RESUMEN

We describe a 58-year-old woman who developed interstitial lung disease (ILD), polyarthritis, and anti-Jo-1 antibodies, with no clinical evidence of myositis. Despite successful treatment with corticosteroid and azathioprine for her arthritis and pulmonary condition, she developed deforming arthropathy of the hands, with periarticular calcinosis. The association of anti-Jo-1 antibodies, ILD, and periarticular calcinosis with subluxing arthropathy sine myositis is rare, with few cases reported. This report expands the clinical spectrum of the antisynthetase syndrome, which is broader than previously reported.

RESUMEN

A case of malabsorption of pancreatic etiology secondary to primary hyperparathyroidism (HPT) is reported. Normalization of calcemia was achieved with parathyroidectomy and oral administration of pancreatic enzymes was initiated. HPT is an infrequent cause of pancreatic inflammatory disease. Although its association is controversial, its inclusion among the causes of exocrine pancreatic failure allows correct diagnosis and therapeutic management with a favorable prognosis.

RESUMEN

A case of angiodysplasia of the colon with clinical manifestation of massive rectal bleeding is presented. The activity and the severity of the life-threatening bleeding episode led to emergency exploratory laparotomy with intraoperative colonoscopy which was not diagnostic. Three years later, following multiple recurrent bleeding episodes and non diagnostic explorations, the localization of the hemorrhage was obtained by abdominal scintigraphy with 99mTc-red blood cells. Surgical removal of the affected area achieved definitive cure.

RESUMEN

We discuss two cases of visceral leishmaniasis (VL) with intestinal invasion in patients diagnosed of infection due to human immunodeficiency virus (HIV). The onset was as diarrheic manifestation associated to a constitutional syndrome, with no fever. One of the patients responded to treatment with antimonials, while the other died. References on this subject are scarce, being found on a literature search two similar cases as the ones described by us. We think that with every diarrheic process in HIV+ patients, VL diagnosis should be considered, and an intestinal biopsy should be performed.

Fifty-three adult patients suffering from various degrees of essential arterial hypertension, which was not controlled with other antihypertensive drugs, participated in this study with the intention: To demonstrate the effectiveness of a 20 mg or 40 mg daily muzolimine dose as complementary treatment. To determine whether the effective dose obtained can be maintained for a long-term administration. To verify its tolerance. Therefore, muzolimine was administered in addition to the basic treatment, which went on unmodified during this study. A laboratory set of examinations, three controls of systolic and diastolic blood pressures and heart rate--each in supine and standing position--were performed before starting the treatment. At the end of the two initial weeks of treatment with administrations of 20 mg daily muzolimine doses, the diastolic blood pressure normalized at values below 90 mmHg in sixteen patients. These patients continued the muzolimine treatment taking the same dose for seven days, being submitted to two more controls during this time. As the normalization was confirmed, laboratory controls were repeated. Thirteen patients of this group continued the same treatment for three months. The remaining thirty-seven patients, who did not normalize their diastolic blood pressure, immediately started a second treatment period of two weeks, receiving a 40 mg daily muzolimine dose. At the end of this period, diastolic blood pressure normalized in twenty-four patients, decreased significantly in six, moderately in five, and remained unchanged in two. At the end of this second period, laboratory tests were repeated. Nineteen patients of this group continued the same treatment for three months.(ABSTRACT TRUNCATED AT 250 WORDS)
