RESUMEN

Philosophy of training students, physicians, and tutors of dentistry schools in medical informatics and the relevant curricula are discussed. Specialized software for differential diagnosis and treatment of the major stomatological diseases (dental caries, pulpitis, periodontitis, mucosal, periodontal, oral diseases, tumors, rapid methods for AIDS screening, etc.) are presented.

RESUMEN

The authors describe the software for a basic computer unified system of working places for dentists specializing in oral, dental surgery and orthodontics with diaries and special entries for each section. The computer system head physician--registration office--primary examination room--rooms of dental treatment, physiotherapy, and x-ray examination--is intended for use as part of local computer network of dental clinics in the MS DOS and Windows systems. Application of the system improves the labor efficiency by up to 35% and the financial efficacy by almost 30%. It also warrants legal protection of both physicians and patients.

RESUMEN

Functional activity of local immunity with regard to dental caries in children is liable to appreciable individual fluctuations which depend on sex, age, and season. In children aged 4 to 5 no association between the titer of antibodies to cariogenic Streptococcus and number of carious lesions was observed, whereas in those aged 9 to 19 these parameters were in inverse relationship. In children aged 11-12 and 14-16 this association is inverse. Regression analysis of nonspecific and specific characteristics of local immunity of the oral cavity in children aged under 12 suffering from caries revealed functional insufficiency of the mechanisms regulating the local immunity, which disappears after fluoroprophylaxis. Test system for screening of mutant Streptococcus cannot be recommended for wide use to detect subjects sensitive to caries, because it has certain limitations as regards the age of examinees.

RESUMEN

The authors demonstrate the potentialities of multidimensional analysis methods: component, cluster, and discriminant as exemplified by clinico-laboratory examinations of 26 patients with medium-severe periodontitis and concomitant diseases. A two-stage modification of multidimensional grouping of biomedical objects is suggested. The first stage implies detection of the principal components and their estimation for each examinee. The second stage consists in realization of multidimensional grouping method according to G. S. Kil'dishev and Iu. I. Abolentsev in relation to individual component values. Different typologically homogeneous groups of patients with a specific microbiologic and biochemical status evidently requiring a differentiated approach to treatment may be singled out within the frames of a current classification of periodontal diseases basing on clinical and laboratory data.

RESUMEN

Ninety-five patients with periodontitis of slight and medium severity were examined, 21 of them without concomitant diseases, 44 with essential hypertension, and 30 with diabetes mellitus. Besides clinical signs of periodontal abnormalities, the examinations revealed using phase-contrast microscopy four groups of microorganisms in the periodontal pouch contents: cocci, immovable and movable bacilli, and tortuous forms. Multidimensional analysis of clinico-laboratory data revealed a relationship between the counts of some microorganisms and severity of periodontal pathology.

RESUMEN

Analysis of components helped reveal the true relationships between the counts of individual representatives of the bacterial association in the contents of 18 periodontal pouches. Though the authors did their best to select a group of patients with the periodontitis course as similar as possible, the multidimensional grouping method has demonstrated the heterogeneity of this group. Four types of periodontal pouches were distinguished, differing by their bacterial status; this probably requires a differentiated approach to its correction.

RESUMEN

Clinical changes in oral mucosa minor salivary glands were examined in 30 patients with grave diabetes mellitus treated by transplantation of pancreatic islet cells without immunosuppressant therapy and in 35 patients suffering from chronic renal failure 18 of these operated on for transplantation of an allogenic kidney with immunosuppressant therapy. Clinical condition and function of the minor salivary glands were assessed according to a scheme developed by the authors with consideration for clinical signs: edemas, hyperemia, hyperplasia, retention, and destruction. Morphologic examinations were carried out in various periods after transplantation.

RESUMEN

Forty-seven adolescents with periodontal abnormalities were examined using clinical, immunologic, and functional tests (altogether 12 signs). Factor analysis helped cut down the number of signs just to 4 major factors. Interpretation of these factors and consideration of the individuals in factor space permitted singling out at least 2 types of periodontitis course, requiring, possibly, different approaches to therapy.
