RESUMEN

A new genus of xyleborine ambrosia beetle, Melanesicus Beaver Petrov gen. n., with type species Xyleborus partitus Browne (1974) is described from the Melanesian region. The following new combinations are proposed: M. caledoniae (Beaver Liu, 2016) comb. n., M. deformatus (Browne, 1974) comb. n., M. granulosus (Schedl, 1975) comb. n., all from Xyleborus Eichhoff, 1864. Melanesicus tishechkini Petrov sp. n. is described from Vanuatu, and Melanesicus nukuruanus sp. n. Beaver from Fiji. The taxonomy, distribution and biology of the species included are briefly reviewed.

RESUMEN

The four species currently placed in the genus Immanus Hulcr and Cognato are reviewed. Immanus papuanus Beaver, Sittichaya Liu sp. n. is described. The following new synonymy is proposed: Immanus desectus (Eggers, 1923) (= Xyleborus insignis Browne, 1984 syn. n.). The following new combinations are proposed: Immanus pseudocolossus (Schedl, 1942) comb. n., I. sarawakensis (Eggers, 1923) comb. n., I. trolaki (Schedl, 1939) comb. n., all originally described in Xyleborus. A key to the females of the eight species now included in the genus is provided. The taxonomy, diagnostic characters, distribution and biology of the species are briefly discussed.

RESUMEN

The genus Diapus Chapuis was erected (Chapuis 1865) for four species of pinhole borer (Curculionidae: Platypodinae) from the Oriental region and New Guinea. It was distinguished from other platypodine genera primarily by the widely separated procoxae (Chapuis 1865). Hopkins (1914) designated Diapus quadrispinatus Chapuis, 1865 as the type species of the genus. The genus is currently placed in the platypodine tribe Tesserocerini, subtribe Diapodina (Alonso-Zarazaga & Lyal 2009). Only two genera are included in the Diapodina, Diapus and Genyocerus Motschulsky (Alonso-Zarazaga & Lyal 2009, Jordal 2015). Diapus is distinguished from Genyocerus by the following characters (Wood 1993, Beaver & Liu 2007): 1. In Diapus, the scutellum is narrower and more sunken, not flush with the elytral surface posteriorly as it is in Genyocerus. 2. The mycangial pores of Diapus are sometimes fused to form a transverse or crescentic bar on each side of the midline of the pronotum. This does not occur in Genyocerus. 3. The antennal club of Diapus sometimes has a median testaceous strip lacking sensillae on the anterior face. This strip is never present in Genyocerus. 4. The males of Diapus never possess a membranous extension of the apical margin of the fourth abdominal ventrite, present in some species of Genyocerus. 5. The females of Diapus often bear deciduous mandibular appendages, which are never present in Genyocerus (Beaver & Liu 2007).

RESUMEN

The present state of knowledge of the scolytine ambrosia beetles of Thailand is summarised to provide a basis for future studies of the fauna and its economic importance in forestry, timber production and crop tree plantations. A checklist of the fauna is provided with information on local and world distribution, host trees, biology and taxonomy. Six faunal elements based on geographical distribution, and the host tree and altitudinal preferences of species are discussed. One hundred and sixty-one species are recorded from Thailand, 67 of them for the first time. The following new synonym is proposed: Euwallacea wallacei (Blandford) (= Xyleborus barbatomorphus Schedl n.syn.). The following new combinations are given: Ambrosiophilus cristatulus (Schedl), Ambrosiophilus latisulcatus (Eggers), Beaverium dihingensis (Eggers), Beaverium lantanae (Eggers) and Immanus desectus (Eggers) are transferred from their present position in Ambrosiodmus; Ancipitis depressus (Eggers) and Ancipitis punctatissimus (Eichhoff) are transferred from Leptoxyleborus; Cyclorhipidion armipenne (Schedl), Cyclorhipidion inarmatum (Eggers), Euwallacea semiermis (Schedl), Fortiborus macropterus (Schedl), Microperus nudibrevis (Schedl) and Wallacellus minutus (Blandford) are transferred from Xyleborus.

RESUMEN

The present state of knowledge of the pin-hole borers of Thailand is summarised to provide a basis for future studies of the fauna and its economic importance in forestry and timber production. A checklist of the fauna is provided with information on local and world distribution, host trees, biology and taxonomy. Six faunal elements based on geographical distribution, and the host preferences of species are discussed. Ninety-two species have now been recorded in Thailand, of which forty-three are recorded here for the first time. Three species are endemic to Thailand. The following new combinations are given: Dinoplatypus piniperda (Schedl), Treptoplatypusfulgens (Schedl), both transferred from Platypus.

We present a series of 35 patients (19 men and 16 women) with a mean age of 64 years (36.7 to 75.9), who underwent total hip replacement using the ESKA dual-modular short stem with metal on-polyethylene bearing surfaces. This implant has a modular neck section in addition to the modular head. Of these patients, three presented with increasing post-operative pain due to pseudotumour formation that resulted from corrosion at the modular neck-stem junction. These patients underwent further surgery and aseptic lymphocytic vaculitis associated lesions were demonstrated on histological analysis. Retrieval analysis of two modular necks showed corrosion at the neck-stem taper. Blood cobalt and chromium levels were measured at a mean of nine months (3 to 28) following surgery. These were compared with the levels in seven control patients (three men and four women) with a mean age of 53.4 years (32.1 to 64.1), who had an identical prosthesis and articulation but with a prosthesis that had no modularity at neck-stem junction. The mean blood levels of cobalt in the study group were raised at 50.75 nmol/l (5 to 145) compared with 5.6 nmol/l (2 to 13) in control patients. Corrosion at neck-stem tapers has been identified as an important source of metal ion release and pseudotumour formation requiring revision surgery. Finite element modelling of the dual modular stem demonstrated high stresses at the modular stem-neck junction. Dual modular cobalt-chrome hip prostheses should be used with caution due to these concerns.

RESUMEN

The use of computer navigation and conventional techniques in total knee arthroplasty remains controversial. Advocates of computer navigated techniques cite better alignment of components and reduced morbidity associated with avoidance of intramedullary instrumentation as a rationale for their use. In contrast, proponents of conventional techniques argue that better alignment does not correlate with a better functional outcome and that the conventional approach avoids the perceived risk of fracture associated with bicortical insertion of navigation tracker pins. All total knee arthoplasties performed at our institution are prospectively monitored for life in a dedicated Joint Replacement Assessment Clinic (JRAC). Patients are reviewed by physiotherapists, independent of the surgeons who performed surgery, both preoperatively and at six weeks, three and six months, and one, two and five years postoperatively (and every five years thereafter). Patients are assessed using validated outcome measures (Knee Society Score, Western Ontario and McMaster Universities (WOMAC) osteoarthritis index, Short Form SF-36 Health Survey (version 2) and a patient satisfaction score). In addition, at 6 months post surgery, a CT scan of each implanted prosthesis is performed using the Perth CT knee protocol. The findings of a single unit's experience of 777 navigated primary total knee replacements are discussed and critically compared to the body of literature that currently relates to this controversial topic.

We previously compared the component alignment in total knee replacement using a computer-navigated technique with a conventional jig-based method. We randomly allocated 71 patients to undergo either computer-navigated or conventional replacement. An improved alignment was seen in the computer-navigated group. The patients were then followed up post-operatively for two years, using the Knee Society score, the Short Form-36 health survey, the Western Ontario and McMaster Universities osteoarthritis index, the Bartlett Patellar pain questionnaire and the Oxford knee score, to assess functional outcome. At two years post-operatively 60 patients were available for assessment, 30 in each group and 62 patients completed a postal survey. No patient in either group had undergone revision. All variables were analysed for differences between the groups either by Student's t-test or the Mann-Whitney U test. Differences between the two groups did not reach significance for any of the outcome measures at any time point. At two years postoperatively, the frequency of mild to severe anterior pain was not significantly different (p = 0.818), varying between 44% (14) for the computer-navigated group, and 47% (14) for the conventionally-replaced group. The Bartlett Patellar score and the Oxford knee score were also not significantly different (t-test p = 0.161 and p = 0.607, respectively). The clinical outcome of the patients with a computer-navigated knee replacement appears to be no different to that of a more conventional jig-based technique at two years post-operatively, despite the better alignment achieved with computer-navigated surgery.

Our aim was to assess the intra- and inter-observer reliability in the establishment of the anterior pelvic plane used in imageless computer-assisted navigation. From this we determined the subsequent effects on version and inclination of the acetabular component. A cadaver model was developed with a specifically-designed rod which held the component tracker at a fixed orientation to the pelvis, leaving the anterior pelvic plane as the only variable. Eight surgeons determined the anterior pelvic plane by palpating and registering the bony landmarks as reference points. The exact anterior pelvic plane was then established by using anatomically-placed bone screws as reference points. The difference between the surgeons was found to be highly significant (p < 0.001). The variation was significantly larger for anteversion (sd 9.6 degrees ) than for inclination (sd 6.3 degrees ). The present method for registering pelvic landmarks shows significant inaccuracy, which highlights the need for improved methods of registration before this technique is considered to be safe.

We have compared a new technique of computer-assisted knee arthroplasty with the current conventional jig-based technique in 70 patients randomly allocated to receive either of the methods. Post-operative CT was performed according to the Perth CT Knee Arthroplasty protocol and pre- and post-operative Maquet views of the limb were taken. Intra-operative and peri-operative morbidity data were collected and blood loss measured. Post-operative CT showed a significant improvement in the alignment of the components using computer-assisted surgery in regard to femoral varus/valgus (p = 0.032), femoral rotation (p = 0.001), tibial varus/valgus (p = 0.047) tibial posterior slope (p = 0.0001), tibial rotation (p = 0.011) and femorotibial mismatch (p = 0.037). Standing alignment was also improved (p = 0.004) and blood loss was less (p = 0.0001). Computer-assisted surgery took longer with a mean increase of 13 minutes (p = 0.0001).

A comprehensive failure analysis was performed on 6 femoral components and 1 tibial component that fractured in service. All were Whiteside Ortholoc II total knee arthroplasty components, manufactured from cast cobalt-chromium-molybdenum alloy and porous coated. Fracture surface analysis revealed fatigue-induced failure in all cases. Most fractures occurred at regions of high stress concentration, such as sharp corners, sintered beads, and thin sections. Metallurgical examination showed significant variation in grain size, interdendritic carbides, and hardness between samples. In some cases, continuous carbide networks and voids were prominent at the bead-substrate interface. Patient weight and surgical placement were identified as contributory factors in component failure. Limitations of cast cobalt-chromium-molybdenum alloy in weight-bearing applications must be emphasized, particularly when important determinants, such as design, metallurgy, and specific clinical factors, are less than optimal.

Electrophysiological recording demonstrated that visuo-tectal projections are topographically organised after optic nerve regeneration in aged Xenopus laevis. 3H-thymidine autoradiography confirmed previous reports [Taylor, Lack, & Easter, Eur. Journal of Neuroscience 1 (1989) 626-638] that cell division had already ceased at the retinal ciliary margin. The results demonstrate that, contrary to a previous suggestion [Holder & Clarke, Trends in Neuroscience 11 (1988) 94-99], continued neurogenesis is not a pre-requisite for the re-establishment of appropriate connections with target cells.

RESUMEN

The management of femoral shaft fractures by retrograde intramedullary nailing is becoming more widespread. There have been no reported intraoperative neurovascular injuries to the surrounding anatomy using the retrograde femoral nailing technique. We report a case of injury to a branch of the profunda femoris artery during placement of the anteroposterior proximal locking screw.

The end-artery retinal vasculature of a marsupial, the fat-tailed dunnart, was defined by India ink injection and studied in wholemounts. In the adult, the vitreal vasculature supplying the ganglion-cell layer has major paired-vessels in a horizontal H shape. These vessels skirt the area centralis and visual streak that are supplied by fine end-loops. A second vascular layer of uniformly distributed endloops arises from the superficial vessels and lies at the inner nuclear/outer plexiform border. During development, vessels enter the eye via the optic nerve head to form the upper vasculature, assuming an essentially mature arrangement prior to the formation of the area centralis and visual streak. Vessels then descend to form the lower bed. Unlike the cat, the dunnart has retinal vessels that are patent throughout development, their growth is interstitial and reductive remodelling is not seen. A retinal end-artery system may have evolved in marsupials because their precocity requires a vasculature that is functional from early stages of development.

This retrospective study examined the outcome of 40 patients 50 or more years of age treated between September 1988 and September 1993 for tibial plateau fractures. Our analysis was based on a review of the medical records of all patients as well a patient self-assessment questionnaire. The 22 women and 18 men studied had a mean age of 60 (range, 50 to 76 years) at the time of injury. A satisfactory overall outcome (based on patients' self-assessment) was observed in only 14 (35%) of the 40 cases. Unsatisfactory outcomes were observed in 23 (72%) of 32 cases treated operatively and 3 (38%) of 8 cases treated nonoperatively. No significant difference in outcome was observed between the 27 patients with AO type B fractures and the 13 patients with AO type C fractures. Similarly, no significant difference in outcome was observed in patients with fractures involving the lateral plateau only, those with fractures involving the medial plateau only, and those with bicondylar fractures. There was no significant relationship between the adequacy of fracture reduction and overall clinical outcome, although a trend of more favorable outcomes was noted in patients with excellent or satisfactory reductions. The results suggest that patients of 50 or more years of age have a high rate of unsatisfactory clinical outcomes regardless of how their tibial plateau fractures are treated.

The financial and medical records of 11 patients diagnosed with a tibial nonunion were evaluated in order to assess the costs associated with the care of this clinical entity. The initial tibia fractures were seen at Hermann Hospital (Houston, Texas) between April 1991 and June 1993. We included only those patients who had a diagnosis of tibial shaft nonunion which we defined as a tibial fracture which was without radiographic or clinical evidence of progressive healing 6 months after the initial injury. A total of 9 patients were available for evaluation and 2 were lost to follow-up.

From a series of experimental studies with pigs (12-16 kg), either pulmonary edema or liver failure emerged as a distinct pathogenetic expression of fumonisin B1 (FB1) toxicosis. The primary determinant as to which pathogenetic consequence developed was the quantity (dose) of the mycotoxin fed or intubated per kilogram of body weight per day. Pigs intubated with a minimum of 16 mg FB1/kg/day developed severe interlobular edema with or without hydrothorax and variably severe pulmonary edema. Pigs intubated with < 16 mg FB1/kg/day or pigs fed diets containing 200 mg FB1/kg of feed developed marked icterus and hepatocellular necrosis. The spectrum of degrees of severity of pulmonary edema observed in the experimental pigs allowed rational speculation regarding evolution of the pathologic changes.

Using sham replacement of the proximal femur in adult mongrel dogs, shear strength at the interface between polymethylmethacrylate bone cement and cancellous bone has been found to be linearly dependent on the depth of penetration of the cement into the bone. Shear strength at the interface was increased by 82% and penetration by 74% when distal bone plugging, pressure lavage, and pressurized insertion of cement were employed. Use of a lower-viscosity cement gave a further 18% increase in penetration and shear strength. There was no film of blood at the cement-bone interface with pressurized insertion of Simplex P and Palacos R cements.

Ninety-two fresh osteochondral allografts were implanted in 91 patients with posttraumatic osteoarticular defects of the knee joint. These patients have been prospectively followed since 1972. An analysis of long-term survival of these grafts has been performed to determine their success rates: 75% at 5 years, 64% at 10 years, and 63% at 14 years. An evaluation was made between unipolar grafts, which involve only one surface of the compartment, and bipolar grafts, which involve both surfaces. The unipolar grafts had a lower failure rate at all time periods compared to bipolar grafts, with 76% survival at 5 years, 69% at 10 years, and 67% at 14 years. While investigating other factors that might affect the survival of the fresh osteochondral allografts, we calculated the influence of the anatomical location of the graft, patient sex, and patient age. There was no meaningful impact on allograft survival by either the location of the allograft or the sex of the patient. However, there was a significant effect on allograft longevity in terms of patient age, with patients under age 60 doing better. The best indication for the use of unipolar fresh osteochondral allografts is for posttraumatic defects in relatively young, active patients. Joint malalignment should be corrected to achieve best results.
