RESUMEN

GD2 is a ganglioside found in the plasma membrane of the neural crest-derived cancer, neuroblastoma. GD2 is shed into the circulation of patients with neuroblastoma and could serve as a tumor biomarker to monitor tumor burden or response to treatment. We developed and validated a high-performance liquid chromatography tandem mass spectrometry (HPLC-MS/MS) method to quantify the D18:1-18:0 (C18) and the D18:1-20:0 (C20) lipoforms of GD2 in human plasma and serum. Human brain derived GD2 containing a mixture of C18 and C20 was used as the analytical standard. Samples were extracted with methanol containing dueterated-GM1 (internal standard), and analytes were separated on a Phenomenex Kinetex C18 column eluted with a gradient mobile phase composed of ammonium acetate buffer, methanol and isopropanol. An AB Sciex 4500 QTRAP mass spectrometer in negative ion mode was used to quantify the doubly charged GD2 C18 and C20 lipoform precursor ions (m/z 836.8 and m/z 850.8) that both yield a product ion of m/z 290.0. The calibration curves were linear from 4-1000 ng/mL and 6-1500 ng/mL for GD2 C18 and C20 lipoforms respectively. Inter-day and intra-day accuracy were within the acceptable validation range in plasma and serum.

Many pre-health students pursue extracurricular shadowing opportunities to gain clinical experience. The Virginia Tech School of Neuroscience introduced a formal course that provides a clinical experience superior to that received by many medical students. This course is composed of weekly 75-minute seminars that cover diseases affecting the nervous system, their diagnosis and treatment, complemented by weekly half-day intensive clinical experiences with unprecedented access to a team of neurosurgeons (in hospital operating rooms, Intensive Care Units, emergency room, angiographic suites, and wards). In the operating rooms, students routinely "scrub-in" for complex surgeries. On hospital rounds, students experience direct patient care and receive in-depth exposure to modern nervous system imaging. Students participate in two 24-hour "on-call" experiences with team providers. After call, students participate in cognitive and psychological studies to assess physiological and psychological effects of call-related sleep deprivation. Students prepare weekly essays on challenging socioeconomic and ethical questions, exploring subjects such as the cost of medicine and inequalities in access to health care. Towards the end of the course, students meet with the admission dean of the Virginia Tech Carilion medical school; they prepare a personal statement for medical school/graduate school applications, and attend a half-day block of mock medical school/graduate school interviews delivered by experienced clinicians. In lieu of a final exam, each student presents to the entire neurosurgery department, an in-depth clinical analysis of a case in which they participated. We provide details on implementation, challenges and outcomes based on experiences from three semesters with a total enrollment of approximately 60 students.

INTRODUCTION: Aboriginal Health Workers (AHWs) play a crucial role in the delivery of primary health care services in underserved rural and remote communities throughout Australia. The Mount Isa Centre for Rural and Remote Health (MICRRH), in Northwest Queensland, Australia, has been involved in training AHWs since 2001. During this time, it has been observed that while there has been interest in pursuing further education in other health careers, the uptake for advanced study by AHWs has been minimal. This exploratory study was designed to assess the career aspirations of local AHWs (both qualified and students) as well as community stakeholder views to identify barriers experienced when undertaking advanced education. METHODS: The study used a descriptive and exploratory design. AHWs and key stakeholders were invited to participate. Open-ended interviews were undertaken with nine participants in two communities in the Mount Isa Health Service District in Northwest Queensland, Australia. FINDINGS: While there was some interest expressed in careers like medicine and nursing, the majority of participants indicated a preference for advancement to management or specialist areas as AHWs. In relation to the barriers faced by AHWs and students in continuing study or career advancement, three main themes emerged: support; infrastructure; and promotion.

Cellular levels of the rapidly degraded c-myc protein play an important role in determining the proliferation status of cells. Increased levels of c-myc are frequently associated with rapidly proliferating tumor cells. We show here that myc boxes I and II, found in the N termini of all members of the myc protein family, function to direct the degradation of the c-myc protein. Both myc boxes I and II contain sufficient information to independently direct the degradation of otherwise stably expressed proteins to which they are fused. At least part of the myc box-directed degradation occurs via the proteasome. The mechanism of myc box-directed degradation appears to be conserved between yeast and mammalian cells. Our results suggest that the myc boxes may play an important role in regulating the level and activity of the c-myc protein.

Three hundred eight nursing students were classified into three perimenstrual severity groups based on their responses to the Moos Menstrual Distress Questionnaire, disregarding the number of symptoms reported. The prevalence of severe perimenstrual symptoms was 44% for strong symptoms and 18% for acute symptoms. Severity was significantly related to perimenstrual absence and to health care seeking for menstrual disorders, dysmenorrhea, and for gynecological disorders unrelated to menstruation. Severity was not significantly related to non-gynecological absence or health care seeking for non-gynecological disorders. Severe menstrual symptoms, particularly dysmenorrhea, had more of an effect on absenteeism and health care seeking than severe premenstrual symptoms.

The contribution of learning to the adult experience of illness was investigated by asking 351 nursing students how their mothers reacted to menstrual symptoms and cold symptoms during their adolescence and how their mothers behaved when they themselves had menstrual symptoms. Mothers of respondents were independently asked the same questions. Nursing students who had been encouraged to adopt a sick role for menses or whose mothers modeled menstrual distress reported significantly more menstrual symptoms, clinic visits, and disability days for these symptoms as adults. Similarly, those encouraged to adopt a sick role for colds or who lived with a chronically ill person reported more clinic visits and disability days for nongynecological symptoms. Specific types of symptom reports and health care appeared to be learned: Encouragement and modeling of the menstrual sick role were more highly correlated with symptom reports, clinic visits, and absenteeism for menstrual than for nongynecological symptoms, and encouragement of the cold sick role was more highly correlated with nongynecological than with menstrual clinic visits and absenteeism.

Two recent item factor analyses of the Minnesota Multiphasic Personality Inventory (MMPI) classified the resulting factors according to a conceptual scheme offered by Norman's (1963) five factor model. The present article empirically evaluates those classifications by correlating MMPI factor scales with self-report and peer rating measures of the five factor model in a sample of 153 adult men and women. Both sets of predictions were generally supported, although MMPI factors derived in a normal sample showed closer correspondences with the five normal personality dimensions. MMPI factor scales were also correlated with 18 scales measuring specific traits within the broader domains of Neuroticism, Extraversion, and Openness. The nine Costa, Zonderman, McCrae, and Williams (1985) MMPI factor scales appear to give useful global assessments of four of the five factors; other instruments are needed to provide detailed information on more specific aspects of normal personality. The use of the five factor model in routine clinical assessment is discussed.
