RESUMEN

Patients presenting advanced breast tumors are usually subject to major resections of the anterior chest wall tissue. Flaps taken from the abdominal wall, such as the TRAM, the external oblique flap, and the thoracoabdominal flap are frequently used for closure of this type of lesion. In this study, a different shape was planned for the skin island from the latissimus dorsi musculocutaneous flap with primary closure in V-Y for the correction of major lesions in the anterior chest wall after mastectomies occasioned by advanced breast cancer. The technique was used on eight female patients, between November of 1998 and July of 1999, victims of advanced breast cancer, who had been submitted to radical mastectomies with major resections of the cutaneous tegument. It was possible to make primary closure of lesions in the anterior chest wall, the preoperative areas of which varied between 15 x 15 and 29 x 14 cm (vertical x horizontal). This technical variant permitted use of the flap without the need to create tunnels for its advancement and rotation. It also proved to be easy to perform and presented a low morbidity rate, with three patients presenting minor complications that did not require correction through any further surgical intervention. Closure was obtained in the donor and recipient sites without the use of skin grafts or other more major procedures. According to the authors, this procedure is a viable alternative in repairing large defects in the anterior chest wall.

A surgical procedure with the transverse rectus abdominis myocutaneous (TRAM) flap for breast reconstruction is presented using parameters from the opposite normal breast to achieve a better cone shape in the new breast to project the nipple-areola complex. This cone projection is obtained through a vertical plication of both skin/fat halves of the TRAM flap and with two supraumbilical fat flaps to avoid cone collapse. The infraclavicular and axillary regions are filled with a de-epithelialized "fish-fin" cutaneous-fat or fat-only flap, which is placed as a lateral TRAM extension. The de-epithelialized lateral extremity of the TRAM flap folded over itself gives a mound shape to the lateral aspect of the new breast, and the inverted umbilical stalk attached to the TRAM flap imitates a nipple. This procedure is based on six breast reconstructions with a 2-year follow-up. The procedure is a simple, safe, and versatile way to mimic the opposite breast. It is mostly indicated for thin patients who have small to moderate breasts without ptosis or hypertrophy who refuse breast implants or request a mastopexy or reduction mammaplasty on the opposite normal breast during the same procedure.

The rejection of allotransplantation of epigastric microsurgical flaps and the effect of immunosuppression have been studied in 58 rats. Three sets of experiments were planned: (1) Wistar Furth isogenic donors and receptors (control set); (2) Brown Norway donors and Wistar Furth receptors (rejection set); and (3) Brown Norway donors and Wistar Furth immunosuppressed receptors (cyclosporin A set). Cyclosporin A (10 mg/kg/d) treated rats had a transplantation survival rate of up to 30 days: 83.3% among isogenic animals and 60% among allogeneic. There was 100% rejection by the 9th day after the transplantation in allogeneic non-immunosuppressed rats. Biopsies embedded with historesin were taken from the flap and normal contralateral skin (used as control) on the 3rd, 7th, 15th, and 30th days after the surgery. A quantitative study of infiltrating lymphocytes in the flaps, with and without cyclosporin A, was done by evaluating the local inflammatory infiltrate. A significant increase in the number of lymphocytes among the rejection and immunosuppressed groups was seen, as compared to the isogenic set. Local lymphocytosis in allogeneic non-immunosuppressed transplantations reached its highest level on the 3rd day after surgery, before gross findings of rejection, which could only be seen by naked eye on the 5th or 6th day. Therefore, we conclude that cyclosporin A is effective in preserving allogenic transplantation in rats. Biopsies of transplanted areas may contribute to earlier diagnosis of the need for immunosuppressive therapy.

From a retrospective study of 250 patients submitted to immediate breast reconstruction with TRAM flap after mastectomy for breast cancer, we reported the occurrence of pregnancy in seven cases from January 1992 to January 1996 at the Women's Health Reference Center and at the University of São Paulo Medical School. There were six immediate and one delayed reconstructions. Mean age was 35 years old. Outpatient follow-up was 13 months (mean 6-42 months). Two of the seven patients developed abdominal bulge, but no hernias were detected in this group. All patients were satisfied with the aesthetic result. Seven of them came to term. No abnormalities were found in their children. During the same period of time bulging was found in 10% of the 250 patients. Breast reconstruction with TRAM flap is safe even if patients become pregnant, and in this study TRAM flaps did not cause dyfunctional labor or significant abdominal wall damage.

RESUMEN

Strie distansae have been studies since the last century. However, many aspects like oxitalanic fiber variations in the striae, were still to be studied. This was the purpose of our work. Our results showed that in the striae, there is a decrease in the number of oxitalanic fibers when compared to normal skin. We also found a reduction in the arborecent aspect of these fibers as well as tissular fragility. The pathogenesis of striae distansae is still obscure, but the study of oxitalanic fibers can contribute to the understanding of its treatment as well as the compression of cutaneous aging.

Os autores estudam a prevalencia de fumantes entre academicos de algumas Faculdades da USP (ligadas a area de saude) do Campus de Ribeirao Preto. De um total de 1327 academicos (sendo 491 da medicina; 304 da enfermagem; 201 da farmacia e 331 da odontologia) levantaram os dados atraves de um questionario em 1146 (ou seja 86,4% da populacao) sobre o habito de fumar e caracteristicas pessoais.Concluiram que: 1. a prevalencia instantanea de fumantes nesta populacao era de 26,9% (sendo de 28,4%; 20,7%; 36,1% e 28,4% respectivamente para academicos de medicina, enfermagem, farmacia e odontologia); 2. Ha pequena diferenca entre prevalencias observadas entre homens e mulheres, (exceto na farmacia); nos primeiros anos academicos as mulheres fumam mais que os homens; 3. a maioria dos academicos fumantes iniciaram com o habito antes dos 17-18 anos de idade; 4. os academicos destas 4 areas tem um gasto anual com cigarros de cerca de Cr$ 3.735.698,40, pagando por isso um tributo ao governo de Cr$ 2.614.988,88, oferecendo um lucro aos varejistas de Cr$ 410.926,82 e contribuem para as companhias produtoras com um valor de Cr$ 709.782,70
