Abstract Objective This study aimed to investigate the effects of the presence of subchorionic hematoma (SH) in early pregnancies with threatened miscarriage (TM) on levels of first-trimester maternal serum markers, pregnancy-associated plasma protein-A (PAPP-A), and free β-human chorionic gonadotropin (β-hCG) levels. Methods The data of TM cases with SH in the first trimester between 2015 and 2021 were evaluated retrospectively. The data of age and gestational age-matched TM cases without SH were also assessed to constitute a control group. Demographic characteristics, obstetric histories, ultrasonographic findings, and free β-hCG and PAPP-A levels of the groups were compared. Results There were 119 cases in the study group and 153 cases in the control group. The median vertical and longitudinal lengths of the SH were 31 mm and 16 mm. The median age of both groups was similar (p=0.422). The MoM value of PAPP-A was 0.088 (.93) in the study group and 0.9 (0.63) in the control group (p=0.519). Similarly, the MoM value of free β-hCG was 1.04 (0.78) in the study group and 0.99 (0.86) in the control group (p=0.66). No significant relationship was found in the multivariate analysis between free β-hCG MoM, PAPP-A MoM, age, gravida, and vertical and longitudinal lengths of the hematoma (p>0.05). Conclusion The level of PAPP-A and free β-hCG were not affected by the SH. Therefore, these markers can be used reliably in TM cases with SH for the first-trimester fetal aneuploidy screening test.

Abstract Objective The availability of reliable and inexpensive markers that can be used to determine the risk of rupture during methotrexate (MTX) treatment in ectopic pregnancies (EPs) is considerable. The aim of the present study is to investigate the role of systemic inflammatory markers such as leukocytes (or white blood cells, WBCs), the neutrophil-to-lymphocyte ratio (NLR), and platelet distribution width (PDW), which are among the parameters of the complete blood count (CBC), in the prediction of rupture of EPs under MTX treatment. Materials and Methods A total of 161 patients with tubal EP who underwent a single-dose methotrexate (MTX) protocol were retrospectively analyzed, and the control group (n = 83) included patients cured by MTX, while the ruptured group (n = 78) included patients who were operated on for tubal rupture during the MTX treatment. The features of EP, beta-human chorionic gonadotropin (β-hCG) levels, sonographic findings, and CBC-derived markers such as WBC, NLR, and PDW, were investigated by comparing both groups. Results The NLR was found to be higher in the ruptured group, of 2.92 ± 0.86%, and significantly lower in the control group, of 2.09 ± 0.6%. Similarly, the PDW was higher (51 ± 9%) in the ruptured group, and it was significantly lower a (47 ± 13%) in the control group (p < 0.05). Other CBC parameters were similar in both groups (p > 0.05). Conclusion Systemic inflammation markers derived from CBC can be easily applied to predict the risk of tubal rupture in Eps, since the CBC is an inexpensive and easy-to-apply test, which is first requested from each patient during hospitalization.


Resumo Objetivo A disponibilidade de marcadores confiáveis e baratos que podem ser usados para determinar o risco de ruptura durante o tratamento com metotrexato (MTX) em gestações ectópicas (GEs) é considerável. O objetivo do presente estudo é investigar o papel de marcadores inflamatórios sistêmicos, como leucócitos (ou glóbulos brancos, glóbulos brancos), a relação neutrófilo-linfócito (NLR) e largura de distribuição de plaquetas (PDW), que estão entre os parâmetros do hemograma completo (hemograma), na predição de ruptura de PEs sob tratamento com MTX. Materiais e Métodos Foram analisados retrospectivamente 161 pacientes com EP tubária submetidas a protocolo de dose única de metotrexato (MTX), sendo que o grupo controle (n = 83) incluiu pacientes curadas com MTX, enquanto o grupo roto (n = 78) incluíram pacientes operadas por ruptura tubária durante o tratamento com MTX. As características de EP, beta-gonadotrofina coriônica humana (β-hCG), achados ultrassonográficos e marcadores derivados de CBC, como WBC, NLR e PDW, foram investigados comparando os dois grupos. Resultados A RNL foi maior no grupo roto, de 2,92 ± 0,86%, e significativamente menor no grupo controle, de 2,09 ± 0,6%. Da mesma forma, o PDW foi maior (51 ± 9%) no grupo roto, e foi significativamente menor a (47 ± 13%) no grupo controle (p < 0,05). Outros parâmetros do hemograma foram semelhantes em ambos os grupos (p > 0,05). Conclusão Marcadores inflamatórios sistêmicos derivados do hemograma podem ser facilmente aplicados para predizer o risco de ruptura tubária na Eps, uma vez que o hemograma é um exame de baixo custo e fácil aplicação, solicitado primeiramente a cada paciente durante a internação.

ABSTRACT Background: Multiple sclerosis (MS) is an inflammatory and neurodegenerative autoimmune chronic neurological disease. Currently, there are no effective serum biomarkers to verify MS diagnosis, to assess disease prognosis, and evaluate response to MS treatment. Objective: The present study is a preliminary assessment of irisin and nesfatin-1 serum levels in patients with relapsing- remitting MS (RRMS). Methods: A total of 86 participants, 42 patients with RRMS diagnosis and 44 healthy controls were included in the study. The serum irisin and nesfatin-1 parameters of the patients and control group members were analyzed. Results: Irisin and nesfatin-1 levels of the RRMS patients were significantly lower than the controls (z: -3.82, p<0.001; z: -4.79, p<0.001, respectively) The cut-off level of irisin is 10.390 (ng/mL) (sensitivity: 84.1%, specificity: 71.4%, AUC: 0.800), and the cut-off level of nestatin-1 is 7.155 (ng/mL) (sensitivity: 68.2%, specificity: 64.3%, AUC: 0.739) in the ROC analysis. For these cut-off levels in the case-control groups, the lower irisin and nesfatin-1 levels are the independent variables for MS patients (OR 9.723, 95%CI 2.884-32.785, p<0.001; OR 3.992, 95%CI 1.336-11.928, p<0.001) respectively. Conclusion: The present study revealed lower irisin and nesfatin-1 levels in patients with RRMS. These findings suggest that the decreased levels of irisin and nesfatin-1 peptides may contribute to MS pathogenesis such as inflammation, oxidative stress, and apoptosis in MS, leading to demyelination, axonal damage with neuronal loss, and gliosis.


RESUMO Antecedentes: A esclerose múltipla (EM) é uma doença neurológica crônica autoimune inflamatória e neurodegenerativa. Atualmente, não há biomarcadores séricos eficazes para verificar o diagnóstico de EM, para avaliar o prognóstico da doença e avaliar a resposta ao tratamento de EM. Objetivo: O presente estudo é uma avaliação preliminar dos níveis séricos de irisina e nesfatina-1 em pacientes com EM recorrente-remitente (EMRR). Métodos: Um total de 86 participantes, 42 pacientes com diagnóstico de EMRR e 44 controles saudáveis, foram incluídos no estudo. Os parâmetros séricos de irisina e nesfatina-1 dos pacientes e membros do grupo controle foram analisados. Resultados: Os níveis de irisina e nesfatina-1 dos pacientes com EMRR foram significativamente mais baixos do que os dos controles (z: -3,82, p <0,001; z: -4,79, p <0,001, respectivamente). O nível de corte de irisina é 10,390 (ng/mL) (sensibilidade: 84,1%, especificidade: 71,4%, AUC: 0,800), e o nível de corte de nestatina-1 é 7,155 (ng/mL) (sensibilidade: 68,2%, especificidade: 64,3%, AUC: 0,739) na análise ROC. Para esses níveis de corte nos grupos de caso-controle, os níveis mais baixos de irisina e nesfatina-1 são as variáveis independentes para pacientes com EM (OR 9,723, IC95% 2,884-32,785, p <0,001; OR 3,992, IC95% 1,336-11,928, p <0,001) respectivamente. Conclusão: O presente estudo revelou níveis mais baixos de irisina e nesfatina-1 em pacientes com EMRR. Esses achados sugerem que os níveis diminuídos de peptídeos irisina e nesfatina-1 podem contribuir para a patogênese da EM, como inflamação, estresse oxidativo e apoptose na EM, levando à desmielinização, dano axonal com perda neuronal e gliose.

Abstract Background and objectives: Sedative and analgesic treatment administered to critically ill patients need to be regularly assessed to ensure that previously stated goals are well achieved as the risk of complications of oversedation is minimized. We revised and prospectively tested the Ramsay Sedation scale (RSS) for interrater reliability and compared it with the Sedation-Agitation Scale (SAS) and the Richmond Agitation Sedation Scale (RASS) to test construct validity during midazolam-remifentanil sedation. Methods: A convenience sample of ICU patients was simultaneously and independently examined by pairs of trained evaluators by using the revised SAS, RSS, and RASS. Ninety-two ICU patients were examined a total of 276 times by evaluator pairs. Results: The mean patient age was 61.32 ± 18.68 years, 45,7% were female (n = 42), 54.3% male (n = 50). Their APACHE values varied between 3 and 39 with an average of 13.27 ± 7.86 and 75% of the cases were under mechanical ventilation. When classified by using RSS (2.70 ± 1.28), 10.9% were anxious or agitated (RSS1), 68.5% were calm (RSS 2-3), and 20.6% were sedated (RSS 4-6). When classified by using RASS (-0.64 ± 1.58), 20.7% were anxious or agitated (RASS+1 to +4), 63.0% were calm (RASS 0 to -2), and 16.3% were sedated (RASS -3 to -5). When classified by using SAS (2.63 ± 1.00), 12% were anxious or agitated (SAS 5-7), 57.6% were calm (SAS 4), and 30.4% were sedated (SAS 1-3). RSS was correlated with the SAS (r = -0.656, p < 0.001) and RASS was correlated with the SAS (r = 0.565, p < 0.001). RSS was highly correlated with the RASS (r = -0.664, p < 0.001). Conclusions: Ramsay is both reliable and valid (high correlation with the RASS and SAS scales) in assessing agitation and sedation in adult ICU patients.


Resumo Justificativa e objetivos: O tratamento de pacientes em estado crítico com sedativos e analgésicos deve ser regularmente avaliado para garantir que as metas pré-definidas sejam atingidas, bem como minimizar o risco de complicações resultantes de sedação em excesso. Conduzimos uma revisão e testamos prospectivamente a Escala de Sedação de Ramsay (Ramsay Sedation Scale [RSS]) para a confiabilidade interavaliador e a comparamos com a Escala de Sedação e Agitação de Riker (Riker Sedation-Agitation Scale [RRSAS]) e a Escala de Sedação e Agitação de Richmond (Richmond Agitation Sedation Scale [RASS]) para testar a validade de construto durante a sedação com midazolam-remifentanil. Métodos: Uma amostra de conveniência de pacientes de UTI foi simultânea e independentemente examinada por pares de avaliadores treinados com o uso das escalas revisadas RRSAS, RSS e RASS. Foram examinados 92 pacientes de UTI por pares de avaliadores em 276 momentos. Resultados: A média dos pacientes foi de 61,32 ± 18,68 anos; 45,7% eram do sexo feminino (n = 42) e 54,3% do masculino (n = 50). Seus escores APACHE variaram entre 3-39, com média de 13,27 ± 7,86, e 75% dos casos receberam ventilação mecânica. Quando RSS foi usada para a classificação (2,70 ± 1,28), 10,9% dos pacientes estavam ansiosos ou agitados (RSS1), 68,5% estavam calmos (RSS 2 a 3) e 20,6% estavam sedados (RSS 4 a 6). Quando RASS foi usada para a classificação (-0,64 ± 1,58), 20,7% dos pacientes estavam ansiosos ou agitados (RASS +1 a +4), 63,0% estavam calmos (RASS 0 a -2) e 16,3% estavam sedados (RASS -3 a -5). Quando RSAS foi usada para a classificação (2,63 ± 1,00), 12% dos pacientes estavam ansiosos ou agitados (RSAS 5 a 7), 57,6% estavam calmos (RSAS 4) e 30,4% estavam sedados (RSAS 1 a 3). Houve correlação de RSS com RSAS (r = -0,656, p < 0,001) e de RASS com RSAS (r = 0,565, p < 0,001). Houve forte correlação de RSS com RASS (r = -0,664, p < 0,001). Conclusões: A RSS é confiável e válida (forte correlação com RASS e RSAS) para avaliar a sedação e agitação em pacientes adultos internados em UTI.

This study was performed to investigate the effects of crude protein (CP) in diet and sex ratio on egg production, egg and hatching chick weight and hatchability traits in pheasants (Phasianus Colchicus). In this study, the treatment groups were constructed as 15% CP-5:1; 15% CP-7:1; 18% CP-5:1; 18% CP-7:1 for CP and sex (male:female) ratio respectively. The eggs were obtained from 48 weeks old pheasant. In this study, it was determined that there was positive effective on egg production, egg and hatching weight and hatchability of fertile eggs (HFE) for 18% CP. In contrast, fertility (FR) (p 0.05) and hatchability rate of total eggs (HR) (p>0.05), EPEM (p>0.05) and MPEM (p>0.05) tended to decrease with increasing CP. It was found that there was egg production, egg (p>0.05) and hatching weight (p>0.05), FR (p>0.05), HR (p>0.05) and HFE (p>0.05) increased with increasing female for sex ratio. In terms of embryonic mortality, the EPEM (p>0.05) and LPEM (p>0.05) rate decreased with increasing female. For protein levels × sex ratio, it was found that hatchability traits were adversely affected for 15% CP-1:7. Also, it was determined that the lowest EPEM, MPEM and LPEM were determined for 7:1 in 18% CP group. Based on these results, to improve hatchability traits and a lower embryonic mortality rate, it may be beneficial to use 15% CP-5:1 or 18% CP-7:1 for pheasant breeding.
