel fundamental na abordagem e no tratamento da pessoa com Enfarte Agudo Miocárdio com elevação do segmento ST (Peterman & Bisgaard, 2010), sendo que os enfermeiros são os profissionais na primeira linha de contacto com essas pessoas, e assim podem influenciar de uma forma decisiva o tempo que decorre até à terapêutica de reperfusão (Underwood et al., 2009; Martin et al., 2014). O referencial teórico que emerge neste relatório é o Nursing as Caring (Boykin & Schoenhofer, 2013) que descreve o cuidado de enfermagem como autêntico e intencional que cresce momento a momento. O crescimento do cuidar no tempo e em cada momento, tem por base uma noção de continuidade no tempo e na relação, e portanto, na evolução da situação clínica destes doentes que envolve todo um processo de abordagem e tratamento tempo-dependente. Este relatório pretende descrever o percurso de estágio que decorreu, numa Unidade de Cuidados Coronários e num Serviço de Urgência, e que teve como foco de atenção a intervenção especializada de enfermagem à pessoa com Enfarte Agudo do Miocárdio com elevação do segmento de ST. Neste percurso formativo destaco a reflexão contínua sobres as atividades realizadas e o desenvolvimento de competências, nomeadamente, ao nível do cuidar da pessoa a vivenciar processos complexos de doença crítica e/ou falência orgânica, assim como a seleção de fontes de informação relevantes para a tomada de decisão, que contribuíram para o desenvolvimento de uma intervenção precisa, eficiente, em tempo útil, eficaz e certamente mais holística à pessoa com Enfarte Agudo do Miocárdio com elevação do segmento ST.


Ischemic heart disease is the most common cause of death and its frequency is increasing (Ibanez et al., 2017). In Europe, it accounts about 1.8 million deaths a year (Townsend et al., 2016). In 2014, there were 4619 deaths, in other words, one person died every two hours in Portugal due to acute myocardial infarction (Instituto Nacional de Estatística, 2016). The existence of a multidisciplinary team plays a fundamental role in the approach and treatment of the STEMI patient (Peterman & Bisgaard, 2010), and nurses are the professionals in the first line of contact with the STEMI patient, and thus can influence the time elapsed until reperfusion therapy (Underwood et al., 2009, Martin et al., 2014). The theoretical framework that emerges in this report is Nursing as Caring (Boykin & Schoenhofer, 2013), which describes nursing care as authentic and intentional, which grows from moment to moment. The concept of continuity in time and relationship and therefore, on the evolution of the clinical situation of this patients that involves a whole process of approach and time-dependent treatment supports the growth of care in time and in each moment. The growth of care in time and in each moment, is based on a notion of continuity in time and relationship, and therefore, on the evolution of the health status of the person with STEMI that involves a whole process of approach and time-dependent treatment. In this training course I give emphasis to the continuous reflection on the activities carried out, the development of competencies, such as, at the level of caring for the person experiencing complex processes of critical illness and/or organic failure, as well as the selection of sources of information relevant to the decision-making, which have contributed to the development of a precise, efficient, timely, effective and certainly more holistic intervention for STEMI patients.
