RESUMEN

Tuberculosis (TB) treatment is highly effective, but response to therapy can vary by geography, race, and ethnicity. We assessed for differences in TB treatment response in a representative and heterogeneous Brazilian population. We estimated genetic ancestry proportion according to major ancestry groups (African, European, and Amerindian ancestry) in the Regional Prospective Observational Research in Tuberculosis (RePORT)-Brazil cohort. RePORT-Brazil is an observational prospective cohort study of individuals with newly-diagnosed, culture-confirmed, pulmonary TB. TB treatment outcomes that were attributed to TB treatment included Grade 2 or higher adverse drug reaction (ADR), Grade 3 or higher ADR, hepatic ADR, and failure/recurrence. Ancestry proportion was the main predictor in logistic regression for each outcome, with adjustments for candidate confounders. There were 941 pulmonary TB patients included in this study. We observed a decreased risk of Grade 2+ ADR when African ancestry proportion increased by 10% (Odds Ratio [OR] 0.41, 95% Confidence Interval [CI] 0.20 -0.85) and an increased risk for Grade 2+ ADR with increasing European ancestry (OR 2.84, 95% CI 1.47 - 5.48). We then performed the same analysis adding HIV status as an interaction term. The decreased risk for Grade 2+ ADR seen for African ancestry proportion did not hold for persons living with HIV; we observed increased risk for Grade 2+ ADR with increasing African ancestry proportion. There were no associations with Amerindian ancestry or for other treatment outcomes. In this Brazilian TB cohort, toxicity risk was associated with African and European ancestry, divergent, and affected by HIV. #RePORT-Brazil Consortium members include: Aline Benjamin and Flavia M. Background: Since 2014, Brazil has gradually implemented the Xpert MTB/RIF (Xpert) test to enhance early tuberculosis (TB) and drug-resistant (DR-TB) detection and control, yet its nationwide impact remains underexplored. Our study conducts an intervention time-series analysis (ITSA) to evaluate how the Xpert's implementation has improved TB and DR-TB detection nationwide. Methods: 1,061,776 cases from Brazil's National TB Registry (2011-2022) were reviewed and ITSA (2011-2019) was used to gauge the impact of the Xpert's adoption on TB and DR-TB notification. Granger Causality and dynamic regression modelling determined if incorporating Xpert testing as an external regressor enhanced forecasting accuracy for Brazil's future TB trends. Findings: Xpert implementation resulted in a 9.7% increase in TB notification and substantial improvements in DR-TB (63.6%) and drug-susceptible TB (92.1%) detection compared to expected notifications if it had not been implemented. Xpert testing counts also presented a time-dependent relationship with DR-TB detection post-implementation, and improved predictions in forecasting models, which depicted a potential increase in TB and DR-TB detection in the next six years. Interpretation: This study underscores the critical role of Xpert's adoption in boosting TB and DR-TB detection in Brazil, reinforcing the case for its widespread use in disease control. Improvements in prediction accuracy resulting from integrating Xpert data are crucial for allocating resources and reducing the incidence of TB. By acknowledging Xpert's role in both disease control and improving predictions, we advocate for its expanded use and further research into advanced molecular diagnostics for effective TB and DR-TB control. The study evoluated an in-house Spike Receptor Binding Domain Enzyme-Linked Immunosorbent Assay (RBD-IgG-ELISA) for detecting SARS-CoV-2 IgG antibodies in infected and vaccinated individuals. The assay demonstrated a sensitivity of 91%, specificity of 99.25%, and accuracy of 95.13%. Precision and reproducibility were highly consistent. The RBD-IgG-ELISA was able to detect 96.25% of Polymerase chain reaction (PCR) confirmed cases for SARS-CoV-2 infection, demonstrating positive and negative predictive values of 99,18% and 91,69%, respectively. In an epidemiological survey, ELISA, lateral flow immunochromatographic assay (LFIA), and electrochemiluminescence immunoassay (ECLIA) exhibited diagnostic sensitivities of 68.29%, 63.41%, and 70.73%, respectively, along with specificities of 82.93%, 80.49%, and 80.49%, respectively. Agreement between RBD-IgG-ELISA/PCR was moderate (k index 0.512). However, good agreement between different assays (RBD-IgG-ELISA/LFIA k index 0.875, RBD-IgG-ELISA/ECLIA k index 0.901). Test performance on individuals' samples were inferior due to seroconversion time and chronicity. The IgG-RBD-ELISA assay demonstrated its effectiveness in monitoring antibody levels among healthcare professionals, revealing significant differences both before and after the administration of the third vaccine dose, with heightened protection levels observed following the third dose in five Coronavirus disease (COVID-19) vaccine regimens. In conclusion, the RBD-IgG-ELISA exhibits high reproducibility, specificity, and sensitivity, making it a suitable assay validated for serosurveillance and for obtaining information about COVID-19 infections or vaccinations.

BACKGROUND: Tuberculosis (TB) treatment-related adverse drug reactions (TB-ADRs) can negatively affect adherence and treatment success rates. METHODS: We developed prediction models for TB-ADRs, considering participants with drug-susceptible pulmonary TB who initiated standard TB therapy. TB-ADRs were determined by the physician attending the participant, assessing causality to TB drugs, the affected organ system, and grade. Potential baseline predictors of TB-ADR included concomitant medication (CM) use, human immunodeficiency virus (HIV) status, glycated hemoglobin (HbA1c), age, body mass index (BMI), sex, substance use, and TB drug metabolism variables (NAT2 acetylator profiles). The models were developed through bootstrapped backward selection. Cox regression was used to evaluate TB-ADR risk. RESULTS: There were 156 TB-ADRs among 102 of the 945 (11%) participants included. Most TB-ADRs were hepatic (n = 82 [53%]), of moderate severity (grade 2; n = 121 [78%]), and occurred in NAT2 slow acetylators (n = 62 [61%]). The main prediction model included CM use, HbA1c, alcohol use, HIV seropositivity, BMI, and age, with robust performance (c-statistic = 0.79 [95% confidence interval {CI}, .74-.83) and fit (optimism-corrected slope and intercept of -0.09 and 0.94, respectively). An alternative model replacing BMI with NAT2 had similar performance. HIV seropositivity (hazard ratio [HR], 2.68 [95% CI, 1.75-4.09]) and CM use (HR, 5.26 [95% CI, 2.63-10.52]) increased TB-ADR risk. CONCLUSIONS: The models, with clinical variables and with NAT2, were highly predictive of TB-ADRs.

Resumo Objetivo Construir e validar um design de telessimulação síncrona e observacional em enfermagem para o atendimento segundo o protocolo de suporte básico de vida intra-hospitalar no adulto. Métodos Estudo metodológico baseado nas fases do procedimento teórico realizado por meio de uma scoping review; fase empírica desenvolvida através da validação de conteúdo do design telessimulado e fase analítica em que adotou-se o índice de validade de conteúdo. Resultados Construiu-se um design para o atendimento segundo o protocolo de suporte básico de vida percorrendo seis etapas: o planejamento, preparação, participação, teledebriefing, avaliação e feedback e a aprendizagem adicional, com Índice de Validade de Conteúdo de 0,96. Conclusão O design desenvolvido foi considerado válido em conteúdo para planejar e executar a telessimulação pela enfermagem e ainda ser adaptado a outros contextos educacionais.

Resultados Se elaboró un diseño para la atención de acuerdo con el protocolo de soporte vital básico que atravesó seis etapas: planificación, preparación, participación, teledebriefing, evaluación y feedback y aprendizaje adicional, con un Índice de Validez de Contenido de 0,96. Conclusión El diseño elaborado fue considerado válido en contenido para planificar y ejecutar la telesimulación por parte de enfermeros y además puede adaptarse a otros contextos educativos.


Abstract Objective To construct and validate a synchronous and observational telesimulation design in nursing for care according to in-hospital basic life support protocol for adults. Methods A methodological study based on the phases of the theoretical procedure carried out through a scoping review; empirical phase developed through the telesimulated design content validity and analytical phase in which the content validity index was adopted. Results A care design was constructed according to the basic life support protocol, going through six steps: planning, preparation, participation, teledebriefing, assessment and feedback and additional learning, with a Content Validity Index of 0.96. Conclusion The developed design was considered valid in content to plan and execute telesimulation by nursing and still be adapted to other educational contexts.

Introdução: A disbiose intestinal parece desempenhar um papel importante em diversas doenças imunomediadas, havendo um interesse crescente em compreender a influência do microbioma intestinal na doença psoriásica. Objetivos: Estudar o microbioma intestinal em pacientes com doença psoriásica grave em comparação com indivíduos sem psoríase. Em paralelo, avaliar as diferenças do microbioma intestinal de indivíduos portadores de psoríase, com e sem doença articular associada. Por fim, avaliar o microbioma dos indivíduos do grupo controle, diferenciando-os quanto a história positiva de psoríase em parentes de primeiro grau. Métodos: Estudo tipo caso controle, onde foi realizado sequenciamento do gene 16S rRNA V3/V4 e análises bioinformáticas com o DNA total extraído de amostras de fezes de 30 pacientes com doença psoriásica grave e 30 controles, pareados por idade e sexo e pertencentes à mesma localização geográfica. A classificação de gravidade respeitou os critérios do consenso, sendo considerados graves aqueles com superfície corporal acometida acima de 10% e/ou escore de PASI maior que 10 e/ou questionário DLQI maior que 10.Resultados: Foram estudados 60 indivíduos. Não se documentou diferença em alfa diversidade entre os grupos (p > 0.05). Entretanto, a análise da diversidade beta do microbioma intestinal mostrou diferentes agrupamentos entre os pacientes portadores de psoríase quando comparados aos controles (p = 0.031). A relação Firmicutes/Bacteriodetes (F/B) foi maior nos expostos (p = 0.05). A abundância diferencial ajustada mostrou aumento da expressão do gênero Sutterella (p < 0.01) e da espécie Sutterella wadsworthensis (p < 0.05) no grupo com psoríase. Quando comparados pacientes com e sem doença articular, não foi possível documentar diferenças em termos de alfa ou betadiversidade. Entretanto, pacientes com artrite psoriásica estabelecida apresentaram maior expressão do gênero Bacterioides (p = 0.02), além da espécie Bacteriodes uniformes (p = 0.03). Não foi identificada qualquer diferença relevante entre os controles, quando comparados indivíduos com e sem história familiar de psoríase. Conclusões: Foi possível demonstrar um microbioma intestinal diferente entre os portadores de doença psoriásica grave, quando comparados aos controles sem psoríase. Verificou-se uma maior expressão, significativa, do gênero Sutterella e da espécie Sutterella wadsworthensis entre os portadores de doença psoriásica, podendo representar uma marca da disbiose relacionada à esta doença. Em paralelo, verificouse diferenças estatísticamente siginificativas entre pacientes com doença articular, sendo, porém, uma marca nestes pacientes a maior expressão do gênero Bacterioides e da espécie Bacteriodes uniformes. Cabe destacar que a razão F/B foi inferior nos expostos. Achado não inédito, porém contrário a maior parte das publicações onde se demonstra predomínio do filo Firmicutes entre doentes. Do mesmo modo, redução de Akkermansia e Ruminoccocus, previamente relacionados à doença psoriásica, não foram verificados. Talvez por baixa leitura em ambos os grupos. Por fim, não houve diferença naqueles indivíduos com história familiar de psoríase em parentes de primeiro grau, em termos de microbioma intestinal. Este trabalho é um dos poucos estudos brasileiros e traz novas evidências sobre microbioma e psoríase. Consideramos que o microbioma merece atenção, especialmente porque traz diferentes oportunidades de intervenção, embora ainda existam alguns pontos a serem confirmados com estudos prospectivos.(AU)


Background: Gut dysbiosis may play a role in immune-mediated diseases. There is a growing interest in understanding microbiome influence in psoriasis. Objectives: To study the gut microbiome in patients with severe psoriatic disease, when compared with individuals without psoriasis. In parallel, to evaluate the differences in the gut microbiome of individuals with psoriasis, with and without psoriatic arthritis. Finally, to evaluate the microbiome of individuals in the control group, differentiating them based on a positive family history of psoriasis in firstdegree relatives. Methods: V3/V4 16S rRNA gene sequencing and bioinformatic analyses were performed with the total DNA extracted from the stool samples of 30 patients with severe plaque psoriasis and 30 age and gender-matched controls from the same geographic location. The severity classification respected the consensus criteria, with those with an affected body surface area above 10% and/or a PASI score greater than 10 and/or a DLQI questionnaire also greater than 10; being considered severe disease. Results: Sixty individuals were studied. No difference in alpha-diversity was documented between the groups (p > 0.05). Beta-diversity analysis showed different clustering of the gut microbiome in severe psoriasis when compared with controls (p = 0.031). Firmicutes/Bacteriodetes ratio was higher psoriasis (p = 0.05). Adjusted differential abundance showed an increased expression of Sutterella gender (p < 0.01) and Sutterella wadsworthensis species (p < 0.05) in psoriasis group. When comparing patients with and without joint disease, it was not possible to document differences in terms of alpha or beta diversity. However, patients with established psoriatic arthritis showed higher expression of the Bacterioides genus (p = 0.02), and Bacteriodes uniform species (p = 0.03). No relevant difference was identified between controls when comparing individuals with and without a family history of psoriasis. Conclusions: It was possible to demonstrate a different pattern of gut microbiome among those with severe psoriatic disease, when compared to controls. There was a significant greater expression of the genus Sutterella and Sutterella wadsworthensis species among patients, which may represent a sign of dysbiosis related to psoriasis. In parallel, there were no statistically significant differences in alpha and beta diversity between patients with joint disease when compared to those without psoriatic arthritis. However, a hallmark in these patients was the greater expression of the genus Bacterioides and the species Bacteriodes uniformes. It is worth noting that the F/B ratio was lower in psoriatic patients. This is not an unprecedented finding, but contrary to most publications that demonstrates the predominance of the Firmicutes phylum among patients. Likewise, reduction of Akkermansia and Ruminoccocus, previously related to psoriatic disease, were not verified in our sample. Perhaps due to low reading in both groups. Finally, there was no difference in those individuals with a family history of psoriasis in first-degree relatives, in terms of gut microbiome. This paper is one of the few Brazilian studies and brings insights into microbiome and psoriasis. Microbiome deserves our attention, especially since it brings different opportunities for intervention, although there are still some key points to be confirmed with prospective studies.(AU)

The community adherence support group (CASG) was one of the first differentiated service delivery (DSD) models introduced in Mozambique. This study assessed the impact of this model on retention in care, loss to follow-up (LTFU), and viral suppression among antiretroviral therapy (ART)-treated adults in Mozambique. A retrospective cohort study included CASG-eligible adults enrolled between April 2012 and October 2017 at 123 health facilities in Zambézia Province. Propensity score matching (1:1 ratio) was used to allocate CASG members and those who never enrolled in a CASG. Logistic regressions were performed to estimate the impact of CASG membership on 6- and 12-month retention and viral load (VL) suppression. Cox proportional regression was used to model differences in LTFU. Data from 26,858 patients were included. The median age at CASG eligibility was 32 years and 75% were female, with 84% residing in rural areas. A total of 93% and 90% of CASG members were retained in care at 6 and 12 months, respectively, while 77% and 66% non-CASG members were retained during the same periods. The odds of being retained in care at 6 and 12 months were significantly higher among patients receiving ART through CASG support (adjusted odds ratio [aOR] = 4.19 [95% confidence interval; CI: 3.79-4.63], p < .001, and aOR = 4.43 [95% CI: 4.01-4.90], p < .001, respectively). Among 7,674 patients with available VL measurements, the odds of being virally suppressed were higher among CASG members (aOR = 1.14 [95% CI: 1.02-1.28], p < .001). Non-CASG members had a significantly higher likelihood of being LTFU (adjusted hazard ratio = 3.45 [95% CI: 3.20-3.73], p < .001). While Mozambique rapidly scales up multi-month drug dispensation as the preferred DSD model, this study emphasizes the continued importance of CASG as an efficacious DSD alternative, especially among patients residing in rural areas, where CASG acceptability is higher.

Background: Genetic polymorphisms have been associated with risk of anti-tuberculosis treatment toxicity. We characterized associations with adverse events and treatment failure/recurrence among adults treated for tuberculosis in Brazil. Methods: Participants were followed in Regional Prospective Observational Research in Tuberculosis (RePORT)-Brazil. We included persons with culture-confirmed drug-susceptible pulmonary tuberculosis who started treatment between 2015-2019, and who were evaluable for pharmacogenetics. Treatment included 2 months of isoniazid, rifampin or rifabutin, pyrazinamide, and ethambutol, then 4 months of isoniazid and rifampin or rifabutin, with 24 month follow-up. Analyses included 43 polymorphisms in 20 genes related to anti-tuberculosis drug hepatotoxicity or pharmacokinetics. Whole exome sequencing was done in a case-control toxicity subset. Results: Among 903 participants in multivariable genetic association analyses, NAT2 slow acetylator status was associated with increased risk of treatment-related grade 2 or greater adverse events, including hepatotoxicity. Treatment failure/recurrence was more likely among NAT2 rapid acetylators, but not statistically significant at the 5% level. A GSTM1 polymorphism (rs412543) was associated with increased risk of treatment-related adverse events, including hepatotoxicity. SLCO1B1 polymorphisms were associated with increased risk of treatment- related hepatoxicity and treatment failure/recurrence. Polymorphisms in NR1/2 were associated with decreased risk of adverse events and increased risk of failure/recurrence. In whole exome sequencing, hepatotoxicity was associated with a polymorphism in VTI1A , and the genes METTL17 and PRSS57 , but none achieved genome-wide significance. Conclusions: In a clinical cohort representing three regions of Brazil, NAT2 acetylator status was associated with risk for treatment-related adverse events. Additional significant polymorphisms merit investigation in larger study populations.

Conclusão: A escalamencionada demonstrou ser aplicável em pacientes adultos e idosos com alteração do nível de consciência, por apresentar confiabilidade interobservador, avaliar o padrão respiratório e reflexos do tronco encefálico e por prever desfechos desfavoráveis como o óbito.


Conclusion: We showed that this scale can predict the mortality of adult and older adult patients' with altered level of consciousness, offering interobserver reliability and assessing their respiratory pattern and brainstem reflexes

BACKGROUND: Successful tuberculosis (TB) treatment is necessary for disease control. The World Health Organization (WHO) has a target TB treatment success rate of ≥90%. We assessed whether the different types of unfavorable TB treatment outcome had different predictors. METHODS: Using data from Regional Prospective Observational Research for Tuberculosis-Brazil, we evaluated biological and behavioral factors associated with each component of unsuccessful TB outcomes, recently updated by WHO (death, loss to follow-up [LTFU], and treatment failure). We included culture-confirmed, drug-susceptible, pulmonary TB participants receiving standard treatment in 2015-2019. Multinomial logistic regression models with inverse probability weighting were used to evaluate the distinct determinants of each unsuccessful outcome. RESULTS: Of 915 participants included, 727 (79%) were successfully treated, 118 (13%) were LTFU, 44 (5%) had treatment failure, and 26 (3%) died. LTFU was associated with current drug-use (adjusted odds ratio [aOR] = 5.3; 95% confidence interval [CI], 3.0-9.4), current tobacco use (aOR = 2.9; 95% CI, 1.7-4.9), and being a person with HIV (PWH) (aOR = 2.0; 95% CI, 1.1-3.5). Treatment failure was associated with PWH (aOR = 2.7; 95% CI, 1.2-6.2) and having diabetes (aOR = 2.2; 95% CI, 1.1-4.4). Death was associated with anemia (aOR = 5.3; 95% CI, 1.4-19.7), diabetes (aOR = 3.1; 95% CI, 1.4-6.7), and PWH (aOR = 3.9; 95% CI, 1.3-11.4). Direct observed therapy was protective for treatment failure (aOR = 0.5; 95% CI, .3-.9) and death (aOR = 0.5; 95% CI, .2-1.0). CONCLUSIONS: The treatment success rate was below the WHO target. Behavioral factors were most associated with LTFU, whereas clinical comorbidities were correlated with treatment failure and death. Because determinants of unsuccessful outcomes are distinct, different intervention strategies may be needed to improve TB outcomes.

RESUMEN

Objetivo: identificar as estratégias pedagógicas utilizadas no ensino da assistência de enfermagem às vítimas de acidente vascular cerebral. Método: revisão integrativa, realizada entre setembro de 2022 e março de 2023 em sete fontes de informações. Utilizou-se o aplicativo Rayyan para a seleção e a análise temática dos 485 manuscritos, destes, sete compuseram a amostra. Resultados: evidenciou-se como estratégias pedagógicas a simulação clínica, instrução interativa assistida por computador, videotape conduzido por instrutor, vinhetas, ferramenta multimídia de e-learning e o workshop. Conclusão: identificaram-se seis estratégias, com destaque para a simulação clínica por ter sido o método mais abordado pelos estudos como a única capaz de desenvolver, em conjunto, as habilidades cognitivas, psicomotoras e afetivas nos estudantes de enfermagem.


Objective: to identify the pedagogical strategies used in teaching nursing care to stroke victims. Method: integrative review, carried out between September 2022 and March 2023 in seven sources of information. The Rayyan application was used for the selection and thematic analysis of the 485 manuscripts, of which seven constituted the sample. Results: Clinical simulation, interactive computer-assisted instruction, instructor-led videotape, vignettes, multimedia e-learning tool and workshop were identified as pedagogical strategies. Conclusion: Six strategies were identified, with clinical simulation standing out as the method most often mentioned in studies as the only one capable of developing cognitive, psychomotor, and affective skills in nursing students.

Se utilizó la aplicación Rayyan para seleccionar y analizar temáticamente los 485 manuscritos, de los cuales siete constituyeron la muestra. Resultados: la simulación clínica, la enseñanza interactiva asistida por ordenador, los vídeos dirigidos por un instructor, las viñetas, las herramientas multimedia de aprendizaje electrónico y los talleres fueron las principales estrategias de enseñanza. Conclusión: Se identificaron seis estrategias, destacando la simulación clínica como el método más mencionado en los estudios como el único capaz de desarrollar habilidades cognitivas, psicomotoras y afectivas en los estudiantes de enfermería.

ABSTRACT Objective: to map the necessary steps and components for operationalizing a synchronous and observational telesimulation design in the context of developing clinical competencies aimed at students and health professionals. Method a scoping review supported by the recommendations of the Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviews: Checklist and Explanation, and by the assumptions of the Joanna Briggs Institute Reviews' manual method. The search was carried out in November 2021 in 13 databases, totaling nine studies in the final sample, which were then analyzed using Thematic Analysis. Results two categories were developed: steps and components for operationalizing a synchronous and observational telesimulation design; and challenges to implement synchronous and observational telesimulation. Conclusion operationalizing a synchronous and observational telesimulation has been supported by a six-step instructional design, characterized by planning, preparation, participation, teledebriefing, learner assessment/feedback collection and additional learning, capable of developing the students' cognitive and affective skills. The relevant challenges to this context were also highlighted, configured by the need to obtain a sufficient technological structure for remotely transmitting the telesimulated scenario and an adequately trained faculty.


RESUMEN Objetivo mapear los pasos y componentes necesarios para la operacionalización de un diseño de telesimulación síncrona y observacional en el contexto del desarrollo de competencias clínicas dirigidas a estudiantes y profesionales de la salud. Método scoping review respaldada por las recomendaciones de Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviews: Checklist and Explanation y por los supuestos del método manual de Joanna Briggs Institute Reviews. La búsqueda se realizó en noviembre de 2021 en 13 bases de datos, totalizando nueve estudios en la muestra final, analizados mediante Análisis Temático. Resultados se desarrollaron dos categorías: etapas y componentes para la operacionalización de un diseño de telesimulación síncrona y observacional; y desafíos para implementar la telesimulación síncrona y observacional. Conclusión la puesta en funcionamiento de la telesimulación observacional y sincrónica ha sido respaldada por un diseño instruccional de seis pasos, caracterizado por planificación, preparación, participación, teledebriefing, evaluación del alumno/ recopilación de comentarios y aprendizaje adicional, capaz de desarrollar las habilidades cognitivas y afectivas del aprendiz. También se destacaron los desafíos relevantes a este contexto, configurados por la necesidad de obtener una estructura tecnológica suficiente para la transmisión remota del escenario telesimulado y un cuerpo docente adecuadamente capacitado.

Realizou-se a busca em novembro de 2021 em 13 bases de dados, totalizando nove estudos na amostra final, analisados por meio de Análise Temática. Resultados desenvolveram-se duas categorias: etapas e componentes para a operacionalização de um design de telessimulação síncrona e observacional; e desafios para implementar a telessimulação síncrona e observacional. Conclusão a operacionalização da telessimulação síncrona e observacional vem sendo sustentada por um design instrucional de seis etapas, caracterizadas pelo planejamento, preparação, participação, teledebriefing, avaliação do aprendiz/coleta de feedback e aprendizagem adicional, capazes de desenvolver as habilidades cognitivas e afetivas do aprendiz. Destacaram-se, ainda, os desafios pertinentes a este contexto, configurados pela necessidade de se obter uma estrutura tecnológica suficiente para transmissão remota do cenário telessimulado e um corpo docente adequadamente treinado.

Most studies obtained good methodological quality in their content validity process.


RESUMEN Objetivos: identificar en la literatura científica qué escenarios clínicos simulados fueron desarrollados y validados para la enseñanza y el aprendizaje en enfermería. Métodos: revisión integradora, realizada en siete fuentes de información. Se utilizó el programa Rayyan para la selección, el análisis de contenido para explorar los hallazgos y la herramienta de evaluación metodológica del proceso de validación, denominada Quality Appraisal tool for Validity Studies. Resultados: inicialmente se identificaron 1.179 manuscritos y 14 formaron parte de la muestra. Se definieron dos categorías: Perfil de escenarios clínicos simulados producidos en enfermería; y Habilidades clínicas desarrolladas y sus mecanismos de evaluación. Consideraciones Finales: hubo predominio de escenarios de alta fidelidad, construidos en Brasil en los últimos cinco años, dirigidos a estudiantes de enfermería sobre las temáticas de urgencia y emergencia, cuidado materno y estomaterapia, abordando la evaluación de las habilidades cognitivas, psicomotoras y afectivas en enfermería. La mayoría de los estudios obtuvieron buena calidad metodológica en su proceso de validación de contenido.

Resultados: identificaram-se, inicialmente, 1.179 manuscritos e 14 compuseram a amostra. Definiram-se duas categorias: Perfil dos cenários clínicos simulados produzidos em enfermagem; e Habilidades clínicas desenvolvidas e seus mecanismos de avaliação. Considerações Finais: houve preponderância de cenários de alta fidelidade, construídos no Brasil, nos últimos cinco anos, voltados a estudantes de enfermagem sobre as temáticas da urgência e emergência, cuidado materno e estomaterapia, abordando a avaliação das habilidades cognitivas, psicomotoras e afetivas em enfermagem. A maioria dos estudos obteve boa qualidade metodológica em seu processo de validação de conteúdo.

Objetivo: analisar os fatores associados e o desfecho das vítimas de tentativa de suicídio atendidas em um serviço hospitalar de urgência e emergência. Método: estudo observacional, longitudinal, retrospectivo e quantitativo, com dados de 191 prontuários de vítimas de tentativa de suicídio atendidas entre 2013 e 2022 em uma unidade de urgência e emergência. Aplicou-se o Acute Physiology and Chronic Health Evalution II (APACHE II) e foram realizadas análises estatísticas descritiva e inferencial. Resultados: a maioria dos pacientes era de homens, adultos, com comorbidades de cunho mental, sendo o mecanismo de autoviolencia mais prevalente o abuso de medicamentos. A grande maioria não possuía tentativas prévias, sobreviveram ao evento e foram encaminhadas após a alta hospitalar para serviços especializados. Conclusão: os fatores associados à tentativa de suicídio são representados pelo sexo masculino, sem companheiro(a) e idade entre 18 e 59 anos. Quanto ao desfecho, aqueles que apresentam elevado escore no APACHE II demonstraram maiores chances de evoluírem para óbito.


Objective: to examine associated factors and outcomes of suicide attempt victims treated at a hospital emergency service. Method: this quantitative, longitudinal, retrospective, observational study considered data from 191 medical records of victims of suicide attempts treated between 2013 and 2022 in an emergency unit. The Acute Physiology and Chronic Health Evaluation II (APACHE II) was applied, and descriptive and inferential statistical analyses were performed. Results: most of the patients were men, adults, with mental comorbidities, and medication abuse was the most prevalent self-harm mechanism. The great majority had made no previous attempts, survived the event, and were referred to specialized services after hospital discharge. Conclusion: the factors associated with attempted suicide are being male, single, and aged between 18 and 59 years. As for the outcome, those with high APACHE II scores were more likely to die.


Objetivo: analizar los factores asociados y la evolución de las víctimas de intento de suicidio atendidas en un servicio de urgencias hospitalario. Método: estudio observacional, longitudinal, retrospectivo y cuantitativo, con datos de 191 historias clínicas de víctimas de intento de suicidio atendidas entre 2013 y 2022 en una unidad de urgencia y emergencia. Se aplicó el Acute Physiology and Chronic Health Evaluation II (APACHE II) y se realizaron análisis estadísticos descriptivo e inferencial. Resultados: la mayoría de los pacientes eran hombres, adultos, con comorbilidad mental, siendo el abuso de medicamentos el mecanismo de autolesión más prevalente. La gran mayoría no tenía intentos previos, sobrevivió al evento y fue derivado tras el alta hospitalaria a servicios especializados. Conclusión: los factores asociados al intento de suicidio están representados por el sexo masculino, sin pareja y con edad entre 18 y 59 años. En cuanto al resultado, aquellos con una puntuación alta APACHE II tenían más probabilidades de morir.

OBJECTIVE: To analyze the association of dysglycemia with clinical, laboratory, and radiographic characteristics of patients with pulmonary tuberculosis (PTB), as well as with their tuberculosis treatment outcomes. METHODS: This was a longitudinal study involving 140 patients diagnosed with PTB (positive cultures for Mycobacterium tuberculosis or positive Xpert MTB/RIF results from sputum samples). Patients were evaluated at diagnosis (M0), after completing the second month of treatment (M2), and at the end of treatment (MEND). At M0, the patients were classified into three groups: normoglycemia+PTB (NGTB); pre-diabetes mellitus+PTB (PDMTB), and diabetes mellitus+PTB (DMTB), in accordance with glycated hemoglobin levels (< 5.7%, 5.7%-6.4%, and ≥ 6.5%, respectively). Treatment outcomes were classified as favorable (cure or treatment completion) and unfavorable (death, loss to follow-up, or treatment failure). RESULTS: In our sample, 76 patients (61.4%) had dysglycemia, 20 of whom (14.3%) had DM at M0. The patients with dysglycemia, in comparison with those in the NGTB group, more frequently presented with positive sputum smear microscopy (94.2% vs. 75.9%; p = 0.003); cavities (80.2% vs. 63.0%; p = 0.03); bilateral lesions (67.4% vs. 46.0%; p = 0.02); and higher median of affected thirds of the lungs (3.0 vs. 2.0; p = 0.03) on chest radiography. No significant differences regarding outcomes were found among the groups, but tuberculosis lethality was higher in the DMTB group than in the PDMTB and NGTB groups (20% vs. 2.2%). CONCLUSIONS: PTB patients with dysglycemia had laboratory and radiographic manifestations indicative of more advanced disease, and the risk of death was higher in the DMTB group. These findings reinforce the recommendation for early screening for DM in patients with newly diagnosed tuberculosis in order to reduce the risk of death during treatment.

Objetivo: Identificar as evidências científicas da literatura sobre a inserção de máscara laríngea por enfermeiros. Método: Revisão integrativa de literatura, realizada em novembro de 2020, cuja busca ocorreu nas bases de dados Cumulattive Index to Nursing and Allied Health Literature, Cochrane, Excerpta Médica Database, Literatura Latino-Americana e do Caribe em Ciências da Saúde, US National Library of Medicine National Institutes Database Search of Healthe Web of Sciencepor meio do programa Rayyan para seleção dos estudos. Resultados: Identificaram-se 1.156 estudos, do quais oito atenderam aos critérios de inclusão. Os estudos foram categorizados em dispositivos de primeira e segunda geração. Conclusão: Conclui-se que o uso de máscara laríngea de primeira e segunda geração por enfermeiros é uma alternativa recomendada por sua rapidez, sucesso e eficácia em garantir a via aérea avançada, em especial, em situações de parada cardiorrespiratória em adultos, porém, recomenda-se verificar os efeitos adversos de seu uso.


Objective: To identify evidence-based literature on the laryngeal mask airway insertion by nurses. Method: Integrative literature review conducted in November 2020, searched in the Cumulative Index to Nursing and Allied Health Literature, Cochrane, Excerpta Medica Database, Latin American and Caribbean Health Sciences Literature, US National Library of Medicine National Institutes Database Search of Health and Web of Science through the Rayyan Study Selection Program. Results: Eight out of the 1,156 studies identified met the inclusion criteria. Studies were categorized into first and second-generation devices. Conclusion: The use of a first and second-generation laryngeal mask airway by nurses is a recommended alternative for its speed, success and effectiveness in ensuring the advanced airway, especially in situations of cardiopulmonary arrest in adults. However, adverse effects of its use should be evaluated.

Sexually transmitted infections (STIs) should form part of the comprehensive healthcare agenda for adolescents and young people (A&Y) and testing plays a strategic role in STI prevention. However, little attention has been paid to the social and programmatic obstacles to this practice among A&Y. The aim of this study is to describe the prevalence of testing for syphilis and HIV reported by a group of A&Y, analyzing its relationship with variables potentially indicative of inequities related to vulnerability to STIs. This was a cross-sectional survey applied to students at public high schools in three cities in the state of São Paulo, Brazil. The responses of 369 young people who reported having started sexual life were analyzed. The analysis included description of the frequencies of demographic and socioeconomic variables and their associations with testing for syphilis and HIV through the chi-square test. The concentration index (CI) was also calculated, with construction of the respective concentration curve for the associated variables. Positive associations were found between occurrence of testing and living in homes with up to three inhabitants, living in a marital situation with a regular or committed partnership, previous use of primary care services and previous use of hospitals or emergency services. The concentration curve for testing STIs showed significant contributions from the associated variables. The findings indicated that situations of inequity need to be addressed in order to reduce A&Y's vulnerability to STIs.
