ABSTRACT
DE LOS DATOS QUE ANTECEDEN, CABE EXTRAER LAS SIGUIENTES CONCLUCIONES: EN LA QUIMIOTERAPIA DE CORTA DURACION, LA RMP CONSTITUYE UN ANTIBIOTICO ESENCIAL EN LA FASE INICIAL Y EN LA FASE DE CONTINUACION. LA CONTRIBU CION DE LA INH AUN NO ESTA BIEN DETERMINADA. SIN EMBARGO, CUANDO NO SE ADMINISTRA ESTE ANTIBIOTICO (O ES INACTIVO), COMO EN EL REGIMEN CON RMP Y EMB, LOS RESULTADOS SON RELATIVAMENTE DEFICIENTES. ES PROBABLE QUE LA EM Y LA PZA CONTRIBUYAN AL EXITO DE LA QUIMIOTERAPIA DE CORTA DURACION COMO SUPLEMENTOS INICIALES O, EN EL CASO DE OTROS TRATAMIENTOS, CUANDO NO SE PUEDA UTILIZAR RMP. EN EL CASO DE PACIENTES CON TUBERCULOSIS BAC TERIOLOGICAMENTE ACTIVA LA DURACION TOTAL DE UN TRATAMIENTO DE CORTA DU RACION, PARA QUE TENGA UNA EFICAIA DEL 100 por ciento, NO PUEDE SER INFERIOR A SEIS MESES. LA DURACION OPTIMA ES DE SEIS A NUEVE MESES. UNA FASE INTEN SIVA INICIAL DIARIA PARECE TAN INDISPENSABLE PARA EL EXITO DEFINITIVO DE LA QUIMIOTERAPIA DE CORTA DURACION COMO LO ES PARA LA QUIMIOTERAPIA CON VENCIONAL

La estreptomicina es un potente elemento antibacteriano que puede administrarse por vía intramuscular o subcutánea o inyectarse directamente en el espacio subaracnoideo, o la cavidad pleural o peritoneal, absorbiéndose muy poca cantidad por el aparato gastrointestinal. La vía intramuscular es la de elección
Después de la inyección intramuscular la estreptomicina es excretada en la orina y bilis. Es necesario mantener una concentración de la droga en la sangre y tejidos que sea por lo menos 4 a 8 veces mayor que la necesaria para inhibir los microbios in vitro. Es necesario repetir las inyecciones cada 3 a 4 horas
La dosis de estreptomicina depende del tipo de microbio infectante. Conviene introducir y mantener una cantidad suficiente en el sitio de la lesión para que la concentración sea 4 a 8 veces mayor que la necesaria para matar los organismos in vitro. En ciertas infecciones agudas pueden requerirse de 1 a 4 gm diarios por espacio de 5 a 14 días
En el tratamiento del empiema o meningitis conviene utilizar la estreptomicina localmente inyectándola directamente en la cavidad pleural o en el espacio subaracnoideo
La estreptomicina ha demostrado su eficacia máxima en el tratamiento de la tularemia, las infecciones por Hemophilus influenzae, las infecciones del aparato urinario debidas a bacilos gram-negativos y las bacteriemias y meningitis debidas a bacilos gram-negativos
