RESUMEN

Los servicios profesionales farmacéuticos asistenciales contribuyen a un mejor control de los problemas de salud, sobre todo en pacientes polimedicados, y a una optimización de los recursos sanitarios, lo cual conlleva una menor prescripción de medicamentos y un menor número de visitas a los centros sanitarios. Formaron parte de este estudio 78 pacientes, siendo el sexo femenino el más prevalente, con una edad media 72,49 ± 13,92 años; de ellos, 60 pacientes eran mayores de 65 años y, por tanto, considerados pacientes geriátricos. Fueron incluidos en un servicio de sistemas personalizados de dosificación y, posteriormente, se realizó un seguimiento farmacoterapéutico. Se detectaron 450 problemas relacionados con los medicamentos siendo los más frecuentes los asociados al paciente: falta de conocimiento de uso, mala adherencia terapéutica, errores en la administración…, siendo la probabilidad de presentarlos mayor en el sexo masculino. Asociados a estos problemas con los medicamentos se detectaron 160 resultados negativos asociados a la medicación; solo 5 de ellos no fueron resueltos durante la fase estudio, los más frecuentes fueron los de inefectividad no cuantitativa, es decir, aquellos que no se resuelven con una modificación de la dosis farmacológica sino incluyendo o eliminando otros principios activos. En conclusión, el servicio de sistema personalizado de dosificación precisa de una revisión de la medicación como, por ejemplo, el seguimiento farmacoterapéutico, ya que ambos permiten optimizar el tratamiento de los pacientes polimedicados, así como, un aumento del control de los problemas de salud.(AU)


Professional pharmaceutical care services contribute to better health problem control, especially in polymedicated patients, and to the optimization of health resources, which leads to less medication prescription and fewer visits to healthcare centers. This study included 78 patients, with the female sex being the most prevalent, with an average age of 72.49 ± 13.92 years; of these, 60 patients were over 65 years of age and, herefore, considered geriatric patients. They were included in a personalized medication dispensing service and, subsequently, a pharmacotherapeutic follow-up was carried out. 450 medication-related problems were detected, the most frequent of which were associated with the patient: lack of knowledge of use, poor therapeutic adherence, administration errors, etc., with the probability of presenting them being higher in the male sex. Associated with these medication problems, 160 negative medication-related results were detected; only 5 of them were not resolved during the study phase, the most frequent being those of non-quantitative ineffectiveness, that is, those that are not resolved with a modification of the pharmacological dose but by including or eliminating other active ingredients. In conclusion, the personalized medication dispensing service requires a medication review, such as pharmacotherapeutic follow-up, as both allow the optimization of the treatment of polymedicated patients, as well as an increase in the control of health problems.(AU)

To determine the diuretic activity of Sambucus nigra L. ssp. palmensis (Link) R. Bolli (SP). SP was evaluated in adult female Swiss mice. Urinary excretion volume was measured and the concentration of sodium, potassium, chloride, pH and specific conductance of 3 doses of aqueous extract (35.0, 52.2 and 70.0 mg kg-1) were determined. SP (70.0 mg kg-1) produced a higher urinary excretion (6.41 mL) and diuretic index (15%) than hydrochlorothiazide (HCTZ) (6.27 mL and 12%, respectively). The saluretic index indicates a lower sodium excretion than HCTZ (13%) and inversely proportional to the dose (8% -5%). The same is observed for potassium excretion (0.0172-0.0162 mEq.K+/100 g/6 h), which achieves a lower value than the control group (0.0166 mEq.K+/100 g/6 h), suggesting potassium retention. These results support the use of this plant species as a diuretic in Canarian folk medicine.


Determinar la actividad diurética de Sambucus nigra L. ssp. palmensis (Link) R. Bolli (SP). SP fue evaluada en hembras adultas de ratones Swiss. Se midió el volumen de excreción urinaria y se determinó la concentración de sodio, potasio, cloruro, el pH y la conductividad específica de 3 dosis de extracto acuoso (35,0, 52,2 y 70,0 mg kg-1). SP (70,0 mg kg-1) produjo una excreción urinaria (6,41 mL) e índice diurético (15%) superior a hidroclorotiazida (HCTZ) (6,27 mLy 12%, respectivamente). El índice salurético indica una excreción de sodio inferior a HCTZ (13%) e inversamente proporcional a la dosis (8% -5%). Lo mismo ocurre con la excreción de potasio (0,0172-0,0162 mEq.K+/100 g/6 h) que alcanza un dato inferior al del grupo control (0,0166 mEq.K+/100 g/6 h), lo que sugeriría retención de potasio. Estos resultados apoyan el uso de esta especie vegetal como diurético por la medicina popular canaria.

Background: This work analyzed the impact of COVID-19 on antidepressant consumption during the first year of the pandemic (2020) taking as a baseline the prescribing trends during the previous 4 years (2016-2019). For this, the province of Santa Cruz de Tenerife and the 4 islands that make it up were taken as a geographical area. Methods: Sales data in community pharmacies were taken as an aggregated database. The dose per inhabitant and day was used as an indicator of consumption. Results: In the islands of EL Hierro and La Gomera, the increase in population does not in itself justify the relevant increases observed in the consumption of antidepressants and possibly COVID-19 and its consequences on the health of the population could be responsible for these increases. On the island of Tenerife, the increase in population could justify, to a large extent, the slight increase in consumption observed. The island of La Palma presents an increase of 1.40%, but lower than the expected value taking as baseline the prescribing trends in the last 4 years, despite an increase in population of 0,95%. All the islands present the same trends in the consumption of the different subgroups of antidepressants and active pharmaceutical ingredients, although with slight variations, with the exception of the island of La Palma which presents a statistically different behavior and trends. Conclusions: These differences could be related to the socio-sanitary and demographic characteristics of each of the islands.

A 62-year-old patient, dependent, with good therapeutic adherence and the presence of psychiatric illnesses that limit his quality of life. Your soon, a caregiver since his personality disorder was diagnosed, goes to the pharmacy asking for help to deal with the situation. Study and evaluation of the case: The patient's health status was evaluated, and possible Medication-Associated Negative Outcomes (NOM) were detected that limited her quality of life. INTERVENTION: He was referred to his primary care physician with a multidisciplinary consultation report indicating possible NOM and possible modifications. RESULTS: The primary care physician accepted the intervention and began with a progressive deprescription of benzodiazepines, with respect to the extrapyramidal symptoms, the NOM was confirmed, and he was referred to his psychiatry specialist. After significantly improving their quality of life, their cardiovascular diseases were addressed, for which hygienic-sanitary measures were taken to reduce their cardiovascular risk. CONCLUSIONS: The coordination of the different health professionals allows an increase in the autonomy of the patient, a deprescription of medications and an optimization of health resources, translating into an improvement in their quality of life.
